  • 광학기술의 의학적 응용은 1900년 루비레이저가 개발되고 이어서 다양한 레이저가 개발되면서부터다. 레이저를 이용하면서 최소침습적 수술이 가능해졌고, 내시경을 사용하여 체내 부분에 대한 시술도 가능해졌다. 최근에는 레이저, 발광다이오드, 램프 같은 광원을 소스로 하고 광검출기를 센서로 하는 광학 기술을 이용한 생리상태 측정에 대한 연구가 진행되고 있다.

  • Minimally invasive surgery (MIS) has several advantages over conventional open surgery. Less pain, less morbidity, early recovery and better cosmetic outcomes are well-known benefits of MIS. However, indications of MIS are highly dependent on the experience of surgeon and the disease status, In the benign diseases with less surgical complexity, MIS can be performed relatively and safely. However. in advanced malignant diseases which require more complex surgical manipulation, the of patients and the principles of oncologic surgery should be considered before the selection of surgical methods, MIS would be performed more and more with aid or technologic advancement. However, long-term result with equivalent oncologic outcomes to open surgery is necessary to be one of standard surgical treatments in thoracic surgery.

  • We present an attractive real time in-vivo endoscopic microscope with a resolution of submicron, in which two kinds of optical correcting plates are inserted to eliminate higher order spherical aberration and field curvature. And, since the conventional objective lens is replaced to GRIN lenses with diameter of 1 mm, the above endoscopic microscope can be effectively utilized to invade minimally for live animals.

  • A 40-year-old woman underwent plication by video-assisted thoracoscopy for left diaphragmatic eventration. Normalization in the position of the left diaphragm and a marked improvement in the left lung expansion were identified on the chest X-ray. This technique offers the patients many advantages of the minimally invasive operation.

  • Recently, the incidence of Achilles tendon rupture has been increasing with an increase in the elderly and the sports population. Various kinds of surgical options have been introduced up to now. Among them, the traditional open repair is most commonly used despite the risk of re-rupture or post-operative infections, which in turn can impair the blood flow to the Achilles tendon. Therefore, minimally invasive methods to overcome these complications have been studied with excellent outcomes.

  • Recurrent deformity following hallux valgus surgery can be technically challenging to treat. In cases of revision surgery, a surgical technique with greater corrective power is often chosen compared to the primary surgery. Therefore, minimally invasive surgery is not commonly performed. On the other hand, minimally invasive surgery minimizes soft tissue damage and allows for greater correction of deformity compared to traditional open approaches. This paper reports four cases of recurrent hallux valgus treated with a minimally invasive distal transverse metatarsal osteotomy - Akin osteotomy (MITA), resulting in significant improvements in the clinical and radiographic outcomes.

  • Hemophilia is a group of gender-linkage inherited bleeding disorders that impair the body's ability to control blood clotting or coagulation. This sex-linked disorder is transmitted on the X chromosome. These genetic disorders have lower blood plasma clotting factor level of coagulation factor. Most common form is Hemophilia A and B. Restorative dental care and simple surgery for the hemophiliac patient are quite often neglected for fear of bleeding during procedures. Even dental specialist avoid these patients and make them severe problem patients. On the dental treatment especially, invasive procedure, special considerations of bleeding control are required. Inter-consultation with the hematologist will provide orientation on the best approach to dental treatment, such as the need of replacement therapy, the modification of antifibrinolytic therapy, the application of local hemostatic methods. In this case reports, we successfully treat early childhood caries of patient with hemophilia B under the general anesthesia.

  • The management of refractory pain and limitation of mouth opening in the temporomandibular joint (TMJ) is both challenging and controversial. Arthrocentesis is a simple technique and it can be used instead of more invasive surgical procedures for patients who failed to conventional conservative treatments. The patients in this case report had complained persistent severe pain and restricted mandibular movement of TMJ, although they had been treated with conservative therapy, e.g., stabilization splint therapy, behavioral therapy, physical therapy, pharmacological therapy. Therefore we employed arthrocentesis for these patients, and they showed significant reduction of pain and improvement of mandibular movement after repetitive arthrocentesis. We recommend arthrocentesis as an effective, minimally invasive technique for patients with continuing severe pain and restricted mobility in the TMJ that is unresponsive to conservative management.

  • Background: Minimally invasive surgery of pectus excavatum by Dr. Nuss is a new technique that allows the repair of this deformity without any cartilage resection or sternal osteotomy We describe the early experiences with Nuss procedure. Material and Method: From December 1999 to January 2001, twenty patients with pectus excavatum underwent repair by Nuss procedure There were 14 males and 6 females whose mean age was 10.1$\pm$7.7 years, ranging from 1 to 33 years. Most patients(N=19) were below 20 years, except 33 years old female patient(N=1). Result: The severity of depression was assessed by computed topography(CT). CT index was mean 4.9$\pm$5.7(ranged from 3.3 to 8). The average operating time was 85.8$\pm$23.7 minutes. The used metal bars were ranged in length from 8 inches to 16 inches(average 11.8$\pm$14.4 inches). Early postoperative complications were pneumothorax in three patients, paralytic ileus in one, and postoperative chest pain requiring analgesics in all patients. Epidural analgesia was used in one adult patient for control of postoperative pain. In our experiences, there were no serious complications posteoperatively. Conclusion: There were good early results with the Nuss procedure that we performed for repairing of pectus excavatum. However, we believe the procedure needs to be observed for the long term results for it to be broadly accepted.

  • Oral mucocele is a common exophytic lesion resulting from the accumulation of saliva due to pathological changes in the minor salivary glands. It is typically asymptomatic and painless and characterized by semipermeable, fluctuant nodules. General treatment methods for mucocele include surgical excision, marsupialization, cryosurgery, and steroid injection. This case report presents the treatment of oral mucocele using micro-marsupialization and a tying method as minimally invasive techniques rather than surgical treatment. Based on this case report, it is suggested that micro-marsupialization and the tying method can be used as alternative methods for the treatment of oral mucocele infants and children with behavioral control problems.
