Effectiveness of $Myprodol^{(R)}$ in ram Management (통증 조절에 있어서 $Myprodol^{(R)}$의 효과)

  • Background : Many analgesics are used to relieve the pain of various causes. Among these, $Myprodol^{(R)}$, a combination analgesic with codeine, ibuprofen and paracetamol, was recently used as a new analgesics. This study was performed to survey the kinds of diseases, side effects and pain relief effect of $Myprodol^{(R)}$ in clincal practice. Methods : This retrograde study surveyed the medical records of 183 patients treated with $Myprodol^{(R)}$ at Pain Clinic, Pusan National University Hospital. From medical records, the disease entities, the pain characteristics, duration of $Myprodol^{(R)}$ medication, the analgesic effect and side effects were evaluated. Results : $Myprodol^{(R)}$ is used in the treatment of cancer pain (64.3%) and non-cancer pain (35.7%). Among side effects of $Myprodol^{(R)}$ medication, nausea with vomiting, constipation and generalized edema were common in cancer pain, but epigatric pain was common in non cancer pain. $Myprodol^{(R)}$ was more effective in non cancer pain than cancer pain. Conclusions: $Myprodol^{(R)}$ was used in cancer and non-cancer pain patients. In cancer pain patients, $Myprodol^{(R)}$ was effective in early cancer pain but, not effective in advanced cancer. $Myprodol^{(R)}$ was also used and effective in non-malignant benign chest pain, lumbago, post-operative pain.

Effects of Kinesio Taping on Craniovertebral Angle and Balance Ability in Subject with Forward Head Posture

  • Forward head posture is one of the most recognized types of poor head and neck alignment. Poor head and neck alignment posture is a major contributor to compromised balance and neck pain, due to abnormal joint position sense and proprioception. Kinesio taping is an intervention method used clinically for the management of pain. Kinesio taping may produce its effects through pain reduction, stimulation of blood circulation, induction of muscle relaxation which provides correction of joint position, and providing stability to the muscles and joints without limiting the range of motion. Many studies have proved that kinesio taping has positive effects on the reduction of pain and improves alignment, on the other hand, some studies have not found. Kinesio taping may provide immediate pain relief and improved alignment following the application, but there is insufficient evidence to support sustained relief beyond that time and they recommended future studies to examine the benefits of kinesio taping as this would have a greater value in clinical practice. Therefore, this study is to investigate the effects of kinesio taping on the alignment of head posture and dynamic balance ability in people with forward head posture.

  • Electrical stimulation is a common method for successful pain management for both acute and a some cases of chronic pain. The incidence of cervical sprain is very high with automobile accidents. Treatment of cervical sprain is consists of analgesic drugs and physical therapy. Lower back pain is a common problem in pain clinics. back pain management are complex, so we have difficulty to choose best treatment modality. The prevalence of herniated lumbar disc(HLD) is 1~3% of lower back pain. The cases of laminectomy varies between 10~20% and postoperative pain is prolonged for several day. We applied Acupuncture like TENS (ALTENS) on the digit for cervical sprain patients and post laminectomy pain patient for three days. The result was very satisfactory. And we found that total hospital days in ALTEND groups are shorter than control group in both disease entities. In conclusion, acupuncture like TENS on finger control gate is an effective method of the pain management.

The Effect of Cancer Pain Management Education on the Pain and the Concerns of Pain Management in Cancer Patients (암성 통증관리 교육이 암 환자의 통증과 통증관리 염려에 미치는 효과)

