The Effects of Treadmill Exercise on Inhibition of Soleus Muscle Atrophy and Improvement Functional Recovery after Spinal Cord Contusion in the Rats (척수타박손상 후 트레드밀운동이 근 위축지연과 기능적 회복에 미치는 효과)

  • Physical activity and exercise can promote sensorimotor recovery from central nerve injury. It has been suggested that the functional recovery promoted by exercise training after spinal cord injury might be associated with insulin-like growth factor-I in the inflicted muscle. To investigate morphological and biochemical change of the soleus muscle after spinal cord injury, all tissues were used for H&E, immunofluorescence staining and Western blot. Also, BBB-test was used to evaluate behavioral improvement after spinal cord contusion. Thirty male Sprague-Dawley rats ($230{\pm}10\;g$; 7week in age) were assigned equally to three different groups; Normal (n=10), SCI (n=10), SCI+TMT (n=10). Every rat in SCI and SCI+TMT groups underwent laminectomy at T9 level and then contusion on the exposed spinal cord site in anesthetized condition. After one week-recovery from contusion, every rat in the SCI+TMT group exercised on a motorized treadmill for 30min/d, 5d/wk for 7wks. TMT followed by injury increased IGF-I induction levels in the soleus muscle and inhibited muscle atrophy. Behavioral scales for 4 and 8 weeks after spinal cord injury were improved in the SCI+TMT group compared to the SCI group. These results suggest that treadmill exercise after spinal cord injury might promote functional recovery along with muscle regrowth through the up-regulation of IGF-1 in muscle tissue.

  • 척수 손상에 의한 사지마비 중증 장애인이 일상 생활을 함에 있어서 가족이나 주변인의 도움을 받지 않고서는 가전제품의 작동이나 전화 통화 등과 같은 작은 일 조차 스스로 할 수 없는 현실에서 국내 여건에 적합한 생활환경 제어장치의 개발이 필요하나 현재 이를 위한 마땅한 제어장치가 없는 실정이다. 각 가정에 널리 보급되어 있는 PC를 활용할 수 있도록 PC기반의 음성인식기를 개발하고 이를 응용한 가전제품 제어기를 개발, 제작하여 사지마비 환자 스스로 간단한 동작만으로 환자와 가족의 신체적, 정신적 부담을 감소 하고자 한다 이장치는 음성인식기 부분과 제어장치 부분 그리고 I/O 인터페이스 부분으로 구성 되어있다. 중증장애인은 이 장치를 사용하여 일상생활에서 기본적인 자유로운 생활의 영위를 도모하며 환자 가족의 부담을 줄일 수 있으며 환자와 가족 모두 심리적인 만족을 얻은 것으로 나타난다. 그러나 기기 사용을 위한 환자의 음성인식을 위한 언어학습과정의 불편함과 인식에러의 과제가 남아있다.

  • This paper deals with gait assisting rehabilitation robot which helps SCI patient walk again. We propose new concept of orthotic for robot considering motions of Hip and Knee Joints, and how to fit the robot to a user in terms of weight balance and comfortable standing. Then we describe our first engineering sample being designed based on the passive orthotic and show how to make the robot work for SCI patient in basic operation.

  • The use of electricity to evoke s skeletal muscle response is FES, which is a form of functional electrical stimulation. In the case of the damaged spinal cord, the technique can supply stimulation to the lower moter neurons and their muscle fiber, which have been disconnected from control of the higher nervous system. Recent advances in electronics, particularly miniaturization, have made possible the design of much improved systems of electrodes and stimulaters for FES. Clinical research has followed two main lines: the use of FES in the upper extremities for producing functional hand rehabilitation in quadriplegics and in the lower extremities for producing standing and gait in paraplegics.

  • To determine whether position affects measured lung capacity of spinal cord injury patients. The study subjects were 45 patients with spinal cord injury (cervical level 15, thoracic level 15, lumbar level 15). Subjects were provided with a full explanation of the experimental procedures and all provided written consent signifying their voluntary participation. We used a spirometer (Spirometer, Micromedical Ltd, UK) to measure pulmonary function in the supine and sitting positions (straightened upper body at an angle of $90^{\circ}$). Forced vital capacity (FVC), forced expiratory volume during the first second (FEV1), tidal volume (TV), and maximum insufflation capacity (MIC) were also measured. FVC, FEV1, TV, MIC (%) were greater in the supine than in the sitting position for those with injury at the cervical or thoracic injury level. On the other hand, FVC, FEV1, TV, MIC (%) were lower in the supine position for those with an injury at the lumbar level. More attention should be paid to the effect of injury level on measured lung capacity.

