AN INVESTIGATION OF THE FREQUENCY AND PATENCY OF ACCESSORY CANALS IN FURCATION AREAS OF PRIMARY MOLARS (유구치 치근분지부 부근관의 빈도와 개통성에 관한 연구)

  • Deep caries in primary molars without early intervention frequently induce a pulpal disease and consequent abscess lesion in surrounding tissue. Especially in many furcation radiolucent lesions of primary molars, accessory canals in furcation areas of primary molars play an important role in theses pathological progress. Although the studies of these communication canals in furcation area of human permanent molar well documented, there has been little research showing their existence in primary molar. For the purpose of understanding the morphological characteristics of accessory canals at furcation areas of primary molars and examining its frequency and patency, 52 specimen teeth were observed under the scanning electro-microscope and tested with dye penetration under vacuum pressure, and obtained the results as follows : 1. In SEM study, the frequency of accessory foramina viewed from the external and internal furcation area was 60% and 35% respectively. Likewise the diameter of the accessory canals was within the range of $11{\sim}107{\mu}m$ and $12{\sim}62{\mu}m$ respectively. 2. The frequency of patent accessory canals evidenced by the dye penetrated under the vacuum suction at furca areas in primary molars was 50% among the 20 specimen teeth tested.

  • Deep caries in primary molars without early intervention frequently induce a pulpal disease and consequent furcation lesion with fistulous openings Up to now, majority of the textbooks on pediatric dentistry and literatures have described that extraction of the inflicted teeth is indicated for these cases and in reality these teeth have usually been extracted in the dental clinics. However when we recognize the excellent capacity of bone regeneration in children and the presence of numerous accessory canals at furcation areas, the removal of infection source in pulp by pulpectomy and inflammatory granulation tissues at furcation areas by furcal curettage might open the possibility of rapid healing at the furcation regions. In this report, 10 cases of primary molars in 3 to 6-year-old children with fistulous openings and furcation lesions in moderate size of 2 to 4mm in depth radiolucency at furcation lesion have been chosen. After pulpectomy and furcal curettage, evident bone regeneration was detected radiographically in all cases. Through the cases, we came to realize that all the cases previously described are not the indications of extraction and this approach could make many cases with pulp and furca combined lesions survive and remain healthy in the children's dental arches. However, in order for this approach to acquire objective appropriateness, it is thought that more scrupulous evaluation is desirable on the various factors regarding the indication such as the extent of furcation lesions, absorption status of teeth, amount of covering bone on succeeding teeth and so on.

Healing Following Transplantation of Periodontitis-Affected Roots with Calcium Sulfate in the Extraction Socket (Calcium Sulfate가 성견 발치와내에서 치주질환이환 치근이식과 치조골 재생에 미치는 영향)

  • 이 연구의 목적은 치주질환에 이환된 성견의 치근을 활택술 후 발치와에 이식하였을 때 calcium sulfate 골이식이 치근흡수와 치근유착을 지연시키는가를 알아보고자 하는 것이다.성견의 제2, 제3 소구치 주위의 치조골을 제거하여 분지부를 노출시키고, 8주간 교정사를 결찰하여 실험적 만성치주염을 유발하였다. 치관을 제거하고, 치근을 반분하고, 활택한 후 발치하였다. 발치된 치근을 둘로 나누어 각각을 우측에는 calcium sulfate와 함께, 좌측에는 이식재 없이 이식하였다. 12주간의 치유 후 조직학적으로 관찰하였다. 대조군에서는 흠의 치관쪽으로 치근흡수와 치근유착이 일어났고, 실험군에서는 성견 결합조직과 calcium sulfate가 위치하였던 공간이 관찰 되었고 치근흡수는 지연되었다. 이 연구에 나타난 바에 의하면, 치근 이식시 calcium sulfate 의 사용은 치조골로부터의 육아조직을 배제하고 치근흡수와 치근유착을 지연할 수 있는 것으로 생각된다.

IMMUNOHISTOCHEMICAL STUDY ON THE DISTRIBUTION OF NERVES IN THE PERIODONTAL LIGAMENT OF A DOG'S PRIMARY TEETH (유치 치주인대 신경분포에 관한 면역조직화학적 연구)

  • The purpose of this study was to investigate the distribution of nerves in the periodontal ligament of a dog's primary teeth by each developing stage. The distribution of nerves in the periodontal ligament were investigated by means of immunohistochemistry for detection of neurofilament protein (NFP). The results were as follows: The NFP-immunoreactive nerve fibers were found to be densely distributed in the apical third of the periodontal ligament, while they were sparse in the coronal two third, in both primary and permanent teeth. In generally the density of distribution and degree of arborization of nerve fibers in periodontal ligament of primary teeth revealed a poor appearance compared with that of permanent teeth. Periodontal ligament in anterior teeth showed more abundant nerve innervation than posterior teeth, and the periodontal ligament of the bifurcation area in posterior teeth roots were not observed to have nerve fiber. The density of nerve distribution in the periodontal ligament of primary teeth was reduced according to the physiological root resorption and nerve fibers were not observed in the surrounding area on the root of the exfoliation stage in primary teeth. The distribution of nerve fibers in mucogingival tissue, was poor innervated according to the aging of the dogs. A more abundant distribution of nerve fiber was represented in the lingual mucogingival tissue than in the labial side. Most of the nerve endings in the periodontal ligament of primary teeth showed a tree-like appearance. However, the typical Ruffini-like nerve endings were not observed.

