A Study on Temperature Changes during Bone Scaling and Cutting of Dental Ultrasonic Scaling/Surgery System (치과용 초음파 스케일러/수술기 통합 시스템의 스케일링 및 절삭 시 온도 변화에 관한 연구)

  • While dental clinics still use the ultrasonic scaling/surgery tool for teeth scaling and cleaning the tool's use is expanding steadily to include treatment of damaged teeth and bone tissue. In this study, a handpiece moving system (HMS) was developed to evaluate bone scaling and cutting in the field of dentistry. The HMS, through a scaling test of bone using a scaler tip, it was able to identify surface damage. Additionally, a thermos-graphic camera was used to observe the temperature distribution that occurred during the bone scaling and cutting process. Consequently, we found that increasing the working load increased the amount of surface damage. Changes in temperature distribution occurred slowly and were maintained within safety bounds for 10 minutes. Going forward, we will compare the HMS performance on scaling and cutting with other devices.

  • Russell-Silver syndrome is a genetic disorder characterized by intrauterine and/or postnatal growth restriction and typical facies. The clinical feature is various due to heterogeneous genetic characters. Their common findings are short stature without catch-up growth, normal head size for age, a distinctive triangular face with prominent forehead and a pointed chin, low set ears and clinodactyly of the fifth fingers. Intraoral features of the syndrome are microdontia, delayed tooth eruption, hypodontia, and crowding. More than 400 case have been reported in the literature, and estimated incidence is from 1 in 3000 to 1 in 100,000. In this case we performed caries treatment under the general anesthesia for the patient with Russell-Silver syndrome. Dentist have to consider microstomia for the management of patients with Russell-Silver syndrome.

A study on the usage satisfaction of multi-type handpieces with equipped air-jet function (Air-jet기능을 장착한 멀티형 복합유닛 핸드피스의 사용 만족도 조사)

  • Purpose: This study was to investigate usage satisfaction of multi type handpiece with equipped air-jet function Methods: The subjects of this study through the survey was conducted for fifty-nine dental technicians in Daegu and Gyeongsangbukdo from the May 7 to 19, after the June 23 to July 4 2014. The survey was about the function and quality through user testing environment by cutting zirconia specimens using appliance for zirconia. The collected date was analyzed by the statistical program SPSS Win Ver 19.0 for the satisfaction of handpiece and control system. To test for significance on each item, p<0.05 had been decided a standard. General characteristics and relationship between the handpiece and control system was performed correlation analysis. Results: The results of this study is as follows. 35 dental technicians had 1~2 years career in zirconia part as the highest 59.3%, 95% of subjects expected that the prospects for the zirconia material have bright prospects. The satisfaction of multi type handpiece and control system design and function showed a significant difference. Subjects satisfied with the design and function of the multi type showed negative correlation with those of Standard. The satisfaction of multi type handpiece showed 3.37 points of air jet, 3.05 points of noise level, 2.69 points of water flow rate, and 1.98 points of cruise function. Sample differences in all parameters showed a statistically higher difference. Conclusion: High-speed cutting mechanism of multi-type is recommended working with zirconia materials and the multi-type unit combined handpiece motor, air-turbine and air-gun will be expected the increase of user due to the high satisfaction of air jet.

The effect of guided flapless implant procedure on heat generation from implant drilling (수술가이드를 이용한 무피판 임플란트 술식에서 임플란트 드릴링이 열발생에 미치는 효과)

  • Purpose: The aim of this study was to evaluate the heat generation in bone in vitro during the guided flapless drilling procedure and the effect of drilling methods on the heat generation. Materials and methods: A model that has missing the first and second mandibular molars bilaterally was used. In group A, classical flap implant surgery was performed. In group B, flapless implant surgery using surgical guide was performed. In group C, flapless implant surgery using surgical guide without up-and-down pumping motion was performed. Temperature was measured with k-type thermocouple and a real-time digital thermometer. The thermocouples were placed at 0.5 mm away from the osteotomy area at the depths of 3 mm and 6 mm. The measured values were evaluated with independent t-test. Results: The mean temperature generated was $27.2^{\circ}C$ ($SD{\pm}2.1^{\circ}C$) and $27.5^{\circ}C$ ($SD{\pm}2.3^{\circ}C$) for groups A and B, respectively. These differences were not statistically significant. In group C, the mean temperature was $37.0^{\circ}C$ ($SD{\pm}3.4^{\circ}C$). There were statistically significant differences between groups B and C with respect to the mean temperature. Conclusion: These findings suggest that guided flapless drilling with up-and-down pumping motion may not significantly increase the bone temperature.

  • Recentrly, early dental care for children is becoming more important, and the number of children patients' dental services is on the rise. So, a variety of seduction therapies are needed to alleviate the tension in children who are anxious about dental care. This paper describes the development process and contents of system that can display the animation of different stories according to the treatment stage by grasping the use state of the treatment device in real time. In the animation, the hreo character and the villain, the cavity character, appear and the hero defeat the villain at each treatment stage. Animation is played when treatment starts and ends after treatment. Current existing one-side played animation of dental clinics is not enough to attract children's attention. If the flow of animation changes depending on the treatment process, it will give children a different enjoyment and consequently ease the tension over the treatment.