  • The development of standards and guidelines of rehabilitation nursing has been the major concerns for providing better nursing to the rehabilitation clients. As the patients with stroke and spinal cord injuries are the most prevalent physical disabilities in Korea, this study focussed on the nursing diagnoses of these two groups of patients. In order to identify the nursing diagnoses frequently used In their practice for the patients with stroke and spinal cord injuries, a survey was done with the questionnaire form developed by the research team. The surveyee were the staff nurses working at rehabilitation wards more than 2 years from 8 general hospitals in Korea, They identified and set the priorities of 13 nursing diagnoses from 79 stroke patients and 10 nursing diagnoses from 35 patients with spinal cord injuries during the periods from March 1 to June 2, 1999. The identified nursing diagnoses for the stroke patients are impaired physical immobility, sensory-perceptual alteration, activity intolerance, self-care deficit, altered defecation, altered urination, risk for injury, unilateral neglect, impaired skin integrity, altered thought processes, pain, altered health maintenance, dysreflexia. The identified nursing diagnoses for spinal cord injuries are altered urination, altered defecation, impaired skin integrity, pain, risk for injury, reflex incontinence, impaired physical immobility, self-care deficit, activity intolerance, knowledge deficit.

  • Brachial plexus injury is regarded as one of the most devastating injuries of the upper extremity. Accurate diagnosis is important to obtain the successful results. Basic preoperative evaluation includes simple radiography, cervical myelography. Magnetic resonance imaging, angiography, electrophysiologic studies and intraoperative studies. Furthermore, proper timing of surgery, surgical indication, plan and sufficient understanding of patients about the prognosis are the key for the satisfactory outcomes. This article provides an overview of the evaluation, diagnosis, intraoperative monitoring, and proper surgical planning for the treatment of posttraumatic brachial plexus injuries.

  • Pulmonary aspergillosis is the most common disease of fungal infection and has lower infectivity. Pulmonary asergillosis is classified by aspergilloma, bronchopulmonary aspergillosis, necrotic and invasive aspergillosis. Invasive aspergillosis is found in immune compromised host, immunosuppressive treatment after organ transplantation, anticancerous chemotherapy, blood abnormality, AIDS patients etc. We reported a case of invasive aspergillosis in an immunocompetent host, with review of literatures.

Effects of the Inspiratory Muscle Strengthening Training on the Respiratory Functions and the Quality of Life in Patients with Cervical Spinal Cord Injury Patients (들숨근 훈련이 경수손상환자의 호흡기능 및 삶의 질에 미치는 영향)

  • For patients suffering from spinal cord injuries, both of the possibility of having complications with the respiratory system and the related fatal rate are still high, while pneumonia is the most important cause of these fatalities. This research was carried out to find out what effects does a breathing exercise have on the respiratory functions and the improvement in the quality of life. The study was carried out for male patients suffering cervical spinal cord injuries for the purpose of inspiratory muscle strengthening. The objects of the study were 17 non-smoking-at-present male patients with C4 to C6 cervical spinal cord injuries. They had practiced inspiratory muscle strengthening training for three times a week for six weeks. The quality of life and the dyspnea (breathing difficulty) were self-measured before and after the training. As a result, the change in respiratory function was only significantly reduced (p<0.01) within the chronic group. In terms of the changes in the quality of life, the sub-acute group has only seen a significant improvement (p<0.05) for the VT(vitality), while the chronic group has seen significant improvements(p<0.05, p<0.01) in: GH(general health); BP(back pain); and VT(vitality). In this research, the inspiratory muscle strengthening has reduced the dyspnea (breathing difficulties) and improved the quality of life.

A Case Study of T10 Spinal Cord Injury due to Varicella Zoster Myelitis Treated with Korean Medicine (대상포진 척수염으로 유발된 척수손상 환자의 한방치료 1례)

  • Objectives: This is a clinical report of one patient with both lower limb paraparesis and numbness as well as sensory impairment caused by a T10 spinal cord injury due to varicella zoster myelitis.Methods: The patient with the spinal cord injury induced by varicella zoster myelitis was treated using Korean medical treatments such as herbal medicine (Palmijihwang-tanggami), acupuncture, and moxibustion.Results: After treatments, improvements in muscular strength and the sensory impairment of both lower limbs were observed as well as improvements in various side effects such as the debridement state and laboratory findings of urine analysis.Conclusion: Given these results, it is considered that Korean medical treatment is effective for patients with spinal cord injury due to varicella zoster myelitis.
