A Study on the Antigastritic Effects of Rheum Species Extracts and Their Active Components (대황 추출물의 항위염 작용과 유효성분에 관한 연구)

  • Rheum tanguticum has long been used in oriental medicine as antipyretics, analgesics, anti-inflammation, aperient, hypertension medicine and medicine for skin disease. This study has investigated the effectiveness of defense for gastritis making use of Rheum tanguticum and its similar plants, Rumex cripus, Rheum officinale, Rhem palmatum and Rheum undulatum. Chysophanol, chrysophanol-8-O-glc, Desoxyrhaponticin desoxyrhaponti-genin, emodin, isorhaponticin, 2-methoxy-4-hydroxyanthraquinone-5-O-glc, physcion, pirace-tannol-3'-O-glc, resveratrol, rhaponticin and rhapontigenin are used as the components of Rheum tanguticum. In HCl ethanol-induced gastritis in rat, the most effective extract was 70 percent ethanol which is of Rheum tanguticum, showing the inhibition of 91.8 percent to the gastric lesions. 70% ethanol extract of Rhem palmatum and Rumex cripus shown inhibition of 75.6 percent and 73.2 percent, respectively. This effectiveness is considered as acid-neutralizing capacity, anti-H.pylori and anti-oxidant activity. 70% ethanol extract of Rheum tanguticum and its component, piracetannol-3'-O-glc exhibited higher free radical scavenging activity than others. These results suggest that Rheum species extracts and their active components could be utilized for the treatment of gastritis. Furthermore, these results are expected to contribute the standardization with objectivity and reliability for Rheum species, medicinal herbs. In addition, it can contribute to the prevention of indiscriminate distribution of imitation, and the rising rate of dependence on imports of medicinal herbs, and mixing prevention of low-quality goods.

Change of RBC Indices with Serum Iron, TIBC Level in Iron Deficiency Anemia Patients Who Treated Oriental Medicine (철결핍성 빈혈 환자에서 한약 투여에 따른 혈청철, 철포합능 및 적혈구 지표의 변화)

  • Anemia are often detected among many patients who are treated in oriental traditional medicine hospitals in Korea through blood tests which are taken to diagnose the patients' general state, and most of them are diagnosed to have iron deficiency anemia. This study amis the effect of herbal medicine on the RBC count, hemoglobin, hematocrits and indices of red blood cells such as mean corpuscular volume(MCV), mean corpuscular hemoglobin(MCH), mean corpuscular hemoglobin concentration(MCHC), serum iron, and total iron binding capacity(TIBC) among patients who were treated in oriental medical hospital and diagnosed to have anemia through blood tests other than main symptoms. The subjects of this study were 29 patients(male 7, female 22, mean age: 56.24${\pm}$19.11years) who were chosen ones, they has below hemoglobin 12 gm/dL, serum iron 70 IU/L, MCV 80 fL on laboratory screening test when they first visits our hospital. As a result, the volume of red blood cell(RBC), hematocrit(HCT), and hemoglobin(Hb) in peripheral blood increased from $348.27{\pm}8.95{\times}10^5$ /UL, 28.82${\pm}$0.56%, 9.24${\pm}$0.22 g/dL to $375.62{\pm}9.11{\times}10^5$ /UL, 31.36${\pm}$0.68%, 10.31${\pm}$0.25 g/dL comparing before and after oriental medicine treatment. After the treatment, MCH, MCHC, and MCV also increased from 26.98${\pm}$0.84 Pg, 32.62${\pm}$0.45 g/dl, 79.26${\pm}$0.17 fL to 27.74${\pm}$0.74 Pg, 32.84${\pm}$0.33 g/dl, 84.15${\pm}$1.67 fL. There was statistically significant increase in MCH, MCV(P<0.05); however, there was no significant change in MCHC(P>0.05). The amount of iron in blood increased from 32.96${\pm}$4.20 ug/dL to 67.22${\pm}$7.18 ug/dL, which showed a statistically significant increase(P<0.05). On the other hand, TIBC decreased from 325.08${\pm}$13.97 ug/dL to 315.61${\pm}$18.79 ug/dL, which was not a statistically significant change(P>0.05). Like this, all the patients showed favorable results in anemia based on the measurements of indices of red blood cells except MCHC. According to the classification of herbal medicines, in strengthening energy group(補氣), replenishing blood group(補血), and replenishing both energy and blood group(補氣血), the volume of RBC, HCT, and hemoglobin showed significant increases. Furthermore, in herbal medicines strengthening blood(補血) and replenishing both energy and blood group(補氣血), serum iron also relatively increased compare to strengthening energy group(補氣).

