Comparison of Demographic Characteristics, Health and Quality of Life between General Adults and Adults Living with Dementia Patients : The 2016 Community Health Survey (치매 환자와 함께 거주하는 성인과 일반 성인 사이의 인구사회학적 특성, 건강 및 삶의 질 비교 : 2016년 지역사회건강조사 원시자료를 이용하여)

  • Objective : The purpose of this study was to compare the demographic characteristics, health and quality of life between general adults and adults living with dementia. Method : The data were collected using raw data of the 2016 community health survey and compared between 2,592 adults living with dementia patients and 225,840 general adults. health were assessed for sleep time, stress level, depression, and subjective health status, and quality of life was measured by EQ-5D. Result : In comparison of demographic characteristics, age and family number of adults living with dementia patients were significantly higher than general adults (p<.001), income and eduation levels were low (p<.001), and marital status was higher rate of living with spouse (p<.05). In comparison of health status, adults living with dementia patients were significantly longer in sleep time than the general adults (p<.001), and stress level was higher (p<.001), the percentage of experience of depression was higher (p<.001), and the subjective health status was worse (p<.001). Adults living with dementia patients were significantly lower in quality of life total score and all sub-domain than general adults (p<.001). Conclusion : Based on the results of this study, it is necessary to seek ways to improve the health and quality of life of dementia patients' families.

  • According to the Clinical Research Center for Dementia, 840,000 Koreans aged 65 or older had dementia patients, with a prevalence rate of 10.39%. The prevalence rate is one in 10 elderly people, but difficult for families to take care of them all day. Judged that possible to manage the conditions and schedules of elderly people living alone by utilizing AI speaker system where schedule management is stored. This paper implements modules for AI schedulers in patients with dementia. Configured to link AWS, a remote IOT, inside the raspberry pi, and to output the schedule to speakers using a calendar from Google API. Through this study, judged that ease of scheduling will help manage and schedule dementia patients.

Bioinformatics Analysis of Gene Expression Regulation by Transposable Elements in Dementia Patients (치매환자에서 transposable elements에 의한 유전자 발현조절의 생물정보 분석)

  • Dementia is a progressive disease of increasing the dysfunction of intellectual and physical ability. In the aging society, many families are suffering from the caring the patients who are diagnosed with dementia. However, dementia is a complex disease affected by the genetic and environmental agents. In the present study, we investigated the transposable elements in relation to dementia. From the analysis of dementia EST (expressed sequence tag) sequences, we found dementia candidate genes, and analyzed expression profiles and repeat elements using bioinformatics tools. This analysis showed that 98 genes were affected in their mRNA sequences by transposable elements expression. Their expressions were affected by the integration of different transposable elements (SINE, LINE, LTR, DNA) during the primate evolution. We believe that our work will be of significant interest to genome scientists, and may help them gain insight into implication of transposable elements expression in dementia.

Effects of Burden and Family Resilience on the Family Adaptation of Family Caregivers of Elderly with Dementia (부양부담과 가족극복력이 치매노인 부양가족의 적응에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the levels of burden, family resilience and adaptation of caregivers of elderly patients with dementia, and further to identify factors influencing their adaptation. Methods: A cross-sectional descriptive study was designed. Data were collected from questionnaires distributed to 131 family caregivers of elderly patients who visited at the Centers for Dementia in Seoul during 2012~2013. The data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation analysis, and stepwise multiple regression. Results: The average age of the care giving subjects was 63.58, and 31.3% were male, and 41.2% were the spouses. Statistically significant relationships were found between burden and adaptation (r=-.38, p<.001), and between family resilience and adaptation (r=.52, p<.001), and between burden and family resilience (r=-.35, p=.001). Thirty percent of adaptation was explained by burden and family resilience. The most influencing factor to adaptation was family resilience which explained about 27% of the variance. Conclusion: The results of the study clearly indicate that family resilience explains better than burden on adaptation of family caregivers. Thus, to develop more effective nursing intervention for family caregivers of elderly patients with dementia, it would be necessary to integrate family resilience in the programs. 27% is not that much and I wonder if we have to do more work to identify the factors that influence care giving.

Respite Care for Family Caregivers of Elders with Dementia : Concept Clarification (재가 치매가족의 휴식 돌봄: 개념명료화)

  • Purpose: Respite care is not a discrete intervention, but encompasses a range of services. This research was conducted to clarify the phenomenon of respite care for family caregivers of elders with dementia from a nursing perspective. Method: The Hybrid Model of concept development was applied to clarify the concept of respite care for family caregivers of elders with dementia. The study was conducted in the following three steps, theoretical phase, fieldwork phase, and final analytic phase. Results: The definition of respite care for family caregivers of elders with dementia was delineated through integration of data analyses in theoretical and fieldwork phase, and has three dimensions; tailored supports for caregivers, tailored supports based on physical and cognitive function of elders with dementia and community interventions related to family care function. Conclusion: Through this study, the concept of respite care for family caregivers of elders with dementia is clarified and reformulated as nursing practice phenomena in the Korean context, which indicates ways to develop caring practice forms for a family living with an elder with dementia in a community setting.

