  • 낭은 상피세포로 싸여있는 병적인 공간으로 내부가 액체나 반액체로 채워져 있다. 악골에서 낭은 다른 골 부위에서보다 더 흔하게 발생하는데 이는 대부분의 낭이 치아 형성 후에 악골에 남아있는 치성잔류물로부터 발생하는 경우가 많기 때문이다. 구강악안면부에서 발생하는 낭은 연조직에서 발생할 수도 있지만 대부분 골내에서 발생하고, 법랑질 상피나 치배와 관련해 악골내에 발생하는 치낭성이다. 치낭성은 WHO분류에서 발생근원에 따라 크게 염증성과 발육성 기원의 두 가지 군으로 분류되고, 발육성 낭은 치성과 비치성으로 세분할 수 있다. 악골에 발생하는 낭은 대부분이 치낭성이며 가장 흔한 악골 낭종은 치근단낭으로 염증성이고, 발육성인 함치성낭과 치성각화낭도 다음으로 발생률이 높은 편이다. 소아환자에서 함치성낭과 치성각화낭의 발병률이 성인보다 더 높았다. 비치성낭 중에서 비구개관낭이 가장 많이 발생하며, 연조직에서 발생하는 낭과 상피세포 경계가 없는 낭성 병소인 가성낭(pseudocyst)은 드물게 관찰된다. WHO에서 치성각화낭과 서회화치성낭은 파괴적 성장을 하고 재발 경향이 높으므로 양성 낭성 종양으로 재분류하고 있다.

RADIOLOGIC MANIFESTATIONS OF ODONTOGENIC TUMORS OF MESENCHYMAL ORIGINS (중배엽성 치성종앙에 대한 방사선학적 연구)

  • 치성종양은 악구강계 영역의 질환 중 중요한 관심거리가 되어왔으며, 이중 상피성 치성종양에 관한 보고는 많으나, 중배엽성 치성종양에 관한 보고는 많이 이루어지지 않았다. 저자는 1975년부터 1985년까지 서울대학교병원 치과진료부에서 방사선학적, 병리학적으로 중배엽성 치성종양인 치성섬유종, 치성점액종, 백악질형성섬유종으로 진단된 증례들을 토대로 하여 연구, 분석한 결과 다음과 같은 결과를 얻었다. 1. 치성섬유종의 연령분포는 32세에서 72세로 광범위 하였으며, 발생위치는 모두 상악구치부이었고, 단방성이 2증례, 다방성이 1증례이었다. 2. 치성점액종의 연령분포는 40세에서 55세로 비교적 중년층에서 호발했고, 4증례가 상악에, 2증례가 하악에 발생하였다. 6증례 모두 방사선학적으로 다방성 방사선투과성을 보였으며, 경계는 비교적 불분명하였다. 3. 백악질형성섬유종은 주로 ?년에서 중년 성인 사이에 많이 발행하였고 40대에 가장 많은 출현율을 보였다. 남녀비는 약 1:2로 여자에게서 빈발하였다. 19증례중 하악에 15증례, 상악에 4증례가 발생하여, 하악이 호발부위였으며, 상하악 소구치, 대구치 부위에 발생하였다. 방사선학적으로 방사선투과성이 2증례, 혼합성이 12증례, 방사선불투과성이 5증례이었다.

DENTIGEROUS CYST OF A MAXILLARY MESIODENS: CASE REPORTS (상악 정중 과잉치에서 발생한 함치성 낭종을 가진 환아의 치험례)

  • Dentigerous cyst is the most common type of odontogenic cyst followed by radicular cyst. Most of dentigerous cysts originate from the permanent dentition and a few of them are associated with mesiodens. The complications of a dentigerous cyst associated with mesiodens are eruption abnomalies of adjacent teeth, root resorption, displacement, rotation, diastema and the secondary infection of the cyst due to adjacent teeth. The treatment of a dentigerous cyst of a maxillary mesiodens is to enucleate a cyst with a mesiodens. Otherwise, when it is close to adjacent teeth, a cyst is to reduce the through marsupialization after removing a mesiodens. There are numerous studies on dentigerous cysts and mesiodentes alone; however, the studies on the association of the two are rare. These are case reports about patients with mixed dentition whose dentigerous cyst of a maxillary mesiodens had been removed. After removal, the patients have been regularly checked up and shown satisfactory progress.