  • The lasers have been used in dentistry for more than 30 years and the application of lasers for drilling dental hard tissue has been investigated since the early developement of lasers. Recently, the Er:YAG laser was invented for hard tissue ablation. The Er:YAG laser, having a wavelength of 2.94um, is highly absorbed in both water and hydroxiapatite, leading to a very effective material for hard tissue removal by bursting off the solid tissue component that is, enamel and dentin are removed by the Er :YAG laser by water vaporization and microexplosion, without any melting of inorganic tissues. Therefore, the Er:YAG laser produced round craters with well defined margins and the surrounding tissues had no cracks and no charring. When used for cavity preparation, pulpal damage should not occur if hear buildup is minimized by careful selection of exposure parameters and by use of a water spray. The present study demonstrated that the Er:YAG laser cut the tooth substance adequately for composite resin restoration, without having undesirable side effects such as harmful effects on the pulp, discoloration or cracking etc. Also, the child patients were well cooperative during laser treatment mainly because of little noise, lesser vibration and minimal pain compared to conventional means of cavity preparation.

The comparative study of user satisfaction on various implant engine system (다양한 임플란트 엔진 시스템에 대한 사용자 만족도 비교)

  • Purpose: Implant engine system is composed of the handpiece, micromotor, control box and foot switch. The aim of this study was to evaluate the satisfaction of the implant engine systems in terms of convenience-design and to examine the relation with the experience of implant surgery. Materials and Methods: Three implant systems were evaluated: SurgicXT/X-SG20L, INTRAsurg300/CL3-09, XIP10/CRB26LX. For this comparative study, 30 dentists were included and the satisfaction was measured using a structured questionnaire. One-way analysis of the variance (ANOVA) and multiple regression analysis were used within and between the groups. Results: Total satisfaction differed from each other (P < 0.05). The convenience score was more associated with the total satisfaction than design score. Moreover, the implant surgery experience affected several assessments. Conclusion: Collectively, in a cross-sectional study model, the design of implant system significantly affects its total satisfaction and the surgery experience can be influential factor in the evaluation of implant engine system.

DC link voltage control method in the sinusoidal current drive system for dental hand-piece PMSM (치과 핸드피스용 고속 PMSM의 정현파 구동을 위한 인버터 직류 링크전압 제어기법)

  • This paper presents a DC link voltage control method to reduce the ripple current and the switching loss in the sinusoidal current drive system for the wide-speed range PMSM. The DC link voltage of the three phase inverter in the sinusoidal current drive system is designed by the back-EMF voltage at maximum speed of the PMSM. In general, the drive systems have used the constant DC link voltage without reference to the motor speed. The current ripple causes hysteresis loss and makes noise. In addition, the switching loss on the inverter increases in proportion to the rise in the DC link voltage. In this paper, we propose the variable DC link voltage control method to reduce the current ripple in the PMSM drive system. We show reduction effect of the current repple and the switching loss through simulation results.

Strength Analyses of New 2- and 3-Axis-Type Small Multiplying Gears in Dental Hand-Pieces (치과드릴 구동용 신 소형 2축 및 3축형 증속기어 강도특성 비교)

  • Two types of very small multiplying gears and arrays have been developed for new dental hand-pieces, and the increased speed ratios, modules, number of teeth, gear diameters, and gear types were calculated based on the dynamics of the machinery. The contacting and bending strengths were evaluated for gear teeth with two design concepts using AGMA equations and finite element analyses, and the contacting stresses on teeth with and without DLC (diamond-like-carbon) coating layers were calculated. Fatigue and tension tests were performed to obtain an S-N curve, the Young's modulus, and the strength of the gear material, and these were utilized in the analyses. Slightly larger stresses were found for 2-axis-type gears than for other types of gears, and the S-N curves showed that a gear lifetime of 109 cycles was satisfied. The contacting stresses in gears coated with DLC were reduced by 30%. A new prototype model of a hand-piece with small gears was successfully fabricated and tested.

Syncope and pneumomediastinum during the maxillary sinus elevation with an air-syringe: a case report (주사기를 이용한 상악동 거상술 시 발생한 실신 및 종격동기종에 관한 증례보고)

  • Pneumomediastinum is a very rare and potentially catastrophic complication of dental procedures. Its common causes are tooth extraction, endodontic treatment, and subgingival curettage using handpieces and high-pressure air/water syringes. We present a case of massive pneumomediastinum with subcutaneous emphysema in a 61-year-old female who underwent bone grafting into the maxilla for pretreatment of dental implantation using a syringe. The patient suffered from abrupt severe odynophagia and loss of consciousness. The patient transferred to emergency department and images work-up revealed a pneumomediastinum and subcutaneous emphysema on the entire face and neck. We performed conservative treatments including prophylactic antibiotics, oxygen inhalation, and fasting meals, and then discharge after 7 days uneventfully. The patient's syncope might be resulting from hypotension and pain shock induced by pneumomediastinum with a sudden chest compression. The pneumomediastinum could be resulting from concurrent perforation and massive air infiltration into the maxillary sinus during bone grafting. We suggest that pneumomediastinum needs prompt diagnosis and management because of the risk of airway obstruction when a patient present syncope in the dental room.
