A Method to Quantify Breast MRI for Predicting Tumor Invasion in Patients with Preoperative Biopsy- Proven Ductal Carcinoma in Situ (DCIS) (유방 자기공명영상법을 이용한 수술 전 관상피내암으로 진단된 환자의 침윤성 유방암을 예측하는 정량적 분석법)

  • Purpose : To determine the quantitative parameters of breast MRI that predict tumor invasion in biopsy-proven DCIS. Materials and Methods: From January 2009 to March 2010, 42 MRI examinations of 41 patients with biopsy-proven DCIS were included. The quantitative parameters, which include the initial percentage enhancement ($E_1$), peak percentage enhancement ($E_{peak}$), time to peak enhancement (TTP), signal enhancement ratio (SER), arterial enhancement fraction (AEF), apparent diffusion coefficient (ADC) value, long diameter and the volume of the lesion, were calculated as parameters that might predict invasion. Univariate and multivariate analyses were used to identify the parameters associated with invasion. Results: Out of 42 lesions, 23 lesions were confirmed to be invasive ductal carcinoma (IDC) and 19 lesions were confirmed to be pure DCIS. Tumor size (p = 0.003; $6.5{\pm}3.2$ cm vs. $3.6{\pm}2.6$ cm, respectively) and SER (p = 0.036; $1.1{\pm}0.3$ vs. $0.9{\pm}0.3$, respectively) showed statistically significant high in IDC. In contrast, E1, Epeak, TTP, ADC, AEF and volume of the lesion were not statistically significant. Tumor size and SER had statistically significant associations with invasion, with an odds ratio of 1.04 and 22.93, respectively. Conclusion: Of quantitative parameters analyzed, SER and the long diameter of the lesion could be specific parameter for predicting invasion in the biopsy-proven DCIS.

  • Amberlite XAD-7 and XAD-4 resins impregnated with DXHQ (5,7-dihalo-8-hydroxyquinoline) were prepared for the adsorption, separation and recovery of heavy metal ions from aqueous solutions. The characteristics of the impregnated resins, DXHQ (X : Cl, Br, I)-XAD were studied to find out the proper pairs of resin and DXHQ for the adsorption of metal ions. The increasing order of the impregnated amount of DXHQ onto XAD-7 resin was as follows: DCHQ < DBHQ < DIHQ. It was observed from the plot of log $K_d$ vs. pH that the optimum pH range for the adsorption of DIHQ onto XAD-4 resin was from 3.0 to 7.0. The stabilities of the DXHQ-XAD resins were investigated by measuring the amount of DXHQ remained on the XAD resin after shaking the DXHQ-XAD resins in various solutions of pH ranging from 2 to 12 and hydrochloric acid solutions. The impregnated resins were considerably stable in both acidic and neutral solutions. The amount of DIHQ leached from DIHQ-XAD-4 resin by eluting with various HCl solutions (1 ∼ 5M) was negligible, but in the case of XAD-7 resin it increases as the concentration of HCl solution increases. The optimum pH ranges, absorption mole ratio (M : DXHQ) and adsorption capacities (mmol metal per gram of resin) for the adsorption of metal ions onto the DXHQ-XAD resins were determined respectively. The stability of metal ion absorbed by the DXHQ-XAD resins was observed as the following order: M-DCHQ-XAD-7 < M-DBHQ-XAD-7 < M-DIHQ-XAD-7. The adsorbed metal ions were quantitatively recovered by eluting with HCl (0.5 ∼ 5M) and DXHQ-XAD resins could be reused over 5 times without re-impregnation of DXHQ.

Radiotherapy of Invasive Cervical Cancer Following Simple Hysterectomy (단순자궁적출술을 시행한 침윤성 자궁경부암에서의 방사선치료)

  • Purpose : Simple hysterectomy is not a standard surgery for invasive cervical carcinoma. This study was performed to investigate the efficacy of radiotherapy following simple hysterectomy in the invasive cervical cancer. Materials and Methods : Retrospective analysis was done for 19 patients with invasive cervical carcinoma who were treated with radiotherapy following simple hysterectomy from April 1989 to December 1993 The median a9e of patients was 47 years old. Patients were treated with external beam radiation therapy alone (17) or external beam radiation therapy plus intra vaginal ovoid irradiation (2). The median follow up period was 46 months. Results : The 4 year overall survival rate was $75\%$. The 4 rear local control rate was $89\%$. Two patients had recurred at the locoregional and distant sites, and another two patients at distant sites. Conclusion : We confirmed that radiotherapy is the adequate treatment modality for patients with invasive cervical cancer who underwent simple hysterectomy.

