FACTORS AFFECTING 6 MONTHS' SHORT-TERM PROGNOSIS OF CONDUCT DISORDER IN THE ADOLESCENTS (청소년 행동장애의 6개월 단기 예후에 영향을 미치는 변인)

  • The purpose of this study is to find out the factors that affect the prognosis of conduct disorder in the adolescents. According to the nature or behavioral problems during 6 months after discharge, the good prognosis group(N=37) and the poor prognosis group(N= 36) were selected and scores of Youth Self Report(YSR), MMPI, KWIS were compared between both groups. The following results were obtained. 1) In family environmental factors, no significant difference was found between both groups. 2) In YSL total problem score, score of externalizing syndrome and score of delinquent behavior were significantly higher in the poor prognosis group. 3) In MMPI, no significant difference was found between both groups. 4) In KWIS, total 1.0. did not show significant differences between both groups. Our hypothesis that the prognosis of conduct disorder in adolescent is poorer in cases with higher quantities of problematic behaviors is certified.

DEVELOPMENT OF KOREAN ADOLESCENT FORM OF SUICIDAL IDEATION QUESTIONNAIRE (한국형 청소년 자살사고 설문 개발)

  • Objectives : This study was done to develop Korean form of Reynolds' SIQ-JR(Suicidal Ideational Questionnaire-Junior). Through this study we evaluated reliability and validity of Korean form SIQ-JR. Methods : Subjects were 1,160 middle and high school students and 114 adolescent psychiatric patients. SIQ-JR and 8 self-reporting scales highly related with suicidal ideation and suicide behaviors were applied to them. Results : In reliability aspect of Korean form of SIQ-JR, internal consistency, test-retest reliability and item-total correlation were statistically significant results. In factor analysis, concerning about other person was extracted as a indepenedent factor. This results reflect our cultural characteristics. There were statistical differences of SIQ-JR scores in groups by sex and age. Clinical group showed significantly(P<0.01) higher scores than general population. Among the 8 suicidal relating scales, depression(r=0.42), aggression(r=0.36), trait anxiety(r=0.35), and anger-in(r=0.31) were highly related with suicidal ideation but impulsivity and hopelessness were not significantly related with suicidal ideation. Conclusion : Korean form of SIQ-JR was useful instrument in reliability and validity. We hope that this instrument widely used in school mental health, community psychiatry and clinical setting for screening tool.

Insufficient Sleep and Visuospatial Memory Decline during Adolescence (청소년기 수면 부족과 시공간 기억력 저하)

  • Objectives: The objective of this study was to investigate the correlation between insufficient sleep and visuospatial memory in adolescents using a computerized neurocognitive function test. Methods: A total of 103 high school students (26 males and 77 females; mean age $17.11{\pm}8.50years$) without a serious psychiatric problem was recruited. All subjects were requested to complete a self-report questionnaire about weekday total sleep time and weekend total sleep time. The epworth sleepiness scale (ESS) and the beck depression inventory (BDI) were administered to measure daytime sleepiness and symptoms of depression. Seven subsets of the Cambridge Neuropsychological test automated battery were examined to assess visuospatial memory. Results: After controlling for age, sex, ESS, and BDI, longer weekend total sleep time was correlated with poor performance on delayed matching to sample (r = -0.312, p = 0.002) and immediate recall on pattern recognition memory (r = -0.225, p = 0.025). Increased weekend catch-up sleep time was correlated with poor performance of delayed matching to sample (r = -0.236, p = 0.018), immediate recall on pattern recognition memory (r = -0.220, p = 0.029), and delayed recall on pattern recognition memory (r = -0.211, p = 0.036) after controlling for age, sex, ESS, and BDI. Conclusion: This study showed that increased weekend catch-up sleep time reflecting insufficient weekday sleep were associated with poor performance in delayed recall tasks of visual memory. This finding suggests that insufficient sleep during adolescence might produce a decline of visuospatial memory.

