목적: 본 연구의 목적은 적층 가공(additive manufacturing)법, 절삭 가공(subtractive manufacturing)법, 전통적인 방법에 따른 임시 수복용 레진의 마모 저항성을 조사하는 것이다. 재료 및 방법: 제작방법에 따라 4개의 군으로 나누었으며, 각 군은 전용의 임시 수복용 레진을 사용하였다: S3P군, Stereolithography apparatus (SLA) 3D 프린터 및 전용의 광경화성 수지로 제작한 군; D3P군, Digital Light Processing (DLP) 3D 프린터 및 전용의 광경화성 수지로 제작한 군; MIL군, Milling machine 및 밀링용 레진 블록으로 제작한 군; CON군, 전통적인 방법 및 자가중합형 레진으로 제작한 군. 한편, 3D 프린팅된 레진 시편을 제작함에 있어 적층 각도와 층 두께를 각각 $0^{\circ}$와 $100{\mu}m$로 설정하였다. 구강내 환경을 재현하기 위하여 열순환 처리와 수평, 수직운동이 가능한 2축 chewing simulator를 사용하였으며, 하부에는 한쪽면이 편평하게 제작된 임시 수복용 레진을, 상부에는 끝이 3 mm 직경을 가지는 원뿔형의 steatite를 고정하여 마모시험 진행하였다(5 kg, 30,000회, 0.8 Hz, $5^{\circ}C/55^{\circ}C$). 임시 수복용 레진의 마모량은 마모 전후의 Standard Triangulated Language (STL) 파일과 전용의 CAD software를 이용하여 부피를 계산하였고, 주사전자현미경으로 마모 양상을 비교하였다. 결과: S3P군, D3P군, MIL군의 마모량은 CON군보다 유의하게 작았으며 (P < .05), S3P군, D3P군, MIL군 사이에는 통계학적으로 유의한 차이가 없었다 (P > .05). 주사전자현미경으로 마모면을 관찰한 결과, S3P군과 D3P군에서는 대합치의 운동 방향에 대해 수직적으로 갈라진 흔적이 발견되었다. MIL군에서는 전반적으로 균일한 마모면이 보인 반면, CON군에서는 대합치 운동 방향으로의 뚜렷한 마모 흔적과 다수의 기포가 관찰되었다. 결론: 본 연구의 한계 내에서, 3D 프린팅된 임시 수복용 레진은 치과용으로서 적절한 마모저항성을 보였다.
우리나라 치과의 CBCT 보급률은 세계적으로 높은 편으로, 최근 1만대 보급을 돌파하고 있는 것으로 파악된다. 또한 최근 각 치과 제조업체마다 DLP방식의 In-House방식의 3D 프린터 보급률 또한 가파르게 상승하고 있다. 이에 맞춰 최근 개원가에서 CBCT와 구강 스캔 정보를 이용한 컴퓨터 가이드 임플란트 수술의 활용도가 증가하고 있는 상황이다. 현재 컴퓨터 가이드를 이용한 임플란트 수술 방법에 대해 문헌상 고찰을 통해 리뷰해보고, 또한 정확성 및 신뢰성이 보장되는 가이드 수술을 위한 고려사항에 대해 정리해보고자 한다.
