TRAUMATIC BONE CYST : A CASE REPORT (외상성 골낭의 치험례)

  • Traumatic bone cyst is a nonodontogenic cyst without epithelial-linig which contains fluid in it's cavity, and it is limited by bone walls with no evidence of infection. Traumatic bone cyst is asymptomatic and appears more frequently in the second decade. Gender distribution is approximately equal, although males are affected slightly often than females. Radiographically the lesion shows a well demarcated radiolucent lesion of variable size and the lesion may have scalloped margins. The adjacent teeth to traumatic bone cyst remains vital. Traumatic bone cyst is usually treated by surgical exploration and currettage of the lesion. In the first case of this case report, the patient was refered from the local dental clinic for the radiolucent area under the left mandibular first molar. From the panorama radiograph at the first visit, the radiolucent area of the left mandible showed a well defined scalloped margin and identified as traumatic bone cyst. In the second case, the patient have visited for the chief complaint of swelling and abcess of right maxillary second premolar. In the radiographic check up with panorama radiograph, the radiolucent lesion with well demarcated scalloped margin was found in the right mandible body, and identified as traumatic bone cyst. In the first case, overinstrumentation was done through the mesial root canal to irrigate the lesion. In the second case, not any treatment was done, and watched the progression of the lesion. And in both cases, after two month, the radiolucency and the size of the lesion has decreased to show healing in progress.

An Analysis of the Factors Affecting Job Satisfaction in Clinical Dental Hygienists: Focusing on Schoolwork and General (임상 치과위생사의 직무만족도 영향요인 분석: 학업병행과 일반을 중심으로)

  • The purpose of this study was to identify the factors affecting job satisfaction in dental hygienists with and without schoolwork, who form professional manpower in dental clinics, and provide basic data that could help develop detailed plans for reducing the turnover intention and improving job satisfaction. For data collection, convenience sampling was performed among clinical dental hygienists at dental clinics in Daejeon and Chungcheong and Gyeongsang Provinces from July 20 to September 20, 2019, and a self-administered questionnaire was used and the data were analyzed by T-test, One-way Anova, Pearson correlation, Stepwise multiple regression analysis, etc. using IBM SPSS/WIN 20.0. For the dental hygienists with schoolwork, job stress was negatively correlated with professional self-concept and job satisfaction, and professional self-concept was positively correlated with job satisfaction; for the dental hygienists without schoolwork, job stress was negatively correlated with professional self-concept and job satisfaction, and professional self-concept was positively correlated with job satisfaction. The factors affecting job satisfaction were professional self-concept, job stress, and age of ≥30 for the dental hygienists with schoolwork and professional self-concept, job stress, and the position as a middle manager for those without schoolwork. To put these results together, professional self-concept and job stress were factors affecting job satisfaction for dental hygienists with and without schoolwork. It is necessary to focus on the formation of good dental organizational culture through positive institutional support as well as through systematic establishment of professional self-concept.

Comparative Evaluation of the Survival Rates of Zirconia Crown and Stainless Steel Crown for Proximal Caries in Primary Molars: a Retrospective Study (유구치 인접면 우식증 수복 시 지르코니아 전장관과 기성금속관의 생존율 비교 평가: 후향적 연구)

  • Due to increasing demand for aesthetics, zirconia crowns have become a popular choice for treating primary molars. However, there is limited literature available comparing the survival rates of zirconia crowns with those of other restorative materials. The objective of this study was to compare the 36-month survival rates of zirconia crowns and stainless steel crowns for proximal caries, as well as to analyze failure types associated with each crown type. Electronic medical records and radiographs of 1,061 primary molars from 498 patients treated with 2 types of prefabricated crowns at Chosun University Dental Hospital and 2 private dental clinics between 2017 and 2019 were collected and analyzed. The survival rate of zirconia crowns was found to be lower compared to that of stainless steel crowns. Regarding the groups without pulp treatment, the survival rate of stainless steel crowns was significantly higher than that of zirconia crowns. However, in the groups that received pulp therapy, no significant difference in the survival rates was observed between the two preformed crowns. Notably, abnormal root resorption or periapical lesions were identified as the primary cause of restorative failure in stainless steel crowns, whereas loss of restoration was the predominant cause in zirconia crowns. This study holds valuable implications for clinicians when selecting preformed crowns for primary molars.

