Ultrasound-guided Extracorporeal Shock Wave Therapy Combined with Multiple Needling for Calcific Tendinitis in Shoulder (견관절의 석회화 건염에 대한 초음파 유도하 다발성 천공술 및 고에너지 체외 충격파 병합치료)

  • Purpose: To evaluate the effectiveness of ultrasonography-guided combined multiple needling and high-energy extracorporeal shock wave therapy (ESWT) for calcific tendinitis of the shoulder. Materials and Methods: We included 42 calcific tendinitis patients who underwent ultrasonograpy-guided multiple needling followed by high-energy ESWT who visited the clinic from January 2010 to June 2013. The average follow up period was 45 weeks. Clinical evaluation was done before and after 12 weeks from treatment, in clinical terms using pain visual analogue scale (P-VAS), ASES, UCLA scores reflecting performance and symptom improvement, and in sonographic terms by studying the changes in size of the calcific nodules. Results: A statistically significant improvement was seen in P-VAS, ASES, UCLA scores and decreased calcification size on sonographic evaluation. Conclusion: Ultrasonography-guided combined multiple needling and high-energy ESWT is considered as a useful method which could provide clinical function improvement and reduction of calcification deposit.

석회화 건염

  • 석회화 건염은 회전근 개중 극상근 건에 석회가 침착되는 질환으로서 진행 양상에 따라 Formative, Resting, Resorptive phases로 나눈다. 치료는 물리 치료, Puncture, 국소 steroid 주사, 체외 충격파 등 보존적 치료에 반응을 잘 하며, 특히 resorptive phases때의 극심한 통증은 석회 침착 부위에 puncture나 needling lavege를 실시하면 즉각적인 동통 감소 효과와 함께 석회 침착의 자연 소실을 기대할 수 있다. 이러한 보존적 치료에 효과가 없을 경우 수술적 처치를 고려할 수 있는데 최근에는 대부분 관절경을 이용한 석회 제거술을 시행한다. 수술전 견관절 충돌 징후가 있거나 수술 소견상 이를 의심할 만한 소견이 있을 경우 견봉하 성형술을 함께 실시하기도 한다.

  • This paper investigates the information related to the radiated sounds during treatment using the extracorporeal shock wave lothotriptor(ESWL). First, we observe the vibration of the objects from the radiated sounds and extract the information about the change of the peak frequency from the relation between the vibration and the radiated sounds. Next, we observe the change of the peak frequency according to the position of the focus and demonstrate that it is more difficult to vibrate objects exactly at the focal point than above or below the focus of the ESWL device. Lastly, we analyze the relation between the cavitation and the radiated sounds and examine the benefiting condition for the object fragmentation.

The Effects of the Extracorporeal Shock Wave Therapy on Complex Pain Scales of Neck-Shoulder in 20s Adults with VDT Syndrome due to prolonged Smartphone Use (장시간 스마트폰 사용으로 VDT증후군을 가진 20대 성인의 체외충격파적용이 목-어깨의 복합적 통증지수에 대한 효과)

  • The purpose of this study was to determine the effect of extracorporeal shock wave therapy for the neck-shouler muscles of complex pain scales of the 20s adults with VDT Syndrome to prolonged smartphone use. Twenty subjects with neck pain due to VDT syndrome participated in the experiment. They were randomly divided into 10 experimental groups and 10 control groups. Experimental subjects were conducted for 4 weeks, 5 days a week. The experimental group was subjected to extracorporeal shock wave therapy 2000 times at an intensity of 5 Hz and 2.0 bar once, and the control group performed self-stretching exercise for 30 minutes once. Subjects measured and compared neck-shoulder pain with VAS and PPT before and after the experiment. Data analysis was performed with SPSS win 18.0, and correspondence comparison was performed to analyze the results of the experimental group and the control group before and after the experiment. After the experiment, the experimental group decreased more in VAS and PPT of the neck-shoulder muscle. but the control group was no difference. Though these results, extracorporeal shock wave therapy was showed a reduction in pain after treatment than before treatment but there are no difference in the pain reduction after self-stretching exercise. Thus, extracorporeal shock wave therapy is expected to be used as a variety of treatments to reduce neck-shoulder pain as a cause of VDT syndrome.

Effect of Extracorporeal Shock Wave Therapy on Pain and Function in Patients with Rotator Cuff Tendinitis (체외충격파치료가 회전근개 건염 환자의 통증 및 기능에 미치는 영향)

  • The purpose of this study was to evaluate whether extracorporeal shock wave (ESWT) is an effective treatment for rotator cuff tendinitis. Study subjects included 32 patients (16 in the experimental group and 16 in the control group). The patients were evaluated by assessing pain and function using visual analog scale (VAS), pressure pain threshold (PPT), Constant and Murley Scale (CMS), and simple shoulder test (SST). The results of this study indicated that ESWT after the subjects in the experimental group experienced significant changes in pain, range of motion, muscle strength, and function when compared to the control group. These results indicate that ESWT could be considered as an effective and efficient treatment for rotator cuff tendinitis.

