Pancreatic polypeptide family-PP, NPY, PYY (위장홀몬에 대한 최신 정보)

  • 위장관에서 분비되는 많은 국소호르몬 중에서 이미 널리 알려진 것을 외에 구조적인 공통점을 가진 새로운 group인 pancreatic polypeptide family에 대한 정보를 요약하여 보았다. PP, NPY 및 PYY는 그 구조적 유사점들에도 불구하고 분비되는 장소가 판이하여, PP는 위와 췌장의 국소호르몬으로서, NPY는 중추 및 말초신경의 신경충동전달체로서, 또 PYY는 하부위장관 점막에서부터 분비되어 전신 호르몬처럼 작용하고 있다. 이들 NPY와 PYY의 혈관수축작용과 혈압상승작용은 우리의 흥미를 끌기에 충분하다. 특히 PYY는 주로 하부장관 특히 결장과 직장의 분비조직에서 많이 발견된다는 점과 돼지의 뇌혈관을 수축시켰다는 점이 주목할 만한 것이다. 우리나라에는 많은 고혈압 환자가 있으며, 고혈압을 비롯한 여러가지 원인에 의한 뇌졸중(뇌출혈 포함) 환자도 많이 발생하고 있다. 민간의 속설에 의하면 출혈성 뇌졸증이 기온이 낮은 계절에 보다 자주 발생하는 경향이 있으며, 이른아침 화장실에서의 배변시 자주 발생한다고 한다. 물론 이러한 속설을 뒷받침할 만한 자료가 없으므로 학문적 견지에서의 신빙성 여부를 논할 가치는 없겠으나, 배변시 비교적 강한 자극을 받고 활발한 운동을 하는 결장과 직장에 PYY의 분비세포가 집결되어 있다는 점과 PYY의 뇌혈관수축작용 및 혈압 상승작용 등을 고려해 볼 때, 출혈성 뇌졸증과 PYY의 상관관계는 추구해볼 만한 사항이라 사료된다.

A Case of Extramammary Paget's Disease (유방외 Peget병 1예)

  • Extramammary Paget's disease is uncommon intraepithelial carcinoma of the skin and frequently associated with a subjacent or a regionally proximate carcinoma. We have experienced a case of extramammary Paget's disease affecting 71 year-old man. The patient has been suffered from a well demarcated, and slowly growing erythematous plaque on the left suprapubic area for 3 years. A biopsy specimen reveals infiltration of typical Paget's cells within the epidermis and the adnexa. We review the literature briefly.

One Case of Incomplete Double Urethra (선천성 불완전 중복요도 1례)

  • Duplication of the urethra within a single penile shaft is a rare anomaly. These can be divided into those that are in the sagittal plane, which is most common, and those that occur side by side. In some cases, the accessory channel is complete, having a separate bladder opening and no communication with the more normal ventral urethra. In other cases, the accessory urethra is incomplete, either ending blindly or communicating with the urethra distal to the bladder neck. We report on a 20-year-old male with incomplete double urethra.

A Case of Hepatitis Developing after Open Heart Surgery used Halothane Anesthesia (개심술 마취후 발생한 급성간염 1례)

  • Although halothane is one of the most widely used inhalation anesthetics, it may cause postanesthetic complications such as halothane hepatitis. Halothane hepatitis has been reported intermittentely with variable incidence. However it is not easy to prove halothane as a causative agent, because there are many factors causing postoperative hepatic dysfunction. The author had a case of acute hepatitis developing after open heart surgery used halothane. 37-year-old female underwent an open heart surgery for ASD repair under halothane anesthesia On the 14th postoperative day, she developed high fever of 38 C. Liver function tests showed marked elevation of SGOT, SGPT, and bilirubin, followed by gross jaundice. HB, Ag(-) and HB, Ab(+) were reported. She died of acute respiratory, hepatic, and renal failure on the 19th postoperative day Possible causes of the hepatitis were considered halothane, blood transfusion, and drugs.

  • Histopathological diagnosis of brain stem glioma should be performed for the purpose of the determination of its management and clinical course, but its surgical biopsy has been followed by high mortality and morbidity. We performed the tissue sampling for histological examination with BRW stereotaxic system under local anesthesia successfully.

  • The upshoot and downshoot in Duane's retraction syndrome is believed to be related to a leash effect from the lateral rectus muscle. When the eye is rotated into the adducted position, the lateral rectus muscle slips over the globe, producing the up-and downshoot on adduction. The splitting of the ends of the lateral rectus into a Y configuration prevents the rotation of the globe up or down by stabilizing the muscle's position on the eye. Three patients with Duane's retraction syndrome demonstrated abnormal vertical movement on adduction and underwent a Y-splitting on the lateral rectus. In all patients, marked decrease in the up-and downshoot is noted after surgery.

  • Splenic abscess is an uncommon lesion and may be present either as a localized area of infection in the spleen or as a part of generalized sepsis. The diagnosis is difficult because of the rather nonspecific clinical picture. Hence, splenic abscess has a high mortality rate and is after diagnosed only at autopsy. Computerized tomography(CT) offers the clinician a reliable tool for the diagnosis of intra-abdominal abscess. A successful outcome is dependent on an early diagnosis and prompt treatment by splenectomy with antibiotic cover. We experienced a care of splenic abscess which was diagnosed by CT and treated by splenectomy with antibiotics. Postoperative course was relatively uneventful.

  • Maternal and fetal effects of anesthesia for 423 cesarian section, performed during the past three and quarter years period in this hospital, have been evaluated. It is emphasized that the selection of anesthetic agent and method should depend upon the physical status of the patients and the ability of both obstetrician and anesthesiologists. Author results were as follows: 1. The incidence of c-section was 13.9% of total deliveries. 2. More than about three fourths of total cases were emergency cases. 3. Almost all cases were operated under general anesthesia. 4. The good physical states of patients resulted in better Apgar score of infants. 5. The shorter the interval from induction of anesthesia to delivery, the better was the Apgar score of infants.

A Case of Convulsive Seizure Following Spinal Anesthesia in a Geriatric Patient with COPO (만성 폐쇄성 폐질환자에서 척추 마취후 발생한 경련발작 1례)

  • In the geriatric patient with COPD, incidence of postoperative pulmonary complication is higher than young patient. Therefore, some anesthesiologists preferred spinal anesthesia to general anesthesia for surgery of the perineum, lower extrimities, and pelvic extra peritoneal organs. But, during spinal anesthesia, the same careful observation is requiered as during general anesthesia. We experienced a case of the convulsive seizure at about 1 hour after spinal anesthesia for open prostatectomy in a 76-year-old male patient wit COPD. It was suspected that his convulsive seizure be resulted from hypercapnea combined with hypoxia following upper airway obstruction. This patient was treated successfully by ultrashort acting barbiturate and controlled ventilation.

A Case of The Krukenberg Tumor (Krukenberg 종양 1예)

  • There have been reported cases that the Krukenherg tumor had been primary ovarian carcinoma, But the Krukenberg tumor is generally known as one special type of metastatic ovarian carcinoma, which histologically consists of nest of mucin filled signet-ring cells in a cellular, nonneoplastic stroma. The most common gastrointestinal tract origin for Krukenberg tumor is the stomach, and the next frequent is the large intestine. Generally the Krukenberg tumor is difficult to diagnose and treat until somewhat enlarging its size. We experienced a case of the Krukemberg tumor on the remained ovary after the previous unilateral adnexectomy, which was metastasized from adenocarcinoma of stomach. We present this case with a brief review of literature.
