A Study on Clinical Feasibility and Practical Strategies for Dental Hygiene Process (ADPIE) (치위생과정(ADPIE)의 임상적용 가능성과 실천방안 모색)

  • This study was aimed to analyze factors affecting intention on clinical application by task autonomy of dental hygienists, expected effect and obstructive factor on clinical application of assessment, dental hygiene diagnosis, planning, implementation, and evaluation (ADPIE). Meanwhile, it proved mediating effects of "attitude toward ADPIE" when it came to "intention on clinical application". The data was collected from 237 dental personnel in capital region from March 28, 2014 to May 2, 2014. To analyze the suitability of a theoretical model and hypothesis testing, SPSS 21.0 and AMOS 18.0 program were used. The theoretical model was accepted as it resulted in ${\chi}^2=421.67$ while showing goodness of fit index=0.858, comparative fit index=0.915, Tucker-Lewis index=0.896, root mean square residual=0.039, and root mean square error of approximation=0.099. The result showed "task autonomy of dental hygienists (${\beta}=0.398$, p<0.05)" and "expected effect on ADPIE (${\beta}=0.363$, p<0.01)" had a positive effect on "attitude toward ADPIE". Also, "attitude toward ADPIE" had a positive effect on "intention on clinical application". In addition, "task autonomy of dental hygienists" and "expected effect on ADPIE" had an indirect influence on "intention on clinical application" via such intermediary as "attitude toward ADPIE". According to the above results, the task autonomy of dental hygienists, expected effect on ADPIE, and attitude toward ADPIE were confirmed to be significant factors when it came to the intention on its clinical application. Therefore in order to settle ADPIE in the clinical practice, improvement of task autonomy for dental hygienists as well as their expectation and attitude on ADPIE must be promoted.

Dental Hygienist-Led Dental Hygiene Process of Care for Self-Support Program Participants in Gangneung (강릉시 자활근로사업 참여자 대상 치위생 과정 사례보고)

  • This study aimed to provide basic data for establishing the clinical basis for dental hygienist-led dental hygiene process of care by identifying multiple risk factors for self-support program participants in Gangneung city; we also compared oral health status and behavioral changes through customized oral health care. Four dental hygienists who were evaluated for degree of conformity provided dental hygiene process of care to eight self-support program participants who were selected as having an oral health risk among people in the self-support center. The clinical indicators measured during dental hygiene assessment and evaluation and behavioral changes due to dental hygiene intervention were compared and analyzed. With respect to clinical indicators, at the time of probe, the retention rate of patients with gingival bleeding decreased from 61.4% to 14.7% after intervention (p=0.004). Furthermore, the retention rate of patients with a periodontal pocket >4 mm decreased from 15.6% to 5.8% (p=0.001). The average modified O'Leary index of the patients improved from 23 to 40 (p=0.002). Previously, all eight subjects used the vertical or horizontal method of brushing; after dental hygiene care interventions regarding method and frequency of toothbrushing, use of oral care products, and individual interventions, they started using the rolling or Bass method of toothbrushing. Four of eight subjects reported using interdental toothbrushes after intervention. As a result of applying the change model to the transtheoretical behavior change of the subject, the result of strengthening the health behavior was confirmed. For promotion of oral health by the prevention-centered incremental oral health care system, dental hygienist-led dental hygiene management and maintenance is essential. It is thought that continuous research, such as for feasibility evaluation, cost benefit analysis, and preparation of legal systems, is needed to establish and activate dental hygiene management.

Analysis of the Fusion Differences in Dental Hygiene Process Charts Applying Dental Hygiene Process Between School and Dental Clinic (학교와 임상에서 치위생과정을 적용한 치위생관리기록부의 융합적 차이 분석)

  • This study is aimed to reduce the gap between hygienists' work in schools and dental clinics and to provide basic data for an oral health program based on the dental hygiene process. The denta hygiene process can spread widely in dental clinics by analyzing the dental hygiene process at each stage. The charts of a total of 199 people (100 people from the clinic and 99 people from the school) were finally analyzed. analysis of dental hygiene problems and dental hygiene plan in dental hygiene process. As a result, among the 17 factors related to dental hygiene problems, 6 factors were similar between the school group and the clinic group. There was a significant difference between groups in the remaining 11 factors. With regard to the 12 items of the hygiene management plan, there was a significant difference in 11 items. There are many differences between the dental hygiene process of the school and the clinic in terms of the diagnosis and planning of dental health. the routine dental health care of patients can be effectively carried out by dental hygienists. Therefore, the dental hygiene process should be adopted by the clinic.