Radiation Therapy Results of Invasive Cervical Carcinoma Found After Inappropriate Hysterectomy (부적절한 수술루 침윤성 자궁경부암의 방사선치료 결과)

  • Purpose : Hysterectomy without lymph node dissection was considered an inadequate treatment method for invasive uterine cervix cancer. Usually the procedure was performed inadvertently on patients who were thought to have benign or premalignant conditions preoperatively. We analysed radiotherapy results of such patients to evaluate survival rates, failure patterns and prognostic factors according to various conditions. Materials and Methods : Sixty one patients undergoing hysterectomy in the presence of invasive cervical carcinoma were reviewed retrospectively. Preoperative diagnosis were carcinoma in situ (38 cases), severe dysplasia (2), myoma (6), uterine bleeding (4), uterine prolapse (2). and early invasive cervix cancer (10) (One patient had myoma and carcinoma in situ coincidently). Patients received postoperative megavoltage radiotherapy from August 1985 to December 1993, and minimum follow-up period was 24 months. Eight patients received ICR only, 6 patients ICR and external radiation, and 47 patients received external radiation therapy only. Results : Overall 5-year survival rate and relapse-free survival rate werer $83.8\%$, $86.9\%$ respectively. For patients with retrospective stage IA, IB, IIB (gross residual after surgery), and vaginal cuff recurrence were $90.9\%$, $88.8\%$, $38.4\%$, and $100\%$ respectively There were 8 cases of treatment failure, most of them (5/8) were in patients with gross residual disease, other patients were full thickness involvement of cervix wall (2/8) except one. Patients with early vaginal cuff recurrence and microinvasive cervical cancer (stage IA) had no treatment related failure Prognostic factors affecting survival by univariate analysis were status of residual disease, tumor histology and retrospective stage. Conclusion : Adjuvant radiotherapy appeared to be effective treatment method for patients with presumed stage IA, IB and early local recurrent disease after inadvertent hysterectomy Survivals for patients with gross disease remained after inappropriate hysterectomy was poor, So, early cancer detection and Proper management with precise pretreatment s1aging is necessary to avoid inadherent hysterectomy especially in the cases of gross residual disease.

Analysis of $^1H$ Magnetic Resonance Spectroscopy Pattern in Invasive Ductal Carcinoma of Breast (유방 침윤성 관상피암에서 수소핵 자기공명분광상의 특성 분석)

  • To evaluate the potential value of $^1H$ Magnetic resonance spectroscopy (MRS) for detecting and characterizing invasive ductal carcinoma of breast. We conducted $^1H$ Magnetic resonance spectroscopy (MRS), using a 3.0T MR scanner, on 40 patients who were histologically diagnosed to have invasive ductal carcinoma (IDC); tumor areas of the patients were designated as experimental samples, and non-tumor areas as control samples. The peak at 3.2 ppm is characteristically intense and observed in 34 cases of the total 40 invasive ductal carcinoma (sensitivity 86.2%; specificity 100%; positive predictive value 100%; negative predictive value 60%). In constrast peak at 1.3 ppm is characteristically intense and observed in normal breast (sensitivity 86.2%; specificity 100%; positive predictive value 100%; negative predictive value 60%). The study shows that $^1H$ MRS can effectively discriminate invasive ductal carcinoma from normal breast in most cases. It also demonstrates the feasibility of localized in vivo $^1H$ MRS technique as a new diagnostic modality in the detection of breast tumor.

