천연 항진균 물질인 황련(Coptidis Rhizoma)은 감염성 피부염을 유발시키는 주 원인균인 Cadida albicans에 항세균 작용이 있다고 알려져 있다. 본 연구에서는 황련유래 단백질(Proteins derived from Coptidis Rhizoma, CRP)의 유효 성분을 추출하여 안정성 도모 및 피부 세포 내에 안정하게 흡수시키는 물질 전달체로서 리포좀을 제조하였다. 황련유래 단백질을 함유한 리포좀 입자의 평균 크기는 187 nm, 표면전하는 3.337 mV이며 포집효율은 33%이었다. 온도, 저장시간에 따른 CRP의 방출 거동을 확인하였으며, 리포좀의 안정성을 증가시키기 위하여 리포좀을 poly(vinyl alcohol) (PVA)로 코팅하였다. 코팅된 리포좀은 코팅 안된 리포좀에 비하여 낮은 방출율을 나타내었다. 또한, 리포좀에 함유된 CRP의 항캔디다 효과는 $50^{\circ}C$ 또는 자외선(UV)으로 24 h 처리한 후에도 지속됨을 확인하였다.
Tenacibaculum maritimum (formerly Flexibacter maritimus)은 tenacibaculosis라 불리며, 해산어에서 궤양 및 괴사성 증상을 나타내는 질병원인체 이다. Tenacibaculosis는 다양한 양식 어종에서 있어서 경제적 측면에서도 중요한 질병 중 하나이며, 피부, 입, 지느러미, 꼬리 등과 같은 어체 표면에 심각한 궤양과 괴사로 인한 손상을 유발시 킨다. 본 연구에서는 tenacibaculosis의 증상을 나타내는 넙치 병어로부터 A-7 균주를 분리하였고, 형태학적 특성, 생화학적 성상 및 유전자 분석 결과를 바탕으로 T. maritimum으로 동정하였으며, 침지법을 통하여 넙치에 인위적으로 감염되는 것을 확인하였다. Tenacibaculosis를 제어하기 위해 약욕에 의한 항생제 투여가 많이 이루어지며, 그 중 oxytetracycline이 주로 사용되고있다. 그러나, 대부분의 양식어민과 소비자들은항생제 잔류와 환경오염 등의 문제를 우려하고있으며, 비싼 항생제의 사용으로 인하여 양식어의 생산 비용도 높아지고 있다. 이러한 문제점들을 극복하기 위하여 본 연구에서는 천연 자생식물 추출물을 항생제 대체제로 사용하기 위한 연구를 수행하였다. 총 35 종류의 천연식물의 항균 활성을 검색하였고, 많은 천연식물 추출물이 T. maritimum에 대하여 높은 항균 효과를 나타낸다는 것을 밝혔다.
황백(Cortex Phellodendri: CP)은 황벽나무(Phellodendron amurense)의 건조된 수피로부터 얻어진다. 이 수피는 한국의 전통 한약제로서 설사, 황달, 무릎과 발의 통증, 요도관 및 피부 감염증에 폭넓게 사용되어 왔다. 이들 기능성 성분의 분리 및 정제는 박층 크로마토크래피, 컬럼 액체 크로마토크래피 및 HPLC와 같은 여러 분석법들이 동양의 약초연구에 이용되어 왔다. 본 연구는 CP로부터 berberine을 분리하기 위해 향류분배 크로마토크래피법(CPC)으로 효과적으로 수행하였다. 두 용매의 CPC 최적조성은 n-butanol: acetic acid: water(4:1:5 v/v/v)이었다. 이동상의 유속은 1,000 rpm 회전력에서 상승법으로 분당 3 mL 속도로 전개시켰다. CPC에서 분리된 분획분은 prep-HPLC로 정제하였다. $^1H$-NMR 스펙트럼은 4.10과 4.20 ppm에서 $3H(-OCH_3)$, 6.10 ppm에서 2H의 ($-OCH_2O-$) proton signal의 공명이 관찰되었다. 2개의 방향족 proton은 이중결합 패턴을 보였다. H-11과 H-12 doublet은 각각 7.98과 8.11에서 나타났다. $^{13}C$-NMR 스펙트럼에서는 C2와 C3의 methylenedioxy group($-OCH_2O-$), C9과 C10에 methoxy group($-OCH_3$)이 4개의 치환된 형태로 보였다. 분리 정제된 berberine의 화학구조는 $^1H$, $^{13}C$-NMR, ESI-MS 데이터 분석으로 확인하였다.
