The Usefulness of Ultrasonography in the Evaluation of Clavicle Fracture Healing (쇄골 골절의 골유합 평가에 있어서 초음파의 유용성)

  • Purpose: This study was conducted to determine the usefulness of ultrasonography in evaluating callus formation in radiologically suspected nonunion or delayed union of clavicle fractures during conservative management. Materials and Methods: Six male patients and one female patient (average age: 38.3 years; range: 7 to 70 years), whose conservatively managed clavicle fractures were suspected of nonunion or delayed union, were examined with ultrasonography. If callus formation was detected, the patient continued to receive conservative management. If callus formation was not detected, the patient received operative management. Results: Six patients, in whom callus formation was observed ultrasonographically, continued to receive conservative management. The final simple radiographies of their fractures showed complete unions. One patient, in whom no callus formation was observed ultrasonographically, received operative management. In that patient, there was no operative finding of any fracture healing. Conclusion: Ultrasonography is a useful tool for evaluating callus formation in clavicle fractures suspected of nonunion or delayed union during conservative management. Ultrasonography can be used to avoid unnecessary surgery.

Prolotherapy for the Lower Extremities (하지에서의 프롤로 치료)

  • Prolotherapy, the technology for strengthening lax ligaments, has found increased acceptance in recent years. Prolotherapy involves the injection of the irritant solutions into the weakened or stretched ligaments which are a source of chronic pain. This induces an inflammatory response which mimics the normal repair sequence. Various musculoskeletal disorders of lower extremities such as degenerative arthrtitis, ligament and tendon injuries can be treated with prolotherapy. Prolotherapy is a very good, powerful nonsurgical conservative treatment in various musculoskeletal disorders. Ultrasound exam is very useful to find and confirm the exact location of disorders and check the progress in prolotherapy practices.

  • 이 연구에서는 24마리의 Japanese White 토끼를 대상으로 양쪽 비골을 인위적으로 골절시킨 후 초음파치료가 골절의 치유에 효과가 있는지를 알아보았다. 초음파 치료 후 대조군의 비골과 실험군의 비골에서의 골소주 비율은 차이가 없었으며 초음파 주파수를 0.875 MHz로 하였을 때와 3 MHz로 하였을 때의 골소주 비율도 차이가 없었다. 따라서 초음파 치료는 토끼의 비골 골절의 치유 효과가 없었다. 그러나 다양한 주파수와 초음파 전달양식을 변화시켰을 때 골절치유 효과가 있는지에 대해서는 지속적인 연구가 필요하다.

  • The incidence of acute Achilles tendon rupture is increasing as sporting activities are becoming more popular nowadays. There are many reports on the merits and disadvantages of surgical or conservative treatment for the injury. We performed a conservative treatment of acute Achilles tendon rupture with a serial casting and observed the progression of tendon healing ultrasonographically.

The Effects of Aromatic Oil on Shoulder Pain in the Middle-aged Woman Patients (중년 여성 환자의 견관절 통증에 대한 아로마 오일의 효과)

  • 대전에 소재하는 C 대학교 병원의 물리치료실에 2003년 5월 9일부터 2004년 10월 20일까지 내원한 환자들 중 견부 통증의 진단으로 치료를 받은 환자 가운데 중추신경계에 손상 병력이 없고, 치료사와 의사소통이 가능한 인지 능력을 가졌으며, 실험에 참가하기로 동의한 환자를 대상으로 내원한 순서에 따라 아로마 초음파군, 아로마군, 초음파군에 각각 30명씩을 배정하여 실험한 결과, 치료의 횟수가 증가함에 따라 통증의 정도를 알아보는 시각적 통증 점수가 감소하였으며(p<0.001), 군 간에 통증 감소의 정도도 차이가 있었다. 사후 검정을 보면 아로마 초음파군과 아로마군의 통증 감소의 정도가 초음파군에 비해 높았다(p<0.001). 또한 치료의 횟수가 증가함에 따라 McGill-Melzac 통증 점수 역시 감소하였으며(p<0.001), 군 간에 차이는 없었지만, 교호작용을 고려하면 아로마를 사용한 군에서 감소 폭이 더 큰 것으로 해석할 수 있다. 압통계의 역치 역시 치료의 횟수가 증가함에 따라 모든 군에서 상승하였으며(p<0.001), 군 간에 역치 증가의 정도에도 차이가 있었는데(p<0.05), 사후 검정을 보면 아로마 초음파군이 아로마군에 비해 역치 증가의 폭이 더 컸다. 따라서 압통계의 역치 증가는 아로마 보다는 초음파에 더 영향을 받는 것으로 생각할 수 있다. 이상의 결과로 볼 때 아로마 오일은 주관적이고, 정서적인 통증을 검사하는 시각적 통증 점수와 McGill-Melzac 점수에 많은 영향을 미치는 반면 조직의 회복에 의해 상승하는 압력 역치를 검사하는 압통계의 수치에는 영향을 적게 미침을 확인할 수 있었으며, 반면 초음파는 주관적, 정서적인 통증의 감소보다는 실제 조직이 회복되면서 감소하는 통증에 더 많은 영향을 미치는 것을 알 수 있었다.

  • Since Endoscopic ultrasound (EUS) was introduced in the 1980s, EUS has evolved from a diagnostic tool to a therapeutic modality for patients with pancreatic neoplasms. Traditionally, treatment policy of pancreatic benign neoplasms (PBN) has been a dichotomous approach to observation or surgery. However, EUS guided treatment provides an alternative option with minimally invasiveness for patients with PBN. This review aimed to provide the role of EUS guided treatment for PBN.

  • Ultrasonographic examination for the rotator cuff disease is dynamic noninvasive study and the results is comparable to MRI on the behalf of recent development. It also can be used as a tool for guided injection and follow up study after rotator cuff repair. The development of equipment, learning curve and knowledge for the disease are the three most important factors for the critical role of the ultrasonography in the diagnosis and treatment of the rotator cuff disease.

  • The sonophoresis, a representative low-intensity ultrasonic therapy, is a technique for delivering the drugs into the epidermis, dermis and skin appendages by using physical vibration and heat effects of the ultrasonic waves. Sonophoresis could increases the delivering and absorption efficiency of the drugs usually consisting of hydrophilic molecules and macromolecules. In addition, it has the advantage of being effective in delivering drugs with relatively large molecular sizes such as insulin or lipid. In this study, we proposed a multi-structure ultrasonic transducer assembly with a large-size single piezoelectric element and a drug delivery function at the treatment site for efficient sonophoresis treatment. Futhermore, a transducer assembly structure capable of raising and maintaining the temperature of the treatment site was proposed and evaluated for effectiveness. The transducer assembly proposed in this study is expected to improve the efficiency of sonophoresis by providing a constant amount of drug, and assisting drug delivery through heating the treatment site.

Fungal Psoas Abscess Successfully Treated by Ultrasonographically Guided Percutaneous Aspiration - A Case Report - (진균성 요근 농양의 초음파 유도하 경피적 천자술을 통한 성공적 치료 - 증례보고 -)

  • Although surgical drainage has been performed in most reported cases of psoas abscess, ultrasonographically guided percutaneous drainage could be effective treatment for psoas abscess. However, utility of percutaneous drainge under ultrasound is less well established. We present a patient in whom fungal psoas abscess was managed by ultrasonographically guided percutaneous aspiration with literature review. Drainage was done by needle aspiration under ultrasound guidance and there was no procedural complication. After aspiration, Candida Albicans was found as a causing organism responsible for abscess and abscess could be successfully treated by repeated aspiration and appropriate antifungal therapy.