INFLUENCES OF PARENTING ATTITUDE ON THE DEPRESSIVE TRAIT IN YOUNG ADOLESCENTS (부모의 양육태도가 중학생의 우울성향에 미치는 영향)

  • Objects : This study was conducted to investigate the relationships between parenting style and depressive trait in adolescents. Methods : 287 Middle school students were asked to complete self-report questionnaires including the Parental Bonding Instrument(PBI) and the Children's Depression Inventory (CDI). Results : Relations between two sets of variables were examined by means of the Pearson's correlation coefficient. It was suggested that the relationship with their mothers had more influence on the depressive trait of adolescents. The mean scores of children's depression inventory were significantly higher in group whose parents had been overprotective and less careful, than whose parents had been careful and not overprotective. Conclusion : The results support that the adolescents' depression is significantly related to the parenting style. Among parental bonding types, the high care-low overprotection type was most favorable and the low care-high overprotection type was worst.

Prevalence and Related Risk Factors of Suicidal Ideation in Urban Adolescents (일 도시지역 청소년의 자살사고 유병률과 위험인자에 대한 연구)

  • Objective: The aim of the present study is to assess the prevalence and related risk factors of suicide ideation among middle and high school students in an urban area. Methods: We surveyed 3,691 middle and high school students (2,159 male, 1,532 female, 11-19 years of age) using a self-report questionnaire that covered basic socio-demographic data, academic achievements, presence of physical or psychiatric illness, sleep duration on weekdays, time spent at private academies on weekdays and weekends, and subjective needs for counseling. The Beck Depression Inventory (BDI) and Reynolds' Suicidal Ideation Questionnaire (SIQ) were included in the survey. Results: The prevalence of students with high suicide ideation ($SIQ{\geq}62$) was 4.6%. In a logistic regression model, female sex (p = 0.002), younger age (p < 0.001), poor academic achievement (p = 0.043) and higher score of BDI (p < 0.001) were associated with a higher SIQ score. In addition, younger age (p = 0.045) and a higher BDI score (p < 0.001) were associated with a higher SIQ score adolescents having high suicide ideation ($SIQ{\geq}62$). Conclusion: Related risk factors of suicide ideation in adolescents were female sex, younger age, poor academic achievement, and a depressive mood. It would be especially helpful to pay more attention to younger adolescents and a depressive mood as a high-risk group. The understanding of these factors will be helpful for providing an effective suicide screening and prevention program for adolescents.

THE EFFECT OF A MENTAL HEALTH PROMOTION PROGRAM AT A MENTAL HEALTH MODEL MIDDLE SCHOOL (정신건강시범 중학교에서 수행된 정신건강 증진 프로그램의 효과)

  • Objectives : This study was performed to evaluate the effects of a mental health program within a mental health model middle school. Methods : Subjects of the study consisted of 748 students from the second grade and third grade students at the middle school chosen for a school mental health program by the Ministry of Education in Jeju. The subjects participated in 12 consecutive sessions of group discussion developed to prevent mental health problems. The authors investigated the effects of the program by evaluating the students with Young's Internet Addiction Scale (IAS), Conners & Wells' Adolescent Self Report Scale(CASS) and Minnesota Multiphasic Personality Inventory (MMPI) before the initial session and after the final session. The data was analyzed by t-test in SPSS PC+ 10.0. The range of significance was p<.05. Results : In MMPI, the percentage of students above clinical range reduced from $12.9\%\;to\;11.0\%$. It reduced in the second grade students, but increased in the third grade students. The scores of paranoia and mania subscales showed a statistically significant reduction. In IAS, the percentage of students above the range of Internee overuse reduced from $16.0\%\;to\;6.8\%$. The percentage of students who showed risk of attention problems in CASS reduced from $22.7\%\;to\;18.3\%$. Also, both IAS and CASS scores showed a statistically significant reduction. The clinical significance of the reduction of IAS scores was within moderate range. Conclusion : The mental health program reduced the percentage of students' risk of mental health problems, internet addiction and attention problems and it was clinically effective on preventing Internet addiction. These results support the effects of a school mental health program to promote students' mental health. The authors suggest to expand this program to other schools, to reconfirm the effect of the program by using proper & specified instruments and to evaluate long-term effect of the program.