The purpose of this study is to compare hepatotoxicity of each treatment for dermatophytosis; one is the administration of the ketoconazole only and the other, ketoconazole with diphenyl-dimeththyl-dicarboxylate. Have chosen the range of 14-24 months of healthy dogs divided by two groups (group 1 and group 2) for the experiment of which test proved positive in dermatophytosis diagnosis and showed normal reaction in terms of physical examination, blood chemistry and especially of liver function. Group 1 was administrated ketoconazole orally at 10 mg/kg/day and of same dose of ketoconazole with diphenyl-dimethyl-dicarboxylate for group 2. After administering, we have tested two groups by blood collecting every one week in order to check the differences of hepatotoxicity state through AST, ALT and r-GTP, the barometers of liver function which lasted for 12 weeks. Moreover, tested Indocyanine Green (ICG), known as susceptible gauge of function of excretion before starting the experiment and tested ICG as well after 12 weeks. The experiment of result the value of group 1 in AST, ALT and r-GTP has been highly rised after administering ketoconazole for 10 weeks meanwhile, of group 2 has shown a steady state troughout the whole experiment. For ICG test, we injected 0.5 mg/kg of ICG into a vein for both groups and tested the retention rate at regular interval of 15, 30, 45 minutes. The results of retention rate in two groups were similar to before the drug administration. However, after 12 weeks the retention rate of group 1 has been delayed, on the other hand, retention rate of group 2 were a steady state. In conclusion, the administration of ketoconazole only for a long period of time induced hepatotoxicity where as, the administration of ketoconazole with diphenyl-dimethyl-dicarboxylate didn't induce hepatotoxicity. Therefore, when doctors prescribes for a dog with dermatophytosis should not administrate ketoconnazole itself but with diphenyl-dimethyl-dicarboxylate and one who has abnormal condition of liver function should not be prescribed ketoconazole treatment. If there is a case needed to prescribe ketoconazole treatment, the regular monitoring should be accompanied by at the same time.
Aeromonas hydrophila는 수생계에 흔히 존재하는 병원체로서 출혈성 패혈증, 수종, 궤양 등의 질병을 유발한다. A. hydrophila는 효과적인 감염과 다양한 환경에서 적응하기 위해 세포 외 물질(Extracellular products, ECPs)을 분비한다. 본 연구에서 사용한 A. hydrophila CK257은 매우 독성이 강한 균주로 균이 분비하는 ECP 역시 쥐, 토끼, 금붕어를 포함한 동물 모델에게 조직 손상을 일으키며 독성을 보였다. 이러한 괴사 반응을 일으키는 원인 요소를 찾기 위해 ECP의 특성을 분석하였고 그 결과, 원인 요소가 단백질이라는 것을 확인하여 이후 단백질 분리 및 정제에 초점을 맞추어 실험을 진행하였다. 균체를 제거한 배양액 상의 분비 단백질을 얻기 위하여 ammonium sulfate 침전법을 사용하였고, 이후 겔 거르기 크로마토그래피(Gel filtration chromatography)를 이용하여 단백질을 정제하였다. 정제 후 단백질은 다섯 개의 분획으로 나뉘었고 이 중 한 개의 분획이 동물 모델에 괴사와 같은 피부 손상을 일으키는 것을 확인하였다. 괴사 활성을 가진 분획의 단백질 4개의 밴드를 MALDI-TOF 시스템을 통해 분석한 결과, Peptidase M35와 Peptidase M28로 밝혀졌으며 이들은 각각 금속촉매효소(metalloprotease)임을 확인할 수 있었다. Peptidase M35와 Peptidase M28의 단백질 분해 효소로써의 기능을 확인하기 위해 카제인 분해 활성을 확인하였고, 다양한 금속 이온을 처리함에 따라 그 활성이 달라지는 것을 관찰할 수 있었다. 또한 조직에 존재하는 단백질의 한 종류인 엘라스틴(elastin) 분해능 역시 확인해 보았다. 본 연구에서는 A. hydrophila가 분비하는 물질 가운데 조직 괴사 활성을 일으키는 것으로 추정되는 Peptidase M35와 Peptidase M28의 두 가지 인자를 확인하여 동정하였다. 이후 두 가지 인자를 결손 시킨 돌연변이주를 구축하여 조직 괴사에 이들이 직접적으로 작용하는지 확인 할 예정이다.
