Effect of Herbal Decoction for Sitz Bath on Dermoepidermal Recovery to Wound Tissue in Rats (한방좌욕제의 창상치유 효과에 대한 실험적 연구)

  • Purpose: This study was to observe the effect of "herbal decoction for sitz bath" on dermoepidermal recovery to wound tissue in rat's skin. Methods: The samples were assigned to 3 groups: control group : without any treatment, positive control group : potarose 10% solution, experiment group : herbal decoction for sitz bath. We made the open wound of $2{\times}2cm^2$ size that cut deep into the dermis. Treating the open wound for 17 days, we observed the size of the wound diminishing. On 17th days, the cell viability was measured by MTT assay. The effect anti-inflammatory and dermoepidermal recovery were examined by H&E staining, immunohystochemical staining for MIP-2, FGF. Results: The experiment group showed more recovery from the open wound comparing the control group and the positive control group on 10th days after wounding. But there was not remarkable difference between the experiment and positive control group after 17th days post-wounding. The number of MIP-2 positive reacted cell were significantly decreased and that of FGF positive reacted cell were significantly increased than positive control group at 17th days. Conclusion: According to these results, we finally concluded that "herbal decoction for sitz bath" could be effective in recovery to wound tissue.

The Effect of Silk Fibroin-Alginic Acid Sponge Treatment as an Artificial Skin on Wound (인공피부용 실크 피브로인-알긴산 해면체의 창상치유 효과)

  • Purpose: This study was designed to compare the wound healing effect of silk fibroin, alginate and fibroin/ alginate blend sponge with clinically used Nu gauze in a rat skin defect model. Methods: Two full thickness excisions were made on the back of Sprague-Dawley rat. The excised wound was covered with either of the silk fibroin(SF), alginate (SA), or fibroin/alginate blend sponge(SF/SA). On the postoperative days of 3, 7, 10 and 14, the wound area was calculated by image analysis software. At the same time, a skin wound tissue was biopsied. Results: Healing time 50% ($HT_{50}$) of SF/SA sponge treated group was dramatically reduced as compared with that of control treatment. We also found that the $HT_{50}$ of SF/SA sponge was significantly decreased as compared with either those of SF or SA treatment. Furthermore, SF/SA treatment significantly increased the size of epithelialization and collagen deposition as well as the number of PCNA positive cells on epidermal basement membrane as comapred with those of control treatment. Conclusion: Our results suggest that the wound healing effect of SF/SA blend sponge is the best among other treatments including SF and SA during the whole wound healing period.

Comparative study of wound healing in porcine urinary bladder with CO2 laser and scalpel incisions (CO2 Laser와 Scalpel을 이용한 절개 시 돼지 방광에서의 창상치유 평가)

  • This study compared the instrument performance and tissue healing of a steel scalpel with a $CO_2$ laser in an animal urinary bladder surgery model. Landrace and Yorkshire mixed breed pigs were used. Two symmetrical incisions were made in urinary bladder of each pig. One incision was made on the left side of ventral aspect on urinary bladder using a steel scalpel, while the other incision was performed on the right side using a $CO_2$ laser with an 8W output power. Each instrument was evaluated clinically for speed, ease of incision, and extent of bleeding. At 7 and 21 days after initial wounding, each wound was taken for histological observations. The scalpel was an easier instrument to use in the confines of the urinary bladder tissue, compared with the laser. However, there was no significant difference between the two groups. The amount of bleeding was less in the laser group but the time of the incisions was shorter with the scalpel. Scalpel incisions showed complete restoration of the epithelium and muscularis. On the other hand, the laser incisions showed incomplete restoration of the epithelium and muscularis. However, most of wound healing in the laser incisions was accomplished according to the time lapse. Although the scalpel produced less damage to the urinary bladder tissue and was easier to handle than the $CO_2$ laser, it did not provide hemostasis that was helpful for use on highly vascular tissue. The $CO_2$ laser provided good hemostasis, but delayed wound healing. In conclusion, the $CO_2$ laser provided better hemostasis and better surgical field than the scalpel. The $CO_2$ laser was used effectively in urinary bladder incision.

