Trends of Dental Treatment under General Anesthesia and Patterns of Repeated General Anesthesia in Children and Adolescents with Severe Disabilities (소아청소년의 전신마취 하 치과 치료의 추세변화 및 치과 영역 중증 장애 유무에 따른 전신마취 재시행 양상에 대한 연구)

  • The aim of this retrospective study was to evaluate the demographic characteristics of pediatric dental patients who underwent dental treatment under general anesthesia (DTGA) at the Seoul National University Dental Hospital from January 2011 through December 2020 and compare the patterns of repeated DTGA between dental patients with severe disabilities (DSD) and non-DSD (healthy or medically compromised patients without DSD). There were 1,857 DTGAs among 1,719 patients (mean age = 5.1 years; males = 59.3%; ASA 2 or above = 52.9%; DSD = 26.8%). Overall, 6.6% of patients underwent repeated DTGA, and the rate of repeated DTGA over a 10-year period was 7.4%. ASA 2 or above (p < 0.0001) and DSD (p < 0.0001) were more likely to undergo repeated DRGA compared to ASA 1 and non-DSD. At both GA1 and GA2, DSD received significantly more restorative treatment on permanent teeth than non-DSD (p = 0.002, p < 0.0001, respectively). There has been an increasing demand for DTGA in pediatric dentistry over the last 10 years. Regular check-ups and preventive oral health care are necessary for pediatric dental patients with severe disabilities to reduce the possibility of repeated DTGA.

Long-term Retrospective Study on Cumulative Survival Rate of Implants with Guided Bone Regeneration (골유도재생술을 동반한 임플란트의 생존율에 대한 연구)

  • Purpose: The purpose of this study was to analyze the cumulative survival rate of dental implants installed with guided bone regeneration (GBR), and also elucidate the factors related with the survival of dental implants. Material and Methods: This retrospective study was conducted on 148 dental implants installed in 76 patients by one specialist (Y.K.) at the Department of Periodontology and Implant Center, Seoul National University Dental Hospital from 2001 to 2010. The cumulative survival rates were obtained by the Kaplan-Meier method. The correlations between various factors and dental implant survival were analyzed by using the log-rank test and Cox proportional hazards model. Results: Among 148 dental implants installed in 76 patients, 8 implants in 7 patients were lost and the cumulative survival rates up to 5-years and 10-years were 97% and 89%, respectively. Gender, smoking status and location of implant were significantly associated with the cumulative survival rate of implants (p < 0.05). Age, history of hypertension and diabetes were not significantly associated with the cumulative survival rate of implants (p > 0.05). Conclusion: The dental implants installed with guided bone regeneration is predictable technique according to the results of cumulative survival rate over 10 years.

Geometric Features Detection of 3D Teeth Models using Approximate Curvatures (근사 곡률을 이용한 3차원 치아 모델의 기하학적 특징 검출)

  • In the latest medical world, the attempt of reconstructing anatomical human body system using computer graphics technology awakes people's interests. Actually, this trial has been made in dentistry too. There are a lot of practicable technology fields using computer graphics in dentistry For example, 3D visualization and measurement of dental data, detection of implant location, surface reconstruction for restoring artificial teeth in prostheses and relocation of teeth in orthodontics can be applied. In this paper, we propose methods for definitely detecting the geometric features of teeth such as cusp, ridge, fissure and pit, which have been used as most important characteristics in dental applications. The proposed methods are based on the approximate curvatures that are measured on a 3D tooth model made by scanning an impression. We also give examples of the geometric features detected by using the proposed methods. Comparing to other traditional methods visually, the methods are very useful in detecting more accurate geometric features.

  • There are various methods for restoring the dentition of completely edentulous patients. Removable complete dentures have the advantage of being relatively economical, but they can be uncomfortable to wear. With the introduction of implant prosthodontics, various options such as implant-supported overdentures and hybrid prostheses have become available. If there is inadequate remaining ridge or limited financial resources, an overdenture supported by a few implants with additional attachments may be more suitable. In this case, due to severe peri-implantitis and other complications, the implants were removed. Subsequently, four implants (two on each side) were placed in the maxilla and a milled-bar with attachment was fabricated for each side of the maxilla.

  • Full mouth rehabilitation is re-organizing the occlusion of the remaining teeth and missing teeth considering the functions, esthetics, and neuromuscular harmony. With the loss of multiple teeth, the patient's occlusal plane gradually collapses and the vertical dimension can be reduced. Since reduced vertical dimension can be a potential etiology of the temporomandibular joint and masticatory muscles, prosthetic restoration with increased vertical dimension is required. This case report is about a 68 years old patient with vertical dimension loss due to worn dentition and multiple loss of teeth. In this case, the loss of vertical dimension is assessed carefully using the digital dentistry technology. Using CAD software in digital analysis step, the occlusal plane was established and evaluated using several criteria. Orienting the position of the bone and teeth using CBCT image, patient's condition was visualized in 3 dimension and treatment planning was possible virtually. The information that matches the patient's condylar position with the articulator, which is the virtual face bow, is reproduced on the actual articulator, and evaluated again. After the evaluation, provisional prosthesis was fabricated and it was confirmed that the patient adapts without any abnormality. This was implemented as a final prosthesis. As a result, the patient obtained satisfying results, utilizing the benefits of digital dentistry technology and traditional methods.