A Clinical Study for Gall Bladder Stones in Children (소아 담석증에 대한 임상적 연구)

  • Purpose: Gall bladder (GB) stones in children are rarely observed, and so reports of them have been quite limited in Korea. Therefore, we tried to assess the epidemiology and clinical features of children with GB stone diagnosed at Severance Hospital. Methods: A clinical study was performed on 18 patients below 15 years of age diagnosed with GB stone by abdominal ultrasonography or CT scan at Severance hospital from January 1999 to May 2005. Results: Concerning patients' profile, their sex ratio of male to female in total 18 cases was 2 : 1 with the mean age of $6{\pm}4.3years$. 15 patients (83.3%) were asymptomatic. Inducing factors were found in 14 children (77.8%) including ceftriaxone therapy in 6 cases (33.3%), infection in 4 cases (22.2%), spherocytosis in 3 cases (16.7%), Down syndrome and abdominal operation in 1 case (5.6%) respectively. Single stone was found in 13 cases (76.5%) and multiple in 4 cases (23.5%). 15 cases (88.2%) had the stones less than 5 mm in size, and 2 cases (11.8%) between 5 and 10 mm. 17 patients received conservative treatment, and one patient had cholecystectomy due to Meckel diverticulum. Conclusion: In this report, Pediatric GB stones are predominant in male children, without typical symptoms, having inducing factors, single and small. Recently the diagnosis of them has been increased because of the development of imaging study. But the reports for them were still rare in Korea. Therefore the study for them is more necessary to find it's clinical characteristics.

Development of a Nutrition Questionnaire and Guidelines for the Korea National Health Screening Program for Infants and Children (영유아 건강검진 영양 문진 및 지침 개발)

  • Purpose: A new health policy, referred to as the National Health Screening Program for Infants and Children, was launched in November 2007 by the Ministry of Health and Welfare and National Health Insurance Corporation in Korea. We have developed a nutrition-counseling program that was incorporated into this project. Methods: We reviewed the nutritional guidelines published by The Korean Pediatric Society and internationally well-known screening programs such as Bright Future in the United States. We also reviewed the recent Korean national surveys on nutritional issues, including the Korea National Health and Nutrition Examination Survey (KNHANES) and the 2005 National Survey of Physical Body and Blood Pressure in Children and Adolescents. The development of questions, pamphlets, computer programs, and manuals for doctors was carried out after several meetings of researchers and governmental officers. Results: We summarized the key nutritional issues according to age, including breastfeeding in infants, healthier complementary feeding, and prevention of iron deficiency anemia, establishment of healthier diets, as well as dietary prevention of overweight children with an emphasis on physical exercise. We have constructed a new Korean nutrition questionnaire and an anticipatory guidance program based on the primary care schedule of visits at 4, 9, 18, 30, and 60 months of age. Five to eight questions were asked at each visit and age-matched pamphlets for parents and guidelines for doctors were provided. Conclusion: We developed a nutrition-counseling program based on recent scientific evidence for Korean infants and children. Further research on this national program for screening the nutritional problems in detail and setting the therapeutic approaches may help identify areas of success as well as those that need further attention.

A study of mothers knowledge of weaning of infants with iron-deficiency anemia (철결핍빈혈 영·유아에서 어머니의 이유지식)

  • Purpose : Iron-deficiency anemia (IDA) is still one of the most common nutrient deficiency disorders, despite improvements in general health and nutrition. This study was designed to investigate the diagnostic values of hematological profiles, including the level of ferritin, and to evaluate the knowledge of mothers on weaning practices for infants and young children with IDA. Methods : This study was conducted on 111 infants and young children from six to 40 months of age with IDA. Their parents completed a questionnaire. IDA was defined as a level of hemoglobin <11.0 g/dL, the presence of microcytosis, a level of ferritin <10.0 ng/dL, transferrin saturation <15%, or an 1 g/dL increase in the level of hemoglobin after iron administration. The questionnaire made inquiries into their weaning practices. Results : In 111 infants and young children aged from six to 40 months, the average level of Hb was $9.5{\pm}1.0g/dL$. The prevalence of ferritin level (>10 ng/dL) was 48.6%, in spite of IDA. Seventy-four infants (66.7%) began to wean between four and six months, and 37 infants (33.3%) after seven months of age. The main food given after weaning was rice gruel. The weaning periods showed a significant relationship to the severity of anemia (P<0.05). There was no significant difference in the severity of anemia in terms of the educational levels of the mothers. Fifty-five mothers (49.5%) gave a wrong answer to a questionnaire describing that breast-fed infants aged over four months need to be fed with iron-sufficient food. Of all mothers, 49.6% took one month or more to complete the weaning process and 20% took three months or more. Conclusion : Many infants and young children with IDA have been provided with non-iron fortified foods and inadequate weaning. To improve nutritional status, especially among infants with iron deficiencies, nutritional education for mothers with infants at the weaning age must be increased and related programs must be implemented effectively.

