DILACERATION CAUSED BY PERIAPICAL LESIONS OF THE DECIDUOUS INCISOR : CASE REPORT (상악 유전치의 치근단 병소로 인한 만곡 영구전치)

  • Dilaceration is a tooth abnormaly occuring in the histo-morphodifferention stage resulting from disturbance between the uncalcified and already calcified portion that affects both the crown and root. The involved tooth is usually the maxillary central incisor and it also shows high prevalence of impaction. The cause of dilaceration can be either from the trauma of the primary tooth, ectopic development of the tooth germ or from cysts. And it is also found in some cases of Otodental syndrome, Hurler syndrome, Cleidocranial dysostosis. The purpose of this study is to show that periapical lesions caused by dental caries can be another factor in causing dilaceration.

Spontaneous Eruption of a Dilacerated Mandibular Central Incisor after Trauma of a Primary Tooth : Two Case Reports (선생 유치의 외상 후 발생한 만곡된 하악 영구 중절치의 자발적 맹출 : 증례보고)

  • Dilacerations generally involve central incisors; most often maxillary incisors rather than their mandibular counterparts. The clinical features of dilaceration include non-eruption of the responsible tooth or prolonged retention of the deciduous predecessor tooth. In Case 1, the tooth showed a dilaceration at the boundary between the crown and the root, more laterally rather than labiolingually. In Case 2, the dilacerated tooth showed a crown dilaceration with a relatively normal orientation of the dental root. In both cases, no significant space losses for eruption were observed. Moreover, it seems that unlike the maxilla with the palate, the mandibular anterior teeth are limited to show severe displacement. From these cases, it is suggested that if a mandibular permanent incisor shows a crown dilaceration or lateral dilaceration at the boundary between the crown and the root, there is a relatively high probability of spontaneous eruption of the dilacerated tooth.

  • Dilaceration is most common in maxillary central incisor. Impaction by dilacerated tooth cause orthodontic problem such as tilting of adjacent tooth, midline deviation. Dilaceration is caused by trauma, cyst, other origin and tooth shape, degree of root curvature, location in the alveolar bone, ability of spontaneous eruption should be considered whether treat or not. Labially and lingually dilacerated tooth is difficult to spontaneous eruption. Thus, Prosthetic treatment alternative with extraction, auto transplantation, orthodontic traction with surgical exposure is included within the treatment alternative. These cases are about impacted central incisor with dilacerated root. We use closed eruption technique and guide impacted tooth into normal position by orthodontic traction.

  • In the case of the impacted teeth, the clinician has to consider development of tooth, site of impaction, eruption path, and cooperation of patient. If there are genetic or general factors to effect the eruption of tooth, the clinician treats these first and then takes the early treatment for eruption guidance. If there are physical factors to intercept eruption, the clinician put them off first. However, if there are no factors to effect eruption of tooth and enough space for eruption, the clinician can consider extraction of deciduous teeth, forced eruption and surgical reposition. In case of surgical repositioning, proper time for root development, proper socket formation, and minimal trauma are important for success. This case presents displaced impacted maxillary central incisor with dilacerated root. The development of root is Nolla's stage 7, and the tooth was treated by surgical repositioning. We can observe no root resorption and good healing pattern.

  • Dilaceration is a developmental disturbance in shape of teeth. It refers to an angulation, or a sharp bend or curve, in the root or crown of a formed tooth. Dilacerations are estimated to occur in 3% of all permanent dentitions. However, the etiology of dilaceration is not yet fully understood. Traumatic injury to the deciduous predecessors and ectopic development of the tooth germ are the two commonly cited causes of this anomaly. The involved tooth is usually the maxillary central incisor and it also shows high prevalence of impaction. At first, We tried to do surgical exposure with orthodontic traction but to no avail. After then surgical repositioning of the dilacerated tooth was performed. Finally, Satisfactory results were obtained at the end of treatment, with improvement in dental esthetics and functional aspects.