THE COMORBIDITY AND EMOTIONAL STATE OF THE ENURETIC CHILDREN (유뇨증 소아의 공존질병 및 정서상태)

  • Enuresis is repeated involuntary or intentional voiding of urine into bed or clothes over age 5. Though it would be a self-remitting disorder, it could be serious problem in emotional and socio-adaptational aspects. The author reviewed the enuretic patients of Child & Adolescence psychiatric section in Chungnam National University Hospital during past 3 years. 46(4.9%) of 936 patients were diagnosed as enuresis in DSM-Ⅳ. The author evaluated their comorbidity by the data of diagnostic review made in two psychiatrists, and emotional aspects(self-concept, anxiety, depression) through the self-rating scales (Piers-Harris children’ self concept scales, RCMAS, state-trait anxiety inventory for children, child’s depresson inventory). Thirty(65.2%) of the 46 enuretic patients had additional diagnoses such as attention deficit hyperactive disorder, mental retardation, encopresis, oppositional defiant disorder, depression, anxiety disorder, autism, somatoform disorder, tic disorder, obsessive-compulsive disorder, sleep disorder, etc. Sixteen enuretic patients had at least one comorbid disorder. Eleven patients had two, and three patients had more than three. Fourteen of 46 enuretic patients were evaluated through self-rating scales of self-concept, anxiety and depression. But we couldn’t obtain meaningful results. Maybe it was due to the small sample size(N=14) and the influence of the comorbid disorders. Finally, it was an impressive evidence that there exist many comorbid disorders in enuresis(esp. attention deifict/hyperactive disorder). In emotional aspects, the author thought that further evaluation should be needed for more meaningful results.

Gender Differences of Direct and Relational Aggression in Children and Adolescents (국내 초등학생 및 중학생의 성별에 따른 직접적 공격성과 관계적 공격성의 차이)

  • Objectives : The aim of this study was to investigate the gender differences of direct aggression and relational aggression in Korean elementary and middle school students. Methods : Parents of 946 elementary school students(age $9.54{\pm}1.72$ years, 485 boys, 461 girls) completed the Child Behavior Checklist(CBCL). Six-hundred-and-three middle school students(age $13.98{\pm}0.93$ years, 301 boys, 302 girls) completed the Youth Self-Report(YSR). Independent t-test, chi-square test, and Fisher's exact test were used. Results : Boys had significantly higher direct aggression scores than girls in elementary school students(p<0.001). There was no significance gender difference of relational aggression in elementary school students(p=0.235). In middle school students, boys had significantly higher direct aggression than girls(p=0.017), and girls had significantly higher relational aggression compared to the boys(p<0.001). Conclusions : Our results suggest the presence of gender differences in manifestation of aggression subtypes in Korean elementary and middle school students. These gender differences should be taken into account in prevention and intervention approaches of aggression in children and adolescents.

Eveningness, Sleep Patterns, Daytime Sleepiness and Fatigue in Korean Male Adolescents (남자 고등학생들의 아침-저녁형 수면 양상과 주간 졸음, 피로도와의 관련성)

  • Objectives: Individuals differ in their biological rhythms. This study investigated the association between the morningness-eveningness and sleep patterns, daytime sleepiness and fatigue in Korean male adolescents. Methods: Participants were 501 eleventh grade male students ($16.9{\pm}0.2$ years of age) in one male high school in Seoul. Each student completed the Korean translation of composite scale (KtCS), Epworth sleepiness scale (ESS), Fatigue Severity Scale (FSS) and a questionnaire about their sleep schedule. Results: The evening group had shorter weekday sleep time than the intermediate (p<0.001) or morning group (p=0.029). The evening group had more daytime napping (16.8%, p<0.001), high rate of caffeine use (58.8%, p<0.001) and snoring (18.5%, p=0.037). The evening group showed more daytime sleepiness (8.7, SD=3.2, p<0.001) and severe fatigue (4.4, SD=1.2, p<0.001) than the intermediate or morning group. Conclusion: Adolescents with eveningness have higher problems in weekday sleep, daytime sleepiness and fatigue. An intervention program for sleep problem in these adolescents should be considered.