  • Most dentigerous cysts are solitary. Bilateral and multiple dentigerous cysts are rare and occur typically in association with a number of syndromes such as Maroteaux-Lamy syndrome, Hunter's syndrome, Basal cell nevus syndrome, Marfan syndrome, cleidocranial dysplasia. The presented case is of bilateral nonsyndromic, dentigerous cysts associated with mandibular right and left first premolars. A marsupialization procedure may be a choice of treatment for a large sized dentigerous cyst rather than an enucleation. The marsupialization procedure is recommended during the age when the erupting force of the teeth is still strong. We can expect the unerupted tooth to erupt normally. Although most of bilateral or multiple dentigerous cysts which are not associated with syndromes are rare, a bilateral dentigerous cyst without syndrome is seen. Therefore, it is wise to explain a possibility of development of new one to patient / parents in advance.

  • Odontogenic keratocyst is classified as a developmental odontogenic cyst and is believed to arise from cell rests of the dental lamina. It accounts for 3% to 11% of all jaw cysts and they occur twice as often in the mandible as in the maxilla. Histologically, the cysts are lined by stratified, keratinizing, squamous epithelium. Daugther cysts or microcysts are often observed microscopically. The recurrence rate has been reported variously, but is known by its high recurrence rate. These lesions are more common in males than in females, occur over a wide age range and are typically diagnosed during the 2nd and 3rd decade. The diagnosis depends on the cyst’s microscopic features and is independent of its location and radiographic appearances. This cyst is a radiolucent lesion that is often multiloculated, has a smooth or scalloped border. The cyst is characteristically located in the body and ramus of the mandible, and often occurs in conjunction with an impacted tooth. This case report describes an odontogenic keratocyst on the lower right molar area of an 8-year-old girl. The cyst was removed under the general anaesthesia, and is being checked regularly for any recurrences.

A Re-examination of the Significance of Devotional Offerings at Daesoon Jinrihoe Temple Complexes (대순진리회 도장 치성의 의미 재검토)

  • This study aims to determine the significance of the devotional offerings held in Daesoon Jinrihoe Temple Complexes (道場 dojang) and to interpret their identity. Firstly, the existing research insists that the significance of these rituals implies that offerings held by Daesoon Jinrihoe embody the order's religious purpose. However, this paper argues that the purpose of devotional offerings is not limited thereto. The reason is that the primary sources of Daesoon Jinrihoe defines the devotional offerings as practiced for purposes of worship, celebration, commemoration, the expression of gratitude, the dissemination of information, and to humbly beg pardon from divine beings. Additionally, the offerings are meant to express the solid faith of devotees. Considering the various purposes of practicing devotional offerings, rather than mostly being about prayer to divine beings, the meaning of the offerings could be better understood as forms of communication and exchange of unique ideas and intentions. Secondly, in the light of Korea's history of rituals, the devotional offerings of Daesoon Jinrihoe obviously differ from the rituals of Confucianism or the ritual for Heaven and Earth (圜丘祭 hwanguje). Indeed, the rituals of Daesoon Jinrihoe are rather alien to both of the aforementioned rituals. Accordingly, the identity of the devotional offerings in Daesoon Jinrihoe do neither succeed to the religious and cultural tradition of Korea nor transform it. It is rather the case that Daesoon Jinrihoe's devotional offerings should be identified as having been newly invented. In this regard, the devotional offerings of Daesoon Jinrihoe should be understood as an invented tradition that began in modern times. In other words, they are 'invented rituals to Heaven.'

