Effect on the Management of Postherpetic Neuralgia (대상포진 후 동통에 대한 치료 효과)

  • Fifteen patients was analyzed on effect of the management of postherpetic neuralgia by local anesthesia on the special region at pain clinic in Youngnam University Hospital. The results were on follows : 1) The frequency of occurrence of sex and the lesion side were similar in all patients. 2) The age of incidence was between 50 and 70 years old. 3) The most frequent site of lesions was the neck. 4) There was no relationship between age and treatment time. 5) Whole patients was done average 7-10 time local injection.

The Results of Hvperfractionated Radiation Therapy Combined with Taxol for Paraaortic Node Recurrence in Cervix Cancer (대동맥주위 림프절에 재발된 자궁경부암에서 Taxol을 병행한 과분할 방사선치료의 결과)

  • Purpose : The aim of this study was to investigate treatment results, toxicity and efficacy of hypefractionated radiation therapy combined with paclitaxel for paraaortic node recurrence in cervix cancer. Materials and Methods: Between September 1997 to March 1999, 12 patients with paraaortic node recurrence in cervix cancer who previously received radical or postoperative radiotherapy were treated with hypefractionated radiation therapy combined with paclitaxel. Of these, 2 patients who irradiated less than 30 Gy were excluded, 10 patients were eligible for this study. Median age was 51 years. Initial FIGO stage was 1 stage IBI, 2 stage IIA, 7 stage IIB. For initial treatment, 7 patients received radical radiotherapy and 3 received postoperative radiotherapy. The paraaortic field encompassed the gross recurrent disease with superior margin at T12, and inferior margin was between L5 and S1 with gap for previously pelvic radiation field. The radiation field was initially anterior and posterior opposed field followed by both lateral field. The daily dose was 1.2 Gy, twice daily fractions, and total radiotherapy dose was between 50.4 and 60 Gy(median, 58.8 Gy). Concurrent chemotherapy was done with paclitaxel as a radiosensitizer. Dose range was from 20 mg/m$^{3}$ to 30 mg/m$^{3}$ (median, 25 mg/m$^{3}$), and cycle of chemotherapy was from 3 to 6 (median, 4.5 cycle). Follow-up period ranged from 3 to 21 months. Results : Interval between initial diagnosis and paraaortic node recurrence was range from 2 to 63 months (median, 8 months). The 1 year overall survival rate and median survival were 75$\%$ and 9.5 months, respectively. The 1 year disease free survival rate and median disease free survival were 30$\%$ and 7 months, respectively. At 1 month after treatment, 4 (40$\%$) achieved a complete response and 6 (60$\%$) experienced a partial response and all patients showed response above the partial response. There was distant metastasis in 6 patients and pelvic node recurrence In 2 patients after paraaortic node irradiation. There was 2 patients with grade 3 to 4 leukopenla and 8 patients with grade 1 to 2 nausea/ vomiting which was usually tolerable with antlemetic drug. There was no chronic complication in abdomen and pelvis during follow up period. Conclusion : hypefractionated radiation therapy combined with paclitaxel chemotherapy diosensitizer showed high response rate and few complication rate in paraaortic node recurrence in cervix cancer Therefore, present results suggest that hypefractionated radiation therapy combined with paclitaxel chemotherapy can be used as optimal treatment modality in this patients.

  • 방사선치료후 재발성 또는 잔존성 두경부암의 수술후에 피부판의 괴사 및 slough, 감염, 누공, 부종과 경동맥 노출 심지어는 경동맥 파열 등의 술후 합병증이 빈발함으로 수술에 큰 어려움을 겪게 된다. Pectoralis major myocutaneous flap은 진행된 두경부암의 광범위한 절제후에 그 결손부를 보충해 주는 데 좋은 방법으로 사용할 수 있을 뿐아니라 방사선치료후에 생긴 합병증에도 이 flap을 이용하여 좋은 치료결과를 얻을 수 있다. 저자들은 수술 전에 방사선치료를 받고 심한 부종으로 후두조직의 괴사 및 기능이 손실된 환자와 방사선 치료후 재발된 상인두암환자의 경우에서 수술 후 생긴 pharyngocutaneous fistula와 피부판감염 및 경동맥노출 등으로 치유가 곤란하여 여기에 pectoralis major myocutaneous flap을 사용하여 pharyngostoma를 재건하고 노출된 경동맥부위를 보호하여 좋은 치료효과를 얻었기에 문헌을 고찰하여 보고하는 바이다.

  • The treatment of fractured teeth in the upper anterior area has various causes and clinical symptoms at different ages. Most patients come to the clinic in emergency state, so if you're treating them with accurate diagnosis criteria, You'll get aesthetic and functionally satisfying results. I'm going to take a look at the treatment of the upper anterior tooth, which is more sensitive than other parts, with the case I've treated before. The case I treated cannot be the answer, but I hope it will be of any help if youencounter similar symptoms in your clinical trial.

