위암환자의 술 전 병기 결정에서 PET-CT의 유용성 (Effectiveness of Positron Emission Tomography in the Pre-operative Staging of Gastric Cancer)

  • 목적: 위암환자의 수술 전 병기 결정에 있어서 PET의 유용성을 알아보기 위해 본 연구를 시행하였다. 대상 및 방법: 2006년 2월부터 2008년 8월까지 본원에서 위암으로 진단받고 수술 전 병기 결정을 위해 복부 컴퓨터 단층촬영(CT) 및 PET-CT를 모두 시험한 환자 70명을 대상으로 복부 CT 및 PET-CT 영상 자료에 의한 림프절 전이 판정과 수술 후 병리 조직검사에 의한 림프절 전이판정으로 민감도, 특이도, 양성예측도, 음성예측도 및 정확도를 조사하였고 위암의 형태, 조직학적 분류, 크기 등과 FDG섭취 정도와의 차이 등을 알아보았다. 결과: 조기 위암 23예의 경우 복부 CT에서 병변이 진단된 경우는 4명(17.4%), PET-CT에서는 12명(52.1%)이었으며, 진행 위암 47예의 경우 CT에서 26명(56.5%), PET-CT에서 45명(95.7%)으로 PET-CT에서 진단율이 높게 나타났다. 병변의 크기에 따른 분류에서도 복부 CT보다 PET-CT에서 진단율이 높은 것으로 나타났으며, 조기 위암의 경우 진행 위암에 비해 FDG 섭취 정도가 낮았지만 3 cm 이상의 조기 위암의 경우 3 cm 미만의 조기 위암에 비해 FDG가 섭취되는 경우가 더 많았다. CT에서 림프절 병기 결정의 민감도는 40.0%, 특이도는 85.7%, 양성예측도는 85.7%, 그리고 음성예측도는 40.0%였으며, PET-CT에서는 각각 55.6%, 81.0%, 86.2%, 45.9%였다. PET-CT를 통해서만 발견된 전이 병소는 1예(간전이)였으며, 중복암은 직장암 1예와 췌장암 1예가 있었다. 또한 세포조직형에 따른 분류에서도 분화영, 미분화형 모두 PET-CT에서 진단율이 높은 것으로 나타났다. 결론: CT에 비해 PET-CT에서 원발 병소의 진단, 림프절 전이 모두 진단율이 높은 것으로 나타났으며 PET-CT는 전이 병소와 중복암의 발견 등 다른 영상 진단법에 비하여 유리한 점이 있다.

Caerulein으로 유발된 흰쥐의 급성 췌장염에 대한 가미청이탕(加味淸胰湯)의 효과 (Effects of GamiChungYi-tang on the Caerulein-induced Acute Pancreatitis in Rats)

  • In this study, we aimed to investigate the effect of GamiChungYi-tang(GCY-t) on caerulein-induced acute pancreatitis (AP). It is performed by detecting oxidative stress markers and observing histopathological examination. Thirty adult male rats(Sprague-Dawley) were divided into six groups as follows: normal (NOR,n=5), caerulein-induced (CON,n=5), caerulein+Cefotaxime Sodium(CT,n=5), caerulein+GCY-t (130 mg/kg, CHA,n=5), caerulein+GCY-t (260 mg/kg, CHB,n=5) and caerulein+GCY-t (520 mg/kg, CHC,n=5) groups. Pancreatic tissues of rats from all groups were removed for apoptosis and light, and electron microscopic examination. Blood of rats from all groups were collected for oxidative stress markers inspection and pathological examination. Pancreatic oxidative stress markers were evaluated by the measurements of leukocyte, serum amylase and platelet activating factor (PAF), Interleukin-6 (IL-6) levels were determined spectrophotometrically. CON group has a significant increase (p<0.05) in amylase compared with NOR, but CT and CHA, CHB, CHC groups reduced the levels of these enzyme. The levels of Platelet activating factor (PAF) were increased in CON compared with NOR, but decreased in CT and CHA, CHB, CHC groups compared with CON. Interleukin-6 (IL-6) levels were increased significantly in CON compared with NOR, but reduced in CT and CHA, CHB, CHC groups. In the observations of Optical microscopy and electron microscopy, The experimental groups showed the significant decreases in pancreatic tissue inflammation, edema, vacuolization, necrosis compared to the control group. After all, GCY-t is potentially capable of limiting pancreatic damage produced during AP by restoring the fine structure of acinar cells and tissue.

