고밀도 인공물 환자에서 반복적 재구성 사용 유무에 따른 3차원 입체조형 방사선 치료 계획 인자의 정량분석 (Quantitative Analysis of 3D-CRT Radiotherapy Planning Factors with or without IR in Patients with High Density Artifacts)

  • 본 연구의 목적은 3차원 입체조형 방사선 치료 계획에 고밀도 인공 물체를 삽입하는 불확실성을 보정하기 위해 반복적 재구성(IR)의 유용성을 평가하는 것이다. 쇠구슬 팬텀과 티타늄이 삽입된 대상자의 CT Simulation 영상에서 IR을 적용한 영상과 IR을 적용하지 않은 영상을 얻었으며 선량 평가 인자인 HI, MU와 체적 평가 인자인 Volume과 PCI를 비교하였다. 쇠구슬과 티타늄 팬텀 실험 결과 IR을 적용할 때 고밀도 인공물질의 부피가 각각 4.850%, 11.456% 줄었으며 MU는 0.924%, 1.181% 감소하였다. HI는 0.106%, 0.272% 감소하였고 PCI는 0.358%, 0.867% 감소하였다. IR을 척추 성형술, 대퇴부 정렬핀, 손목 정렬핀 등 대상자의 CT 영상에 적용했을 때 인공물의 부피는 47.76%, 23.841%, 49.339% 감소하였다. MU도 0.924%, 0.294%, 1.675%, HI는 1.232%, 0.412%, 1.695% 감소하였으며 PCI는 4.022%, 0.512%, 13.472% 감소하였다. 결론적으로 3차원 입체조형 방사선 치료 계획시 IR을 적용했을 때 고밀도 인공물이 삽입된 팬텀과 대상자 케이스에서 선량과 체적이 모두 감소되었다는 것을 확인할 수 있었다.

요가 수련을 통한 한발서기 자세의 안정화 연구 (A Study of the Stability on Standing posture of Single leg in Yoga practicing)

  • 이 연구의 목적은 요가의 한발서기자세를 수련 후 신체안정화에 어떠한 영향을 미치는지를 규명하는 것이다. 요가수련 경험이 없는 20대 여성 13명을 대상으로 24개월간 수련 전·후의 3D동작분석과 근육 생체 신호를 측정하였고 분석 결과는 다음과 같다. 첫째, 댄서포즈에서 왼쪽 무릎의 y축과 오른쪽 발목의 x축, 그리고 트리포즈에서 오른쪽 발목의 x축, 왼쪽 발목 y축의 관절 움직임 범위가 작아지며 통계적으로 유의한 차이를 보였다(p<.05). 둘째, 몸통과 골반의 정렬각은 댄서포즈와 트리포즈 모두에서 유의한 차이가 없었다. 셋째, 질량중심 이동거리는 트리포즈의 Y, Z방향에서 작아지며 유의한 차이를 보였다(p<.05). 넷째, 트리포즈 동작에서 양쪽 척추기립근, 복직근 및 왼쪽 대퇴사두근의 근활성도가 커지면서 유의한 통계적 차이를 보였다(p<.05). 이러한 결과는 요가 훈련이 특히 발목과 질량중심 움직임을 작게하고, 코어근육을 강화시켜 안정적 자세를 만드는데 중요한 역할을 한다고 할 수 있다. 결론적으로 요가 훈련은 자세 안정화에 효과가 있으며, 자세교정에 영향을 미친다고 할 수 있다. 추후 요가훈련을 통해 자세안정화를 볼 수 있는 정렬각의 변인을 통해 얻을 수 있는 정보에 대한 연구가 더 필요할 것으로 사료된다.

목과 어깨근육 운동프로그램이 전방머리자세의 척추-골반 정렬 변화에 미치는 영향 (Effects of Neck and Shoulder Exercise Program on Spino-Pelvic Alignment in Subject with Forward Head Posture)

  • Purpose : Excessive computer use frequently results in musculoskeletal disorders of the neck and shoulder such as forward head posture (FHP). The purpose of this study was to investigate effects of neck and shoulder exercise program on spino-pelvic alignment and the correlation between change in head and neck posture and spino-pelvic alignment in FHP. Methods : The study included 44 participants with FHP. The participants performed the exercise for correction of FHP 2-3 times a week for 4 weeks. We examined whole spine X-ray images in the lateral standing position with both arms crossed. We measured anterior head translation distance (AHT), craniovertebral angle (CVA), cervical lordosis (CL), thoracic kyphosis (TK), lumbosacral lordosis (LSL), sacral slope (SS), pelvic tilt (PT), and pelvic incidence (PI) of the subjects. The association between change in AHT and each spino-pelvic parameter was also subjected to Pearson's correlation coefficient analysis. Results : There were statistically significant differences before and after exercise in the parameters of AHT, CVA, and SS (p<.05). Significant negative correlation was observed between the change in AHT and CVA (r=-.768, p<.001), and CL (r=-.388, p<.05). There was significant positive correlation between the change in AHT and SS (r=.328, p<.05), and PI (r=.333, p<.05). However, no significant correlation was observed in change in AHT with that of TK, LSL, and PT. Conclusion : Based on the above results, we conclude that there is a relationship between change in AHT, which is a parameter associated with forward displacement of the head, and that of CVA, CL, SS, and PI after exercise in cases of FHP.