  • Pain management is a major issue in caring of cancer patients. Because pain management cancer patient of does not control effectively, it is important to educate reporting pain and using analgesics for having cancer patient's concerns and anxiety. The purpose of this study was to identify the effect of cancer pain management education on the pain and concerns of pain management in cancer patients. This study was a quasi-experimental as nonequivalent control pretest-post test design. The subjects of this study consisted of 50 (experimental group 25, control group 25) patients hospitalized in K university hospital in Busan. The data were collected from December 1, 2001 to April 12, 2002. The measurement tool for the concerns of pain management had used questionnaires interpretated by Kim(1999) developed by based Ward(1993) and pain nominal scale. The collected data were analyzed frequency, percentage, mean, SD, $X^2$-test, t-test, ANCOVA. The results of this study were as follows: 1. The 1st hypothesis : "The experimental group which had received the cancer pain management education were lower than the control group in the score of pain" was not supported (p>0.05). 2. The 2nd hypothesis : "The experimental group which had received the cancer pain management education were lower than the control group in concerns of pain management" was supported (F=5.285, p<0.01). In conclusion, the cancer pain management education can know what was effective to decrease in the concerns of pain management in cancer patients. Therefore, Pain Management Education must be positively utilized in clinical situation.

A meta-analysis of intervention studies on the effects of self-management in knee osteoarthritis (무릎 골관절염 대상자에게 적용한 자가관리 중재의 효과 메타분석)

  • The purpose of the this study was to provide objective evidence through a systematic review and meta-analysis regarding the effects of self-management in knee osteoarthritis. Articles published between 1999 and 2013 from periodicals indexed Ovid Medline, CINAHL, PubMed, RISS, KISS and other databases were selected, using the following key words: Osteoarthriti* OR OA, self-(management OR care OR help). A selection of 11 studies(7 randomized controlled trials and 4 quasi-experimental interventions that were used were exercise, education, massage and self-help group. As a result of mera-analysis, self-management significantly effected pain, physical function, activities of daily living, and self-efficacy. In particular, self-management showed great effect on ADL, and moderate effect on pain and physical function. Based upon these result, it is necessary to develop a standardized self-management program for knee osteoarthritis patients.

Analysis of Pain Records for Cancer Patients Complaining of Moderate or Severe Pain (중등도 이상의 통증을 호소하는 암환자의 통증간호기록 분석)

  • Purpose: This is a retrospective study that investigated cancer patients' complaints of moderate or severe pain to analyze pain characteristics, pain relief interventions and their effects. Methods: The participants of this study were 363 patients who were hospitalized in the cancer ward for three to 30 days and scored 4 points or higher on the pain severity assessment. Results: The most frequent region of pain was the abdomen. The most frequent factor that exacerbated pain was movement. The most frequent pain alleviating factor was administration of analgesics. The most frequent pain type was breakthrough pain, and the most frequent non-pharmaceutical intervention for pain control was heat therapy. Among all, analgesics were routinely prescribed for 52.2% of the participants. Morphine sulfate was the most frequently used analgesic while Gabapentin was the most frequently used non-narcotic analgesic. At the time of discharge, 82.5% of the participants marked their pain intensity as 3 points or lower. Conclusion: For cancer patients complaining of moderate or severe pain, it is important to actively control pain from the beginning of admission. Thus, it is necessary to educate not only cancer patients using narcotic analgesic for pain control and their families but nurses about the effects and side-effects of drugs. Moreover, patients and their families need to learn how to assess and record pain at home to collect data that can be referred for future treatment.

The Analgesic Effects of Transcutaneous Electrical Nerve Stimulation and Interferential Currents on the Experimental Ischemic Pain Model: Frequency 50 Hz (실험적 허혈 통증 모델에서 경피신경전기자극과 간섭파전류의 진통 효과 비교 : 주파수 50 Hz를 중심으로)

