  • The distance between the natural teeth and the implants is an important factor in preserving the periodontal tissues and esthetics. And abnormal positional displacement and tilting of the teeth during restorative procedure may require intentional root canal treatment and may affect masticatory function. This report is to present a successful full mouth rehabilitation of a patient with uneven dentition and collapsed occlusion using orthodontic and implant treatment. The patient had no symptoms or discomfort of temporomandibular joint disorder such as pain or sound. The orthodontic treatment was continued until implant provisional prosthesis delivery. And the vertical height of occlusion was elevated 2mm on anterior basis for anterior teeth protection and esthetics. After the orthodontic treatment, the implant abutments and natural teeth were finally restored with porcelain-fused-to-metal crowns and bridges. Satisfactory function and esthetic outcomes are observed after 6months of follow up.

  • Purpose: The aim of this study is to investigate the characteristics of patients and teeth referred to a university dental hospital for endodontic problem. Materials and Methods: From January, 2017 to December, 2018, patients who were referred to a university dental hospital for endodontic problem were collected from clinical records. A total of 1171 patient records were analyzed. The status of the referred teeth was divided into three groups according to whether they were treated endodontically based on radiographs and clinical records at the time of referred visit. Results: 69.9% of the referred teeth were maxillary and mandibular first and second molars. The average time from referral to actual visit is 9.03 days and 65.6% of the case referred with referring letter. The most primary reasons of referral were persistent clinical symptom (pain, swelling, and sinus tract) (37.9%), diagnosis difficulty (16.7%), blockage of canal space (13.8%) and difficult tooth anatomy (11.4%). In the case of referral before endodontic treatment, the most primary reason of referral was failure to make a proper diagnosis. If the teeth were referred in the middle of endodontic treatment, the most primary reason of referral was persistent clinical symptom and blockage of canal space. In the case of referral after root canal filling, the most primary reason of referral was persistent clinical symptom. Conclusion: In the case of molars, the rate of persistent clinical symptom and blockage of canal space were the most primary reason of referral, and the rate of apical surgery and management of trauma was high in the case of anterior teeth.

  • The objectives of this study was to investigate the amount of tooth ablation and the change of intrapulpal temperature by Er:YAG laser as it relates to pulse energy and pulse repetition rate at the identical power and, thereby, to reveal which of the two parameters strongly relates with ablation efficiency and intrapulpal temperature. Extracted healthy human molar teeth were sectioned into two pieces and each specimen was irradiated within the combination of pulse energy and pulse repetition time at the same power of 3W; $300mJy{\times}10Hz$ group, $200mJy{\times}15Hz$ group, and $150mJy{\times}20Hz$ group. Each specimen comprised ten tooth specimens. A laser beam with conjunction of a water flow rate of 1.6 ml/min was applied over enamel surfaces of the specimens during 3 seconds and the ablation amount was determined by difference in weight before and after irradiation. To investigate the temperature change in the pulp according to the above groups, another five extracted healthy human molar teeth were prepared. Each tooth was embedded into resin block and the temperature-measuring probes were kept on the irradiated and the opposite walls in the dental pulp during lasing. When the power was kept constant at 3W, ablation amount increased with pulse energy rather than pulse repetition rate (p=0.000). Although intrapulpal temperature increased with pulse repetition rate, there were no significant differences among the groups and between the irradiated and the opposite pulpal walls, except at a condition of $150y{\times}20Hz$ (p=0.033). Conclusively, it is suggested that ablation efficacy is influenced by pulse energy rather than pulse repetition rate.

  • The purpose of this study was to examine the influence of type of malocclusion and orthodontic treatment awareness on quality of life among orthodontic patients in the region of Busan as well as to develop an educational program tailored to the type of malocclusion as a way to improve quality of life. A survey was conducted for approximately 6 months from December, 2015, and the answer sheets from 472 respondents were analyzed. The most common painful area was the teeth, and this case was most predominant in the respondents with level 2 malocclusion, who differed from others in that regard (p<0.001). Regarding the relationship between satisfaction with orthodontic treatment and quality of life, respondents who were more satisfied currently and who were neither quite confident nor quite unconfident were ahead of their counterparts in quality of life. Concerning the reason for receiving orthodontic treatment, quality of life was lower among patients who started to receive treatment due to pronunciation problems (p=0.013), chewing difficulty (p<0.001), and temporomandibular joint click sound (p<0.001). With regard to influential factors on oral health-related quality of life, time for starting to receive orthodontic treatment was most influential (p<0.001), followed by current satisfaction (p<0.001), changes in confidence (p=0.003), self-rated teeth status (p=0.008), and type of occlusion (p=0.019). Therefore, accurate analysis of the oral health status of orthodontic patients and customized oral health education are required to improve quality of life even during the period of orthodontic treatment.
