Distortion of Magnetic Resonance Imaging for Different Types of Orthodontic Material (치과 교정 물질에 따른 자기공명영상의 왜곡)

  • To evaluate the effects of an artifact by metal material for orthodontics in Magnetic Resonance Image (MRI) examination, wires and brackets used in orthodontics were selected and compared. Using a head coil, a $T_2$-weighted image, $T_1$-weighted image and FLAIR image were obtained. With obtained images, the sizes of the artifacts were measured and compared using Image J Program. In the research, the material with the biggest artifact in the wires and brackets for orthodontics was stainless steel wire. In the future, selecting and developing metal for correction should be considered also in other fields along with the purpose of orthodontics.

Effect of metal primer and thermocycling on shear bonding strength between the orthodontic bracket and gold alloy (치과용 금합금에 대한 금속 프라이머 처리와 열순환 처리가 교정용 브라켓의 전단결합강도에 미치는 영향)

  • Objective: The aim of this study was to evaluate the effect of metal primers and thermocycling on shear bond strength between the orthodontic bracket and gold alloy. Methods: For this study, 80 specimens made of dental gold alloy were divided into 8 groups based on the combination of metal primers (none, Alloy primer, Metaltite, V-primer) and thermocycling (with and without thermocycling). Shear bond strength testing was performed with a universal testing machine. Bond failure sites were classified by a modified ARI (Adhesive Remnant Index) score. Results: All metal primer treated groups showed a significantly higher shear bond strength than the only sandblasting treated group without thermocycling (p < 0.05). There were no significant differences on shear bond strength in the groups with thermocycling (p > 0.05). Bond failure sites of the metal primer treated group without thermocycling occurred at gold alloy/adhesive interface, whereas there were no differences on bonding failure sites in the groups with thermocycling. Conclusions: These findings suggest that using metal primer on gold alloy enhances the initial bracket bond strength. But, this effect was not shown with thermocycling.

MOLAR RESTORATION WITH AN ORTHODONTIC BAND (교정용 밴드를 이용한 구치부 수복)

  • The use of stainless steel crowns are indicated for restoration of primary or permanent molars with proximal dental caries, extensive dental caries, or previous pulp treatment with increased danger of tooth fracture. Stainless steel crowns were introduced by Humphrey in 1950. For their improved durability, longevity, and success rate, they have been strongly considered for restoring extensive and multi-surfaced dental caries of molars in pediatric dentistry. However, they also have shortcomings, such as possibility of pulpal exposure or damaging proximal surface of adjacent teeth. In addition, when oversized stainless steel crowns are used, eruption of the adjacent permanent teeth may be disturbed by their prominent margin. As a means to compensate the shortcomings of stainless steel crowns, use of orthodontics bands may be considered. It is an alternative restoration method, where an orthodontic band is placed on a tooth first and cavity is restored with filling material, such as composite resin, glass ionomer, or amalgam. The use of an orthodontic band is indicated for molar restoration with cervical dental caries, extensive dental caries, enamel hypoplasia, or previous pulp treatment. Because it requires shorter chair time compared to stainless steel crown, its application is very useful for children with poor behavior. However, restoration using an orthodontic band requires good oral hygiene after its application. This case report illustrates the conservative restoration of primary molars and permanent molars with extensive dental caries using orthodontic bands.

  • The purpose of this study was to evaluate effects of time on shear bond strengths of a light-cured glass ionomer cement and chemically cured resin cement to enamel, and to observe the failure patterns of bracket bondings. Shear bond strength of a light-cured glass ionomer cement were compared with that of a resin cement. Metal brackets were bonded on the extracted human bicuspids. Specimens were subjected to a shear load(in an Instron machine) after storage at room temperature for 5 and 15 minutes; after storage in distilled water at $37^{\circ}C$ for 1 or 35 days. The deboned specimens were measured In respect of adhesive remnant index. The data were evaluated and tested by ANOVA, Duncan's multiple range test, and t-test, and those results were as follows. 1. The shear bond strength of light-cured glass ionomer cement is higher than that of resin cement at 5 and 15 minutes. 2. The shear bond strengths of both light-cured glass ionomer cement and resin cement increase with time. There was no significant difference in those of both 1 day group and 35 day group 3. Light-cured glass ionomer cement is suitable as orthodontic bracket adhesives