  • The purpose of this study was to examine the tissue response to experimental furcation perforations immediately treated with Super EBA, Ketac Silver, MTA and Emdogain using surgical microscope. Forty experimental furcation perforations were created in the mandibular and maxillary premolars and molars of 4 adult dogs and immediately repaired with experimental materials. The animals were sacrificed after 16 weeks and radiographic and histologic results were evaluated. The results were as follows. 1 All materials tested in this experiment revealed a certain degree of extrusion of the filling materials and infiltration of inflammatory cells into the periodontal space. Except MTA group, epithelial down-growth of the surrounding gingiva was found in all experimental groups. 2. Both Ketac Silver and Emdogain group showed the greatest degree of inflammatory reaction and bone resorption. 3. Super EBA group showed moderate inflammation and newly bone formation under the perforation area. 4. MTA group showed minor inflammation, new bone regeneration toward restorative materials and partially cementum growth onto the surface of the material. This group demonstrated a favorable prognosis.

  • The purpose of this study was to compare the force, the displacement and the stress distribution on the maxillary first molars altered by the application of various asymmetric head-gear. For this study, the finite element models of unilateral Cl II maxillary dental arch was made. Also, the finite element models of asymmetric face-bow was made. Three types of asymmetric face-bow were made : each of the right side 15mm, 25mm and 35mm shorter than the left side. We compared the forces, the displacement and the distribution of stress that were generated by application of various asymmetric head-gear, The results were as follows. 1. The total forces that both maxillary first molars received were similar in all groups. But the forces that mesially positioned tooth received were increased as the length of the outer-bow shortened, and the forces that normally positioned tooth received were decreased as the length of the outer-bow shortened. 2. In lateral force comparison, the buccal forces that normally positioned tooth received were increased as the length of the outer-bow shortened, and the buccal fortes that mesially positioned tooth received were decreased as the length of the outer-bow shortened. Though the net lateral force moved to the buccal side of normally positioned tooth as the length of the outer-bow shortened, both maxillary first molars received the buccal force. That showed 'Avchiai Expansion Effect' 3. The distal forces, the extrusion forces and the magnitudes of the crown distal tipping that mesially positioned tooth received were increased as the length of the outer-bow shortened, and the forces that normally positioned tooth received were decreased as the length of the outer-bow was shortened. 4. The magnitude of the distal-in rotation that normally positioned tooth received were increased as the length of the outer-bow was shortened. But, mesially positioned tooth show two different results. For the outer-bow 15mm shortened, mesially positioned tooth showed the distal-in rotation, hut for the outer-bow 25mm and 35mn shortened, mesially positioned tooth showed the distal-out rotation. Thus, the turning point exists between 15mm and 25mm. 5. This study of the initial stress distribution of the periodontal ligament at slightly inferior of the furcation area revealed that the compressive stress in the distobuccal root of the normally positioned tooth moved from the palatal side to the distal side and the buccal side successively as the length of the outer-bow shortened. 6. This study of the initial stress distribution of the periodontal ligament at slightly inferior of the furcation area revealed that the magnitudes of stress were altered but the total stress distributions were not altered in the mesiobuccal root and the palatal root of normally positioned tooth, and also three roots of mesially positioned tooth as the length of the outer-bow shortened.

Experimental study on the effect of direct microcurrent to periodontal regeneration in class III furcation defects (3급 치근분지부 골결손에서 미세전류자극이 치주조직의 재생에 미치는 영향에 관한 실험적 연구)

  • Several experimental studies showed that the application of small amounts of electric current to bone stimulated osteogenesis at the site of the cathode and suggests that the application of electrical currents to periodontal defects could promote bone and cementum formation. The purpose of this study was to determine the effect of direct microcurrent to the periodontal regeneration of class III furcation defects in dogs. Class III furcation defects were surgically created on the third and the fourth premolars bilaterally in the mandibles of nine mongrel dogs. Experimental periodontitis were induced by placing small cotton pellets into the created defects for 3 weeks. The experimental sites were divided into three groups according to the treatment modalities: Group I-surgical debridement only; Group II-allogenic demineralized freeze dried bone grafting; Group III-allogenic demineralized freeze dried bone grafting and electrical stimulation. For fluorescence microscopic evaluation, calcein, oxytetracycline HCI and alizarin red were injected 2, 4 and 8 weeksfS days prior to sacrifice) after surgery. The animals were sacrificed in the 1st, 2nd, 4th and 8th week after periodontal surgery and the decalcified and undecalcified specimens were prepared for histological and histometrical examination. After the first and the second weeks, gingival recession was more severe in group I than groups II and III. After the fourth and the eighth weeks, there was no difference in the width of junctional epithelium and connective tissue attachment among the three groups, but the width of connective tissue attachment increased in group II at the eighth week, compared to the fourth week. The amount of bone repair in new attachment was significantly greater in group III, compared to groups I and II. New attachment formation was significantly greater in group III, compared to groups I and group II. These results suggest that electrical stimulation using microcurrent generator could be a useful tool for periodontal regenerative therapy in class III furcation defect.