The usefulness of Forward IMRT for Head and Neck Cancer (두경부(Head & Neck)종양에서 Forward IMRT 유용성에 관한 고찰)

  • I. Purpose The dose distribution in normal tissues and target lesions is very important in the treatment planning. To make the uniform dose distribution in target lesions, many methods has been used. Especially in the head and neck, the dose inhomogeneity at the skin surface should be corrected. Conventional methods have a limitation in delivering the enough doses to the planning target volume (PTV) with minimized dose to the parotid gland and spinal cord. In this study, we investigated the feasibility and the practical QA methods of the forward IMRT. II. Material and Methods The treatment plan of the forward IMRT with the partial block technique using the dynamic multi-leaf collimator (dMLC) for the patients with the nasopharyngeal cancer was verified using the dose volume histogram (DVH). The films and pinpoint chamber were used for the accurate dose verification. III. Results As a result of verifying the DVH for the 2-D treatment plan with the forward IMRT, the dose to the both parotid gland and spinal cord were reduced. So the forward IMRT could save the normal tissues and optimize the treatment. Forward IMRT can use the 3-D treatment planning system and easily assure the quality, so it is easily accessible comparing with inverse IMRT IV. Conclusion The forward IMRT could make the uniform dose in the PTV while maintaining under the tolerance dose in the normal tissues comparing with the 2-D treatment.

Preliminary Results of Stereotactic Radiosurgery Using Stereotactic Body Frame (정위 체부 고정틀을 이용한 체부 방사선수술의 예비적 결과)

  • Purpose : To evaluate efficacy and complication of stereotactic radiosurgery using stereotactic body frame. Methods and Materials :From December 1997 to June 1999, 11 patients with primary and metastatic tumors were treated with stereotactic radiosurgery using stereotactic body frame(Precision TherapyTu). Three patients were treated with primary hepatoma and seven with metastatic tumor from liver, lung, breast, trachea and one with arteriovenous malformation on neck. We used vacuum pillow for immobilization and made skin marker on sternum and tibia area with chest marker and leg marker. Diaphragm control was used for reducing movement by respiration. CT-simulation and treatment planning were peformed. Set-up error was checked by CT-Simulator before each treatment. Dose were calculated on the 80$\~$90$\%$ isodose of isocenter dose and given consecutive 3 fractions for total dose of 30 Gy (10 Gy/fraction). Results :Median follow-up was 12 months. One patient (9$\%$) showed complete response and four Patients (36$\%$) showed partial response and others showed stable disease. Planning target volumes (PTV) ranged from 3 to 111 cc (mean 18.4 n). Set-up error was within 5 mm in all directions (X, Y, Z axis). There was no complication in all patients. Conclusion :In Primary and metastatic tumors, stereotactic body frame is very safe, accurate and effective treatment modality.

Canine Dermatitix Associated with Malassezia pachydermatis (개에 있어서 Malassezia pachydermatis에 의한 피부염 1예)

  • 지방친화성 진균인 M. pachydermatis가 두 마리의 수캐에서 피부병의 원인균으로 증명되었다. 두 마리의 환축에서 흥반, 인설, 탈모 등이 여러 부위에서 발생하였으며, 광역항생물질과 코르티코스테로이드의 45일간 투여에 대해 아무런 임상적 반응을 나타내지 않았다. 발아를 나타내는 난원형 내지 원주형의 효모균이 KOH 및 PHOL 방법에 의해 환축의 피부병변으로부터 다수 증명되었고, 이들은 형태학적으로 M. pachydermatis와 일치하였다. 두 환축의 피부 병변부위 시료를 chloramphenicol을 첨가한 %37{\diamond}C$의 Saubouraud dextrose agar에 배양하면 이 진균이 반복하여 배양되었다. ketoconazole의 경구투여와 sulphur lime의 국소적용에는 치료반응을 잘 나타내었다.

  • Anchorage plays an important role in orthodontic treatment. Endosseous implants may be considered adequate firm anchorage. However, clinicians have hesitated to use endosseous implants as orthodontic anchorage because of limited implantation space, high cost, and long waiting period before osseointegration occurs. Recently, some clinicians have tried to use titanium miniscrews and microscrews in treatment due to their many advantages such as ease of insertion and removal, low cost, immediate loading, and the ability to place microscrews in any area of alveolar bone. The author treated a case with skeletal cortical anchorage using titanium microscrew implants. During six months of orthodontic force application from skeletal cortical anchorage, the author could get 4 mm bodily retraction and intrusion of upper anterior teeth. The most outstanding result was a 1.5 mm posterior refraction of the upper posterior teeth. The titanium microscrew implants had remained firm and stable throughout treatment. These results indicate that skeletal cortical anchorage might be a very good option.

  • The definition of fascial spaces are latent spaces between fascial planes. If infections which spread from dental origin to soft tissue are mild, they are restricted by fascial planes. But, when infections are severe, fascial spaces are often used as a natural pathway which spread to the deep cervical region. If they are not treated at early stage, they may result in the fatal complications as followings; airway obstruction, septicemia, cerebral abscess, and thrombophlebitis etc. The early treatment of fascial space abscess is very important for young children. These case reports present the successful result of fascial space abscess treatment through intracanal drainage without surgical excision. It is proven that the treatment through intracanal drainage has some benefits to the surgical excision, which are as follows: 1) It is economic to the patients or their parents avoiding admission. 2) The treatment procedure is more simple. 3) Childrens can avoid the fearful environment.
