Factors Influencing the Burden Felt by Main Family Caregivers of Elderly Patients with Brain and Spinal Diseases (뇌.척추질환 노인 환자 주 가족수발자의 부담감에 영향을 미치는 요인)

  • Purpose: This study of this study was to identify factors influencing the burden of main family caregivers who take care of elderly patients with brain and spinal diseases. Methods: This was conducted as descriptive research and data were collected from 255 main family caregivers who were taking care of elderly patients with brain and spinal diseases from 4 hospitals in Daegu and Gyeongbuk Province. Stepwise-multiple regression was used to identify the influencing factors of burden felt. Results: As the score of burden felt by the main family, economic, social, physical, interdependent and emotional burdens were high in order. Factors influencing burden felt by main family care givers taking care of elderly patients with brain and spinal diseases were changed relation with patient after hospitalization, daily life ability, marital status, education and family caregiver's personality (explanatory power of 24.6%). Family caregivers felt a heavier burden when their relation with the patient was changed negatively or when the patient's activity of daily living was low. Conclusion: Based on these results, we need to develop coping measures and interventional programs for reducing the burden felt by the main family caregivers of elderly patients with brain and spinal diseases.

  • 한국과학기술정보연구원과 가톨릭대학교 의과대학 가톨릭응용해부연구소에서는 기초기술연구회 National agenda project 일환으로 척추 연구자들이 쉽게 사용할 수 있는 기초 자료를 구축하고 있다. 컴퓨터 시뮬레이션 연구에 활용할 수 있는 모델 제작을 위한 형상 정보를 제공하기 위해 60대, 70대 기증시신 20여 표본을 활용하여 고해상도 척추 (whole spine) CT (pixel dimension : 0.4xmm, thickness: 0.6mm)를 촬영하고 이를 3차원 모델링 소프트웨어(Mimics, Ver.14, Materialise, Belgium)를 사용하여 3차원 형상 모델(shell model, STL format)로 구축하고 있다. 또한 3차원 형상 모델을 활용하여 목, 등, 허리 척추의 주요 부위를 계측하여 수치화 하였다. 시신 기반 자료의 한계를 극복하기 위해 척추센터를 내원한 환자 중 고령자 호발 질환을 중심으로 선정하여 X-Ray, 3D CT, MRI, BMD 자료를 구축하여 보강하고 있다.

  • 최근 센서기술과 스마트폰의 융합을 통한 다양한 건강 모니터링을 할 수 있는 기능의 시스템 개발이 대두되고 있다. 본 논문에서는 3축 가속도 센서와 안드로이드향의 스마트폰 어플리케이션을 이용하여 척추 관련 질환을 가진 환자들이 게임을 통해 허리운동을 할 수 있는 시스템을 개발하기 위한 기초 연구이다. 제안된 시스템은 척추 주변에 착용할 수 있는 두 개의 3축 가속도 센서와 센서 데이터를 무선으로 전송할 수 있는 블루투스 모듈 그리고 허리운동용 스마트폰 게임으로 구성되어 진다. 현재 척추예방을 위한 여러 운동방법들이 존재하지만 본 시스템은 게임을 즐길 수 있도록 엔터테이먼트적인 요소를 가미함으로써 운동강도의 적당성과 언제든지 허리 운동을 할 수 있는 환경을 조성하는 데에 활용될 것으로 판단된다.

  • Abnormal spinal disease and function, in addition to simple musculoskeletal problems, can disrupt homeostasis and cause direct and indirect physiological side effects. Part or all of the immune function can be compromised, exposing you to more disease, especially if the nerves running through your spine do not deliver the proper signals to the organs that regulate your immune system. This study focuses on basic anatomic and physiological knowledge and seeks to consider potential mechanisms by which spinal function may potentially help maintain or improve immune function. To this end, we examine the roles of the spine in relation to hematopoiesis, stress, respiration, spine-nerve relationships, and the immune system, and confirm that these roles may influence immune function.

  • 목 디스크는 경추(목뼈) 사이의 디스크가 삐져나오거나, 뼈조직이 비정상적으로 자라 목으로 지나가는 척추신경을 압박하여 통증이 발생하는 질환이다. 목디스크는 뒷목 통증부터 저림, 감각 둔화 등의 증상이 서서히 나타나기 때문에 환자 스스로 질환을 자각하기가 쉽지 않아 적절한 치료시기를 놓치는 경우가 많다. 그러나 목디스크는 중증으로 발전할 경우 중추신경 마비로 인한 호흡곤란, 전신마비 등 치명적인 증상을 일으키기도 하므로 주의해야 한다.

Surgical Decision for Elderly Spine Deformity Patient (노인 척추 변형 환자의 수술적 결정)

  • Globally, the elderly population is increasing rapidly, which means that the number of deformity correction operations for elderly spine deformity patient has increased. On the other hand, for aged patients with deformity correction operation, preoperative considerations to reduce the complications and predict a good clinical outcome are not completely understood. First, medical comorbidity needs to be evaluated preoperatively with the Cumulative Illness Rating Scale for Geriatrics or the Charlson Comorbidity Index scores. Medical comorbidities are associated with the postoperative complication rate. Managing these comorbidities preoperatively decreases the complications after a spine deformity correction operation. Second, bone densitometry need to be checked for osteoporosis. Many surgical techniques have been introduced to prevent the complications associated with posterior instrumentation for osteoporosis patients. The preoperative use of an osteogenesis inducing agent - teriparatide was also reported to reduce the complication rate. Third, total body sagittal alignment need to be considered. Many elderly spine deformity patients accompanied degenerative changes and deformities at their lower extremities. In addition, a compensation mechanism induces the deformed posture of the lower extremities. Recently, some authors introduced a parameter including total body sagittal alignment, which can predict the clinical outcome better than previous parameters limited to the spine or pelvis. As a result, total body sagittal alignment needs to be considered for elderly spine deformity patients after a deformity correction operation. In conclusion, for elderly spine deformity patients, medical comorbidities and osteoporosis need to be evaluated and managed preoperatively to reduce the complication rate. In addition, total body sagittal alignment needs to be considered, which is associated with better clinical outcomes than the previous parameters limited to the spine or pelvis.

Calcium Pyrophosphate Dihydrate Crystal Deposition Disease Involving the Ligamentum Flavum of the Cervical Spine with Intense Enhancement on MRI: A Case Report (자기공명영상에서 뚜렷한 조영증강을 보이는 경추 황색인대의 칼슘수산화인회석 결정침착질환: 증례 보고)

  • Calcium pyrophosphate dihydrate (CPPD) crystal deposition disease is characterized by chondrocalcinosis, which mainly affects the knees, wrists, pelvis, and rarely, the spine. According to previous reports, CPPD crystal deposits display heterogeneous enhancement on MRI. When combined with inflammation of the surrounding soft tissue, strong enhancement by CPPD crystal deposition may appear similar to imaging features of other conditions such as infectious spondylitis. In these conditions, CT plays an important role in differential diagnosis. Here, we present a case of CPPD crystal deposition disease in the ligamentum flavum of the cervical spine that showed intense enhancement on MRI.
