암성통증관리 만족도 (Patient Satisfaction with Cancer Pain Management)

  • 목적 : 본 연구는 국내 암성통증관리지침이 제시된 후 환자들의 통증관리에 대한 만족도, 만족 및 불만족 요인, 통증관리전략을 규명하여 앞으로의 통증관리에 있어 의료인이 지향해야할 세부적인 방향을 제시하기 위함이다. 방법 : 2002년 7월부터 11월까지 서울소재 2개 대학병원 혈액종양내과에 입원 또는 외래치료중인 암환자 59명을 대상으로 하였으며, 미국통증학회의 Patient Outcome Questionnaire(APS-POQ) 및 여러 선행연구를 참고로 연구자들이 구성한 설문지 및 의무기록 열람을 통해 자료를 수집하여 분석한 조사연구이다. 결과 : 1) 대상자의 특성 : 연구대상자의 24시간 동안 가장 심했을 때 통증 평균은 6.74점($0{\sim}10$점 범위), 24시간 평균 통증의 평균은 3.80점이었으며, 통증조절이 이루어진 후 느낀 통증의 정도는 평균 2.93점이었다. 일상 생활에 지장을 주는 정도 합계 평균은 $25.03{\pm}12.82$점($0{\sim}50$점 범위)으로 중등도의 지장을 느끼고 있었으며, 통증에 대한 환자의 염려 항목 중 3점 이상($0{\sim}5$점 범위)인 항목은 질병악화, 중독, 그리고 내성에 대한 항목이었다. 2) 암성통증관리현황 : 진통제를 적절히 복용하고 있는 대상자는 66.1%(39명)이었다. 대상자의 33.9%만 통증조절을 위해 약물이외의 간호중재방법을 사용한 적이 있었고, 의료진으로부터 통증관리에 대한 교육을 받아본 대상자도 35.6%로 나타났다. 3) 통증관리에 대한 환자의 만족도 및 그 이유 : 통증관리에 대한 평균 만족 정도는 $4.19{\pm}1.14$ ($1{\sim}6$점 범위)이었으며, 72.9%(43명)의 대상자가 만족한다고 응답했다. 불만족 하는 이유는 '통증조절 후에도 통증이 감소되지 않았다' '통증을 호소했을 때 빨리 혹은 시기적절하게 대처해주지 않았다', '환자가 통증을 호소할 때, 무관심하며 형식적으로 대했다', '약물 투여방법, 작용시간, 부작용 등 통증 관리에 대한 정보제공이 없었다' 이었으며, 만족하는 이유는 '통증조절 후 통증이 감소했다', '통증을 호소할 때 의료진이 관심을 가져주었다', '의사나 간호사가 신속하게 통증조절을 해주었다' '의사를 신뢰하기 때문' 이었다. 4) 암성통증관리의 만족 또는 불만족에 영향을 미치는 요인 : 만족 집단과 불만족 집단의 통증정도 및 일상생활에 지장을 미치는 정도에 있어서 두 그룹간 통계적으로 유의한 차이가 없었다. 통증관리에 대한 환자의 염려 항목 중 '훌륭한 환자는 통증을 호소하지 않는 자이다'에 있어서 만족 집단의 평균점수가 불만족 집단의 평균점수보다 통계적으로 매우 유의하게 높았다. 결론 : 선행연구들에 비해 암환자의 통증관리에 대한 만족도는 증가하였으나 아직도 30%정도의 대상자는 만족하지 못하는 것으로 나타났다. 암환자의 통증 관리에 대한 만족도 향상을 위해 통증관련 약물, 통증 완화를 위한 간호중재방법 및 환자들의 통증과 관련된 잘못된 지식을 개선하는 내용이 포함된 환자교육이 절실히 요구된다.

간호학생의 통증관리에 관한 지식 (Nursing students관 Knowledge on Pain Management)

  • Pain management of postoperative patients or patients suffering from terminal cancer became one of the most important task of nurses in recent years. This study analyzed basic knowledge of 175 student nurses in Taegu on the pain assessment and management. The questionnaires included five areas : Assessment, pharmacological actions, medication, classification of opioids, and psychological dependence of analgesics. The results indicated that nursing students, in general, lacked knowledge on pain management : Correct responses on pain assessment varied between 9.7 and 100% , classification of opioids was in ranges of 61.7 to 95.4% ; Pharmacological actions was 16.6 to 80% : and medication was 14.9 to 85.7%. Less than 12.6% of students correctly identified the frequency of psychological dependence. These results suggested that the need of reinforcement on the pain management in formal nursing education.