일반적으로 쌍떡잎식물 국화과 허브의 한 종인 에키네시아는 미국 북미 대평원에 서식하는 야생 식물로 수세기 동안 감기나 또 다른 바이러스 감염에 의한 질병에 대하여 면역 기능을 증진시키는 전통 식물 약제로 널리 사용되어왔다. 본 연구에서는 생물반응장치를 이용하여 조직 배양된 에키네시아 식물체에 대하여 화장품 성분으로써 응용 가치를 평가하였다. 이미 몇몇 보고된 논문에서 에키네시아는 조직재생, 상처 치유, 그리고 염증 억제 등의 약리학적 활성에 대한 연구가 진행되었다. 본 연구에서는 생물반응장치를 이용하여 조직 배양된 에키네시아 추출물에 대하여 화장품 성분으로서 효능, 효과를 평가하였다. 실험 결과 조직배양된 에키네시아 추출물은 항산화 효과 및 단백분해효소 발현 억제 효과가 자연 상태의 에키네시아 추출물과 비교하여 우수한 결과를 보였다. 이러한 결과들은 피부보호를 위한 화장품 성분으로서 응용 가능성을 제공할 수 있을 것으로 사료된다.
혈관염이 특징적인 질환으로 구강궤양, 음부궤양, 안구염증과 관절, 피부, 중추신경계, 위장관 등 여러 장기를 침범할 수 있는 만성 염증성 질환이다. 베체트병의 발병원인이나 기전에 대해 확실히 밝혀지지는 않았으나 유전적인 소인이 있는 사람에서 감염 등 환경적인 요인이 면역 반응에 이상을 일으켜 질병의 여러 증상이 발현된다. 주조직복합체 (major histocompatibility complex, MHC)와 non-MHC gene등 다양한 유전자들이 베체트병의 병인으로 관여한다. 이 연구에서는 IL-6의 유전적 다형성이 한국인 베체트병의 감수성에 관여하는지를 확인하였다. 유전자 다형성은 VNTR (variable number of tandem repeat), RFLP (restriction fragment length polymorphism), DHPLC (denaturing high performance liquid chromatography) 방법을 이용하여 확인하였다. 실험 결과, 베체트 환자와 대조군에서 IL-6prom의 유전적 다형성은 관련성이 없었지만 IL-6vntr에서는 유전형과 대립형질의 빈도가 다르게 나타났다. IL-6vntr*C 대립유전자가 한국인 베체트병과 연관성이 높게 나타났다. 이런 결과를 확인하기 위해 앞으로 여러 집단과 다른 유전자의 연구가 필요하다.
목적: 본 연구는 Wolter 금속판을 이용한 제 2형 원위부 쇄골 골절 치료의 임상적 결과를 평가하고자 하였다. 대상 및 방법: 2004년부터 2007년까지 Wolter 금속판으로 치료한 제 2형 원위부 쇄골 골절을 가진 16명의 환자를 대상으로 하였다. 평균 나이는 32.6세 이었고, 수술 후 평균 추시 기간은 22.9 개월이었다. 정복 및 골유합의 평가는 수술 직후 및 최종 방사선 소견을 이용하였고, 기능적 평가는 Kona의 기능적 평가 기준 및 Constant 점수를 사용하였다. 결과: Kona의 기능적 평가기준에 의해 우수 12예, 양호 3예, 보통 1예의 결과를 보였으며 Constant 점수는 평균 90점을 보였다. 16예 모두에서 골 유합을 확인하였으며, 합병증으로는 1예에서 금속판의 갈고리 부분이 돌출되어 피부에서 촉지 되었으며, 1예에서 견봉 골절이 발생하였으나 2예 모두 만족할 만한 골유합 소견을 보였으며, 그 외 견봉-쇄골 관절의 관절염, 감염증 등 다른 합병증은 관찰되지 않았다. 결론: 제 2형 원위부 쇄골 골절에서 Wolter 금속판을 이용한 고정법은 쉽게 만족스런 정복과 견고한 고정을 얻을 수 있고, 불유합의 발생이 적으며, 임상적 결과가 우수한 수술 방법이라고 생각된다.
