Comparative evaluation of two sets of complete dentures fabricated by digital and conventional workflow in a single patient: A case report (단일 환자에서 디지털 및 전통적인 워크플로우로 제작한 두 쌍의 총의치를 장착하여 비교평가한 증례 보고)

  • Application of digital technology in dental treatments can simplify the treatment process and reduce patient discomfort. In regards of digital complete dentures, several commercial systems are available. However, these systems are not being routinely applied in Korea. In this case report, comparative analyses were performed regarding the two sets of complete dentures fabricated in digital and conventional workflow in a single patient. Complete dentures made with digital workflow were able to form an ideal occlusion because customized teeth were used. Compared to that, there were also limitations such as lower retention of mandibular dentures, so we would like to report this.

  • The safety and success of dental sedation for children depend mainly on respiratory status of patients. A special condition, that is, nasal breathing in supine position with their oral airway blocked by rubber dam, should be considered. Therefore, irrespective of medical consultation, pediatric dentists themselves should do respiratory assessment especially adenotonsillar hypertrophy, nasal obstruction, posterior nasal drainage and airway hypersensitivity. Patients with sinusitis, allergic rhinitis, asthma, snoring and OSAS(obstructive sleep apnea syndrome) can induce the sedation failure and complete management of these can improve the safety of dental sedation.

  • Implant-assisted removable partial denture (IAPRD) can be considered as a simple and cost-effective treatment approach for an edentulous patient with anatomical or financial limitations. Recently, it was reported that the application of IARPD with implant supported fixed prostheses covered by the National Health Insurance Service (NHIS) were increasing. This case report describes the treatment of maxillary fully edentulous patient with anterior four-implant-supported fixed prosthesis and distal extension IARPD. This treatment approach may be advantageous over maxillary implant overdentures in some circumstances. The patient was satisfied with improved function and esthetics in the anterior area and financial benefit from the NHIS. Further long-term clinical studies are needed to establish clinical validity of the treatment approach described in this case report.

  • Special Health Care Needs (SHCN) patients need regular follow-up because of high incidence and severity of oral disease. The aim of this study was to evaluate the dental treatment outcomes of SHCN patients according to follow-up patterns. SHCN patients who were treated under general anesthesia (GA) at Kyung Hee University Dental Hospital from 2006 to 2014 were included in this study. The final samples comprised of 53 patients that were divided into regular (33 patients) and irregular (20 patients) follow-up groups according to their follow-up patterns. The type of dental treatment after GA during the follow-up periods were compared. In the irregular group, aggressive treatment including endodontic, prosthetic treatment, and extraction were predominant, compared with the regular group (p < 0.05). In addition, all patients who had dental treatment under GA in follow-up periods were in the irregular group (p < 0.05). In conclusion, the results of this study provide the importance of regular follow-ups with SHCN patients and emphasize responsibilities of dentists for educating patients and their guardians.

Trend Analysis and Prediction of the Number of Births and the Number of Outpatients using Time Series Analysis (시계열 분석을 통한 출생아 수와 소아치과 내원 환자 수 추세 분석 및 예측)

  • The purpose of this study was to analyze the trend of the number of births in Gwangju and the number of outpatients in Pediatric Dentistry at Chonnam National University Dental Hospital over the past 10 years (2010 - 2019) and predict the next year using time series analysis. The number of births showed an unstable downward trend with monthly variations, with the highest in January and the lowest in December. The average number of births in 2020 was predicted to be 682 (595 to 782, 95% CI), and the actual number of births was an average of 610. The number of outpatients was relatively stable, showing a month-to-month variation, with highest in August and the lowest in June. The average number of patients in 2020 was predicted to be 603 (505 to 701, 95% CI), and the average number of actual visits was 587. Despite the decrease in the number of births, the number of outpatients was expected to increase somewhat. Due to the special situation of COVID-19, the actual number of births and patients was to be slightly lower than the predicted values, but it was that they were within the predicted confidence interval. Time series analysis can be used as a basic tool to prepare for the low fertility era in the field of pediatric dentistry.