신생아중환자실 간호사의 통증간호중재에 영향을 미치는 요인 (Factors Affecting Nursing Interventions for Pain among Nurses in Neonatal Intensive Care Unit)

  • Purpose: The purpose of this study was to investigate NICU nurses' nursing interventions for pain and factors affecting nursing interventions for pain. Methods: Participants were 120 NICU nurses from 5 university hospitals located in Daejeon Metropolitan City and Chungcheong Province. The collected data were analyzed using descriptive statistics, independent t-test, one-way ANOVA and Duncan test, Pearson correlation coefficients and multiple regression analysis with SPSS Windows 23.0 IBM program. Results: The factors affecting nursing interventions for pain included knowledge about non-pharmacological nursing interventions for pain and self-efficacy about nursing interventions for pain. These variables explained 28.3% of nursing interventions for pain. Conclusion: The results of this study suggest that it is necessary to develop education programs in which effective nursing interventions for neonatal pain are considered. The programs should be made available to NICU nurses.

89톤급 선미식 안강망어선의 선형치수에 관한 연구 (A Study on the Hull-dimension of 89 ton class Stow-net Vessel with Stern-fishing)

  • 근해안강망업업에서 경제성이 있고 안전성이 향상된 어선의개발에 중요한 기본자료가 될 최적한어선의 규모 및 기본치수를 선정하게되는 본 연구는 그 내용을 최적규모에 영향을 주는 어업 및 어선의 기본성능분석과 최적규모 결정을 위한 시스템의 개발로 크게 구분할 수 있다. 최적규모에 여향을 주는 어업 및 어선의 기본성능분석에서는 근해안강망어업의 어업자원 평가, 어업실태조사 등을 하고, 기본성능의 분석은 대표적인 톤급에서 빈번히 살생되는 안전성, 조업성등과 어선의 기분수치와의 인과관계를 ISM기법으로 분석하여 최적규모 결정모델에 이용하였다. 한편 최적규모결정을 위한 시스템을 경제성 평가 및 최적화 모델 구성, 어선의 초기설계조직의 구성, 및 시스템분석 S/W개발로 구분하여 연구를 수행하였으며, 최종적으로 규모 및 기분치수를 찾았다. 근해안강망어업의 개발대상어선의 대표적인 톤급으로 냉동설비를 성치한현축식 89톤급 및 선미식 89톤급으로 구분하여, 즉 어선(I)군과 어선(II)군으로 분류아여 항해속력 9~11노트에서 최적규모 및 기본치수를 구하였다. 즉 어선(I)군과 어선(II)군에 대한 경제성 분석을 통하여 얻어진 결과에 의햐면 어선(I)군에서는 향해 속력 10노트에 현측식 89톤 근해안강망어선(냉동설비 설치)이 경제적인 것으로 평가된다. 특히 어선(II)군의 향해속력 10노트인 {어선(II-2)군}대상어선은 분석된 모든 어선중에서 가장 경제성이 좋고 안전성을 만족하고 있는 것으로 최정적인 결론을 얻게 되었따. 이와 같은 연구로부터 다음과 같은 결론을 도출하였다. 즉 우리나라 안강망어업에 적합하게 이용될 수 있다고 사료되는 89톤급 선미식 안강망어선의 기본치수는 배의 길이(Lpp):27.0~28.0m, 배의 폭(B):6.6~6.8m, 배의 깊이(D):2.75-2.85m,방형 계수$C_b$:03685-0.698, 항해 속력(Vs):10노트인 것으로 분석된다. 감소되지 않았다' '통증을 호소했을 때 빨리 혹은 시기적절하게 대처해주지 않았다', '환자가 통증을 호소할 때, 무관심하며 형식적으로 대했다', '약물 투여방법, 작용시간, 부작용 등 통증 관리에 대한 정보제공이 없었다' 이었으며, 만족하는 이유는 '통증조절 후 통증이 감소했다', '통증을 호소할 때 의료진이 관심을 가져주었다', '의사나 간호사가 신속하게 통증조절을 해주었다' '의사를 신뢰하기 때문' 이었다. 4) 암성통증관리의 만족 또는 불만족에 영향을 미치는 요인 : 만족 집단과 불만족 집단의 통증정도 및 일상생활에 지장을 미치는 정도에 있어서 두 그룹간 통계적으로 유의한 차이가 없었다. 통증관리에 대한 환자의 염려 항목 중 '훌륭한 환자는 통증을 호소하지 않는 자이다'에 있어서 만족 집단의 평균점수가 불만족 집단의 평균점수보다 통계적으로 매우 유의하게 높았다. 결론 : 선행연구들에 비해 암환자의 통증관리에 대한 만족도는 증가하였으나 아직도 30%정도의 대상자는 만족하지 못하는 것으로 나타났다. 암환자의 통증 관리에 대한 만족도 향상을 위해 통증관련 약물, 통증 완화를 위한 간호중재방법 및 환자들의 통증과 관련된 잘못된 지식을 개선하는 내용이 포함된 환자교육이 절실히 요구된다. 지구조 판별도에서 지판이 침강 섭입하는 지판 경계부(destructive plate margin) 중 화산호의 조구적 영역에 도시된다. 본역의 화산암을 생성시킨 마그마는 $Al_2O_3$와 CaO 함량의 관계도, mode에서 나타나는 사장석 반정, 분화가 진행될수록 부의 Eu 이상이 증가하는 것 등에서 사장석의 분별이 우세한 분별결정작용을 거쳤음을 알 수 있다. 안산암질암을 관입한 중상 암맥에서 측정한 암석 년령은 $97.0{\pm}6.8~94.5{\pm}6.6$, 데사이트질암은 $68.9{\pm}4.8,\61.5{\pm}4.9~60.7{\pm}4.

