The purpose of this study is to understand the importance of exterior environments and to identify the design guidelines for the therapeutic garden for cognitively impaired seniors with Alzheimer's disease and related dementias. The data was collected from 4 Adult Day Care Centers, 2 Nursing Homes, & 1 Assisted living in the U.S.A. from 2001 to 2002 by interview and the floor plan about the outdoor space. Although some may believe that patients with Alzheimer's disease and related disorders are unresponsive to environment, evidence shows that environments especially designed for cognitively impaired seniors can maintain or increase their level of functioning. The use of specially designed exterior environments nay have in reducing incidents of aggressive behavior, and contributing significantly to a wanderer management program. The checklist includes 6 dimensions: safety environment, various sensual stimulations, social interactions, wandering, privacy, and support orientation and reality. The results indicated that the exterior environment have to support wandering behavior, orientation and reality.
Apartments recently built have been strategically designed in many ways, and one of the most striking aspects is to utilize the idea of environmental friendliness and the application to the living environment. Especially, the number of environmentally friendly apartments that concern the health of residents have been increasing and their landscape is notably differentiated. The purpose of this research study was to examine the landscape of the environmentally friendly apartments that have been built in the past five years, and to assess its features in the aspects of therapeutic garden. Based upon the review of previous studies, this study constructed a checklist that was used to evaluate the planning components of the landscape during site visits. This study found that overall landscape selected in the study was well-planned. However, it was suggested that more features of therapeutic garden such as vegetation area which both encourages interaction with residents and produces therapeutic benefits be adopted in planning the advanced therapeutic garden of environmentally friendly apartments.

Purpose : Depending on a variety of therapeutic areas using MVCT(Megavoltage computed tomography) intra-and interfraction is to evaluate changes in patient position. Materials and Methods : In this study, head and neck, thorax, liver, and prostate tumor patients, 48 people with a full course of treatment for patients with various MVCT scan was performed. Average 60 per patient MVCT images were acquired and analyzed. Results : Interfraction error thorax and abdomen, and pelvis in the longitudinal difference was more than 3 mm. Intrafraction to see a change after the end of treatment MVCT taken, and the results confirmed Intrafraction 2 mm in motion around the longitudinal difference. In addition, due to changes in the movement before and after treatment, Inter-and Intrafraction difference was found. Conclusion : In the present study, the Head and Neck, Thorax, liver, prostate cancer in patients with inter-and intrafraction changes and target the appropriate fixtures to complete the outline of the margin would be helpful is considered.

Purpose : This study is to see how much proportion of the patients receiving radiation therapy drop out during radiation therapy and to analyze the reason for the incomplete treatment. Materials and Methods : The base population of this study was 1,100 patients with registration numbers 901 through 2,000 at Department of Radiation Oncology, Samsung Medical Center, Seoul, Korea. Authors investigated the incidence of incomplete radiation therapy, which was defined as less than 95$\%$ of initially planned radiation dose, and the reasons for incomplete radiation therapy. Results : One hundred and twenty eight patients (12$\%$) did not complete the planned radiation therapy. The performance status of the incompletely treated patients was generally Poorer than that of the base population, and the aim of radiation therapy was more commonly palliative. The most common reason for not completing the planned treatment was the patients' refusal of further radiation therapy because of the distrust of radiation therapy and/or the poor economic status. Conclusion : Careful case selection for radiation therapy with consideration of the socioeconomic status of the patients in addition to the clinical indication would be necessary for the reduction of incomplete treatment, especially in the palliative setting.
The patient dose in advanced radiotherapy techniques is an important issue. These methods should be evaluated to reduce the dose in diagnostic imaging for radiotherapy. Especially, the Computed Tomography in radiotherapy has been used widely; hence the CT was evaluated for dose and image in this study. The evaluations for dose and image were done in equal condition due to compare the dose and image simultaneously. Furthermore, the possibility of dose and image evaluations by using the Monte Carlo simulation MCNPX was confirmed. We made the iterative reconstruction for low dose CT image to elevate image quality with Maximum Likelihood Expectation Maximization; MLEM. The system we developed is expected to be used not only to reduce the patient dose in radiotherapy, also to evaluate the overall factors of image modalities in industrial research.
Chronic pulmonary artery thromboembolism is a relatively rare phenomenon causing hypoxia and pulmonary hypertension that eventually leads to respiratory failure and right heart failure. Patients with acute pulmonary artery thromboembolism are generally treated with antithrombotics and thrombolytics. However, in cases with chronic pulmonary artery thromboembolism the fibrinized thrombus is so strongly adhered to the pulmonary artery wall that medical treatment becomes ineffective and surgical treatment must then be considered. We report a 47year old patient, with a history of repeated admission due to unresolved chronic obstructive pulmonary disease and chronic pulmonary artery thromboembolism at a local hospital, who underwent a successful endarterectomy of pulmonary artery thromboemboli using intermittent total circulatory arrest.
For treatment of Total Skin Electron beam Therapy (TSET), measurement of dose at various conditions is need on the contrary to usual radiotherapy. When treating TSET with modified Stanford technique based on linear accelerator, the energy of treatment electron beam, the spatial dose distribution and the actual doses deposited on the surface of the patient were measured by using EBT2. The measured energy of the electron beam was agreed with the value that measured by ionization chamber, and the spatial dose distribution at the patient position and the doses at several point on the patient's skin could be easily measured by EBT2 film. The dose on the patient that was measured by EBT2 film showed good agreement with the data measured simultaneously by TLD. With the results of this study, it was proven that the EBT2 film can be one of the useful dosimeter for TSET.