  • 통상적인 근관형성 과정에서 근관형태의 직선화 경향을 발견할 수 있으며, 그 결과 만곡이 심한 근관에서 이상적인 근관형태를 얻기가 어렵다. 이를 극복하기 위한 많은 기구들과 근관 성형법들이 개선되고 소개되었다. 본 연구에서는 최근에 개발된 직사각형의 단면을 가지는 stainless steel file인 RT file과 Flex-R file, K-file을 이용하여 만곡근관 형성 후 최종 근관형태를 비교하여 각 file을 평가하고자 한다. 본 실험은 Bramante등의 방법을 변형하여 술 전의 근관형태와 술 후의 근관형태를 비교하였다. Schneider의 방법을 변형하여 3차원적 만곡을 계산하여 12도에서 36도 이내에 만곡도를 가진 근관을 선택하였다. 49개의 발거된 상악대구치 협측근관이나 하악대구치 근심근관을 3개의 군으로 나누고 레진과 플라스틱으로 제작된 mold에 투명한 교정용 레진으로 매몰하였다. 근첨에서 2.5, 5, 8mm지점에서 절단하여 각 mold에서 재조립한 후 다음과 같이 근관형성을 시행하였다. 제 1군은 SS K-file을 이용하여 step-back 방법 ; 제 2군은 Flex-R file을 이용하여 balanced force 방법 ; 제 3군은 RT file을 이용하여 step-back 방법으로 근관 형성하였다. 술 전과 술 후에 각 시편들을 체현미경으로 사진촬영하여 근관 중심 위치 이동률, 근관형성으로 삭제된 상아질양, 근관형성 후 모양을 Sigma scan/ image software program으로 계산하고 One way ANOVA로 통계적 유의성을 검증하여 다음과 같은 결론을 얻었다. 1. RT file이 치근단에서 K-file보다 유의성 있게 근관의 중심을 유지하는 것으로 보였다. 치아 중앙부에서는 RT file과 Flex-R file이 K-file에 대해 유의성 있게 우수한 것으로 나타났다. 2. 근관형성 후 삭제된 상아질양에서는 치근단에서 RT file이 적게 나왔으나 유의성은 없었다. 3. 근관형성 후 절단면 모양은 원형, 타원형, 불규칙한 형태들이 다양하게 나타났고 각 군간에 유의성 있는 차이는 없었다.

  • The pulp infection of primary tooth is often caused by dental caries or trauma. But, if it is not managed properly, it can be produce the periapical lesion. The periapical lesion can cause some complications such as enamel hypoplasia, displacement, root dilaceration and impaction. Treatment options of displaced permanent successor are clinical and radiological follow-up after extraction of primary teeth, surgical opening, orthodontic traction, transplantation and extraction. In these cases, the premolars with abnormal eruption path caused by periapical lesion of the primary tooth have shown successful spontaneous eruption just as a result of extraction of infected primary tooth and space maintenance.

Crown angulations of posterior teeth of normal occlusion measured from marginal ridge plane (변연융선평면을 계측기준으로 한 정상교합자의 구치부 치관경사도에 관한 연구)

  • In the previous studies about prescription of preadjusted appliance, occlusal plane was used as a reference plane for crwon angulation (tip) measurement. But this reference plane is not parallel to the line connecting the facial axis points at which the centers of brackets are positioned (Andrews' plane), due to the curve of Spee. Therefore, we developed a new reference plane unaffected by the curve of Sun and more parallel to the Andrews' plane. It is an imaginary line connecting mesial and distal marginal ridges of each posterior tooth, and we named it 'marginal ridge plane'. In this study, crown angulations of posterior teeth of 29 normal occlusion samples were measured and measurements from both reference planes were compared. Crown angulation measurements measured from occlusal plane were different from crown angulation measurements from marginal ridge plane in the upper and lower 2nd molars (p<0.01), md 1st premolars (p<0.05). These results were analyzed as the crown angulation measurements from occlusal plane were affected by the curve of Spee. Crown angulations should be varied according to the amount of curve of Spee to maintain the continuity of marginal ridges. To solve this problem, determining bracket angulation as the bracket slot is parallel to the marginal ridge plane of each posterior teeth is recommended.

  • Odontoma is defined as a benign odontogenic tumor containing enmel, dentin as well as cementum. It has come to mean a growth in which both the epithelial and the mesenchymal cells exhibit complete differentiation. Most authorities accept the view today that the odontoma represents a hamartomatous malformation rather than a true neoplasm. The etiology of odontomas is uncertain but hypothesized to involve local trauma, infection, inheritance or mutant gene. The odontomas often cause various disturbances in the eruption and position of the teeth. The steps in removal of an odontoma in close relation to an adjacent impacted normal tooth should comprise 1) removal of odontoma and 2) exposure of the impacted tooth. Orthodontic therapy may be applied. Before treatment, the necessary space for the impacted tooth should be evaluated. If there is lack of space in the dental arch, orthodontic treatment should be carried out before operation.

  • Maxillary central incisor impactions occur infrequently. Their origins include various local causes, such as odontoma, supernumerary teeth, space loss, and disturbances in the eruption path, also trauma and apical follicular cysts. Impacted teeth can cause serious dental and aesthetic difficulties as well as psychological problems especially in anterior regions. Although the impaction of maxillary incisor occurs less frequently than that of the maxillary canine, it is of concern to parents during the early mixed dentition stage because of the uneruption of the tooth. Forced eruption of impacted teeth should be considered in young patients because this technique can lead to suitable results from a periodontal, occlusal, and esthetic perspective at an earlier stage better than with other treatment options. This report presents the surgical and orthodontic treatment of cases with horizontally impacted and dilacerated maxillary central incisors. For each patient, we used the closed eruption method, placed an attachment on the impacted tooth on surgery, and fully closed the flap. Traction was applied immediately. The impacted tooth erupts through the healed tissue in a manner resembling normal eruption.