  • In patients with myasthenia gravis, it is difficult to manufacture and maintain dentures because the muscles and nervous system in the oral cavity are not properly adjusted. In addition, excessively extended or thick dentures may cause muscle weakness by stimulating the muscles, so dentures should be manufactured in consideration of this. In this case, a denture was fabricated using the closed mouth technique in a patient with myasthenia gravis with mouth opening limitation. Using the closed mouth technique, the patient's masticatory pressure and muscle movements were reproduced and denture with good retention were manufactured.

  • As the number of elderly population increases, the proportion of edentulous patients is increasing, and the demand for traditional dentures will also increase. Arranging the artificial teeth according to the relationship and shape of the upper and lower alveolar ridge is an important factor increasing the stability of the denture. This case is a 79 year old female patient who wishes to make denture covered by national health insurance for psychological and economical reasons. With the alveolar bone severely atrophic, the dental CAD software was used for accurate diagnosis and the posterior teeth were arranged in a cross bite according to the interalveolar crest line angle by the program. After completion, the denture showed adequate stability and maintenance in periodic examinations and treatment, and the patient also expressed high satisfaction.

Impact of Dentists' Attitudes and Dental Hygienists' Services on Dental Anxiety (치과의사의 태도와 치과위생사의 서비스가 치과불안에 미치는 영향)

  • The purpose of this study was to investigate the factors affecting dentists' attitudes and dental hygienists' services on dental anxiety in adults. The subjects were 300 adults older than 20 years of age living in Seoul, Gyeonggi, Daejeon, and Daegu. Data were collected using structured questionnaires. Among the distributed questionnaires, 225 respondents were selected as subjects, excluding 74 people who did not answer and 1 person who was not faithful. Data were analyzed using statistical software with a t-test, one-way ANOVA, and multiple regression. As a result, the gender was slightly higher in women (54.7%) than in men, and the last dental visit was less than one year in 59.6% of respondents. Most of the respondents' educational level was higher than college level (79.1%), and the monthly income was less than 2 million won in 53.8 of respondents. This study showed that distrust of dentists affected dental anxiety and anxiety stimulation. Higher reliability of the dentist was correlated with less dental anxiety in patients. Dental anxiety showed statistically significant results in dentist subcategories of patient slight and dentists' trust (p<0.01). Additionally, the factors affecting dental anxiety and anxiety stimulus were knowledge of dental hygienist and distrust of dentist (p<0.01). According to this study, dentists' and dental hygienists' trust of dental staff show the importance of oral health professionals' role in reducing dental anxiety in patients. It is also suggested that efforts should be made to improve public awareness of oral health experts. It is believed that dentists, and dental hygienists need to promoted to become professionals. In addition, a variety of programs have been developed to reduce dental anxiety, so patients need to be comfortable to receive dental treatment.

  • 총의치 교합에는 관계되는 많은 개념, 기술, 철학이 있다. 이 교합의 기본 법칙들을 이해하고, 어떤 system의 교합이 사용되던 간에 이를 현명하게 잘 적용해야할 것이다. 어떤 교합이, 인공치를 어떻게 배열해야만이 환자에게 가장 안전하면서 효율적인가를 알아내 이를 환자에 적용토록 부단히 노력하는 것이 치과의사의 책임일 것이다.

  • 치과 진료에 있어서 무통치료가 요구됨은 재론의 여지가 없으나, 날로 증가하는 성인병환자나 노인환자에서 주의해야 할 사항이 많은 점등이 술자에게 어려움을 주는 것 또한 사실이다. 이에 하치조신경의 정확한 전달마취법, 실패시의 보완법, 그리고 그 후유증 등에 대하여 다시 고찰하여 보는 것도 의미있는 일이 아닐까 사료된다.