가성 낭종을 동반한 만성 췌장염 환자에 대한 증례보고 (A Case Report of Chronic Pancreatitis with Pseudocyst)

  • Objectives : The purpose of this case is to report the improvement after treatment on the patient who has chronic pancreatitis with pseudocyst. Methods: We treated the patient with oriental and western medical treatment from 22nd February 2007 to 24th March 2007. We checked the patient with VAS score and Abdomen CT. Results : After treatment Pain, dyspepsia, itching sense and general body weakness were improved, VAS score changed from 10 to 0 and cystic lesion decreased on Abdomen CT. Conclusion : This case shows oriental medical treatment has useful effect on chronic pancreatitis. More research of chronic pancreatitis is needed.

췌장 가성낭종 환자 치험 1례 (A Case of Pancreatic Pseudocysts with Oriental Medical Treatment)

  • A pancreatic pseudocyst is the most common cystic lesion in the pancreas. There are several treatment methods with variable results. Recently, conservative treatment has been preferred because it has fewer complications. In this case, a 37-year-old male patient had pancreatic pseudocysts with upper abdominal pain, left upper back pain, diarrhea, weight loss, and general weakness. We prescribed him Banchongsangamibang (蟠蔥散加味方). For about 1 year, he was treated with oriental medicine. All of his pancreatic pseudocysts disappeared on abdominal CT follow-up and the symptoms mostly subsided. There had been no recurrence of pancreatic pseudocysts on abdominal CT follow-up after 1 year.

통합 의학 치료로 5년 생존 및 완전 관해에 도달한 췌장암 증례 보고 1례 (Case Report on the Five-year Survival and Complete Response of a Patient with Pancreatic Cancer Treated with Integrative Medicine)

  • Objectives: This is a five-year survival and complete response (CR) report on pancreatic cancer treated with western medicine and Korean traditional medicine. Method: A 59-year-old woman diagnosed with pancreatic cancer visited ○○ Korean traditional medicine hospital after neoadjuvant chemotherapy and pylorus-preserving pancreaticoduodenectomy. She was treated with Korean traditional medicine, including acupuncture, abdominal moxibustion, wild ginseng pharmacopuncture, and herbal medicine, which was based on integrated medicine therapy (IMT), from March 2018 to September 2022. The tumor size was measured by scanning with computed tomography (CT), magnetic resonance imaging, and positron-emission tomography/CT. Adverse events were evaluated using laboratory conclusion and National Cancer Institute Common Terminology Criteria for Adverse Events version 5.0. Result: During four years and three months of treatment, IMT maintained safety. The patient finally reached five-year survival without any recurrence or complication (CR) on October 26, 2022. Conclusion: We suggest that an integrative approach including Korean traditional medicine can be a meaningful treatment option for pancreatic cancer. Further studies should be performed to establish the proper treatment protocol of integrative medicine for pancreatic cancer.

  • Objective: This long-term case report details a case of pancreatic cancer with lung metastasis suppressed by integrative cancer medicine (IMT). Methods: A 64-year-old woman diagnosed with pancreatic cancer visited a Korean medicine hospital complaining about the side effects of chemotherapy for lung metastasis. She received IMT involving Korean traditional medicine and Western immunotherapy from May 2017 to June 2023. Tumor dimensions were assessed through computed tomography (CT) and positron-emission tomography/CT scans, while tumor markers and safety were monitored by laboratory tests. Results: IMT suppressed the progression of cancer, as observed by imaging and laboratory tests. The patient achieved 5-year survival, even after discontinuing chemotherapy. Conclusion: This case presents a potential therapeutic alternative for patients who are ineligible for chemotherapy or surgical interventions.