A COMPARATIVE STUDY ON THE PHYSICAL PROPERTIES OF ORTHODONTIC PLIERS ACCORDING TO TYPES OF STERILIZATION (멸균 방법에 따른 교정용 플라이어의 물성 변화에 대한 비교 연구)

  • Sterilization has received much attention in orthodontic practices over the past several years. The present study was undertaken to investigate the effects of sterilization on the physical properties of orthodontic pliers-AEZ, Unitek, and Dentronix ligature cutters. This study was designed to examine the tips of ligature cutters before and after 200 and 400 sterilization cycles using the Bowmar RHT-1000, the Dentronix DDS-5000, and the Eschmann SES-2000. The tip surface and the fracture surface were observed with a scanning electron microscope. The microstructure was observed with an optical microscope. The hardness test was carried out with the mic개-Vickers hardness tester and the Rockwell C Scale hardness tester. The chemical composition was analyzed by energy dispersive X-ray spectrometer. The results of this study were as follows : 1. The number and the size of corrosion products on the tip surface and the proportion of cleavage planes in fractured specimen increased, but the hardness of the tip decreased in proportion to sterilization cycles. From these observations, it was considered that mechanical properities decreased in proportion to sterilization cycles. 2. The number and the size of chromium carbides increased in proportion to sterilization cycles. Coarse microstructure decreased mechanical properities. 3. The AEZ and Unitek ligature cutters were Fe-Cr stainless steels, but the Dentronix ligature cutter was Co-Cr alloy. There were many differences among manufactures, but the chemical composition was .not changed after sterilization cycles. 4. The tip edge of ligature cutter used in a clinic revealed microcracks with the SEM observation. Clinical experience confirmed that ligature cutters were gradually degraded by sterilization.

  • 치과재료에 사용되는 금속재료는 주로 스테인리스강, 타이타늄, Ni-Ti, Co-Cr등과 같은 특수합금이 주로 사용되고 있다. 이들 재료는 치과 보철물과 교정재료 및 충전재료로 주로 사용되고 있으며 그 외 치과에서 사용되는 기기나 기구에도 많이 활용되고 있다. 특히 치과 보철물을 사용하여 치료를 원하는 환자가 최근에 급격히 증가하면서 임플란트 고정체와 나사 등을 이용한 치료법의 연구와 개발이 필요하게 되어 세계적으로 연구와 투자가 활발히 진행되고 있다. 그러나 재료의 설계나 합금의 설계 및 제품의 설계상의 문제로 인하여 생체조직과 결합하는데 많은 문제점이 임상적으로 발생되고 있다. 즉 임플란트 표면의 생체적합성부여, 고정체와의 결합시 파절이나 풀림현상, 골에 고정체로 사용하는 나사의 강도와 내마모성문제 등이 개선되어야 할 문제점으로 남아있다. 또한 총의치에 사용되는 자석 어태치먼트의erosion-corrosion문제, 교정선의 탄성 과 마모저항문제 등은 앞으로 계속적인 연구를 행하여야할 과제로 남아있다. 또한 국소의치에서 사용되는 frame은 정밀주조법을 통하여 제조하며 주조상의 결함 등으로 인한 클라스프의 파절 문제점이 발생되고 있다. 따라서 본 연구에서는 치과재료로 사용되는 임플란트 고정체, 나사, 교정용 선등의 문제점을 고찰하고, 지금까지 이루어진 연구를 중심으로 최적의 개선 조건을 찾고자하였다. 최근첨단소재 및 금속재료를 사용하여 치과재료 합금을 설계할 수 있는 연구가 활발히 진행된다면 수입에 의존하고 있는 고가의 치과재료를 값이 싼 고성능의 제품으로 대체할 수 있는 효과가 클 것으로 생각된다.>$\rho$$\sub$0/=1.8 %.   • Metal brackets were recycled by variable methods for economic reason. Such recycling methods had a great effect on bracket slot width and bonding strength. Therefore, the recycling methods that don't change the properties of original bracket were suggested. In this study, debonded brackets were recycled with 30 kinds of solvents and bracket surfaces were examined by S.E.M. (Super IIIA, ISI International Scientific Instruments, Japan) and Stero Microscope (Sz-Tr, Olympus Tokyo, Japan) methods. The following results were obtained. 1. Thermosetting resin adhesives (eq. $Monolok^{(R)}$, $Concise^{(R)}$) were swelled most in sulfuric acid (assays 95%) and slightly in alcohol groups. 2. The solvent was exchanged every 24 hours during the brackets were recycled with sulfuric acid (assays 95%). As the passage of time, the adhesives were removed more clearly, and after 72 hours adhesives were nearly detached from bracket base. 3. Chemical recycled metal bracket surface showed no irregular structure by S.E.M. method.