Comparison of ADL and Symptoms in Elders with Dementia according to Family Resiliency (가족 복원력이 높은 집단과 낮은 집단간 치매노인의 일상활동능력 및 치매증상 비교)

  • Purpose: The purpose of this study was to identify family resiliency, ADL in elders with dementia, and symptom for families with an elder with dementia. Method: The participants were 102 people who were caregivers to elders with dementia and who visited the out-patient department at S. hospital for follow up care. Data were collected from August to October 2003 using a questionnaire. The collected data were analyzed using descriptive statistics and t-test aided by the SPSS/PC. Result: Family resiliency for the whole sample was within normal limits but when the sample was quarter-divided by good and poor family resiliency, based on established cut-off scores, there were differences for some characteristics according to good/poor family resiliency. These differences were for perceived health state of caregivers (t=-2.78, p=.008), economic state of family (t=-3.34, p=.002), and ADL (t=-2.56, p=.014). Conclusion: Future research should focus on the way in which family resiliency can be enhanced and on the effects of interventions for those caregivers who report poor family resiliency.

  • The purpose of this study was to understand the adult day care center as a place for the elders and adults and to develop an initial understanding of the services and activity programs for adult day care for the cognitively-impaired in the U.S.A. The data were collected from 12 Adult Day Care Centers(ADC) from 2001 to 2002 by personal interview and the documents about the programs. ADC programs provided primarily social, recreational, and health activities and services in a group setting. Centers offered participants to socialize, enjoy peer support, and receive health and social services in a stimulating and supportive environment that promotes better physical and mental health. For this purpose those centers offered not only professional health care, occupational.speech.physical therapies but also socio-recreation and therapeutic-recreation services.

Behavioral Variant Frontotemporal Dementia Phenocopy Syndrome (행동증상 아형 전측두엽 치매 표현형모사 증후군)

  • Objectives : The aim of this study was to draw attention toward so called 'behavioral variant frontotemporal dementia(bvFTD) phenocopy syndrome', which is difficult to discriminate with the primary psychiatric disorders, showing poor response to conventional therapeutic drugs, leading to higher risk to misdiagnoses and legal problems. Furthermore, the author insisted that our interest and study on them must be continued. Methods : English articles published during 2000 thru 2016 had been searched by internet with the combination of words such as 'frontotemporal', 'phenocopy' and 'behavioral', and reviewed. Besides, two clinical vignettes were described. Results : Precise diagnosis is important because patients' behavioral symptoms can influence on their families and community. However, disease-modifying treatment for bvFTD are not developed until now, and recent therapeutic drugs are only good for specific symptoms, while deterioration progresses in spite of proper psychiatric management. The possible bvFTD patients are not progressed into probable bvFTD clinically, showing no decline of cogntive and social function, no decrease of activity function, longer survival time, and normal neuroimaging for several years. Conclusions : Rather than expected, there are much more patients having clinical symptoms, course and diagnostic findings including neuroimaging, which are atypical to classical frontotemporal dementia and primary psychiatric disorders. If our knowledge and discriminating ability is improved, discovery rate of that cases will be increased. However, the identity of these atypical features are not clarified until now, it must be further actively investigated.

A Study on the Effect of Elderly Dementia Caregiver's Stress to their Suicidal Ideation -Mediating Effect of Self-efficacy- (치매노인 보호자의 부양스트레스가 자살샘각에 미치는 영향 -자기효능감의 매개효과-)

  • The purpose of this study is to examine the effects of care-giving stress of dementia elderly on the caregiver's suicidal ideation. In addition, mediating effect of self-efficacy was examined. In order to accomplish these purposes, a total of 336 who are caregivers of dementia elderly using self-report questionnaires. In this study, 325 elderly patients were used for final analysis. Date were analyzed by frequency and descriptive analysis, regression model analysis with SPSS 22.0. The analysis result of dementia elderly caregiver's symptoms showed that prevalence of suicidal ideation were 32.6%. Regression analysis was conducted under the control of dementia elderly's disease period, ratings, general characteristics and age, sex, educational background, occupation status, health status, living standard level of caregivers who were the subject of the survey. As a result, it was confirmed that the care giving stress had a statistically significant effect on suicidal ideation. Also, the self-efficacy of caregivers was found to partial mediate the relationship between care-giving stress and suicidal ideation(***p<.001). Based on this result, I suggested the various policies and the necessity of social welfare intervention to restore the mental health of caregivers of the elderly with dementia.