Comparative study of wound healing in canine stomach with CO2 laser and scalpel incisions (CO2 레이저와 scalpel을 이용한 위 절개 시 개에서의 창상 치유 평가)

  • The objective of this study was to compare wound healing in stomach with $CO_{2}$ laser and scalpel incision by measuring the extent of bleeding, the ease of gastric incision, incision time, degree of adhesion and wound healing degree in dogs. Sixteen healthy dogs were used. Two symmetrical incisions were made in ventral aspect of the stomach between the greater and lesser curvatures were made with scalpel and 0.2 mm spot diameter $CO_{2}$ laser (8W, continuous wave) in sixteen dogs. And then each wound was closed with absorbable suture in a two-layer inverting seromuscular pattern. At 3, 7, 14 and 21 days after initial wounding, each wound was taken for histological observation. On surgery, the extent of bleeding, the ease of incision and incision time showed significant differences between the groups. The $CO_{2}$ laser provided better hemostasis (p < 0.05) and smaller postoperative adhesion compared with the scalpel. However, the scalpel produced faster speed of incision and was easier to handle than the $CO_{2}$ laser (p < 0.05). Although there was no considerable difference between the two groups in histological observation, necrosis and calcium deposit tended to be larger in the $CO_{2}$ laser than in the scalpel.

EFFECT OF FIBRIN ADHESIVE ON HEALING OF FULL-THICKNESS WOUNDS IN THE RAT'S HARD PALATE (백서 구개 전층 창상의 치유에 미치는 피브린 접착제의 영향)

  • The purpose of this study was to examine the effects of the fibrin adhesive (Beriplast$^{(R)}$) on healing of full-thickness wounds in the rat's hard palate. Twenty Spraque-Dawley strain white male rats, each weighing 250~300 gm were used. Creation of full-thickness wounds of $4{\times}4mm$ in size were performed on the hard palate. Beriplast$^{(R)}$, a wound dressing material, was applied immediately in the experimental group, but not applied in the control group. All wounds were protected with palatal resin splints. The animals were sacrificed on the 2nd, 4th, 7th, 14th, and 28th day after the operation for macroscopic and microscopic examinations. Results obtained were as follows ; 1. On the 7th day after the operation, epithelial proliferation was greater in the experimental group than that in the control group. 2. The inflammatory reaction of the experimental group was less than the control group on the 2nd and 4th day after the operation. Beriplast was resorbed on the 7th day after the operation. 3. In the control group, the epithelial proliferation occurred from the 7th to the 14th day after the operation, and in the experimental group, epithelial proliferation occurred from the 4th day after the operation. 4. On the 14th and 28th day after the operation, there was no prominent difference between the two groups in histological findings. These results suggest that the use of fibrin adhesive (Beriplast$^{(R)}$) as a palatal wound dressing results in greater epithelial proliferation and less inflammation in the early stage of wound healing.

The Effect of Micro-current Electrical Therapy on Muscle Atrophy and Delayed Wound Healing Process Induced Rat Caused by Traumatic Peripheral Nerve Injury (외상성 말초신경 손상으로 인한 창상 치유 지연 및 근위축이 유발된 소동물에서의 미세전류 자극 효능 평가)

  • This study was carried out to evaluate the effect of micro-current therapy on muscle atrophy and delayed wound healing process caused by traumatic peripheral nerve injury. For this, twenty-five 5-week-old Sprague Dawley rat were used and assigned to five groups including the normal group (NOR, n=5), the wound group (WD, n=5), the wounded and treated with micro-current electrical therapy group (WD+MET, n=5), the sciatic nerve denervated and wounded group (WD+DN, n=5), and the sciatic nerve denervated, wounded and treated with micro-current electrical therapy group (WD+DN+MET, n=5). In order to assess the changes in length of incisional wound for 12 days and the muscle volume for 2 weeks, the ImageJ analysis of macroscopic analysis and micro-CT data were obtained and analyzed. As a result, significant delay in the process in wound healing were observed and micro-current therapy suppress the postponement of healing process. Furthermore, there were significant changes in muscle volume between electrically treated group and non-treated group. These result shows that electrical stimulation may improve the delayed healing process and muscle atrophy at once.

Treatment of Diabetic Mouse Wounds using Blood Bank Platelet Concentrates (혈액은행의 혈소판농축액을 이용한 당뇨생쥐의 창상치유)

  • Purpose: Many clinical trials have shown the effectiveness of platelet releasate on chronic wounds. However, a large volume of blood must be aspirated from a patient and a platelet separator is required. Here, we hypothesized that platelet concentrate obtained from a blood bank (PCBB) would be also effective at stimulating wound healing. The purpose of this study was to investigate the effectiveness of PCBB on accelerating healing of diabetic wounds in vivo. Methods: Round wounds of 5 mm diameter were made at four sites(two wounds on the left and two on the right side) on the backs of nine diabetic mice. Three hundred million platelets suspended in 0.05 ml fibrinogen were dispersed on each wound on left sides. Same amount of fibrinogen without platelets was dispersed on right side control wounds. Thereafter, 0.05 ml thrombin was applied to the each wound. Ten days after wound treatment, healed wounds were excised and the extent of wound healing in each group was compared. Results: Quantitative histologic analysis of epithelial gap distances revealed that PCBB treatment had greatly accelerated wound healing. Mean epithelial gap distances for PCBB treated and control wounds were $2.5{\times}0.6mm\;and\;3.6{\times}0.5mm$, respectively(p<0.05). Conclusion: Our results suggest that PCBB has potential to accelerate the healing of diabetic wounds.