수동적 어깨뼈정렬운동과 유지이완기법이 대학생의 머리척추각(C1~C7)에 미치는 영향 비교 (A Comparsion of the Influences on the University Students Craniovertebral Angle of Passive Scapular Alignment Exercise and Hold and Relaxation Technique)

  • Purpose: This study purpose to comparison the effects of passive scapular alignment exercise and hold & relaxation technique on craniovertebral angle (C1~C7) with forward head posture due to using variety bag and using computer in healthy subjects. Design: Randomized Controlled Trial. Methods: 22 subjects were divide into two groups: PSAEx group and H/R group two groups. After exercise, the CVA were measured using C7 and tragus. Collected data were statistically analyzed by SPSS 20.2 Results: There were significant differences pre and post the exercise. Conclusion: PSAEx and H/R technique was effective to correct a abnormal CVA posture. The result of this study may helpful as basic data for orthopedic physical therapy.

어깨 안정화운동과 복부 안정화 운동에 의한 전방머리자세 성인의 머리척추각도와 어깨뼈 정렬 변화 (Change of Craniovertebral Angle and Scapula Alignment in Adults with Forward Head Posture by Shoulder and Abdominal Stabilization Exercise)

  • Purpose: This study aimed to adjust the craniovertebral angle and shoulder alignment through shoulder and abdominal stabilization exercises in adults with a forward head posture. Methods: The study participants were 29 adults with a forward head posture, and they were randomly divided into the following groups: 14 participants in a combined exercise group that used shoulder and abdominal stabilization exercises and 15 participants in a shoulder exercise group that used just shoulder stabilization exercises. The participants performed the stabilization exercises for 30 minutes per day, three times a week for five weeks. Results: There were significant differences in the craniovertebral angle after intervention in the shoulder stabilization exercise group (p < 0.05). There were significant differences in the craniovertebral angle and location of the right root of the spine and both inferior angles before and after intervention in the shoulder and abdominal stabilization exercise group (p < 0.05). There was a significant difference in the location of the right root of the spine and the left inferior angle between the groups at the post-test (p < 0.05), and there was a larger change in the shoulder and abdominal stabilization exercise group. Conclusion: There was a significant difference in the craniovertebral angle and a partially significant difference in shoulder alignment before and after intervention in both groups.

오심 구토를 동반한 틱 장애 환자에 대해 턱관절균형요법을 병행한 한방 치험례 (A Case Study of the Tic Disorder Patient with Nausea and Vomiting Treated by Korean Medical Treatment with Temporomandibular Joint Balancing Therapy)

  • 본 증례에서는 틱 장애 증상과 오심 구토를 동반한 환자에게 턱관절균형요법을 병행한 한방 치료를 적용하여 증상의 호전을 관찰하였다. 따라서, 이와 같은 경우에 턱관절균형요법을 적용하여 중추신경계와 자율신경계를 안정화시켜 틱장애 증상을 개선시킬 뿐만 아니라 전체적인 척추 정렬을 바로잡음으로써 소화기계와 관련된 흉추의 안정화를 통하여 관련된 증상의 호전에도 효과가 있을 것으로 사료된다.

자세이상과 관련된 국내 연구동향 분석 -시상면상의 척추 정렬 이상을 중심으로- (Research Trends of Abnormal Postures in Korean Literature -Based on Abnormal Spinal Alignment in the Sagittal Plane-)

  • Objectives The purpose of this study is analyzing internal research trends of abnormal postures and providing problems of researches forward. Methods 6 Korean databases were searched for articles of abnormal postures, irrespective of publication year and 30 research were systematically reviewed. An analytical method such as descriptive statistics and an acutal number and percentage was used. Results We collected 30 studies. 16 studies were published within the last four years. 2012 saw 7 studies, the highest number of studies. Clinical research is the major research method. 26 studies were clinical research. The studies can be categorized according to the characteristic of the research content such as research of related factors or physical characteristics, research of treatment or programs, etc. 16 studies were about research of treatment or programs, the most number of studies. 11 studies were about research of related factors or physical characteristics, and 3 studies were about other issues. Conclusions Since the connection among postural abnormality, pain, and musculoskeletal system disorder has been raising, it is important to conduct a continued, methodical study.
