  • 새 천년을 여는 이 시대의 화두는 인터넷과 디지털이라 해도 과언이 아닐 것이다. 이에 발맞추어 디지털을 치과 임상에 적용하고자 하는 많은 시도들이 있으며, 특히 치과 방사선 분야에서 다양한 결과들을 실제 임상에서 많이 이용하고 있다. 또한 많은 임상의들이 디지털 카메라를 이용하여 임상사진을 체득하고 활용하려는 시도들을 많이 하고 이용하고 있으나, 디지털 카메라에 대한 정확한 이해 없이 일반 광학 카메라와 동일시 하는 생각으로 수많은 시행착오를 겪고 결국 애써 장만한 디지털 카메라를 사장시키거나 제대로 이용하지 못하는 경우들을 주위에서 많이 보아왔다. 필자도 처음 디지털 카메라를 장만하였을 때 많은 실행착오와 좌절을 경험했지만, 그 동안 수집한 정보를 비교 분석해서 얻어낸 결과와 실제 촬영을 통해 완성시킨 다양한 테크닉을 중심으로 이 글을 서술한다. 디지털 카메라를 소장하고 있거나 구입 의향이 있는 분들에게 유용한 정보가 되기를 바라면 6월과 7월 2회에 걸쳐 본 글을 게재한다.

Case report - An esthetic restoration treatment based on digital mockup (Case report - Digital Mockup을 활용한 심미 수복 치료 증례보고)

  • Digital dentistry is becoming a mainstream of dental practice. We are already used to intraoral scan, guided implant surgery, manufacture of prosthesis (CAD-CAM) in our daily practice; those are not sensational anymore. However, treatment planning with digital simulations is not spotlighted enough. In this report, a digital mockup was utilized to visualize the final goal of maxillary rehabilitation including all anterior and posterior teeth, and persuade a patient who wanted restoration of only anterior teeth. We will also discuss the use of digital mockup and its actual copy as the guide of altered vertical dimension through the clinical process.

  • The inflow of digital systems into the dental field has had a great impact on dentists and dental technicians. and it brought improved accuracy, convenience, and work efficiency than before. However, digital doesn't solve everything, and it certainly has limitations. Therefore, it is recommended that clinicians clearly understand the strengths and weaknesses of the traditional method and the digital method and design their own workflow that harmonizes the two methods. In this part, I introduce how I use and apply 3D printers in clinical field.

A case report of single crown restoration using an intraoral scanner for occlusal evaluation (단일 치관 수복 시 구강스캐너를 이용한 교합평가 활용 증례보고)

  • The evaluation of occlusion using digital methods is easier and simpler in terms of recording, comparison, analysis, and objectivity compared to existing methods such as articulating paper and occlusion foil. The purpose of this case report was utilizing the digital method for evaluating occlusion. The occlusion of patient requiring full veneer crown restoration was evaluated using an intraoral scanner (i500, Medit, Seoul, Korea) at every visit. The occlusion was also assessed using conventional articulating paper and a digital occlusal analysis system (Dental prescale II, GC corp., Tokyo, Japan) for comparison. Throughout the treatment process, the intraoral scanner and the conventional articulating paper method showed similar outcomes. The results suggest that the use of digital evaluation system is highly probable in the near future.

  • Application of digital technology in dental treatments can simplify the treatment process and reduce patient discomfort. In regards of digital complete dentures, several commercial systems are available. However, these systems are not being routinely applied in Korea. In this case report, comparative analyses were performed regarding the two sets of complete dentures fabricated in digital and conventional workflow in a single patient. Complete dentures made with digital workflow were able to form an ideal occlusion because customized teeth were used. Compared to that, there were also limitations such as lower retention of mandibular dentures, so we would like to report this.

All-on-6 implant fixed prosthesis restoration with full-digital system on edentulous patient: A case report (무치악 환자에서 완전 디지털 시스템을 활용한 All-on-6 임플란트 고정성 보철물 수복 증례)

  • All-on-six concept can be used as one of the treatment options to maximize the use of available residual alveolar bone for implant-supported fixed prosthesis on edentulous patients. But this process is complex and cumbersome. Digital system can be used at multiple steps, from implantation to prosthetic restoration, to overcome this shortcoming. In this case of a maxillary edentulous patient aged 76, digital system was used for restoration of 1-piece design, screw retained fixed prosthesis from diagnosis, implant surgery to fabrication of provisional and final prosthesis. For preoperative diagnosis and treatment planning stage, intra-oral information of a patient was digitalized by direct intra-oral scan. Surgical guide and immediate provisional prosthesis was designed based on this digitalized data. Patient's inconvenience was minimized by applying immediate provisional prosthesis, which was delicately fabricated according to the location data of six implants on most suitable residual alveolar bone. Then, final prosthesis was designed and fabricated going through new interim prosthesis which was newly designed and fabricated, considering patient's requests, stable vertical dimension and occlusion, and esthetic factors using digital system. We hereby report a case successfully applying digital system to multiple steps including implant surgery to fabricating prosthesis, to simplify existing complicated implant treatment procedure to an edentulous patient.