A Survey of Nutritional Status on Pre-School Children in Korea (학영기전아동(學齡期前兒童)의 영양실태조사(營養實態調査))

  • The primary purpose of this study is to evaluate the correct nutritional status on pre-school children in Korea. Furthermore, it made an attempt to find and define nutrional problems, and assist in establishment on their nutritional improvement plan. For this, food intake and health condition (physical, clinical, biochemical and parasitological) survey on 109 Pre-school children in both sexes, randomly selected from Yang-Gu area in Gang-Won province and Rea-ju area in Kyong-gy Province, were conducted by means of three-day records, during the two periods of Spring and Fall season in 1975. The results obtained are summerized as follows: 1. The food intake; Average food intake of the subjects per day were $508{\sim}647g$ ($83{\sim}91%$ in vegetable foods and $5.5{\sim}11.7%$ in animal foods) in Yang-gu area, and $587{\sim}698g$ ($88{\sim}89%$ in vegetable foods and $6.3{\sim}7.6%$ in animal foods) in Rea-ju area. 2. The intake of energy and nutrients; a) Calory intake. Average energy intake of subjects per day in Yang-gu area$(1120{\sim}1415kcal)$ were all lower than the Korean Recommended Dietary Allowances (RDA) in either Spring and Fall survey, whereas the subjects in Rea-ju area were lower intake $(1213{\sim}1418kcal)$ than the RDA in the Spring but higher intake$(1516{\sim}1755kcal)$ than the RDA in the Fall, and the average intake were similar level with that of RDA. b) Protein intake. Average protein intake of the subjects per day in Yang-gu area $(33{\sim}43g)$ girl subjects in Rea-ju area $(35{\sim}39g)$ were lower than the RDA in either Spring and Fall survey, whereas the boy subjects in Rea-ju area$(36{\sim}38g)$ were lower in Spring and higher $(49{\sim}57g)$ in the Fall than that of the RDA, but the average $(43{\sim}47g)$ were similar level with the RDA. The protein intake from animal sources in all subjects were much lower $(5.5{\sim}11.7\;of\;total\;protein)$ than the RDA. c) Fat intake. Average fat intake were very lower in all subjects of both area $(14{\sim}24g\;in\;Yang-gu,\;10{\sim}12g\;in\;Rea-ju)$ than that of RDA which is recommended $12{\sim}14%$ of total energy to be supplied from fat. d) Calcium intake. Average calcium intake were very low in all subjects of both area $(264{\sim}355mg\;in\;Yang-gu\;and\;283{\sim}429mg\;in\;Rea-ju)$, especially, these in Spring were about a half level of the RDA, and it was much increased in the Fall due to increased intake of milk, but it was still not enough than the RDA. e) Vitamin A intake. Average intake of V.A ($703{\sim}1465\;IU$ in Ynag-gu and $750{\sim}1521\;IU$ in Rea-ju) were also lower than the RDA, moreover their V-A sources were mainly vegetable, so that the V-A supply might be critical one for the subjected. f) Riboflavin intake. Average riboflavin intake on all subjects in both area except boys in Rea-ju area in Fall, were very lower than the RDA. 3. The physical status; a) Average weight and height of boys aged 4 and 5 in Yang-gu area and girls of aged 5 in Rea-ju area were lower than those of Korean Standard of 1967 report, but those by age of girls in Yang-gu area and boys in Rea-ju area were a little heigher than the Korean Standard. It is, hower, present Korean standard of physical status might be somehow heigher than the 1967, since the socio-economical situation has been much improved during past a decade. So that, if one considered on this sense, the physical status of the subjects on this survey might be somehow lower than those of present Korean standard. b) Average upper arm circumference in both area were no difference each other, and their mean values of age 4, 5 and 6 in boy and girl were 15.6, 16.5, 16.4 and 15.5, 16.5, 16.4cm respectively. c) Average chest girth of boys were similar to those of Korean standard whereas the girls were smaller than the Korea standard. The average head circumference also showed similar tendency with the chest girth. 4. The clinical findings; The most popular clinical signs were angular stomatitis and dental caries, and boys had more heigher incidence then the girls. 5. The biochemical findings; a) Hemoglobin and anemia Average Hb value of boys and girls were 11.4 and 10.9g per 100 ml of blood respectively. The incidence of anemia (Hb value below 11 g/100 ml, by WHO) was increased by age, and girls had more heigher incidence than the boy (34% : 48%). The incidence of anemia in age of 4,5, and 6 in boys and girls were 28%, 41% 34%, and 33%, 50%, 49% respectively. The degree of the anemia was not severe, and the anemia of there subjects may be caused mainly low intake of better quality protein and low iron intake as well. b) Hematocrit. Average Ht value of whole subject were $39.9{\sim}41.6%$. c) Blood plasma protein. Average blood plasma protein contents of whole subjects were $6.6{\sim}7.4gm$ per 100 ml. The incidence of deficient range (<6.0g%, by ICNND) was only one girl of age 4 in yang Gu area. 6. Parasitological findgs; The most popular parasitism were asicris lumbicoides and trichocephalus trichiura, and about 2/3 of the whole subjects were suffering one or more of these parasitism.