Optimal Concentration of Thrombin to Activate Platelet for Wound Healing (창상치유 목적의 혈소판 치료를 위한 Thrombin의 최적 농도)

  • Purpose: Platelet transplantation is a novel therapeutic strategy for acceleration of wound healing. When applying platelets, efficacy of adding thrombin to stimulate growth factor release from platelets has already been proved. However, no quantitative data of the thrombin treatment has been reported. The purpose of this study was to determine the optimal thrombin concentration to maximize growth factor release of platelets. In particular, this study was designed to quantify levels of platelet derived growth factor(PDGF)-BB, which is a major growth factor contained in the platelets, in vitro. Methods: Fresh platelets were obtained from a blood bank. They were suspended in DMEM/F - 12 and incubated with thrombin of various concentrations. The concentrations of thrombin tested were 0, 6.25, 12.5, 25, 50, 100, 200, and 400 IU/ml. After 30 minutes, 1, 3, 5, and 7 days, the levels of PDGF - BB were measured using enzyme linked immunosorbent assay. Platelets from four donors were included in this study. Each sample was tested in triplicate and the mean value was used as a data for each sample. Results: The addition of thrombin increased the level of PDGF - BB. Increases in storage time of platelets resulted in decreased levels of PDGF - BB. Higher levels of PDGF were detected in consort with increased thrombin concentrations. However, there was no significant difference between samples of 200 and 400 IU/ml concentrations. Conclusion: The results indicate that adding thrombin accelerates the release of growth factors from platelets and the optimal thrombin concentration to maximize this function is 200 IU/ml.

Effects of Chitosan on Wound Healing in Monkeys (원숭이에서 피부 창상 치유에 대한 키토산의 효과)

  • The objective of this study was to investigate the effects of implanted chitosan applied to surgically created wound in Japanese Macaque monkeys. 4 healthy Japanese Macaque monkeys were used. A 4 cm straight skin incision was made and undermined skin ($4{\times}4cm$) over on the 2 monkeys both sides of the dorsal midline, and a 4 cm circular skin incision was made on 2 monkeys both sides of the dorsal midline. One wound (left side) was implanted 1 mg (straight incision) and daily 0.2 mg (circular incision) of cotton type chitosan and the other wounds were treated with normal saline (3 ml) in monkeys. Each straight wound was closed with two interrupted sutures of 2-0 sutures. The monkey's circular skin incision is opened. At 14 days after initial wounding, each wound was taken for histological observations in monkeys. The inflammatory cells in the chitosan group are observed less than the control group, the collagen and the fibrin in the chitosan are observed more than the control group in monkeys. So the wound healing is moderately enhanced for chitosan treatment. The fibroblasts and the capillaries increased for chitosan treatment. The treatment of chitosan in wound is to promote healing.

Comparison of Bone Marrow Stromal Cells with Fibroblasts in Wound Healing Accelerating Growth Factor Secretion (골수기질세포 및 섬유아세포의 창상치유 촉진 성장인자 분비능 비교)

  • Cryopreserved fibroblast implants represent a major advancement for healing of chronic wounds. Bone marrow stromal cells, which include the mesenchymal stem cells, have a low immunity-assisted rejection and are capable of expanding profoundly in a culture media. Therefore, they have several advantages over fibroblasts in clinical use. The ultimate goal of this study was to compare the wound healing accelerating growth factor secretion of the bone marrow stromal cells with that of the fibroblasts and this pilot study particularly focuses on the growth factor secretion to accelerate wound healing. Bone marrow stromal cells and fibroblasts were isolated from the same patients and grown in culture. At 1, 3, and 5 days post-incubating, secretion of basic fibroblast growth factor(bFGF), vascular endothelial growth factor (VEGF), and transforming growth factor beta(TGF-${\beta}$) were compared. In TGF-${\beta}$ secretion fibroblasts showed 12~21% superior results than bone marrow stromal cells. In contrast, bFGF levels in the bone marrow stromal cells were 47~89% greater than that in fibroblasts. The VEGF levels of the bone marrow stromal cells was 7~12 fold greater than that of the fibroblasts. Our results suggest that the bone marrow stromal cells have great potential for wound healing accelerating growth factor secretion.