Extracorporeal Shock Wave Therapy for Postoperative Pain after Ankle Surgery (족관절 수술 후 발생한 통증에 대한 체외충격파의 치료 효과)

  • Purpose: To evaluate the effectiveness of extracorporeal shock wave therapy (ESWT) for postoperative pain after surgery around ankle. Materials and Methods: We included 21 patients who performed an operation around ankle from 2009 to 2013. 4 times ESWT were applied to the patients who have tenderness more than visual analog scale (VAS) point 4. We evaluated the VAS at each sessions and final follow-up, and American Orthopedic Foot and Ankle Society ankle-hindfoot score (AFOAS) was checked at preoperative and final follow-up. Patient's satisfaction and complications were surveyed. Results: Preoperative VAS was mean 4.7, postoperative pain VAS was mean 6.0 at 4.5 months follow-up. The VAS after ESWT each session at 1, 2, 3 weeks and final follow-up were 4.8, 3.2, 2.3, 2.9, respectively. Mean final follow-up period was 9.4 months. Final VAS were significantly decreased (p<0.001). Final VAS were increased from VAS at last session of ESWT, however, not significant (p=0.189). AOFAS significantly improved from preoperative 60 to final follow-up 86 (p<0.001). Excellent was 12 patients (57%), good was 4 patients (19%), no change was 3 patients (14%) and poor was 2 patients (9%). 1 patient complained a dizziness and nausea during ESWT. Conclusion: ESWT for postoperative pain after ankle surgery shows satisfactory pain reduction in 76% of all patients without severe complication.

  • Purpose : The purpose of this study was to assess the effectiveness of Extracorporea Shock-wave therapy(ESWT) on the pain and improvement of wrist extensor strength which is one of the available medical treatment in patients with lateral epicondylitis. Object and Methods : The subjects of this study were consisted of 15 patients with lateral epicondylitis, These medical treatments was conducted total 6 time for 3 weeks. In the ESWT application group. To investigate the effectiveness of the treatment after ESWT treatment, Pain was measured by visual analogue scale(VAS) which is divided into 10 ranks and wrist extensor strength was measured by Aneroid phygmomanometer Results : 1. It was showed that VAS and wrist extensor strength before treatment and after two treatment(1 week) were no significant differences(p<0.05). 2. It was showed that VAS and wrist extensor strength before treatment and after four treatment(2 week) were significant differences(p<0.05). 3. It was showed that VAS and wrist extensor strength before treatment and after six treatment(3 week) were significant differences(p<0.05). Conclusion : We think that ESWT for the patients with lateral epicondylitis. is effective to wrist extensor strength and decrease of pain.

Effect of Extracorporeal Shock-wave Therapy on Pain, Grip Strength, and Upper-extremity Function in Patients with Lateral Epicondylitis (체외충격파치료가 팔꿉관절 가쪽위관절염 환자의 통증과 악력 및 팔 기능에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the effects of extracorporeal shock-wave therapy (ESWT) on pain, grip strength, and upper-extremity function in patients diagnosed with lateral epicondylitis and to provide an effective intervention method for lateral epicondylitis. Methods: Twenty patients with lateral epicondylitis were randomly assigned to the ESWT group (n = 10) and the stretching exercise group (n = 10). Interventions in both groups were performed six times twice a week for three weeks. The visible analog scale (VAS) was used to measure pain change. A dynamometer was used to measure grip strength (GS). Patient-rated tennis elbow evaluation (PRTEE) was used to measure the upper-extremity function. Results: There were significant differences in pain, grip strength, and upper-extremity function in both groups before and after intervention (p < 0.05). There were also significant differences in pain, grip strength, and upper-extremity function between the groups after intervention (p < 0.05). Conclusion: This study showed very positive improvement in pain, grip strength, and upper-extremity function after ESWT in patients with lateral epicondylitis. Therefore, ESWT can be recommended for patients with lateral epicondylitis.

Chronic Intractable Calcific Lateral Epicondylopathy Treated with Ultrasound-Guided Barbotage Combined with Extracorporeal Shock-Wave Therapy (석회성 외측 상과병증의 초음파 유도 하 세척흡인술과 체외충격파치료의 병행)

  • Lateral epicondylopathy represents pain in the lateral (radial aspect) of the elbow caused by degeneration in the common extensor tendon. Calcium deposit frequently developes in lateral epicondylopathy, with the prevalence up to 46%. There are considerable debates on its treatment protocols for lateral epicondylopathy so far, likewise, the effective treatment method for calcific lateral epicondylopathy has not been established. We report here a case of chronic intractable calcific lateral epicondylopathy treated with ultrasound-guided barbotage and focused type extracorporeal shock wave therapy (ESWT). To our knowledge, this is the first report of calcific lateral epicondylopathy treated by ultrasound-guided barbotage combined with ESWT.
