Case report: Full mouth rehabilitation for patient with heavy occlusal force and excessive abrasion (강한 교합력과 과도한 마모를 보이는 환자의 전악수복 증례)

  • Many of the patients with extensive abrasion need comprehensive restorative treatment. The abrasion is usually caused by attrition, besides of it, there are many reasons for it. The plan of treatment should be started on assessment of the type of attrition and the etiologic analysis. Patient with well-developed masticatory muscle, alveolar process, and high occlusal force and also with little muscle length difference between the stable and the contracted state should be carefully assessed for the vertical dimensional loss and the restoration should be carefully designed. Decrease of tooth length can be compensated by the growth of the alveolar bone height; therefore, consistency of the occlusal vertical dimension is maintained. Accordingly, a careless increase of the vertical dimension can produce muscle fatigue, depressed tooth and pain, and fracture of the restoration. In this case, the patient with multiple tooth abrasion and clenching habit, the edentulous maxillary area is restored with amalgam inserted RPD, and the dentulous area of the maxilla and mandible are treated with fixed restoration accompanying with the increase of vertical dimension. Consequently, we are going to report about the satisfying result in both functional and esthetic aspects.

A Study of Legibility by Monitor Letter Color with an Eye Tracker (아이트래커를 이용한 모니터 글자 색상에 따른 가독성에 대한 연구)

  • Purpose: This study will be a basic research providing data which can reduce eye strain using monitor and electric media by measuring legibility of text color on monitor. Methods: Total experimental subjects who don't have color anomaly and best corrected visual acuity was over 1.0 were 50 and their mean age was $22.83{\pm}1.47$. Reading speed test were carried out with 163 words and subjects who could read over 123 words were selected for legibility examination. Monitor background color was white and letter colors were black, blue, red, and yellow for the legibility examination. Results: In case of text color of black, blue, red, and yellow, Event duration(sec) was measured to 41.89, 42.89, 45.32, and 56.28, respevtively, and reading error (number) was measured to 1.94, 2.74, 3.36, and 5.14. Average of fixation duration(sec) was measured to 0.25, 0.26, 0.27, and 0.32. Conclusions: In this paper, event Duration, reading error and average of fixation duration which can measure the legibility was reduced in a sequence of black blue, red, and yellow.

Symptom Clusters in Advanced Cancer Patients (진행암 환자의 증상군)

  • Advanced cancer patients tend to present multiple concurrent symptoms which are often moderate or severe in intensity. To date, the majority of studies have focused on either a single symptom, such as pain, fatigue, or depression or associated symptoms. While this approach has advanced understanding of some symptoms, it has offered clinicians not much guidance for treating several multiple concurrent symptoms in cancer patients. So in recent years, a few symptom management studies attempted a new approach of focusing on symptom clusters instead of individual symptoms. A symptom cluster is defined as two or more concurrent symptoms that are related to each other. If we better understand symptom clusters, interrelations of symptoms, and their common mechanisms in advanced cancer patients, clinicians can more effectively control multiple, concurrent symptoms and reduce drug side effects. And clinicians can also predict any other symptoms, functional performance, and the relationship between symptom clusters and survival in advanced cancer patients. At present, there is inconsistency in symptom clusters due to many unexplained mechanisms and various means to assess and analyze symptoms. Still, with further study, the approach to symptom clusters rather than individual symptoms could more effectively control symptoms and improve patients' quality of life.