  • Interferential currents (IFC) and transcutaneous electrical nerve stimulation (TENS) are used for pain management. This study compared the analgesic effects of IFC and TENS on experimentally induced ischemic pain in otherwise pain-free subjects using a modified version of the submaximal-effort tourniquet technique. The subjects were 14 volunteers (7 male, 7 female) without known pathology that could cause pain. Their mean age was $26.7{\pm}2.0$ years. A single-blind, sham controlled, parallel-group method was used. The primary outcome measure was the change in the self-report of pain intensity during 1 of 3 possible interventions: (1) IFC, (2) TENS, or (3) sham electrotherapy. The IFC and TENS were administered on the forearm, and the sham electrotherapy group received no current output via a dummy stimulator. The results show that a 2-way repeated-measures analysis of variance revealed that there was no change in pain intensity during treatment when all 3 groups were considered together. The mean to pain intensity with the IFC intervention was no different than with TENS. Thus, Statistical analysis showed that both interventions decreased the pain intensity ratings significantly and the difference between interventions was not simply insignificant. IFC has been shown to be more comfortable than TENS in present studies and is likely to be better accepted and tolerated by patients, clinical investigation is warranted.

Effects of the Education about the Self-application of the Kinesio-Taping for the Adult Women with Chronic Low Back Pain on Pain, Satisfaction and Gait (키네시오 테이핑 자가적용 교육이 만성요통 성인 여성들의 통증, 만족도 및 보행에 미치는 영향)

  • We aimed to analyze the effects of Kinesio-Taping on pain, satisfaction, and gait, and to provide basic data on the self-application of this method. The participants were 30 adult women with chronic low back pain. Kinesio-Taping was applied for 2 weeks, followed by 4 weeks of self-application education and self-application. The participants' pain levels were analyzed using the visual analog scale, satisfaction was analyzed using the Canadian Occupational Performance Measure, and gait was analyzed in terms of stride length, step length, step width, and gait time using the Walkway MG-1000 gait analyzer. The results of the pain showed the statistically significant decrease steadily (p<0.001) and the results of the satisfaction showed the statistically significant increase steadily (p<0.001) from the application of the Kinesio-Taping. Also, according to the In-Object verification between pre-test, taping application (after 2 weeks) and Self-application (after 4 weeks), the results of the satisfaction showed statistically significant difference (p<0.001). According to the gait analysis, the results of the stride length and step length showed the statistically significant increase steadily (p<0.001) and the results of the step width and gait time showed the statistically significant decrease steadily (p<0.001) from the application of the Kinesio-Taping. Also, according to the In-Object verification between pre-test, taping application (after 2 weeks) and Self-application (after 4 weeks), the results of the step width showed statistically significant difference (p<0.001). Thus, the self-application education about the Kinesio-Taping should be considered to reduce the pain and improve the performance level of the activities of daily life through the improvement of the self-management ability and to induce the active participation in the self-management activities and increase the satisfaction.

A study on the treatment of muscle pain using a high-frequency pain treatment device (고주파 통증치료기를 이용한 근육통 치료에 대한 연구)

  • After the recent COVID-19 outbreak, more and more people are paying attention to health care. Many people feel uncomfortable in life due to muscle pain after sports or exercise as a hobby and often take a break from exercise. After exercising for a long time or exercising excessively, pain appears throughout the body within 24 to 48 hours after exercise. This pain is called delayed onset muscle soreness (DOMS). In this study, the effectiveness of the pain relief of delayed onset myalgia was verified by using a high-frequency pain therapy device that generates deep heat. EMG was measured before and after pain treatment at ordinary times, RMS values were obtained through analysis, statistical analysis was performed using the SPSS program, and it was judged to be statistically significant. In addition, statistical analysis of the RMS value between normal and after pain treatment was conducted to confirm that there was no significance, and to confirm how much it returned to normal after pain treatment. In addition, since the standards for pain are different for each person even if the size of the same pain is the same, the NRS questionnaire for the size of pain was conducted, and the size of pain felt by the subjects was analyzed using the SPSS program and statistical significance was obtained. Therefore, as a result of this study, the high-frequency pain therapy device generated deep heat in the pain area to raise the temperature, and expanded the arteries and capillaries to increase blood flow, thereby increasing blood circulation and metabolism, and alleviating the pain of delayed onset muscle pain.