A Study on the Scratch Resistance by Additives of PDMS and Grain Pattern (PDMS계 첨가제와 Grain pattern에 따른 PP Compound의 내스크래치성 연구)

  • In this study, we have investigated mechanical properties and scratch resistance of polypropylene compounds by two types of masterbatch with different molecular weight of PP. Masterbatches were prepared with polypropylene(PP) and polydimethylsiloxane(PDMS) by melt mixing. Mechanical properties and scratch behavior were examined with UTM and polarizing microscope. The morphology surface of grain pattern was investigated Color 3D Laser Scanning Microscope. Mechanical properties of two masterbatches containing PP compounds showed a little bit difference in accordance with amount of M/B. Scratch resistance was much improved by adding masterbatches used high-molecular weight PP. Scratch resistance was excellent when grain pattern of the surface is deep, large, round and irregular.

  • The neoplastic variant of calcifying odontogenic cyst has various designation, and its malignant counterpart has been reported as aggressive epithelial ghost cell tumor or odontogenic ghost cell carcinoma. Odontogenic ghost cell carcinoma(OGCC) is a rare carcinoma first documented in 1985. It is composed of varying sized islands of anucleated cells with homogenous, pale eosinophilic cytoplasm, so called ghost cells, were admixed with nucleated cells. We report a case of maxillary OGCC developed from odontogenic epithelial tumor in a 25-year-old man with literature review.

  • Ameloblasotma is slowly growing, locally invasive neoplasm with a potentially destructive behavior. The epithelium of ameloblastoma is thought to have an intrinsic growth potential and has been shown to present a higher rate of proliferation as compared to odontogenic cysts with low local recurrence rate. The molecular mechanisms that regulate the cell growth and invasion of ameloblastoma cells are unknown. Bcl-2 protein, which prevent apoptosis, is expressed in immortalized ameloblastoma cell line(AM-1)(Harada et al 1998). Expression of bcl-2 protein occurs in tooth germs, whose epithelial component may act as the histogenic precursor of ameloblastoma. Bax is considered as a main effector of apoptosis. Bax forms homodimers and also heterodimers with bcl-2. p53 tumor supressor gene participates not only in cell proliferation control but also in induction of apoptosis. The objective of the present study was to evaluate the apoptosis related protein expression in odontogenic cyst and ameloblastoma. A total of 10 dentigerous cysts and 16 ameloblastomas were used in the present study. Dentigerous cyst showed negative or slight positive for p53 and bcl-2 but strongly positive for bax, ameloblastoma, on the other hand, strongly positive for p53 and bcl-2 but weekly positive for bax. Bcl-2 was expressed for ameloblastoma mainly in outer layer or whole layer of epithelium and for dentigerous cyst mainly in basal layer. The difference in expression of apoptosis related protein in dentigerous cyst and ameloblastoma might explain the peculiar aggressive growth pattern of ameloblastoma.

CALCIFYING ODONTOGENIC CYST ASSOCIATED WITH COMPLEX ODONTOMA (치아종과 동반한 석회화 치성낭)

  • The calcifying odontogenic cyst(COC) showed diverse terminology or classification, clinicopathologic features as well as in its biologic behavior, although it was recognized as a distinct clinicopathologic entity. The epithelial lining of a COC appears to have the ability to induce the formation of dental tissues in the adjacent connective tissue wall, and that other odontogenic tumors may sometimes be associated with it. This case is a COC associated with a complex odontoma involving an impacted left maxillary lateral incisor in a 5-year-old female child. Radiographic examination revealed a well-demarcated radiolucent lesion partially occupied by a radiopaque mass, involving the left lateral incisor crown. The histologic sections showed a cystic cavity lined with ameloblastic epithelium containing ghost cell masses with admixed with complex odontoma components. The presence of mixed radiolucent-radiopaque lesion in children as observed in this case, the possibility of COC must be considered. In this case, there was no recurrence 1 year after enucleation and the space control is ongoing now.
