Special Practitioner exists based on the need for special fields that are required to manage difficult patients with specialized skills distinguished from General practitioner. The purpose of dental specialist system is to manage patients who are not treatable by general practitioner by training doctors specializing in one specific field, thereby, enhancing health of the public. Oral Medicine deals with orofacial pain, oral soft tissue disease, forensic dentistry, oral diagnosis. In these days, living environments of city life bring about increase in stress which ultimately lead to increase in prevalence of temporomandibular disorder, oral soft tissue disease, neuropathies such as trigeminal neuralgia, and hence, the number of patients seeking help of those symptoms tend to increase. The purpose of this study was to analyze the rate of referrals and the routes of patients seeking help by investigating 3,707 patients who visited Department of Oral Medicine, College of Dentistry, Yonsei University, from August, 2004 to April, 2005. The results were as follows: 1. The types of disease referred to the Department of Oral Medicine were orofacial pain, oral soft tissue disease, forensic dentistry, oral diagnosis. 2. The rate of referrals was 58.51% which implies that more than half of the patients visiting the Department of Oral Medicine were referred. 3. The types of institution which made referrals were analyzed, and as a result, dental clinics made most of the referrals with the rate of 83.23%, while that from medical or oriental medical institutions was 16.78%. 4. Among the patients who visited the Department of Oral Medicine on their own, 30.52% gained the information about the Department of Oral Medicine from internet and mass media, and from neighbors. From the above results, most of the patients visiting the Department of Oral Medicine were found to be referred not only from the field of dentistry but also from the field of medicine. These suggest that Oral Medicine manages patients who are not treatable by general dental practitioners, and medical specialists tend to cooperate with the Department of Oral Medicine to manage diseases associated with Oral Medicine. Therefore, the role of Oral Medicine can be considered to be important as a special field, and the results of this study should be considered when working out a policy of Dental specialist system on demand and supply of residents in the future.
This article is to report a new technique for reconstruction of the anteromedial and posterolateral bundles of anterior cruciate ligament by separate tensioning and fixation of the each bundle. Method : Tibial and femoral tunnels were made with conventional technique of anterior cruciate ligament reconstruction. Tibial tunnel was enlarged $5\~7$ mm in anterior-posterior direction to make oval it in cross section. When preparing the Achilles tendon allograft, bone plug portion was trimmed as the conventional technique. The tendinous portion was trimmed as two separate bundles by dividing the tendinous portion longitudinally, so the graft is shaped like 'Y'. The bone plug portion of allograft was inserted into the femoral tunnel and fixed with absorbable cross pins. Two ligamentous portionss of the distal part of the grafts were tensioned separately at the external orifice. Anteromedial bundle was fastened under maximum tension with the knee flexed 90 degrees by post-tie method. The posterolateral bundle was fixed by the same technique with the knee in full extension. Then, an absorbable interference screw was inserted between the two bundles upto the upper end of the tibial tunnel, to get more initial rigidity of the reconstructed graft as well as to locate the two bundles in more anatomic position.
In order to investigate the effects of heat treatment of three animal by-products(feather meal, tallow meal, viscera meal) on in situ ruminal degradation characteristics and gastrointestinal availability of dietary crude protein(CP), three ruminally and duodenally cannulated dry Holstein cows were employed. Cows were fed a diet containing 60% concentrate and 40% orchard grass hay, and had free access to water and mineral block. Experimental feeds were processed for 4 hr at 149$^{\circ}C$ in a forced-air oven, and were passed through a 1-mm screen. Degradation kinetics of feed protein in the rumen were fitted to an exponential type model, and intestinal availability was estimated by the mobile nylon bag technique. Effective CP degradabilities in the rumen for feather meal, tallow meal and viscera meal were 30.2%, 75.0% and 56.4% at 5% passage rate per hour(k=0.05), respectively. In addition, heat treatment increased effective ruminal CP degradability on feather meal and viscera meal treatments, whereas decreased in tallow meal treatment(P$<$0.05). Gastrointestinal CP disappearances of feather meal, tallow meal and viscera meal were 56.2%, 18.6%, and 37.9%, respectively. In addition, heat treatment decreased the gastrointestinal CP disappearance on feather meal and viscera meal treatment, but increased in tallow meal treatment(P$<$0.05). Intestinal availability of rumen undegradable protein(A-UDP) was 80.4% for feather meal, 83.8% for tallow meal and 86.9% for viscera meal. In addition, heat treatment increased A-UDP on feather meal and tallow meal treatment, 94.0% and 91.3%, respectively, but decreased on viscera meal treatment, 76.5%(P$<$0.05).
