A study on the types of attitude toward senile dementia (노인성 치매에 대한 태도 유형 연구)

  • As the number of the elderly increases, concerns about senile dementia and serious game for dementia are growing. The purpose of this study is to explore the types of attitude toward senile dementia. To identify public attitudes toward senile dementia, we conducted Q methodology. A Q set of potential public attitudes toward senile dementia was identified through literature review. From this, 52 potential public attitudes toward senile dementia were identified. Twenty subjects ranked these explanations as possible public attitudes. The attitudes toward senile dementia were classified as 4 types : 'type1 : idealistic & expectant', 'type 2 : aversive & defensive', 'type 3 : active & realistic', and 'type 4 : responsible & compensatory'. This study revealed that people have different explanatory frameworks for the attitude toward senile dementia.

Research for Recognition Levels of Students in Occupational Therapy on the Function of the Dementia Care Center (치매안심센터 기능에 대한 작업치료 전공 학생들의 인식 수준 조사)

  • The purpose of this study was to investigate the recognition levels of students in occupational therapy on the function of the dementia care center. The study included 357 students in occupational therapy and the study was conducted from September 2019 to December 2019 based on the questionnaire about the function of the dementia care center. The major results of this study were as follows: Among the types of projects of the dementia care center, 'consultation and registration management' scored the highest. There were significant differences in recognition levels of 7 projects of dementia care center according to 'age' among general characteristics. There were significant differences in recognition levels of 6 projects except 'a visiting model' according to 'grade'. There were significant differences in recognition levels of 'rest area business for dementia' and 'support for families with dementia' according to 'region'. There was a significant differences in recognition levels of 'dementia care village' according to 'experience of using dementia care center'. There were significant differences in recognition levels of 'consultation and registration management', 'support for families with dementia', 'dementia care village' according to 'experience of clinical practice'. This study will provide basic data for the study of community based rehabilitation and help to establish the curriculum of occupational therapy.

A Review of Music Intervention Studies With Elderly Patients With Moderate to Severe Dementia (국내·외 중등도 및 중증 치매 노인 대상 음악 중재 연구 고찰)

  • This review was conducted to identify the research trends in music intervention studies with elderly patients with dementia. Korean- and English-written studies on music intervention for this population were searched and analyzed. Seventeen studies were included in the final analysis. The results showed that behavioral and psychological symptoms were the main dependent variables. In terms of the type of music intervention employed, interventions requiring the patient's active participation in producing music were more common than interventions that required the patient to only listen to music. The majority of studies provided live music, selected music that was familiar to and preferred by the patient, and utilized rhythm-based performance activities. In the studies using active music production as the intervention, the participants were asked to express the emotions evoked by group instrument playing or singing along with music, which doesn't reflect the functional limitations (e.g., cognitive impairment) of elderly patients with moderate to severe dementia. The results of this review point to the need in the field to develop music intervention programs for the elderly with moderate to severe dementia that meaningfully engage these patients in music-related behaviors that target their specific symptomology.

Different Patterns of Behavioral and Psychological Symptoms of Dementia and Caregiver Burden according to Dementia Type in Nursing Home Residents (요양시설 치매노인의 치매유형에 따른 행동심리증상 특성과 간호제공자의 업무 부담감)

  • The purpose of this study was to examine different patterns of behavioral and psychological symptoms of dementia (BPSD) and caregiver burden according to dementia type. Data were collected from June 2014 to December 2014 from five nursing homes. In all, 214 patients [131 patients with Alzheimer's disease (AD) and 83 patients with Vascular dementia (VD)] were included in the study. BPSD and caregiver burden data were examined using NPI-NH (Neuropsychiatric Inventory Nursing Home Version). According to the results, both the AD and VD group showed the highest frequency in apathy/indifference and the frequency of anxiety, elation/euphoria, and irritability/lability was significantly higher in the AD group than in the VD group. The difference in total BPSD composite scores between the AD and VD groups was not significant but agitation/aggression was significantly higher in the AD group than in the VD group. The mean score for caregiver burden was significantly higher in the AD group than in the VD group, particularly in agitation/aggression. In conclusion, BPSD differed according to dementia type, and nurses should provide specific interventions to control BPSD; a program to reduce caregiver burden according to dementia type is also needed.

Social Network Type and Quality of Life of Elderly People with Dementia (사회 연결망 유형과 치매노인의 삶의 질)

  • The aim of this study is to classify the social network types among elderly people with dementia, and to examine the relationship of network type to quality of life. This is to identify influence of social network on quality of life of elderly people with dementia. And the study seeks to identify the differences of quality of life among types of social network. The data of 222 elderly people used in this study were collected from the health center and elderly people with dementia living in the community. The interview was conducted from July 17 to August 31 in 2012 using a structured questionnaire. Descriptive statistics, one way ANOVA, multiple regression analysis and also cluster analysis with the SPSS 18.0 program were used to analyze the data. As a results, three network types were classified.:(1) inactive isolated, (2) active independent (3) inactive dependent networks. Respondents in the different network type are found to have different degrees of quality of life. Respondents inactive independent network is reported to have the highest quality of life, while those with inactive isolated network are the lowest. The results of the study suggest that the classification of network types allows consideration of the interpersonal environments of elderly people with dementia. The relative effects on quality of life for elderly people with dementia, evident in the current analysis, are the case in point. Therefore Social service programs should focus on different groups based on social network type.