Use of CDHC-applied educational curricula by dental hygiene departments in Korean universities (국내 대학 치위생(학)과의 포괄치위생관리(CDHC) 과정 적용 교육과정 현황)

  • Objectives: This study surveyed the application of Comprehensive Dental Hygiene Care (CDHC), based on non-surgical periodontal therapy within the educational curricula of Korean universities and examined whether they recognized the need for CDHC. Methods: This study analyzed data from professors of dental hygiene practicum related subjects in 75 Korean universities. The collected data were analyzed using a Chi-square test, a Mann-Whitney U-test, and a Kruskal Wallis test via SPSS (Ver. 21.0). Results: First, the application scope of CDHC was higher in four-year universities than three-year ones with the scope of several characteristics increasing along with enrollment quota. The application scope of dental hygiene plans and actions increased with the number of patient practices in terms of periodontal evaluations. Second, the needs of the dental hygiene courses were more than 4 points in all characteristics. Although four-year universities exhibited higher points than three-year ones, there was no significant difference found except other test. Finally, all universities based their dental hygiene courses on case histories, oral and maxillofacial information, hard tissue modules, periodontal issues, and other tests, with needs also being high. Meanwhile, the application scope of the dental hygiene course evaluations was lowest in all characteristics. Conclusions: There were no significant differences in dental hygiene curricula among the universities. However, highly qualified and standardized educational courses and lectures should be developed by the Korean evaluation center for dental hygiene curricula using various studies as the basis of theory and practical classes, the total number of terms, number of case studies used, number of students per class, ratio of students to professor, and so on.

Validity assessment for competency development in clinical dental hygiene education based on dental hygiene process of care (치위생과정에 기반한 임상치위생학 교육과정 역량 개발을 위한 타당도 평가)

  • Objectives: Before implementing a competency-based clinical dental hygiene curriculum, it is essential to establish competency development as a foundational educational objective. Therefore, this study aimed to develop the competency of clinical dental hygiene with secured validity using the Delphi survey method. Methods: Dental hygiene competencies were categorized within the dental hygiene process stages, and questions were formulated accordingly. A Delphi survey involving ten qualified experts was conducted to refine the final items based on their review opinions. Results: The expert Delphi survey confirmed that all items met stability criteria, with CVI values of 0.80 or higher, CVR values of 0.60 or higher, and a CV coefficient of variation of 0.5 or less. In total, 42 items were derived. Conclusions: Clinical dental hygiene can contribute to developing specialized dental hygienists if the competencies derived from this study are well applied to the standardized curriculum and operations.

Analysis of case reports based on dental hygiene process (치위생과정 기반의 임상치위생 증례보고서 분석)

  • Objectives : The purpose of this study was to analyse case reports performed through a dental hygiene process and provide basic data on clinical education of dental hygiene. Methods : 154 case reports which collected for six years were analysed. This study applied dental hygiene process model in dental hygiene diagnosis. Dental hygiene diagnosis was more cleared by dental a hygiene process model. Data analysis was performed by the Frequency statistics using SPSS 12.0 for Windows. Results : 1. The clients are mainly comprised 20's university student(91.9%). 2. In assessment phase, clients finished 100% test of subjective data. 3. When applied a dental hygiene process model in dental hygiene diagnosis, students have identified 23 type of dental hygiene problem and analysed dental hygiene problem frequently used as bleeding of gingiva, calculus and deposit of dental plaque. 4. In case of plan of dental hygiene intervention, Fluoride application showed the most high level(98.1%) in clinical intervention. 5. Results of intervention showed that performance rate(98.7%) of scaling is the most high level. Conclusions : Dental hygiene process model is more useful than other diagnostic models in clinical practice based on dental hygiene process.