A Case of Developing a Competency model for the revision of the dentistry education curriculum on the competency-based (역량기반 치의학 교육과정 개편을 위한 역량모델 개발 사례)

  • The purpose of this study is to develop a competency model for the revision of the dentistry education curriculum based on the competency of W College of Dentistry. For this, this study made progress of the competency model development procedure in three stages. The preparation stages of the competency model was developed by performing an analysis about the awareness investigation of the members, and the development stages were progressed through the competency level and behavioral index, etc. The content validity was verified by an expert for the evaluation stage. As a result, content validity index was high with CVI of .91 and a competency model was developed with the 4 key competencies, which are the common competencies of the university linked with job competency, the major competency. This study holds significance by the fact that the competency model development case developed through this procedure provided basic data for research that requires the development of the competency model and the education curriculum revision based on the competency of W College of Dentistry is planned to also be applied.

  • 치의학 분야에서 디지탈 근적외선 체열측정 장치(digital infrared thermographic imaging; DITI)의 응용이 미미한 것은 과거 열측정장치의 기술력이 부족한 것이 주된 요인이었다. 그러나 최근들어 기술이 진보함에 따라 실시간 열영상을 재현할 수 있게 되었고 이는 치의학 및 의학계에 많은 관심을 일으키고 있다. 지금까지의 연구를 보면 두개하악장애시 구강영역의 온도변화의 임상적 효용 가능성을 제시하였으나 안면 및 두경부에 대한 실제 기기 및 측정방법의 신뢰도에 대한 연구는 이루어지지 못한 상태이다. 본 연구에서는 DITI에 대한 신뢰도를 조사하였다. DITI의 모니터 상에서 비활동성 발통점의 부위를 찾아내기가 쉽지 않다. 따라서 모니터상에서 안면발통점(joint, temporalis anterior, masseter anterior, masseter inferior)의 부위를 찾아 온도를 찾아내는 방법과 미리 발통점 피부상에 링모양의 마크를 부착하여 모니터상에서 쉽게 찾아내는 방법을 사용하여 신뢰도를 조사하여 비교한 결과 다음과 같다. 1. 조사한 발통점 중에서 temporalis anterior, joint, masseter anterior과 masseter inferior의 순서로 피부온도가 유의하게 온도가 높았으나(p<0.05) masseter 내에서는 유의한 차이를 보여주지 않았다. 2. 4개의 발통점 모두에서 높은 열적 대칭성을 보였으며, 좌우 온도차이는 $0.1^{\circ}C$미만이었다. 3. 다른 시기에 조사한 조사자내 신뢰도는 두 조사자 모두 높은 상관관계를 보여주지 않았으며 대조군과 마크를 사용한 군간에는 뚜렷한 차이를 볼 수 없었다. 4. 동일한 시기에 조사한 조사자간 신뢰도는 두 시기 모두 joint 부위의 일부를 제외하고 높은 상관관계를 보여주었으며, 마크를 사용한 경우 사용하지 않은 경우 보다 훨씬 신뢰도가 높은 상관관계를 나타내었다. 이상의 결과로 보아 DITI는 두개하악장애환자에 대한 두경부 발통점의 온도변화를 측정하는 데 있어 시간에 따른 신뢰도는 결여되었으나 동일 시기의 측정에는 매우 유익할 것으로 사료되며, 특히 발통점에 대한 표시를 사용하는 경우 매우 정확한 온도를 조사할 수 있을 것으로 판단된다.

The use of implant-assisted removable partial denture in the partially edentulous maxilla with a few unilateral remaining teeth and implant overdenture in the mandible: A case report (상악 편측 소수 잔존치에서 임플란트 융합 국소의치와 하악 임플란트 피개의치의 수복 증례)

  • Successful cases of the implant-assisted removable partial denture and implant overdentures are reported in which a few implants are additionally placed to secure the maintenance, support, and stability of the denture when there are a few residual teeth. When the lateral force applied to the tooth abutment and implant surveyed crown is minimized, the horizontal and rotational movement of the denture is significantly reduced which is an effective method that can improve the address in patients who complain of reduced retention and stability of their dentures. In this case, a small number of implants were placed to fabricate an implant-assisted removable partial denture with implant surveyed crown in the maxilla and implant overdenture with Locator® attachment in the mandible to improve the retention, stability, and support of the dentures. The patient was satisfied with both functional and aesthetic aspects after the final dentures were delivered.

Comparison of Microleakage and Compressive Strength of Different Base Materials (여러 치과 와동 기저재용 재료들의 미세누출 및 압축강도 비교)

  • This study compared the microleakages and compressive strengths of various base materials. To evaluate microleakages, 50 extracted permanent premolars were prepared. The teeth divided into 5 groups of 10 each according to the base materials. Cavities with a 5.0 mm width, 3.0 mm length, and 3.0 mm depth were formed on the buccal surfaces of the teeth. After filling the cavities with different base materials, a composite resin was used for final restoration. Each specimen was immersed in 2% methylene blue solution and then observed under a stereoscopic microscope (× 30). To evaluate the compressive strength, 5 cylindrical specimens were prepared for each base material. A universal testing machine was used to measure the compressive strength. The microleakage was highest in the Riva light cureTM group and lowest in the BiodentineTM and Well-RootTM PT groups. For the compressive strengths, in all groups, acceptable strength values for base materials were found. The highest compressive strength was observed in the Fuji II LCTM group and the lowest strength in the Well-RootTM PT group.