Nowadays a large amount of wastewater containing metal working fluids and cleaning agents is generated during the cleaning process of parts working in various industries of automobile, machine and metal, and electronics etc. In this study, aqueous or semi-aqueous cleaning wastewater contaminated with soluble or nonsoluble oils was treated using ultrafiltration system. And the membrane permeability flux and performance of oil-water separation (or COD removal efficiency) of the ultrafiltration system employing PAN as its membrane material were measured at various operating conditions with change of membrane pore sizes and soil concentrations of wastewater and examined their suitability for wastewater treatment contaminated with soluble or insoluble oil. As a result, in case of wastewater contaminated with soluble oil and aqueous or semi-aqueous cleaning agent, the membrane permeability increased rapidly even though COD removal efficiency was almost constant as 90 or 95% as the membrane pore size increased from 10 kDa to 100 kDa. However, in case of the wastewater contaminated with nonsoluble oil and aqueous or semi-aqueous cleaning agent, as the membrane pore size increased from 10 kDa to 100 kDa and the soil concentration of wastewater increased, the membrane permeability was reduced rapidly while COD removal efficiency was almost constant. These phenomena explain that since the membrane material is hydrophilic PAN material, it blocks nonsoluble oil and reduces membrane permeability. Thus, it can be concluded that the aqueous or semi-aqueous cleaning solution contaminated with soluble oil can be treated by ultrafiltration system with the membrane of PAN material and its pore size of 100 kDa. Based on these basic experimental results, a pilot plant facility of ultrafiltration system with PAN material and 100 kDa pore size was designed, installed and operated in order to treat and recycle alkaline cleaning solution contaminated with deep drawing oil. As a result of its field application, the ultrafiltration system was able to separate aqueous cleaning solution and soluble oil effectively, and recycle them. Further more, it can increase life span of aqueous cleaning solution 12 times compared with the previous process.

Temporomandibular joint arthritides is divided into osteoarthritis, osteoarthrosis and polyarthritis. Because the signs, symptoms and radiographical features of osteoarthritis and osteoarthrosis are similar without arthralgia, diffenential diagnosis is difficult. Also non-radiographically change in early Osteoarthritis leads to misdiagnose. Planar bone scan and SPECT are useful to detect bone change early. This study was carried out in order to make diagnostic criteria of planar bone scan and SPECT. Three hundred and four temporomandibular joints were examined with clinical examination, computerized tomograph, planar bone scan, and SPECT. The obtained results were as follows. 1. If temporomandibular joint simple uptake ratio of patient in twenties is over 1.397%, it's condition may be osteoarthritis. And simple uptake ratio over in thirties-fourties may mean osteoarthritis. 2. It may mean osteoarthritis of temporomandibular joint that the number of coronal and transverse SPECT frame with hot spot is over four. 3. Destructive stage may goes on, if simple uptake ratio is over 1.370% in tweenties and over 1.104% in thirties-fourties. 4. If the number of coronal SPECT frame with hot spot is over four, temporomandibular joint may be on destructive stage in tweenties, thirties-fourties. And if the number of transverse SPECT frame with hot spot is over three, it may be on destructive stage in all ages. 5. When patient complains subjective arthralgia and palpation arthralgia, bone change may be more active than each arthralgia. 6. Osteoarthritis may progress gradually worse in 4.5 anamnesis. And then it may be stable gradually and turn to osteoarthrosis.

Background : Bronchial artery embolization has been established as an effective means to control hemoptysis, especially in patients with decreased pulmonary function and those with advanced chronic obstructive pulmonary disease. We evaluated the effect of arterial embolization in immediate control of massive hemoptysis and investigated the clinical and angiographic characteristics and the course of patients with reccurrent hemoptysis after initial succeseful embolization. Another purpose of this study was to find predictive that cause rebleeding after bronchial artery embolization. Method : We reviewed 47 cases that underwent bronchial artery embolization for the management of massive hemoptysis, retrospectively. We analyzed angiographic findings in all cases before bronchial artery embolization and also reviewed the angiographic findings of patients that underwent additional bronchial artery embolization for the control of reccurrent hemoptysis to find the clauses of rebleeding. Results : 1) Underlying causes of hemoptysis were pulmonary tuberculosis(n=35), bronchiectasis(n=5), aspergilloma(n=2), lung cancer(n=2), pulmonary A-V malformation(n=1), and unknown cases(n=2). 2) Overal immediate success rate was 94%(n=44), an6 recurrence rate was 40%(n=19). 3) The prognostic factors such as bilaterality, systemic-pulmonary artery shunt, multiple feeding arteries and degree of neovascularity were not statistically correlated with rebleeding tendency (p value>0.05). 4) At additional bronchial artery embolization, Revealed recannalization of previous embolized arteries were 14/18cases(78%) and the presence of new deeding arteries was 8/18cases(44%). 5) The complications(31cases, 66%) such as fever, chest pain, cough, voiding difficulty, paralytic ileus, motor and sensory change of lower extremity, atelectasis and splenic infarction were occured. Conclusion : Recannalization of previous embolized arteries is the major cause of recurrence after bronchial artery embolization. Despite high recurrence rate of hemoptysis, bronchial artery embolization for management of massive hemoptysis is a effective and saute procedure in immediate bleeding control.