Purpose: This study aimed to confirm the applicability of gloss polishing using automatic barrel finishing with respect to three-dimensional (3D)-printed resin specimens. The surface roughness and grinding angle of the 3D-printed resin specimens were observed with respect to gloss polishing time using automatic dental barrel finishing. Methods: Herein, experiments were conducted on four types of 3D-printed resin specimens. The specimens, with a thickness of 100 ㎛ each, were printed using a 3D printer. Subsequently, light polymerization was performed on these specimens for 15 min. Post this surface treatment, the specimens underwent grinding for 25 min. This process was followed by gloss polishing at 5-min intervals for up to 25 min using automatic dental barrel finishing. The specimens were photographed using a 3D optical microscope, and their surface roughness and grinding angle were measured. Results: The Ra (centerline average roughness) values of all the specimens, except for crown & bridge 10 group and those in the control group that were not polished using automatic barrel finishing, were <0.2 ㎛. However, polishing time needs to be controlled to realize the desired surface roughness and grinding amount considering the hardness of the resin used. Conclusion: Gloss polishing of 3D-printed resin can be realized using automatic dental barrel finishing. Objectives: The purpose of this study is to assess the accuracy of provisional restorative resins fabricated using dental three-dimensional (3D) printers. Methods: Provisional restorative resins were fabricated using the first molar of the right mandibular. Three groups comprising a total of 24 samples of such resins were fabricated. The prepared abutment was scanned initially and then designed using a computer-aided design (CAD) software. The conventional subtractive manufacturing system was employed to fabricate the first group of resins, while the second and third groups were fabricated using a digital light processing (DLP) 3D printer and a stereolithography (SLA) 3D printer, respectively. The internal surfaces of the resins were scanned and 3D measurements of the resins were taken to confirm their accuracy. Results: The root-mean-square deviation (RMS±SD) of the accuracy of the resins fabricated using the conventional subtractive manufacturing system, DLP 3D printer, and SLA 3D printer were 68.83±2.22 ㎛, 74.63±6.23 ㎛, and 61.74±4.09 ㎛, respectively. A one-way analysis of variance (ANOVA) test showed significant differences between the three groups (p < 0.05). Conclusions: Provisional restorative resins fabricated using DLP and SLA 3D printers demonstrated clinically-acceptable results.
Purpose: The purpose of this in vitro study was to measure and compare the thickness-dependent color dimensions of digital light processing (DLP) three-dimensional (3D) printer and conventional interim restorative resin. Methods: Specimens (N=60) were fabricated using either subtractive manufacturing (S group) or DLP 3D printing (D group) material. All milled and 3D-printed specimens were allocated into three different groups (n=10) according to different thicknesses as follows: 1.0, 1.5, and 2.0 mm. Color measurements in the CIELab coordinates were made using a spectrophotometer under room light conditions (1,003 lux). The color differences (𝚫E*) between the specimen and control target data were calculated. Data were analyzed using the oneway analysis of variance (ANOVA). Post hoc comparisons were conducted using Tukey's honestly significant difference method (α=0.05 for all tests). Results: The 𝚫L*, 𝚫a*, 𝚫b*, and 𝚫E* values of interim restorative resin produced by DLP 3D printing were obtained in terms of the specimen's thickness increased compared with the increases by subtractive manufacturing. When the thickness was similar, the color difference between subtractive manufacturing and DLP 3D printing was ≥5.5, which is a value required by the dentist for remanufacturing. Conclusion: Color was influenced by the thickness of the interim restorative resin produced by DLP 3D printing.
Objectives: The present study aimed to compare the accuracy of removable partial denture (RPD) frameworks fabricated by selective laser sintering (SLS) before and after electropolishing. Methods: A partially edentulous mandibular model was used as the working model. Scanning of the model was performed using a dental scanner. The framework was designed using CAD software. The metal framework was formed using an SLS 3D printer. 3D scans of the two fabricated prototypes produced before and after electropolishing were overlapped with reference data. The fit was calculated based on Root Mean Square (RMS). Fabrication accuracy was verified using the paired t-test to compare the discrepancy before and after electropolishing. Results: The mean (SD) values of RMS before and after electropolishing were 126.6 (34.19) and 75.86 (21.36), respectively. There was a statistically significant difference before and after electropolishing (p<0.05). Conclusions: Metal frameworks made with SLS 3D printers showed clinically acceptable fit after electropolishing.
적절하지 않은 위치에 식립된 임플란트의 보철물은 심미적, 기능적 문제를 나타낼 수 있으나 이를 해결하기는 어려운 때가 많다. 이러한 경우에 재식립 혹은 연조직 및 경조직 이식을 포함하는 외과적 방법을 고려해 볼 수 있지만 결과 예측이 힘들며 긴 치유 기간 및 술 후 불편감과 추가적인 수술 비용이 동반된다. 반면, 보철적 방법으로는 치은 색상의 세라믹이 포함된 고정성 보철물을 제작하거나 가철성 보철물인 gingival mask 제작 등을 생각해볼 수있다. 본 증례에서는 80세의 남환이 과도하게 구개측으로 식립된 임플란트로 인한 상순과 보철물 사이의 음식물 저류에 대한 불편감 및 심미적인 부분에 대한 불만족을 주소로 내원하였고, gingival mask를 제작하여 단 기간 내에 만족스러운 결과를 얻었기에 이를 보고하고자 한다.
