Salivary Gland Carcinoma Mimicking Secretory Breast Cancer (분비성 유방암과 유사한 침샘 암종)

  • Mammary analogue secretory carcinoma (MASC) of the salivary gland is a newly classified pathologic entity since 2010. Prior to its recognition, MASC was diagnosed as low-grade cystadenocarcinoma, acinic cell carcinoma, and mucoepidermoid carcinoma. MASC shares common histological and genetic characteristics with secretory carcinoma of the breast and has a distinct feature of the ETV6-NTRK3 fusion gene. Treatment of MASC in salivary gland is mainly wide surgical resection of the tumor. Prognosis of MASC is similar to other low-grade salivary gland carcinomas. Herein, we report a case of MASC developed in a parotid gland with a review of the literature.

  • Pleomorphic adenoma is also called a mixed tumor and it most commonly occurs in the salivary gland. This neoplasm has a low grade malignant potential, but it may also show aggressive clinical behavior like recurrence or metastasis. We report here on a case of a tumor that was confirmed to be primary pulmonary carcinoma ex-pleomorphic adenoma by the pathologic examination after complete resection, and it had the characteristics of malignant neoplasms, such as multiple metastases.

  • Diffuse infiltrative lymphocytosis syndrome is an autoimmune syndrome that is characterized by the oligoclonal expansion of CD8+ T-lymphocytes in response to human immunodeficiency virus (HIV) antigens. The clinical manifestations include bilateral enlargement of the parotid glands, lymphocytic interstitial pneumonitis, lymphocytic hepatitis, neurological involvement and systemic lymphadenopathies. In addition to a positive HIV test, the diagnostic histopathological findings are CD8+ T-lymphocytic infiltrations in the lymphnodes, liver, lung, muscle and the salivary or lacrimal glands without granulomatous or neoplastic involvement. We report a case of pulmonary involvement of diffuse infiltrative lymphocytosis syndrome that was associated with a human immunodeficiency virus infection.

  • Tumors rarely arise in the sublingual salivary glands. They should be considered malignant until proved otherwise. Adenoid cystic carcinoma is most commonly encountered malignant tumor of the sublingual gland. We report a case of adenoid cystic carcinoma arising from the sublingual gland. Moreover, the sublingual gland had anatomic variation of main duct(Bartholin's duct) which is connected to Wharton's duct.

  • Since the majority of cases with unilateral peripheral facial palsy are idiopathic, radiological studies such as CT or MRI are not usually recommended for further evaluation. We report a patient with peripheral facial palsy caused by minor salivary gland tumor which was demonstrated by appropriate imaging study.

  • Although fine needle aspiration cytology (FNAC) has become one of the primary tools for diagnosing salivary gland lesions, some of these methods continue to confuse pathologists. The most common problems occur so-called basaloid neoplasms. Basal cell adenomas are frequently misdiagnosed as pleomorphic adenomas, and in worse cases, as adenoid cystic carcinomas. The cytologic diagnostic accuracy of basaloid neoplasms could be increased by a better understanding of the histology and the nature of the tumor cells. These are displayed well in aspiration smears. A consideration of differential points on the basis of the epithelial-stromal relationship is offered in this paper.

  • Fine needle aspiration cytology (FNAC) is a very useful tool in the preoperative diagnosis of lesions of the salivary gland. Surgical therapy of high-grade malignancies (salivary duct carcinoma, mucoepidermoid carcinoma, squamous cellcarcinoma, carcinoma ex pleomorphic adenoma, small cell carcinoma, and sebaceous carcinoma) is different from that of benign lesions or low-grade malignancies. Therefore, the recognition of high-grade malignancies is important in salivary gland FNAC. Although recognition of high-grade malignancies of the salivary gland by FNAC is not difficult, precise classification of these malignancies is often impossible. Additionally, because of its rarity, FNAC of spindle cells and mesenchymal lesions of the salivary glands is a tool that is not familiar to many cytopathologists. The characteristic cytomorphologic features of these lesions are reviewed here with a discussion of specific diagnostic problems.

  • Chronic sclerosing sialadenitis, also known as Kuttner tumor, is a benign chronic inflammatory lesion of the salivary gland. Here, we describe a case of chronic sclerosing sialadenitis with mucinous ductal metaplasia in a parotid gland, which was confused with low-grade mucoepidermoid carcinoma on aspiration cytology.

  • 주로 작은 침샘에서 발생하는 샘낭암종의 치료 방침은 수술과 수술 후 보조적 방사선치료가 주로 행해져 왔다. 그러나 설근부에 발생한 샘낭암종에 대해서는 수술적 치료가 가져오는 삶의 질의 저하가 크기 때문에 수술적 치료를 적용하기 쉽지 않다. 또한 샘낭암종의 치료에 있어서 항암제의 역할이 거의 없는 상황에서 방사선치료가 중요한 역할을 할 수 있겠다. 이에 본 저자들은 설근부에 발생한 샘낭암종 세 증례의 방사선치료 결과를 보고 하며 샘낭암종의 치료에 대해서 문헌고찰을 통해 논의하고자 한다.

Recent Trends in the Incidence of Salivary Gland Malignancies (침샘 악성종양의 발생에 대한 최신 경향)

  • Due to the low incidence and histologic diversity of salivary gland cancer, analyzing the incidence of salivary gland cancer is necessary to understand the macroscopic aspects. We intend to investigate the international trend of the reported incidence rate of salivary gland cancer. Using the Korea Central Cancer Registry data, the domestic change in the incidence rate was examined. As a result, a significant increasing trend was confirmed, consistent with the United States and Japan trends. The etiology of the change is unclear, and various factors that may influence the direction are reported. Additional research is needed to understand the pathophysiology of salivary gland cancer, and further efforts are required to understand salivary gland cancer.