Shear bond strength between gold alloy and orthodontic metal bracket using light emitting diode curing light (Light emitting diode를 이용한 광중합 시 금합금과 교정용 금속 브라켓의 전단접착강도)

  • Objective: The need to bond orthodontic brackets onto various alloys has increased because of the increasing demand for adult orthodontic treatment. This study tried to evaluate the shear bond strength between gold alloy and metal bracket using light emitting diode (LED) light curing after metal primer and silicoating surface conditioning. Methods: Half of the type III gold alloy plates were treated with sandblasting with aluminum oxide and metal primer containing 4-META. the other half were treated with silica and silane. Metal brackets were bonded with Transbond XT light curing adhesive on these plates and shear bond strength were evaluated 1 hour, 6 hours, and 24 hours later. The differences of shear bond strength between groups were evaluated with two-way ANOVA. Results: The results showed higher bond strength in the silicoating group and a tendency of bond strength increase over time. Conclusions: When using LED curing lights for metal bracket bonding to alloy surfaces, long curing time and silicoating can produce a reliable bonding strength.

Evaluation of the Cytotoxicity and Mechanical Strength of Dental Duplex Stainless Steel Orthodontic Wire (치과 교정용 듀플렉스 스테인리스 스틸 와이어의 기계적 강도 및 세포독성 평가)

  • The stainless steel wire is extensively used for the orthodontic treatment. But, the stainless steel wire that has commonly superior corrosion resistance has caused hypersensitive reaction or allergy as side effects because of corrosion in the oral environment. For improving the problem of corrosion, we was evaluated the suitability of the duplex stainless steel(DSS) as orthodontic wire through this study. The DSS wire was evaluated the mechanical strength and bio-stability for suitability and bio-compatibility as orthodontic wire. In this work, the DSS and stainless steel(SS) as common use of medical grade were prepared for the tensile strength test. The DSS wire were treated by heat. and Temperature conditions of the heat treatment were $28^{\circ}C$, $500^{\circ}C$, $600^{\circ}C$, $700^{\circ}C$, $800^{\circ}C$, $900^{\circ}C$, respectively. And the DSS wires that treated by heat on the optimum temperature condition were conducted the bending moment test and calculated the S-Max value and the modulus of elasticity. For evaluating the bio stability, each materials were conducted in vitro test for measuring the cell survival rate. The most interesting results was that the tensile strength test of SS wire($8.17\times10^4\;N/mm^2$) and DSS wire($8.05\times10^4\;N/mm^2$) that treated at $500^{\circ}C$ by heat were similar in mechanical strength. In the bio-stability study, the DSS has no cytotoxicity (p=0.05) Thus, we could make a conclusion that the duplex stainless steel wire has vastly superior corrosion resistance was suitable as orthodontic wire.

Effect of storage condition of resin cement on shear bond strength of the orthodontic bracket (레진시멘트의 보관 조건이 치열교정용 브라켓의 전단접착강도에 미치는 영향)

  • Purpose: For orthodontic bracket bonding, light curing resin cement is widely used because the process is convenient, and it can be polymerized at the desired time. This study compared the difference of bonding strength of orthodontic resin cement according to storage condition. Materials and Methods: After acid etching the bovine enamel surface with 37% phosphoric acid, 15 orthodontic brackets for mandible incisors were bonded with Ortho Connect and Orthomite LC according to following three conditions; 1) Immediate after 4℃ refrigeration for 3 months (IR), 2) One day room temperature after 4℃ refrigeration for 3 months (OR), 3) Room temperature for 3 months (RT). The shear bond strength was measured with a universal material tester and failure pattern of the specimen was observed. Two-way ANOVA and One-way ANOVA were used at the 95% significance level. Results: Ortho Connect that was applied immediately after refrigeration showed the maximum shear bond strength. Orthomite that was applied immediately after refrigeration showed the lowest shear bond strength, and the group stored at room temperature for three months showed the highest shear bond strength, and the difference between the two groups was significant. Conclusion: Ortho Connect can be used without worrying about bond strength even if it is used immediately after refrigeration, but Orthomite should be kept at room temperature sufficiently after refrigeration.