Objectives: This study aimed to understand the definitions, types, and principles of computer-aided design/computer-aided manufacturing (CAD/CAM) and scanners due to the introduction of digital workflows. Methods: This study was based on information from the government's law and articles published in academic journals. Results: CAD/CAM is a technology that measures the shape three-dimensionally, saves it as data, designs it into the desired shape, and processes the product. Scanners, which are classified as intraoral and extraoral scanners, measure teeth and the intraoral environment three-dimensionally and convert them into three-dimensional (3D). A 3D printer is a machine that creates a 3D object by layering materials based on a 3D drawing. It can be classified into four types according to the method: extrusion, powder bonding, lamination, and photopolymerization methods. The most used 3D printer methods in dentistry are stereolithograhpy and digital light processing, and they are widely used in prosthetic, surgical, and orthodontic fields. Conclusions: As the dental system is digitized, it is expected that the government will classify the dental hygienist scope of work and the universities will reflect the curriculum; it is necessary to develop excellent dental hygienists, diversify the educational pathways, and establish policies to meet the needs of the increasing number of patients.
총의치는 무치악 환자의 치료에 있어 가장 기본적인 치료방법 중 하나이다. 전통적인 총의치의 제작과정은 예비인상 채득, 최종 인상 채득, 악간관계 채득, 납의치 시적, 의치 장착의 단계를 거치게 된다. 여러 번의 내원 과정과 복잡한 기공 과정이 필요하며, 각 단계별로 오차가 발생할 수 있다. 최근 디지털 기술이 발전함에 따라 총의치 치료과정에도 디지털 기술이 도입되고 있으며, CAD를 이용해 더 정밀한 작업이 가능하며 기공 시간을 단축시키는 것을 물론 환자의 내원 횟수도 줄일 수 있다. 본 증례에서는 디지털 방식을 활용하여 환자의 기존 의치의 전치 배열을 최종 의치로 이행하였다. 3D 프린터를 이용해 시적 의치를 제작하여 환자에게 시적하여 구순 지지도와 전치부 배열을 포함한 심미성 및 구강내 유지력을 평가 후 최종 의치 제작하였다. Computer-Aided Design/Computer-Aided Manufacturing 기술을 활용하여 내원 횟수를 줄이는 동시에, 만족할 만한 의치의 유지력 및 안정성을 얻었으며 심미적인 회복을 이루었기에 본 증례를 보고하고자 한다.
이 연구의 목적은 3D 프린팅 레진의 연마 방법에 따른 색조 안정성과 표면 조도를 시간에 따라 비교하는 것이다. 3D 프린터와 TC-80DP 레진을 이용하여 3D 프린팅 레진 시편을 제작하였고, 각 시편은 연마하지 않은 군, Enhance®군, Sof-LexTM군으로 분류되었다. 각 군은 연마 직후 기준점을 측정하고, 다시 인공 타액과 오렌지 주스에 나누어 침전시킨 뒤, 침전 1, 7, 30, 60일에 CIE L*a*b* 값의 색차(ΔE*)와 각 시점의 표면 조도가 측정되었다. 연구 결과, 60일 침전 후 인공 타액에서는 육안으로 확인 가능한 정도의 색조 변화는 일어나지 않았으나 오렌지 주스에서는 육안으로 확인 가능한 색조 변화가 나타났고, 용액에 따른 차이는 유의미했다. 표면 조도는 Sof-LexTM군에서만 오렌지 주스가 인공 타액보다 유의하게 높았다. 연마 방법에 따른 색조 변화의 차이는 없었으나 표면 조도는 Sof-LexTM군이 연마하지 않은 군과 Enhance®군보다 유의하게 낮았으며, 모든 군에서 치태 침착 임계 조도보다는 높았지만, 임상적으로 허용 가능한 정도였다. 이번 연구 결과, 유전치 전장관 수복을 위해 영구 수복용 3D 프린팅 레진 사용 시 착색제 섭취에 유의해야 하며, SofLexTM를 이용하여 연마하는 것이 권장된다.