The study aimed to evaluate the efficacy of treatment plans using full Arc and Partial Arc Coplanar volumetric modulated arc therapy and Non-Coplanar volumetric modulated arc therapy to minimize radiation treatment side effects, such as pneumonia, and protect normal organs in esophageal cancer radiotherapy. 30 patients who underwent Concurrent Chemoradiotherapy for esophageal cancer were included. Compared planning target volume, lung, heart, spinal cord and total monitor units among three treatment plans: fVMAT(2 Full Arc), pVMAT(4 Partial Arc), and ncVMAT(2 Partial Arc + 2 Non-Coplanar Arc). All plans met the PTV criteria, showing uniform distribution. The average dose to the heart was 5.8 Gy for fVMAT, 6.97 Gy for pVMAT, and 7.6 Gy for ncVMAT, with the lowest value in fVMAT, which was statistically significant. However, the average lung dose was 9.01 Gy for fVMAT, 7.71 Gy for pVMAT, and 7.12 Gy for ncVMAT, with V5Gy(%) values of 52.22%, 38.61%, 36.35% and V10Gy(%) values of 37.8%, 27.33%, 24.15% respectively. ncVMAT showed the lowest values, while fVMAT had the highest, with statistical significance. In conclusion, ncVMAT effectively reduces lung radiation exposure in esophageal cancer radiotherapy, potentially reducing the incidence of side effects such as pneumonia. However, considering factors like setup accuracy and treatment time, applying an appropriate treatment plan may lead to better outcomes.
This study is to evaluate thedosiemtric leaf gap (DLG) at different depths for dynamic intensity-modulated radiation therapy (IMRT) in order to evaluate the absolute dose and dose distribution according to the different positions of tumors and compare the measured and planned the multileaf collimator (MLC) transmission factor (T.F.) and DLG values. We used the 6 MV and 15 MV photon beam from linear accelerator with a Millenium 120 MLC system. After the import the DICOM RT files, we measured the absolute dose at different depths (2 cm, 5 cm, 10 cm, and 15 cm) to calculate the MLC T. F. and DLG. For 6 MV photon beam, the measured both MLC T. F. and DLG were increased with the increase the measured depths. When applying to treatment planning systemas fixed transmission factor with its value measured under the reference condition at depth of 5 cm, although the difference fixed and varied transmission factor is not significant, the dosiemtric effect could be presented according to the depth that the tumor is placed. Therefore, we are planning to investigate the treatment planning system whichthe T. F. and DLG factor according to at the different depths can be applied in the patient-specific treatment plan.
Objective: All patients who have the disc displacement with reduction (DDwR) are always not progressive but some of them with significant catching or locking progress to disc displacement without reduction (DDw/oR). In this study, we suggest opening exercise therapy using Locking-free Appliance (LA) to be helpful to prevent progressive derangement. Methods: Retrospectively, a total 37 patients who received exercise treatment using LA was analyzed from January 2010 to December 2011. Patients had been instructed in proper exercise technique to practise for opening and closing of mouth wearing the appliance 30 times a day in locking-free position. Results: None of the patients took a turn for worse to DDw/oR (37 patients). The 16 patients (43.2%, locking-free group) of them could open their mouth without intermittent condylar locking. Measurement of data was analyzed using and Mann-Whitney test. There was no statistically significant difference for age, gender, presence of palpational pain, locking period, VAS quantitative pain score between locking-free group and locking group. Conclusions: In this study, some patients could open their mouth without intermittent locking and did not take a turn for worse to DDw/oR after opening exercise therapy with LA. Furthermore, LA had advantages which were short wearing time, low costs and minimizing patients' uncomfortable compared to conventional treatment method. This therapy can be more appropriate new treatment to prevent progress to DDw/oR.