Improving Effects of Multigrain Feed on Endurance (혼합곡 식이의 지구력 향상 효과)

  • This study evaluated the effect of multigrain supplementation on exercise-related changes in running time and glycogen storage capacity of male SD rats engaged in treadmill exercise. Thirty-six 6-week-old rats were divided into six groups: normal (Nor) fed normal feed, control (Con) fed with 20% normal feed and 80% milled rice, test group one (GI) fed with multigrain feed I, test group two (GII) fed with multigrain feed II, test group three (GIII) fed with multigrain feed III, and test group four (GIV) fed with multigrain feed IV. Endurance tests by treadmill machine were administered after 24 days of multigrain feed supplementation and adaptive training. Running time was extended and glycogen storage capacity increased in the multigrain-treated group compared to the non-treated group. Also, the fatigue indicators of inorganic phosphorous, CPK, and lactate concentration were all reduced in the multigrain feed group compared to the control group after 25 min and/or exhausted exercise. But there was no difference in GOP, GTP, lactate, or LDH concentrations between the groups. Our results demonstrated that endurance improved with multigrain feed in rats. Specifically, running time, glycogen storage capacity, inorganic phosphorous, CPK, and lactate serum concentration increased. Importantly, the improvements in endurance brought about in the GII group fed with waxy barley was the greatest among the experimental groups.

Test-bed Development for Long-term Monitoring of Small Bridge Asset Management (소규모 교량 자산관리를 위한 계측 테스트베드 구축)

  • While Category 1 and Category 2 bridges are intensively inspected and carefully managed, many small bridges that are not in the Category 1 and 2 are often forgotten until they decay in serious condition. Since many of these small bridges located in the populated city, failure of them would cause huge negative impact on the community. This paper focuses on the small size concrete bridges for timely repair and rehabilitation work for the effective operation and management. Test bed is developed to define the key parameters to forecast the long term performance of the bridges, mostly concrete box bridges. Key parameters suggested in this paper are cumulative fatigue due to repetitive heavy traffic loads and the acid attacks for concrete material deterioration. The cumulative fatigue is measured by the use of the mileage concept. For the long term data collection and inspection, stable and easy to use data collection system is installed as a test bed. The contribution of this research work is on the development of the test bed to define the key parameters of bridge deterioration.

  • Electric Pole is a supporting beam used for power transmission/distribution which is sensitive to external force change of environmental factors. Therefore, power facilities have many difficulties in terms of maintenance/conservation from external environmental changes and natural disasters that cause a great economic impact. The aerial wire cause elasticity due to the influence of temperature, or factors such as wind speed and wind direction, that weakens the electric pole. The situation may lead to many safety risk in day-to-day life. But, the safety assessment of the pole is carried out at the design stage, and aftermath is not considered. For the safety and maintenance purposes, it is very important to analyze the influence of weather factors on external forces periodically. In this paper, we analyze the acceleration data of the sensor nodes installed in electric pole for maintenance/safety purpose and use Kalman filter as noise compensation method. Fast Fourier Transform (FFT) is performed to analyze the influence of each meteorological factor, along with the meteorological factors on frequency components. The result of the analysis shows that the temperature, humidity, solar radiation, hour of daylight, air pressure, wind direction and wind speed were influential factors. In this paper, the influences of meteorological factors on frequency components are different, and it is thought that it can be an important factor in achieving the purpose of safety and maintenance.

Application of Prefabricated Retaining Walls with Steel Lagging (강재 요소를 적용한 조립식 흙막이 벽체에 관한 연구)

  • It has been known that the conventional retaining wall system with timber lagging and H pile has several problems such as the irregular gap between H-piles, cutting or adding to standard timber, back fill over first step excavation, and especially break-down accident at the disjoint of wall system. In the practical excavation, these problems may lead to worker's accident and the inefficiency of construction economy. To solve the above problems, a new method using prefabricated retaining wall was proposed and verified. The characteristics of the new method is to replace timber wall as free-sliding steel-lagging and connector. To check its verification and application, laboratory tests such as bending strength, tensile strength, and fatigue strength were carried out. Also, a pilot test in the field and numerical simulations under various ground conditions were performed. From the researches, it is found that the prefabricated retaining wall plate can be superior to the conventional timber lagging plate in the strength. It is also found that the proposed methods can be effective in the reuse of retaining wall plate and safe in the disjoint of wall system. Finally, it is desired that the proposed method will be effective in the reduction of the imported timbers and helpful in the safety of retaining wall construction.