A Study on the Characteristics of Errors Type for Wellness of Alzheimer's Dementia Patients in the Naming Task (알츠하이머성 치매환자의 웰니스를 위한 명명하기 과제에서의 오류유형 특성 연구)

  • The purpose of this study was to investigate the characteristics of error types in naming task for 8 questionable demeatia groups, 9 definite dementia groups, and 10 normal groups. The items of naming error analysis were classified into visual perception errors, semantic association errors, semantic non-correlation errors, phoneme errors, Don't Know, and No Response. For the analysis, descriptive statistics analysis, analysis of variance, and multivariate analysis of variance were conducted using SPSS 21.0. As a result, there was a significant difference in the error rate between groups according to the error type. The errors that showed significant differences between the normal group and the other two groups were visual perception errors and semantic non-related errors. The error of non-response was different from the dementia confirmation group, but there was no significant difference from the dementia suspicion group. These results showed that Alzheimer's patients had a defect in confrontation naming ability. Also, it was found that it is appropriate to provid other clues when the defects caused by the degeneration of a specific step during the information processing process become severe.

Analysis of the Types of Dementia Patients for Development of Clothes for Dementia Patient in Nursing Homes (요양시설 치매환자복 디자인 개발을 위한 치매환자의 유형 분석)

  • This study aims to obtain basic data to develop clothes for dementia patients by classifying types of dementia patients. Data was collected from those dementia patients who entered a nursing home. This study analyzed a total of 221 sheets. Furthermore, descriptive statistics, cross-tabulation, and K-means clustering were performed for statistical processing using Minitab 14. As a result, dementia patients were classified into four types: inactive-dependent, active-problematic behavior, activity-autonomy, and inactive-offensive. Inactive-dependent type was a group with the most severe disability in cognitive and daily activity functions; however, they lacked behavioral and psychological symptoms and problematic behavior with clothes. Active-problematic behavior type showed the most behavioral and psychological problems and problematic behavior with clothes. Activity-autonomy type was a group without any problematic behaviors. Moreover, the inactive-offensive type had very good cognitive function toward humans. The study imply that it is necessary to provide clothes with proper functions based on the types of patients rather than providing them uniform clothes because clinical and clothes behaviors differ significantly depending on the types of dementia patients.

The Effect of Social Participation on Cognitive Impairment Degree among Korean Older Adults: Comparative Study between Urban and Rural Area (사회참여활동이 고령자 인지기능 손상 정도에 미치는 영향: 도시와 농촌 간 차이 분석)

  • The purpose of this study is to analyze the effect of social participation types on cognitive impairment degree(normal, mild, severe cognitive function) by urban and rural difference. The research was performed by multinomial logistic regression with the data of elderly 5,668 over 55 years of age from the sixth wave(2016) and the seventh wave(2018) of the Korean Longitudinal Study of Ageing. Results show that social participation reduces the risk of cognitive impairment on both urban and rural older adults regardless of types, but the types of social participation, which are effective to reduce the risk of cognitive impairment, are different between urban and rural area. According to residential area, types of social participation, which are effective to decline the risk of being mild or severe cognitive impairment and the risk of being severe cognitive impairment relative to mild, are different. These findings suggest that, in order to prevent cognitive impairment and reduce the severity, social participation is strongly recommended and appropriate supports are required. This implies policy and intervention efforts considering the urban and rural difference are needed.

Differences in Verbal Fluencies and Discourse Comprehension Abilities associated with Working Memory in Alzheimer's Disease and Vascular Dementia (알츠하이머와 혈관성 치매 환자 선별에서의 작업기억 능력 관련 구어유창성 및 이야기이해 능력의 차이)

  • The present study was conducted to examine the differences and correlations between verbal fluency and story comprehension according to the working memory(WM) capacity, and to find out what WM factors influence the linguistic competence in Alzheimer's disease(AD) and vascular dementia(VaD) groups each consisting of 15 patients. The results of their performance produced firstly significant differences in phonemic fluency, story comprehension, delayed recall and recognition task between the two groups. Further analysis shows that VaD group had significant correlations between the scores of story comprehension and the recognition test scores additionally. These findings suggest that it is possible to differentiate the two groups even by story comprehension tasks and WM. In conclusion, the clinical application of the results is likely to contribute to appropriate treatment plans and effective interventions for elderly with AD and VaD as well as to improve the classification criteria for both types of dementia.

Effects of Exercise on Sleep Quality and Dementia-related Factors of Dementia Patients (운동이 치매 환자의 수면 관련 인자, 치매 관련 인자에 미치는 영향)

  • It is well known that exercise may have a positive effects on dementia. However, most of dementia-related studies were performed about symptom care, diverse treatment and drug treatment. Although regular physical exercise plays an important role of brain function and dementia prevention, only a few research studies showed the effect of physical exercise on dementia and brain function. This study is to investigate about the sleep-related factors (melatonin, serotonin), dementia-related factors (BDNF, beta-amyloid, IGF-1, Irisin, GLUT) with physical exercise. This study revealed that Myokine (Irisin, IGF-1), GLUT and Melatonin is an important factors to improve or sometimes delay the symptoms of dementia through BDNF activation following endurance exercise. However, these results were controversal following the various kinds of exercise type, mode, duration and exercise intensity and so forth. Therefore, further mechanism study about exercise and dementia is urgently needed in this area.