일개 병원 간호사의 암환자 통증관리에 대한 지식과 태도 (Nurses' Knowledge about and Attitude toward Cancer Pain Management: A Survey from Korean Cancer Pain Management Project)

  • Purpose: This study was conducted to examine the nurses' knowledge about and attitude toward cancer pain management. Method: Study subjects of this retrospective cross-sectional survey study were 343 nurses sampled conveniently in a tertiary teaching hospital in Seoul. Seven measurement tools were used to collect data on demographic data, knowledge about and attitude toward cancer pain management, barrier to cancer pain management, knowledge about and concerns for the opioid use, and professional education of cancer pain management. Result: Nurses showed knowledge deficit when their knowledge on the cancer pain management and the opiod use was evaluated using two case scenarios and the 15-item questionnaire respectively. Ninety-five percent of the nurses believed that cancer pain management is a major problem. However, inadequate staff knowledge of pain management was rated as the single most important barrier to adequate pain management by 20.4% of nurses. Only 10.9% and 23.2% of the nurses replied that nursing school's education and professional education in cancer pain management respectively, were adequate. Conclusion: This study confirms the existence of knowledge deficits and attitudinal barriers among Korean nurses that can impede cancer pain management. This study suggests a need for professional education for nurses on cancer pain management.

일개 종합병원 입원 암 환자와 암 환자 가족원의 암성통증관리에 대한 지식 및 태도 (Pain Management Knowledge and Attitude of Hospitalized Patients with Cancer and Their Family Caregivers)

  • Purpose: The aim of this study was to investigate pain management knowledge and attitude in cancer patients and their family caregivers. Methods: The subjects were 52 hospitalized cancer patients and 52 their family caregivers in a hematooncologic unit in one general hospital. Data were collected via self-reported questionnaires and interviews in 2008. Results: 46.2% of the patient participants reported pain and 50% of them were taking opioid analgesics. Levels of knowledge about cancer pain and its management in both patient and caregiver participants were low, whereas, the scores of knowledge of the caregivers were significantly higher than that of the patients. The attitudes toward cancer pain were not significantly different between patients and caregivers. Both cancer patients and caregivers had some misconcepts in using opioid analgesics and about cancer pain. Patients' and caregivers' level of knowledge about cancer pain and its mangement demonstrated positive correlation. Conclusion: Education about cancer pain and its management should target both cancer patients and their family caregivers. Also the education should focus on intervening the misconceptions that patients and their caregivers have regarding cancer pain and its management.

간호사의 아동 통증 중재에 대한 지식 및 태도 (Nurses′ Knowledge and Attitude to Pain Management in Children)