On the purpose to improve the milling efficiency as well as head-rice percentage after milling, an experiment to improve the removal ability of immature kernels in the immature brown rice separator (IBRS) was performed focused on varietal characteristics. The removal ability of immature grains by the IBRS was absolutely depending on kernel thickness of brown rice. The kernel thickness of the tested rice varieties distributed from 1.79 mm in Nonganbyeo to 2.16 mm in Daeribbyeo 1. Although there were some variation among rice varieties, it was roughly suggested that the suitable sieve-slit widths for good separation of the immature kernels were 1.9 mm for the varieties thicker than 2.08 mm in thickness, 1.8 mm for the varieties with 2.00-2.08 mm thickness, 1.7 mm for the varieties with 1.90-2.00 mm thickness, and 1.60-1.65 mm for the varieties thinner than 1.7 mm. It was found out that the higher the proportions of immature kernels in brown rice, the more conspicuous the improvement of milling efficiency as well as head rice rates by their removals. With increasing the sieve slit-widths beyond an optimum range, the losses of mature grains increased sharply. For effective separation of immature kernels, it was suggested that the optimum sieve-slit width should be set up depending on both of the kernel thickness and the critical loss limit of mature kernel.
Constructed wetland system was studied to treat wastewater from industrial complex in rural area. Pilot plant at the Baeksuk Nongkong Danzi in Chunahn-City was used for field study. For the DO, the effluent concentration was higher than the influent concentration and it implies that natural reaeration supplies enough oxygen to the system. For the SS, the effluent concentration was consistently lower than the water quality standard even though the influent concentration varied significantly, which showed that SS was removed by the system effectively which is consist of soil and plants. For the BOD and COD, the average removal rate of them were 56% and 43%, respectively, therefore, the effluent concentration could not meet water quality standards when influent concentration was high. The removal rate of BOD and COD can be improved by supplemental treatment in addition to this system if necessary. For the T-N and T-P, the influent concentration of them were lower than the water quality standards than no further treatment was needed. Overall, the result showed that constructed wetland system is a feasible alternative for the treatment of wastewater from industrial complex in rural area. For actual application of this system, further study on design factors including loading rate, removal mechanism, and temperature effects is required to meet water quality standard consistently. Compared to existing systems, this system is quite competitive because it requires low capital cost, almost no energy and maintenance, and therefore, very cost effective.

The worldwide effects of COVID-19 have led to a surge in online shopping and contactless services. The consumption pattern has caused the issues such as the environmental pollution together with the increase of plastic waste. Reducing the reliance on the petroleum based plastic use for the package and replacing it with environmentally friendly material are the simple ways in order to solve those problems. Paper is an eco-friendly product with high recyclability as the food packaging materials but has still poor barrier properties. A barrier coating on surface of the paper can be achieved with the proper packaging materials featuring water, gas and grease barrier. Polyethylene (PE) or polypropylene (PP) coatings which are generally laminated or coated to paper are widely used in food packaging applications to protect products from moisture and provide water or grease resistance. However, recycling of packaging containing PE or PP matrix is limited and costly because those films are difficult to degrade in the environment. This study investigated the recyclability of modified acrylic emulsion coating papers compared to PE and PP polymer matrixes as well as their mechanical and gas barrier properties. The results showed that PE or modified acrylic emulsion coated papers had better mechanical properties compared to the uncoated paper as a control. PE or PP coating papers showed strong oil resistance property, achieving a kit rating of 12. Those papers also had a significantly higher percentage of screen reject during the recycling process than modified acrylic coated paper which had a screen rejection rate of 6.25%. In addition an uncoated paper had similar value of a screen rejection rate. It may suggest that modified acrylic emulsion coating paper can be more easily recycled than PE or PP coating papers. The overall results of the study found that modified acrylic emulsion coating paper would be a viable alternative to suggest a possible solution to an environmental problem as well as enhancing the weak mechanical and poor gas barrier properties of the paper against moisture.
This study was carried out to assess the roles of soil and vegetation on the nutrient cycling in paddy ecosystem where excessive amounts of animal wastes were flowed in due to the rain. Experimental sites included one abandoned and four cropping paddy fields which were moderately terraced under a small farm village raising 90 milk cows and 35 deer under open-air condition. The watershed covered 4 ha with every 50% of uptown and fodder crops. Concentrations of $NH_4-N$ and $P_2O_5$ in waste water flowed into the abandoned paddy field, enforced by the rain of $56.4mm\;day^{-1}$, were $8.3mg\;{\ell}^{-1}$ and $1.8mg\;{\ell}^{-1}$, respectively. Total mass of rainfall inflow to abandoned field during rice growing period (1 May to 30 Sept.) was $20,900Mg\;ha^{-1}$. Total amounts of $NH_4-N$ and $P_2O_5$ contained in that inflow were estimated as 173 kg and 38 kg, respectively. Concentrations in the outflow water through one abandoned and four rice paddy fields were reduced by 92% for $NH_4-N$ and 95% for $P_2O_5$, as compared to those in the inflows. The reserved portions of nutrients in the abandoned paddy field ecosystem, which were the summation of the uptake by weed and residues in soil, were 29% of the inflow amount for $NH_4-N$ and 30% for $P_2O_5$. These results demonstrated that soil and vegetation in paddy field ecosystem reduced the excessive nutrients from the animal waste inflow to the extents that might be suitable not only for the better growth of rice plant, located at the lower paddy fields, but also for preservation of the downstream from eutrophication.
