Effect of Extracorporeal Shock Wave Therapy on Pain and Function in Patients with Rotator Cuff Tendinitis (체외충격파치료가 회전근개 건염 환자의 통증 및 기능에 미치는 영향)

  • The purpose of this study was to evaluate whether extracorporeal shock wave (ESWT) is an effective treatment for rotator cuff tendinitis. Study subjects included 32 patients (16 in the experimental group and 16 in the control group). The patients were evaluated by assessing pain and function using visual analog scale (VAS), pressure pain threshold (PPT), Constant and Murley Scale (CMS), and simple shoulder test (SST). The results of this study indicated that ESWT after the subjects in the experimental group experienced significant changes in pain, range of motion, muscle strength, and function when compared to the control group. These results indicate that ESWT could be considered as an effective and efficient treatment for rotator cuff tendinitis.

  • Calcific tendinitis is characterized by inflammation around calcium hydroxyapatite crystal deposits. Minimally invasive extracorporeal shock wave therapy (ESWT) has been postulated to be an effective treatment option for treating calcific tendinitis. In clinical practice, shock waves usually are aimed at the painful area after palpation and not focused. It has been known that exact fluoroscopic focusing of ESWT at the calcific deposit for treatment of calcifying tendinopathy is highly effective. Ultrasound is a simple, inexpensive and radiation-free diagnostic tool that has been used to demonstrate tendinopathy including calcific tendinitis. However, focusing of shock wave under ultrasound is less well established. We present a patient in whom large calcific tendinitis of gluteus medius was completely resolved by exact focusing of ESWT by ultrasound with literature review.

Extracorporeal Shockwave Therapy for Medial or Lateral Epicondylitis of the Elbow (주관절 내, 외상과염에 대한 체외충격파 치료의 임상적 결과)

  • Purpose: Medial or Lateral epicondylitis is one of the most common causes of elbow pain and recently ESWT (Extracorporeal Shockwave Therapy) is welcomed as a new treatment modality and has been performed in many clinics. The purpose of this study is to report the clinical result after the ESWT treatment for the lateral or medial epicondylitis of the elbow. Materials and Methods: From Nov. 2005 to Jan. 2009, seventy four cases of seventy two (11 male, 61 female) patients diagnosed as medial or lateral epicondylitis of the elbow are included in this study. The average age was 52.4 years old. Authors used visual analogue scale (VAS) to assess the degree of pain at rest and during work, comparing the score before the ESWT treatment with that of $6^{th}$, $12^{th}$ and $24^{th}$ week after the treatment. Authors also checked the number of awakeness during sleep due to pain and compared them before and after the ESWT treatment. Results: Average resting pain score improved from 4.60 before the treatment to 2.01 at $6^{th}$ week, 0.43 at $12^{th}$ week and 0.16 at $24^{th}$ week. Average pain score during working also showed quite an improvement with time from 7.20 before the treatment to 4.05 at $6^{th}$ week, 2.01 at $12^{th}$ week and finally 0.36 at $24^{th}$ week. We checked the frequency of sleep arousal during a week, and also found similar significant treatment efficacy as the average frequency decreased from 9.64 per week before the treatment to 1.21, 0.08 and 0.09 per week at $6^{th}$ week, $12^{th}$ week and $24^{th}$ week, respectively. Conclusion: ESWT for medial or lateral epicondylitis is thought to be one of the effective treatment modalities for those patient group not quite responsive to other conservative treatment.

The Effects of the Extracorporeal Shock Wave Therapy on Complex Pain Scales of Neck-Shoulder in 20s Adults with VDT Syndrome due to prolonged Smartphone Use (장시간 스마트폰 사용으로 VDT증후군을 가진 20대 성인의 체외충격파적용이 목-어깨의 복합적 통증지수에 대한 효과)

  • The purpose of this study was to determine the effect of extracorporeal shock wave therapy for the neck-shouler muscles of complex pain scales of the 20s adults with VDT Syndrome to prolonged smartphone use. Twenty subjects with neck pain due to VDT syndrome participated in the experiment. They were randomly divided into 10 experimental groups and 10 control groups. Experimental subjects were conducted for 4 weeks, 5 days a week. The experimental group was subjected to extracorporeal shock wave therapy 2000 times at an intensity of 5 Hz and 2.0 bar once, and the control group performed self-stretching exercise for 30 minutes once. Subjects measured and compared neck-shoulder pain with VAS and PPT before and after the experiment. Data analysis was performed with SPSS win 18.0, and correspondence comparison was performed to analyze the results of the experimental group and the control group before and after the experiment. After the experiment, the experimental group decreased more in VAS and PPT of the neck-shoulder muscle. but the control group was no difference. Though these results, extracorporeal shock wave therapy was showed a reduction in pain after treatment than before treatment but there are no difference in the pain reduction after self-stretching exercise. Thus, extracorporeal shock wave therapy is expected to be used as a variety of treatments to reduce neck-shoulder pain as a cause of VDT syndrome.

  • Purpose: We report here on two cases of calcific tendinitis at the origin of the common extensor tendons of the forearm. Materials and Methods: A 42 year-old female complained of left elbow pain and flexion contracture. After obtaining the simple radiographs and MRI, surgical excision of the calcific deposits was done under the diagnosis of calcific tendinitis. A 25 year-old female complained of right elbow pain and a limited range of motion. Simple radiographs showed a calcific deposit just distal to the lateral epicondyle, and then she was managed with extracorporeal shock wave therapy. Results: The pain disappeared completely and both patients regained a full range of motion after treatments. Conclusion: Calcific tendinitis at the origin of the common extensor tendons of the forearm should be included in the differential diagnosis of the lateral elbow diseases that cause pain and a limited range of motion.

  • A method has been proposed for the fully automatic detection of left ventricular endocardial boundary in 2D short axis echocardiogram using geometric model. The procedure has the following three distinct stages. First, the initial center is estimated by the initial center estimation algorithm which is applied to decimated image. Second, the center estimation algorithm is applied to original image and then best-fit elliptic model estimation is processed. Third, best-fit boundary is detected by the cost function which is based on the best-fit elliptic model. The proposed method shows effective result without manual intervention by a human operator.

  • This study investigated the effect of extracorporeal shock wave therapy on nerve regeneration in nerve injured rats. In this study, we used 30 male Sprague-Dawley rats weighing 230-280g and 6 weeks old. Study groups were divided into two groups using a random sampling method: experimental group (n=15) treated with extracorporeal shock wave after sciatic nerve injury and control group (n=15) treated without extracorporeal shock wave after sciatic nerve injury. In this study, extracorporeal shock wave therapy equipment (OPTIMUS, SALUS TALENT 3, Korea) was used to apply extracorporeal shock wave therapy and applied to the sciatic nerve crush area of the right hind limb using low intensity. We measured the stance time and stride distance of the affected side using dartfish software. There was a statistically significant difference in the change of stance time and stride distance of the affected side between the experimental group using extracorporeal shock wave therapy and the control group without extracorporeal shock wave treatment. In conclusion, extracorporeal shock wave therapy has a positive effect on nerve regeneration.