  • Objectives : Forsythiae Fructus has been reported to have anti-inflammatory effects in many diseases, and Corydalis Tuber has been used as a pain suppressor in Eastern Asia. However, the protective effects of individual water extract of Forsythiae Fructus (FF) and Corydalis Tuber (CT) and the mixture of FF and CT (FC) on chronic pancreatitis (CP) were not well-investigated. Therefore, we investigated the protective effects of FF, CT, and FC on CP in mice. Methods : To induce CP, cerulein was injected 6 times a day, 4 times a week for 3 weeks. 1 h before the every cerulein injection, 200 mg/kg of FF, CT, or FC was intraperitoneally injected to mice. Histological analysis of pancreas was examined by hematoxylin and eosin stain and collagen deposition was examined by Masson's trichrome stain. Fibrogenic parameters such as α-smooth muscle actin (α-SMA), extracellular matrix (ECM) deposition, and fibrotic cytokines such as transforming growth factor (TGF)-β1 and platelet-derived growth factor (PDGF) were also analyzed by immunofluorescence stain and real-time PCR. Results : Histological damages in pancreas were inhibited by pre-treatment of FF or FC but not CT. α-SMA and ECM in pancreas were inhibited by pre-treatment of CT or FC but not FF. Moreover, the expression of TGF-β1 and PDGF in pancreas were inhibited by FF, CT or FC. Conclusions : Our results suggest that FC have protective effect on CP in mice through inhibition of α-SMA, ECM, TGF-β1 and PDGF in pancreas, and these findings could suggest new clinical strategy for CP.

췌장의 유두상 낭성암 2예 보고 (Two Cases of Papillary Cystic Neoplasm of the Pancreas)

  • Two cases with papillary cystic neoplasm of the pancreas are reviewed and discussed. Up to recently, the tumors have been misclassified as nonfunctioning islet cell tumor or carcinoma, acinar cell carcinoma, papillary cystadenocarcinoma, or pancreatoblastoma. It frequently has been managed with aggressive surgery such as pancreatoduodenectomy. The tumors are well encapsulated and the cut surfaces are characteristically solid and hemorrhagic. Ultrasonography and CT scan are the most useful tools for the diagnosis. The neoplasms usually behave like a very low grade malignancy, so complete removal is the treatment of choice for the tumor arising anywhere in the pancreas. We have a boy and a girl who have papillary cystic neoplasm. The boy was 12 years old and the girl was 14 years old. Both underwent distal pancreatectomy and the progress were uneventful. 급성 괴사성 췌장염에서 발생한 결장 합병증 -1 예 보고- (Colonic Complication of Acute Necrotizing Pancreatitis - a Case Report -)

  • We present a case of a colonic involvement associated with necrotizing pancreatitis, with a review of the literature. A 10 year old boy had an appendectomy at the local clinic ten days ago. On admission, he complained nausea, vomiting and severe constipation. His abdomen was distended and he had tenderness on the left abdomen. Laboratory and radiologic studies revealed findings consistent with acute pancreatitis with colonic complication. He was treated conservatively for 30 days but did not improve. On hospital 30th day, abdominal pain developed and his vital sign changed. Abdominal CT suggested ischemic change of the transverse colon. At laparotomy, the left colon showed stenosis. The greatly distended transverse colon was resected and a transverse end colostomy was done. He was discharged at postoperative 45th day with improvement and colostomy closure was performed 8 months later.

소아에서 발생한 췌장의 유두상 낭성 종양 2예 (Solid and Papillary Cystic Neoplasm of Pancreas in Children)

  • Solid and papillary cystic neoplasm of pancreas is an uncommon low grade malignant tumor. It is predominant in young female between the second and third decades of life, and amenable to cure by surgical treatment. The authors report two cases of solid and papillary neoplasm of pancreas pathologically verified at Kyung Hee University Hospital. The first case was an 11-years old female patient and the other case was a 12-years old male. Symptoms were abdominal discomfort, nausea and vomiting in both cases and abdominal pain in the female patient. CT finding included a solid and papillary neoplasm of pancreas. The mass was well-demarcated with solid and cystic necrosis components. In the female patient, a large hematoma was found. Gross findings revealed apparent encapsulation, cystic degeneration and hemorrhagic necrosis. Microscopically the tumors were characterized by distinctive solid and papillary patterns of cellular arrangement without local invasion. Both patients were discharged after surgery and followed up without any problem.
