부인두강(Parapharyngeal space)는 비인두강과 구인두강에 인접하여 위로는 두개저, 아래로는 설골사이에 위치하는 해부학적 잠재공간으로 이곳에 발생하는 종양은 매우 드무나 다양한 병리조직학적 소견을 보이며 인접한 중요장기들로 인한 수술적 어려움으로 두경부 외과의사의 관심이 되어 왔다. 최근 CT나 MRI의 도입으로 이학적 검사가 어려웠던 이부위의 진단에 많은 도움을 얻게 되었고, 발달된 수술방법의 사용으로 크기가 매우 큰 종양도 절제가 가능하게 되었다. 저자들은 1990년 9월부터 1993년 8월까지 한림대학교 이비인후과학교실에서 술전 CT나 MRI를 시행한 후 조직검사로 확진된 부인두강 종물 22례의 후향적 임상분석을 시행하여 다음의 결과를 얻었다. 1) 양성종양이 11례(30.0%), 악성종양이 11례(50.0%)로 빈도에 차이는 없었다. 2) 종양의 조직학적 기원은 타액선종양이 10례(45.5%)로 가장 많았으며 신경종양이 4례(18.2%)였고 기타 종양이 8례(36.4%)로 다양하였다. 3) 방사선학적 진단방법으로는 18례(81.8%)에서 CT를 시행하였고 필요한 경우에 MRI를 5례(22.7%), 혈관조영술을 3례(13.6%)에서 시행하였다. 4) 치료방법으로는 악성종양은 수술 및 방사선치료 또는 항암약물요법을 병리조직검사 결과에 따라 단독 또는 병합하여 시행하였고, 양성종양의 경우에는 모두 수술을 시행하였는데 수술방법은 경이하선 접근법이 가장 많이 사용되었다. 5) 추적조사는 평균 20개월로 양성종양의 경우는 모두 종양의 재발이 없이 치료되었으나 악성종양 중 전이암 1례와 악성 임파종 1례가 사망하였다.
Epithelial-myoepithelial carcinoma is a rare neoplasm comprising approximately 1% of all salivary gland neoplasms. Histopathologically, the carcinomas are characterized by a dual cell population of an inner duct-forming epithelium and an outer myoepithelial cell. They are characterized by their variable clinical course and a lack of features that predict clinical outcome. The following report describes our experience with this type of tumor. We suggest that radiation therapy and long term follow-up will be necessary because this tumor have a high risk of locoregional aggressiveness.
Non-lymphoid mesenchymal neoplasms of salivary gland origin are rare, accounting for 1.4% of major salivary gland tumors. Of this group 30% were hemangiomas. Hemangiomas are the most common benign tumors in children, but are rare in old ages. 90% of hemangiomas arise in the first three decades of life. Hemangiomas are classified as capillary(including juvenile type), cavernous, or mixed in type. Salivary gland hemangioma is more common in the parotid(90%) and extremely rare in submandibular gland. We present two adult patients, 69 and 60 years of age, who were referred for palpable mass in the parotid area and submandibular area, respectively. After surgical removal of the lesion, histopathologic examination showed characteristic feature of cavernous hemangioma. The rarity of such a lesion in these locations in old ages have prompted this case report.

Salivary gland tumors comprise almost 5% of head and neck malignancies, and minor salivary gland tumor which account for 10-15% of all salivary gland neoplasm are infrequently malignant. The mucoepidermoid carcinoma (MEC) is second most common tumor in minor salivary gland. It usually presents as a painless, rubbery-hard or soft mass, which may be fixed or mobile into the underlying structure. The predilection sites of intraoral MEC are palate, cheek, mandible, lip, and tongue, etc. There are very few published reports of MEC occurred in retromolar trigone. Only one case has been reported so far. Recently, we experienced a-70-year old man with a mass in retromolar trigone, which was finally diagnosed as MEC. We report the unique case with literature review.
Objectives: The salivary gland tumor shows heterogeneity in histologic patterns and biological behavior. The aim of this study is to elucidate the relationships between apoptosis and expressions of apoptosis-related factors(bcl-2, bax, M30), p53 and MIB-1 in the salivary gland tumors. Methods: Immunohistochemical stains for apoptosis-related factors, p53 and MIB-1 and TUNEL study for apoptosis were performed in 46 cases of salivary gland tumors 02 benign and 34 malignant). Results: Twenty(43.5%) of 46 cases showed positive reaction for apoptosis, and the expression rates of bcl-2, bax, M30, p53 and MIB-1 were 85.3%, 68.8%, 65.9%, 39.1% and 26.1%, respectively. A significant difference between benign and malignant tumors was only noted in MIB-1 expression(p=0.0167). In malignant tumors, apoptosis showed no significant relationships to expressions of apoptosis-related factors. There were inverse relationships between p53 and bcl-2 expression(p=0.0375), and between M30 and MIB-1 expressions(p=0.0379). No significant differences of apoptosis, bcl-2, bax, M30, p53 and MIB-1 expression rates according to the tumor size, lymph node status, recurrence and survival were found. Conclusion: In the development of benign and malignant salivary gland tumors, apoptosis might be associated, however, apoptosis and expressions of apoptosis-related factors seemed to be not reliable prognostic factors in malignant salivary gland tumors.