The survey on the infection control of noncritical instruments used in dental treatment (치과 치료에 사용되는 비위험 기구의 감염 관리 실태 조사)

  • Purpose: The aims of this study were to evaluate the dentist's awareness and the actual status of infection control of noncritical dental instruments. Materials and Methods: 40 dental clinics in Daejeon, South Chungcheong, North Chungcheong and North Jeolla provinces were surveyed. The questionnaire was delivered to the dentists belonging to those clinics, and the awareness and the practice of infection control were examined. The microbial contamination on the surface of five noncritical instruments (impression gun, light curing unit, 3-way syringe, shade guide, and dental floss dispenser) used by them was measured with an ATP luminometer. Correlation analysis between the awareness and the actual state of infection control was conducted. Results: Awareness and frequency of infection control was highest in the 3-way syringe. Surface disinfection using disinfectant was most frequent in all instruments. 3-way syringes and shade guides were less contaminated than impression guns, light curing units, and dental floss dispensers. Conclusion: 3-way syringes had a significant correlation between user awareness of infection control and surface contamination, and the higher awareness, the lower the contamination measurement was shown.

Diffusion of Knowledge Related to Systemic Disease among Dental Hygienists (치과위생사의 전신질환 관련 지식 확산)

  • This study was aimed to identify the diffusion of systemic disease-related knowledge among dental hygienists. Two hundreds and eleven dental hygienists were selected for the study. Data was collected between August 31, 2015 and September 20, 2015 by handing out questionnaires. Returned questionnaires were analyzed by descriptive statistics, t-test, and ANOVA. Dental hygienists showed overall mean of 0.64 on the systemic disease related knowledge. Age, education level, career, and educational experience showed significant influence on the systemic disease knowledge (p<0.01). Dental hygienists revealed overall mean of 0.61 (full mark=1) on the usefulness recognition of systemic disease related knowledge. Age (p<0.001), education level (p<0.01), career (p<0.001), and educational experience (p<0.01) showed significant effects on the usefulness recognition of systemic disease knowledge. Degree of how dental hygienists apply the systemic disease-related knowledge to the service was mean 0.86 (full mark=2). Age (p<0.01), education level (p<0.01), career (p<0.01), and educational experience (p<0.001) had significant impacts on how dental hygienists apply the systemic disease related knowledge to the service. The diffusion of systemic disease knowledge among dental hygienists was 2.17, which represented 'persuade'. These results indicated that a study is needed to understand the diffusion of systemic disease knowledge among dental hygienists and to identify obstacles in the process. Moreover, an education program should be developed to offer practical training on the systemic disease-related knowledge. Further studies should evaluate the effectiveness of the program.

The influence that the medical service quality of the dental clinic affects to the patient satisfaction and use intention (치과 병·의원의 의료서비스 품질이 환자만족도와 재이용 의도에 미치는 영향)

  • Purpose: This aim of this paper is to research the influence of customer satisfaction and reutilization on Dental hospitals and clinics of medical service quality. Methods: The study examines 140 Patients who received treatment in Daegu from 1 July 2012 to 30 July 2012. The methods of research were used to factor analysis, reliability test using Cronbach's ${\alpha}$ factor, correlation analysis, multiple regression analysis and spss/win18.0. Results: The factor of quality of care has the highest impact on customer satisfaction. the influence of reutilization depends on the highest reliability and the physical environment was significant. Conclusion: To improve customer satisfaction and reutilization is required to maintenance of hospital facilities, medical services mind and quality of care. The medical service providers are important to develop patient education programs, self-development and customer satisfaction training techniques.

The relationship between social values and health system values in dental healthcare hygienist (치과병의원에 근무하는 치과위생사의 사회적 가치관과 보건의료체계에 대한 가치관의 관련성)

  • Objectives : This study was to examine the social value of dental hygienists, their values about the health system and the relationship of all the related variables. Methods : The subjects in this study were 205 dental hygienists who worked in dental clinics and hospitals on Seoul. A survey was conducted from August 12 to October 15, 2010. The questionnaire consisted of nine items about general characteristics, two items about social values and 11 about values of the health system. The items related to social values and values of the health system were prepared by translating the items used in David et al's study, and the Cronbach alpha coefficient of those items respectively 0.80 and 0.76. Results : The dental hygienists got 3.94 in social values, which was above the average. In terms of values about the health care system, their values of the treatment delivery system(3.92) rated highest, followed by values of patient rights(3.79) and values of institutional restrictions(3.25). Their socal values had a closest positive correlation to their values of the treatment delivery system, and their values of patient rights had a strong positive correlation to those of the treatment delivery system and was positively correlated to those of institutional regulations as well. And there was a positive correlation between their values of the treatment delivery system and institutional regulations(r=.276). Conclusions : The above-mentioned findings illustrated that the social values of the dental hygienists had a positive correlation to their values of the health system. Therefore the kinds of educational programs that help dental hygienists to build their social values and values of the health system should be developed to improve their job efficiency as oral health experts.