Purpose: $Allevyn^{(R)}$(Smith & Nephew, England) is a type of polyurethane foam material with good wound discharge absorption. $Acticoat^{(R)}$(Smith & Nephew, England) is a silver coated dressing material which is effective in infected wound. The purpose of this study is to compare the effects of dry gauze, $Acticoat^{(R)}$ and $Allevyn^{(R)}$ on infected full-thickness wound healing in rat. Methods: One hundred and twenty rats were divided into 3 groups: group I(dressing with dry gauze, n=40), group II(dressing with $Allevyn^{(R)}$, n=40), group III(dressing with $Acticoat^{(R)}$, n=40). A $15{\times}15mm$ square full-thickness wound was made on the dorsum and left open for 24 hours. The size of wound defects were measured each dressing changes. The histological evaluation was performed on the 3rd day, 7th day, 14th day, 21th day. Results: After the wound was left open for 24 hours, typical findings of bacterial infection was observed. After the 7th day, group III showed larger area of epithelialization, smaller defect size compared to those of two other groups. Complete replacement by fibrotic scar tissue was observed in group III with no signs of inflammation on the 14th day. By day 21, the average defect size in group III was decreased from initial 100% to 3.63%. while in group I and II, it was decreased to 62.66% and 53.62%, respectively. Conclusion: $Acticoat^{(R)}$ is an effective tool in the treatment of infected wound.
Purpose: Aquacel Ag$^{(R)}$ is a hydrofiber wound dressing integrated with ionic silver. Sorbact$^{(R)}$ is a hydrophobiccoated dressing that uses the hydrophobic interaction with microbes. In this study, we compared the wound healing effects and the antibacterial effects of Medifoam$^{(R)}$, Betadine soaked, Aquacel Ag$^{(R)}$ and Sorbact$^{(R)}$ dressings against MRSA-infected wounds. Methods: Eighty rats were divided into four groups: Medifoam$^{(R)}$; Betadine soaked; Aquacel Ag$^{(R)}$; and Sorbact$^{(R)}$. A $1.5{\times}1.5cm$ square full-thickness wound was made on the dorsum of each rat and infected with MRSA. Twenty-four hours thereafter, each dressing was applied to the wound and changed every other day. One, 3, 7, 11 and 15 days after the wound infection, swab culture grade, wound bed appearance score, and wound defect size change were evaluated, and 7 and 15 days after, histologic evaluation was compared between the groups. Results: The bacteria load of wounds in the Sorbact$^{(R)}$ group decreased earlier than in the other groups. The wound bed appearance score of the Sorbact$^{(R)}$ group also increased quicker, compared with the other groups. However, the size of wounds of the Aquacel Ag$^{(R)}$ group decreased more rapidly, compared with other groups. From the histologic point of view, there was no significant difference between Betadine soaked, Aquacel Ag$^{(R)}$ and Sorbact groups. Conclusion: The hydrophobic dressing using Sorbact$^{(R)}$ showed a more rapid reduction in the MRSA load and an elevation in the wound bed appearance score, but a slower decrease in wound size change due to detachment of wound bed tissue when the dressing was eliminated in the low exudate wound. The silver-containing hydrofiber dressing using Aquacel Ag$^{(R)}$ was more effective in ultimate wound size reduction, but some debris was trapped in the wound tissue and induced foreign body reaction in the high exudate wound. Thus, ongoing selection process of treatment based on the evaluation of the infectious wound state will be very important.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