Comparative evaluation of the effects of oil pulling on periodontal indices - Using generalized estimating equation model (오일 풀링 중재에 따른 치주질환 임상 지표 비교 -일반화추정 방정식 모형의 적용)

  • Objectives: This study was to analyz the effects of comparative evaluation of the effects of oil pulling on periodontal indices. Methods: A total subjects were 38; control group (19 subjects) and experimental group (19 subjects). In the experimental group, coconut oil was provided and rinsed the mouth for about 10 minutes once a day. We evaluated possession rate of pocket depth (≥4 mm), possession rate of bleeding on exploring and possession rate of gingival recession (≥1 mm). Results: The possession rate of pocket depth (≥4 mm), bleeding on exploring and gingival recession (≥1 mm) significant difference in interaction between the group and measurement time. Conclusions: The findings of this study indicate that dental hygiene process and oil pulling specialists in care of periodontal diseases were effective mediation.

An Analysis of Pre-dental Hygiene Curriculum of Dental Hygiene Programs in USA and in Canada (미국과 캐나다의 전공전 치위생 교육과정 분석)

  • The objective of this research was to analyze and evaluate pre-dental hygiene curriculum of dental hygiene education program in USA and Canada. Sixty one programs were searched through their website. The curriculum include content in the four areas: general education, biomedical science, dental science, dental hygiene science. Pre-dental hygiene curriculum include content in general education and biomedical science. General education content include oral and written communications, psychology, and sociology. Biomedical science content include anatomy, physiology, chemisrty, biochemistry,immunology,general pathology, nutrition, and pharmacology. Pre-dental hygiene curriculum content provide the foundational knowledge and skill such as competencies to communicate effectively, to participate community health program, and to make decisions regarding dental hygiene service.

Evaluation of the Specialized Elective Course for Dental Hygiene Student's Career to Pursue Provided by Yonsei University in Korea (치위생학과 학생의 사회 진출을 모색하기 위한 특성화 교육프로그램의 평가)

  • The purpose of this study is to evaluate the outcomes of the specialized elective course program to be designed to help dental hygiene student to pursue the career by providing the opportunities of attendance to the diverse social settings. Students participating the specialized elective course were required to attend one of the related dental or dental hygiene institutes of their own choice during 2 weeks. After that to evaluate satisfaction of the specialized elective course, 70 of 103 students who have graduated dental hygiene school in Yonsei University responded questionnaire that consist of satisfaction on the specialized elective course, influence on career, change of attitude for career etc. The most students satisfied the specialized elective course(very good 21.4%, good 47.1%, and neutral 24.3%) and satisfaction on this program was significant among related dental institutes. The majority of students(87.1%) responded the specialized elective course to have Influence on their career. Aspect to change of attitude for career, 63 percent of students started serious consideration making a decision on career. This findings indicate that the most students satisfy the specialized elective course, and this program providing the opportunity of attendance to the diverse social settings seems to help dental hygiene student to pursue the career.

Practice Status of Dental Hygiene Assessment in Dental Clinic (치과 임상에서의 치위생사정 수행 실태)

  • This study was conducted to investigate practice rate of dental hygiene assessment and to understand the performance ratio according to hospital type. A questionnaire survey was conducted on 195 dental hygienists, and the dental hygiene assessment items examined were 6 types of dental history, medical history, vital signs measurement, extraoral examination, intraoral soft tissue examination, andtissue examination. As a result of the study, the item with the highest percentage of performers was 92.8% of dental history, and the lowest item was extraoral examination 57.9%. And, dental clinics were higher than dental hospitals and university hospitals in all dental hygiene assessment categories, and in particular, performance rates of extraoral examination and intraoral soft tissue examination were higher in dental clinics. Considering the overall high rate of dental hygiene assessment through this study, it is necessary to discuss the legal work of dental hygienists and to reflect the insurance fee.