  • Many domestic users complain about severe pain to the waist, neck, and shoulder as well as work performance because the domestic chair design was developed with western anthropometry dimension and design criteria. Ergonomic design standards are needed for office chair design to reduce stress and poor physical posture for various user body types. In this study, we have suggested design dimensions recommended from previous studies and Korean anthropometry data. We also have reviewed users' preference dimension through measured subjects and analyzed differences between users' preference dimension and the previous design criteria to verify physical appropriateness. We evaluated general office chairs and adjustable chairs which can adjust to fit each person. we also analyzed how each design dimension was reached and affected the human body by evaluation of physical discomfort and comfort. We have found seat height is very important in a workstation. If the seat height is high, it effects the thigh. If width, height, and angle of the backrest are wrong, fatigue to the shoulder, neck, and waist, etc.. As a result of this experiment, we suggested that the height of a seat for Koreans be 425mm for the fixed type and 365-484mm for the adjustable type. Also other design recommendations were suggested in the thesis. In conclusion, our research will be very important in the database because it provides adjustable ranges to fit user's body types in the various design fields.

The Effect of Self-Help Program for Promotion of Health for Arthritis Patients at various Health Centers in Seoul (서울시 보건소에서 실시한 관절염환자 자조관리과정 평가)

  • The purpose of this study was to evaluate the effects of self-help programs which had been conducted at each health center of Seoul using the protocol developed by Korean Rheumatology Health Professionals Society(KRHPS). This program was developed to initially increase self-efficacy, and then to decrease the symptoms and finally to increase the functions of the patients. This study was designed as one group pretest-posttest design. Data were collected by self-administered questionnaires for 140 out of 303 program participants, and only 102 subjects were analyzed because of missing values. The subjects were composed of 25.5% of rheumatoid arthritis patients and 60.8% of degenerative arthritis patients. The results showed that arthritis self-help program was effective in increasing self-efficacy and then improving pain, fatigue, depression, and activities of daily living. But the number of pain sites was not decreased. This result explains that degenerative arthritis patients composed of more than half of the participants has less number of pain sites than rheumatoid arthritis, and their pain was not completely relieved. As a result, it was confirmed that arthritis patients' physical and psychological states can be improved by self-help program in the community. These findings emphasize the implication of this program for each institute to promote health of arthritis patients.

The Comparison of Perceived Stress, Quality of Life between Patients with HIV Positive and Patients with Pulmonary Tuberculosis (HIV감염자와 폐결핵환자 간의 스트레스지각과 삶의 질의 비교)

  • Objectives: The object of this study was to compare perceived stress and quality of life among patients with HIV infection, patients with pulmonary tuberculosis and normal controls. Methods: Stress response inventory(SRI) and Symptom checklist 90-Revised(SCL-90-R) were used to measure perceived stress responses and psychopathology. Smithklein Beecham quality of life scale was used to measure quality of life. Results: Patients with HIV infection scored significantly higher on scores of tension, anger, depression, fatigue and frustration subscale of the SRI than those with pulmonary tuberculosis and normal controls. Scores of the SCL-90-R, somatization, depression, anxiety, hostility, phobic anxiety, paranoid ideation and psychoticism subscale were also significantly higher in patients with HIV infection than those with pulmonary tuberculosis and normal controls. Patients with HIV infection scored significantly lower in quality of life than those with pulmonary tuberculosis. In patients with HIV infection, age had a significantly negative correlation with scores of somatization, obsessive-compulsive, interpersonal sensitivity, phobic anxiety, paranoid ideation and psychoticism subscale of the SCL-90-R. but the level of education had a significantly positive correlation with somatization, obsessive-compulsive, interpersonal sensitivity, phobic anxiety, paranoid ideation and psychoticism subscale of the SCL-90-R. Conclusion: The results suggest that patients with HIV positive were likely to have higher levels of perceived stress response and psychopathology, and lower quality of life than those with pulmonary tuberculosis and normal controls.