Background and Objectives: Nodal metastasis is one of the prognostic factors in salivary gland cancer. The purpose of this study is to identify risk factors of nodal metastasis considered as a predictor of poor prognosis in patients with salivary gland cancer. Material and Methods: The authors retrospectively studied 82 patients with salivary gland cancer who underwent surgery from 1992 to 2002. We analyzed age, sex, tumor size, histologic type and 5-year survival rate to compare patients with and without nodal metastasis. Results: Mean age was 55.4 years, with a male-female ratio of 1 : 1.28. The overall 5-year survival rate was 85.5%. Among the 82 patients, 14 patients had nodal metastasis. There was no nodal metastasis in low grade malignancy. In patients without nodal metastasis, mean age was 52.5 years and the overall 5-year survival rate was 94.2%. In patients with nodal metastasis, mean age was 69.4 years and the overall 5-year survival rate was 42.9%. In patients with less advanced cancer (T1-T2 stage), the nodal metastasis was 7.5% and with advanced cancer (T3-T4 stage), 33.3%. Conclusion: Nodal metastasis significantly decreases survival in patients with salivary gland malignancy. High grade malignancy, large tumor size and old age are important risk factors of nodal metastasis. Nodal metastasis is more common in submandibular gland cancer compared with parotid gland cancer.
Background and Objective: Even though major salivary gland tumor is a rare disease, the diversity of histopathologic characteristics makes treatment decisions difficult. The aim of this study is to analysis the clinical experience of our major salivary gland tumor and to suggest a guideline of treatment. Materials and Method: Sixty-eight major salivary gland tumors and tumor-like glandular enlargements treated at Sanggye Paik Hospital during the past seven years between June, 1995 and January, 2002 are analyzed for histopathologic diagnosis, treatment modality, clinical manifestation, local control, and treatment-related morbidity, recurrence rate, retrospectively. Results: In twenty-eight patients the swellings were diagnosed as non-tumorous condition. The clinical features, diagnostic and surgical management of fourty salivary neoplasms involving the parotid and submandibular glands are correlated with their histological features. Conclusion: We have concluded that salivary gland neoplasms are needed multimodal treatment, because of their highly variable biologic behavior in each tumor type. Thirty-four cases were benign and six cases were malignant. Most of benign cases were pleomorphic adenoma and they showed wide age-distribution. In six malignant cases, there were acinic cell carcinoma, adenocarcinoma, carcinoma ex-plemorphic adenoma, myoepithelioma, and adenoid cystic carcinoma.

Objectives: Angiogenesis is the process of new blood vessel development from preexisting vessel. The growth of tumor is dependent on angiogenesis. This study was aimed to determine the role of the VEGF expression and MVD(microvessel density) in salivary gland tumors and to analyze the correlation between the expression of VEGF/MVD, and clinical manifestation. Material and Methods: We analyzed the expression of VEGF and average MVD and neo-MVD in the 57 cases of salivary gland tumors by immunohistochemistry. Results: VEGF expression was higher in malignant tumors than in benign tumors. Average and neo-MVD were not different in benign tumor and malignant tumor. There was a tendency of positive correlation between VEGF expression and MVD but it was not statistically significant. Stage, T stage and recurrance rate were not correlated with VEGF expression, average-MVD and neo-MVD. Expression of VEGF, average-MVD and neo-MVD were significantly increased in Warthin's tumor compared with pleomorphic adenoma. Conclusion: Overexpression of VEGF has an important role in malignant salivary gland tumors.

Extramedullary plasmacytoma is extremely rare, constitute 3 to 5 % of plasma cell malignancies and commonly occur in the upper aerodigestive tract. Several case studies of extramedullary plasmacytoma occurring in unusual location are reported; stomach, bladder, central nervous system, breast, thyroid, testis, salivary gland and skin. Here, we present a case of an extramedullary plasmacytoma of the right gluteus maximus muscle in a 49-year-old man.

Epithelial-myoepithelial carcinoma(EMC) of the salivary glands is a rare tumor first discribed in 1972. The EMC comprises approximately 1% of all salivary gland tumors. It is characterized by tubular and solid growth pattern with a dual cell population including an inner layer of epithelial cells, which is peripherally bounded by a layer of clear myoepithelial cells.