Computer-Aided Diagnosis Parameters of Invasive Carcinoma of No Special Type on 3T MRI: Correlation with Pathologic Immunohistochemical Markers (3T 자기공명영상에서 비특이 침윤성 유방암의 컴퓨터보조진단 인자들과 병리적 면역조직화학 표지자들과의 상관성)

  • Purpose To investigate the correlation between computer-aided diagnosis (CAD) parameters in 3-tesla (T) MRI and pathologic immunohistochemical (IHC) markers in invasive carcinoma of no special type (NST). Materials and Methods A total of 94 female who were diagnosed with NST carcinoma and underwent 3T MRI using CAD, from January 2018 to April 2019, were included. The relationship between angiovolume, curve peak, and early and late profiles of dynamic enhancement from CAD with pathologic IHC markers and molecular subtypes were retrospectively investigated using Dwass, Steel, Critchlow-Fligner multiple comparison analysis, and univariate binary logistic regression analysis. Results In NST carcinoma, a higher angiovolume was observed in tumors of higher nuclear and histologic grades and in lymph node (LN) (+), estrogen receptor (ER) (-), progesterone receptor (PR) (-), human epidermal growth factor 2 (HER2) (+), and Ki-67 (+) tumors. A high rate of delayed washout and a low rate of delayed persistence were observed in Ki-67 (+) tumors. In the binary logistic regression analysis of NST carcinoma, a high angiovolume was significantly associated with a high nuclear and histologic grade, LN (+), ER (-), PR (-), HER2 (+) status, and non-luminal subtypes. A high rate of washout and a low rate of persistence were also significantly correlated with the Ki-67 (+) status. Conclusion Angiovolume and delayed washout/persistent rate from CAD parameters in contrast enhanced breast MRI correlated with predictive IHC markers. These results suggest that CAD parameters could be used as clinical prognostic, predictive factors.

Anti-invasive Activity of Human Breast Carcinoma Cells by Genistein through Modulation of Tight Junction Function (인체유방암세포의 tight junction 기능 조절을 통한 genistein의 암세포 침윤 억제 효과)

  • Tight junctions (TJs) that act as paracellular permeability barriers play an essential role in regulating the diffusion of fluid, electrolytes and macromolecules through the paracellular pathway. In this study, we investigated the correlation between the tightening of TJs, permeability and the invasive activity of genistein - a bioactive isoflavone of soybeans - in human breast carcinoma MCF-7 and MDA-MB-231 cells. The inhibitory effects of genistein on cell proliferation, motility and invasiveness were found to be associated with the increased tightness of the TJs, which was demonstrated by an increase in transepithelial electrical resistance and a decrease in paracellular permeability. Additionally, the immunoblotting results indicated that genistein repressed the levels of the proteins that comprise the major components of TJ, claudin-3 and claudin-4, which play a key role in the control and selectivity of paracellular transport. Furthermore, genistein decreased the metastasis-related gene expressions of insulin like growth factor-1 receptor and snail, while concurrently increasing that of thrombospondin-1 and E-cadherin. In addition, we demonstrated that claudins play an important role in the anti-motility and invasiveness of genistein using claudin-3 small interfering RNA. Taken together, our results indicate a possible role for genistein as an inhibitor of cancer cell invasion through the tightening of TJs, which may counteract the up-regulation of claudins. In addition, our results indicate that this may be beneficial for the inhibition of tumor metastasis.

RADIOGRAPHIC STUDY ON THE RESORPTION OF IODOFORM PASTE IN THE APICAL LESION (치근단병소에서의 요도포름호제(iodoform paste)의 흡수에 관한 X-선학적 고찰)

  • 저자는 18세부터 24세의 남녀 5명의 치근단병소가 있는 전치에 iodoform paste로 근관과잉충전을 한후 X-선상으로 그 흡수 현상을 관찰 하였던 바 다음과 같은 결론은 얻었다. 1) 비록 동종의 근관충전용 호제(paste)로 근관충전치료를 하였어도 각치아에서의 흡수율에는 큰 차이가 있었다. 2) 요도포름호제의 흡수율은 3,4주에서 보다 1,2주에서 더 빠르게 나타났다. 3) 치근관내에서의 흡수율은 상당히 느렸으며 2주후에 흡수가 나타난 예도 있었다. 4) 치료후 4주간에서는 확산성, 침윤성, 침윤성증상이 X-선상에 나타났으나 3개월후에는 뚜렷한 경계를 가진 병소부를 볼 수 있었으며, 치조골구조의 변화는 병소부의 주위에 신생골의 정밀한 증식으로 연속적인 백선이 형성된 것을 볼 수 있었다.