Analysis of the Association between COVID-19 and Dental Visits in Children and Adolescents through Big Data (빅데이터를 이용한 소아청소년에서의 코로나 바이러스 감염증-19와 치과 방문의 연관성 분석)

  • This study was conducted to investigate changes in dental visits in children and adolescents due to COVID-19. Based on the data provided by the Korea Health Insurance Review and Assessment Service, the number of dental visits among children and adolescents from January 2019 to August 2020, and the rate change according to Korean disease classification in 2019 and 2020 were analyzed by month and region. From January to August 2020, compared to the same period in 2019, the total number of visits to dental clinics and dental hospitals among children and adolescents decreased by 642,202 times (16.3%) in the 0 - 9 years old group, and 313,488 times (9.2%) in the 10 - 19 years old group. During the same period, the decreases due to Z29 (Need for other prophylactic measures) decreased by 118,219 times (34%) in the 0 - 9 years old group and 83,944 times (31%) in the 10 - 19 years old group, showing the greatest change. It is analyzed that overall dental service of children and adolescents has decreased due to COVID-19, and this may lead to deterioration of oral health of children and adolescents in the future, and this study can be used as a reference in case of an infectious disease such as COVID-19 in the future.

Understanding of the Workers in the North Area of Ulsan on the Oral Health and Hygienic Status (울산시 일부 생산직 근로자의 구강보건 인식에 대한 조사연구)

  • For a month of September, 2004, a questionnaire regarding dental hygiene was distributed to 165 workers of Ulsan area. A total of 125 questionnaire were analyzed: 102 collected of the distributed 165 pieces, and 23 questionnaires reported by patients of a dental clinic in Dong-gu, Woolsan. The result is as follows; (1) Labor workers of Ulsan have thought that they are in good dental health, 44.1% of respondents, following by those who they believe they are in moderate health and those who they think are in poor condition. 89.0% of total workers have experienced dental treatment. (2) Regarding the location of dental treatment, 85.0% of respondents have pointed out dental hospitals and clinics, while 3.9% have indicated that they had treatment from unlicensed dentists. Specifically, workers in their 40s over have ever experienced dental treatment by unlicensed dentists. Those who think they are in poor dental condition have tended to have more dental treatment by unlicensed dentists. (3) Regarding the reason that they put off the treatment, 59.1% of respondents have indicated that they have no time to have the treatment, following by economic burden and the lack of confidence in dentists. When they have tooth aches or the gum ache, 60% of respondents have suggested that they endure the ache or take a medicine to alleviate the pain, showing a lower understanding of early dental treatment. (4) Regarding the reason why they are unwilling to go to dentists, respondents have revealed the anxiety against pain, 50.4%, following by the difficulty of scheduling and unsatisfied services. (5) 47.2% of respondents have brushed their teeth 3 times everyday, following by those who have done it 2 times a day and those one a day. Regarding how to brush the teeth, most of respondents have brushed their teeth in the upper, lower, and the both side, 61.4%. The younger respondents and those who think they are in good dental health have tended to brush their teeth up and down. (6) Regarding the cause of the gum disease, 67.7% of respondents have thought that it is caused by poor dental hygiene, following by natural occurrence due to age, hereditary nature, and physical predisposition. (7) 67.7% of respondents have experienced dental scaling, while 32.3% of them have had no experience in it. 53.5% of workers have thought that scaling may not be recommended since they have the teeth scaled periodically once they had dental scaling, a higher rate than 46.5% who think scaling is good for the gum health.

The dentists' perceptions and current prosthodontic treatment status for the disabled in Korea (치과의사의 장애인 보철 진료 실태와 인식)

  • Statement of problem: In spite of increasing dental treatment for the disabled, there was no collected data on prosthodontic treatment status and dentists' perceptions in Korea. Purpose: The purpose of this study was to research the dentists' perceptions about current prosthodontic treatment status for the disabled in Korea and to suggest public health policies for the disabled. Material and methods: Total of 68 dentists who have regularly treated the disabled were asked to fill questionnaires regarding prosthodontic treatment for the disabled. Results: 1. 45.6% of dentists felt the necessity of an esthetic prosthesis for the disabled. However, 54.3% of dentists reported the improvement of access to implant,orthodontic and esthetic treatment is needed. 2. Most (79.4%) of the dentists thought it is imperative for the government to support the disabled to maintain adequate oral health by providing at least 50% of charge in their prosthesis. Conclusion: Through this research, 97.1% of dentists answered that the current prosthodontic treatment status for the disabled in Korea is poor. This indicated the necessity of the financial support from the government for the disabled.
