CONSERVATIVE TREATMENT OF A UPPER CENTRAL INCISOR WITH POOR PROGNOSIS (예후가 불량한 상악 중절치의 유지)

  • The incisors function as instruments for biting and cutting food during mastication. They also support the lips and face and maintain vertical dimension. In addition, they contribute to overall normal arch appearance. They play important role during the articulation of speech and assist in guiding jaw closure. Extraction and space maintenance are the most common treatment for a tooth with poor prognosis. However, in the mixed dentition, extraction of the upper permanent incisors results in many complications, such as resorption of alveolar bone, poor esthetics, pronunciation, and mastication. Considering these various roles of incisors in oral cavity, approach for traumatized incisors, even the ones with poor prognosis, should be considered first prior to simple extraction. The dentist must take into account the age of the patient, growth potential, occlusion, oral hygiene status, economic status and motivation towards dental health in addition to patient compliance. In this case, although the prognosis was predicted to be unfavorable due to short root and mobility, we could save the central incisor using conservative treatment, reposition by orthodontic appliance instead of extraction.

Patient related and tooth related risk factor of tooth loss after periodontal surgical treatment - prospective study (치주수술 후 치아상실에 대한 환자관련, 치아관련 위험요소 - 후향적 연구)

  • Purpose: The purpose of this study is to analyze the prognostic factors related to tooth loss after 5 years of periodontal surgery in periodontal disease patients. Materials and Methods: From January to December 2017, 22 patients and 124 teeth who underwent periodontal treatment through periodontal surgery were targeted. At the time of treatment, the measured values were evaluated after recording the maximum probing depth, average periodontal probing depth, number of root, furcation involvement, pulp vitality, and prosthesis state on the day of periodontal surgery. Based on the initial records at the time of visit, patient-related factors were gender, age, smoking, tooth loss due to periodontal disease at the time of first visit, diabetes, and maintenance period. The influence of each factor on tooth loss was evaluated. Results: As a result of examining the influence of tooth-related factors on tooth loss, the maximum probing depth depth (P: 0.000), bone loss (P: 0.021) was found to have a significant effect on tooth loss. Conclusion: As a result of examining the influence of patient-related factors on tooth loss, any variables had no significant effect. Bone loss, maximum probing depth acted as statistically significant prognostic factors for tooth loss in patients who underwent periodontal surgery.

Prognosis of Replanted Permanent Incisors after Avulsion Injury: A Retrospective Study (완전탈구 후 재식한 영구 절치의 예후에 대한 후향적 연구)

  • Many studies on the prognosis of avulsed teeth were published but follow-up studies on Korean children and adolescents are insufficient. The aim of this study was to identify clinical factors determining the healing and prognosis of permanent incisors that were replanted after avulsion injury. This study included 184 permanent teeth in 142 patients aged 6-14 years. The clinical and radiographic data of the patients were analyzed from electronic medical records. Immature teeth had a higher probability of pulp revascularization than mature teeth. When extra-alveolar time was longer than 60 minutes, the incidence of inflammatory root resorption (IRR) was higher in immature teeth than mature teeth. However, the incidence of replacement root resorption (RR) was higher in mature teeth than immature teeth under the same condition. The incidence of IRR was increased when pulp was extirpated more than 20 days after replantation. IRR had a more significant influence on the decrease of survival expectation than RR. There was no significant correlation between the root development stage and survival expectation. However, when RR occurred, immature teeth survived shorter than mature teeth. Long term follow-up results from this study are expected to be used as fundamental data for the treatment guidelines and evaluation of the prognosis of replanted teeth.

  • Dentinogenesis imperfecta (DI) is a hereditary disorder of dentinal defect. It is generally inherited as a single autosomal dominant trait. DI usually affects both the primary and permanent dentition. Affected teeth have various types of discolorations, rapid destruction of the dentin, and severe attrition. In radiologic view, the affected teeth have bulbous crowns, short roots and narrow or closed pulp chambers. The treatment objective is to prevent additional attrition and recover the vertical dimension of occlusion. The aim of this report was to present the long-term prognosis in 15 years in a pair of siblings. Both the patients had DI with tooth attrition and discoloration. Different treatment procedures were used, depending on the difference in the timing of intervention. The first patient saved most of his teeth. The second patient had all of her teeth extracted. This report could be helpful for early diagnosis and overall treatment of DI.

A SURVEY ON THE AWARENESS OF ELEMENTARY SCHOOL TEACHERS ABOUT THE FIRST AID TREATMENT OF TRAUMATIZED TEETH (어린이의 치아손상 대처방법에 대한 초등학교 교사들의 인식도 조사)

  • For the tooth traumas of children, instant and appropriate treatment is one of the important factors to determine prognosis of traumatized teeth. Especially in case of avulsed tooth, prognosis is greatly affected by instant and appropriate emergency treatment. The objective of this survey was to investigate elementary school teachers? knowledge of first aid treatment for tooth traumas of children. We asked for 520 elementary school teachers in Busan to answer questionnaires and 455(87.5%) were returned. The result of the survey supports that knowledge of tooth is little understood except general information about the treatment of tooth traumas. And it also shows that practical treatment is poor and instruction of medical treatment is not generally known Furthermore most teachers agreed that expert education for prevention and cure of tooth traumas is necessary (81.8%) and mouth protector is also necessary (71.9%) for the prevention. The result leads to the conclusion that educations on the prevention and emergency treatment of tooth traumas of elementary children who are physically active are needed.