  • Purpose: The purpose of this study was to examine the nurses' knowledge and attitude to pain management in children and explore the status of pain management in clinical practice. Method: The subjects were 131 nurses enrolled in 3 university affiliated hospitals. Questionnaire method was utilized for data collection. Data was analyzed by SPSS statistical program. Result: The result was as follows: 1. The overall mean score of the pain knowledge was 16.88 (58.2%). The percentages of correct response in test subsections were pain assessment 45.5, choice of medication and drug action 48.7, and pain statement 79.6 respectively. 2. 70.3% of the nurses agreed or strongly agreed that the procedural pain should always be eliminated. And, 44.3% of the nurses agreed or strongly agreed that post-operative treatment of pain in children should always aim at eliminating the pain completely. 3. There was a significant relationship between pain attitude and knowledge related facts about pain(r=.217, p<.01). 4. When asked to identify the point of self-reported pain on a 0-10 scale at which the nurse would give medication, point 7 was mostly identified (29%). 5. Only 19.8% of the nurses utilized pain rating scale to assess children's pain. 6. The nonpharmacologic interventions nurses used most were position change, emotional support, and massage. 7. 68.7% of the nurses indicated that they learned about pain management from their current working environment. Also, Mostly helpful resource identified in increasing their pain management knowledge and skills was the continuing education program. Conclusion: The findings showed that there was a need to develop educational program for adequate pain management for children. Further research study is recommended to examine the effectiveness of the intervention methods for children's pain.

수술 후 통증에 대한 간호사의 지식 및 태도 (The Knowledge and Attitudes of Nurses on Post-Operative Pain)

  • Purpose: To examine knowledge and attitudes of nurses on post-operative pain, and to find the factors that hinder pain management by the nurses. Method: Data was collected using a questionnaire from all the nurses working in the surgical units and intensive care units in a hospital in Seoul between March 12 and 22, 2007. Data was analyzed with descriptive statistics, t-test, $x^2$ test, and Pearson Coefficient Correlation. Result: The average knowledge score on pain was $9.33{\pm}1.55$, and that for analgesics was $6.89{\pm}2.00$. There was a significant difference in knowledge of analgesics in terms of career(p=0.012), present work place(p=0.024) and education(p=0.042). The knowledge on pain etiology was significantly different in career. Around 61.1% of respondents answered that they would administer analgesics immediately if patients complaint pain, and 94.1% re-administer analgesics if the VAS score is over 5.69.3% thought that their knowledge was adequate for pain management. The attitudes of pain management were significantly different in career. Conclusion: We found that a further improvement on nurses' knowledge on pain management and analgesics is necessary. This study also suggests a need for professional education for nurses on post-operative pain management.

슬관절 전치술 노인 환자의 수술 전 자가통증조절기 사용 교육이 수술 후 환자에게 미치는 효과 (The Effect of Pre-Operative Patient Controlled Analgesia Education on Elderly Patients with Total Knee Arthroplasty)

  • Purpose: The purpose of this study was to investigate the effects of preoperative patient controlled analgesia (PCA) education on total knee replacement arthroplasty (TKRA) patients' PCA usage, level of pain, the frequency of pro re nata (prn) administrations, number of ambulations/day and continuous passive motion (CPM) angle after TKRA. Methods: This research used the non-equivalent control group pre-test and post-test design. Forty-five TKRA patients at a hospital in Seoul, Korea were included for in the study. For the experimental group, a 20-minute education session was provided a day before surgery. Data were analyzed using Windows SPSS Statistics 21.0 program. Results: Knowledge was higher in the experimental group than in the control group (p<.001). Incidence of nausea was lower in the experimental group (p=.01). No significant differences were found in post-operative pain scores, the frequency of prn analgesics administrations, dizziness, number of ambulations/day and CPM angle. Conclusion: This study showed that pre-operative PCA education could be an effective nursing intervention for increasing patient knowledge on PCA and nausea reduction after TKRA.

아동간호사의 통증관리 수행에 영향을 미치는 요인 (Factors Influencing Pain Management Practice in Pediatric Nurses)

  • Purpose: The purposes of this study were to explore knowledge, barriers, and self-efficacy in relation to pain management practice, and to identify factors influencing pain management practice in pediatric nurses. Methods: A descriptive correlational study was conducted. The participants were 237 pediatric nurses from a metropolitan city. Data were analyzed using t-test or analysis of variance and Pearson correlation and multiple regression analyses. Results: The mean percentage of correct answers on the children's pain management knowledge scale was 58.8%. Child and parent related factors were the main barriers for pain management. Self-efficacy to assess children's pain across developmental stages was particularly low. Pain management practices for assessing pain and non-pharmacological interventions were relatively low. Factors significantly affecting children's pain management practice were current conditions of work department and self-efficacy in pain management, and these factors accounted for 37.5% of the variance in pain management practice. Conclusion: The results suggest that an integrative education program needs to be developed to improve self-efficacy in children's pain management practice. Moreover, good communication, building cooperative relationships with children and parents, and a more active role by pediatric nurses are required to carry out more effective pain management.
