Medical Services for Cesarean Section Cases in One DRG Pilot Study Hospital (질병군별 포괄수가제(DRG 지불제도) 시범사업에서 제왕절개산모의 의료서비스 - 서울시내 한 종합병원을 대상으로 -)

  • One Diagnosis Related Group(DRG) pilot study participating hospital was measured and analyzed to see if there were any changes after the DRG program. It was implemented in consideration of medical service utilization, hospital charges, and non-covered medical service charges by insurance in all Cesarean section cases by reviewing medical records for 3 years, including 1 year before pilot study as well as 1 and 2 years after, respectively. The results were as follows: First, the use of intramuscular antibiotics decreased statistically significantly, whereas intravenous use did not. Second, the administration period and charges of antianemic medication decreased significantly, where the prescription was appropriate. Third, the length of hospital stay decreased statistically significantly. Fourth, there were significant statistical differences in cost sharing between the insured and the insurer: cost sharing of the insured was reduced, whereas the share of the insurer increased. However, there was no change in the quality of care. Fifth, there were no statistically significant changes in the Cesarean section rate. As a result, if the fee schedule is reasonably high, hospitals can provide quality care. This DRG pilot study resulted expected outcomes: by paying a higher fee schedule than fee-for-service, then hospitals can provide quality care to their patients and increase hospital profits.

  • Sepsis and meningitis caused by Neisseria meningitidis are rare in neonates, but neonatal sepsis and meningitis are associated with a high rate of mortality. Meningococcal disease is commonly reported in older children and adolescents and is known to be more prevalent in community settings. In this study, a 16-day-old neonate was diagnosed with serogroup B meningococcal sepsis and meningitis. The baby was treated with antibiotics at the early stages of the infection and was discharged in good condition without any complications. This case report can serve to raise awareness of the incidence and importance of meningococcal infection in neonates, especially serogroup B.

A Study on the Food Habits and Attitudes of Cerebrovascular Accident Patients in Daegu S Medical Center (대구 S병원 뇌졸중 환자의 생활 습관 및 식습관에 관한 조사 연구)

  • Stroke is a serious disease despite recent improvements in the medical treatments available. Thirty-six stroke patients were interviewed as case and 36 non-stroke patients were interviewed as controls between February 2005 and August 2005 at Daegu S Medical Center. Information regarding the subjects' food habits was collected using the recall method from 5 years before the onset of stroke. Using interview techniques, we investigated how dietary habits and attitude influence nutrient intake. These results were analyzed by the $X^2$, Student's t-test and Chi-square test using the SPSS 12.0 program. This case-control study was performed to demonstrate the relationships among general quality factors (BMI, WHR, and family history of stroke) and lifestyle factors (smoking, exercise and alcohol drinking). The results were as follows. Analysis of the percentage of nutrient intake in terms of meal pattern showed that WHR was significantly higher in the case group than in the control group. Hypertension and diabetes were the most common preceding diseases associated with the risk of stroke. The frequencies of smoking, alcohol consumption (p<0.001) and exercise (p<0.05) were higher in the cases than in the controls. Thus, the findings of this study are consistent with those of previous studies and suggest that people should be advised to control hypertension, smoking, alcohol drinking and obesity in order to prevent the occurrence of stroke as these factors are major risk factors for stroke.

The Nutritional Status and Dietary Pattern by BMI in Korean Elderly (노인에서 체질량지수(BMI)에 따른 영양상태 및 식생활 태도)

  • This study was performed to document the association between obesity, a major risk factor for chronic diseases, and dietary pattern in Korean elderly. The subjects were 595 men and women aged 60-89 years. The subjects were classified into 4 groups based on BMI: under weight, BMI < 18.5, normal weight, 18.5 $\leq$ BMI 22.9; overweight, 23.0 BMI 24.9; and obese BMI $\geq$ 25.0. Dietary intakes by 24-hr recall, blood pressure, anthropometric parameters and health eating index (HEI) were measured. Underweight group was excluded for data analysis doe to small number of subjects, and age-adjusted measurements were compared among normal, overweight and obese groups. The mean anthropometric values for males and females were 23.7 and 24.8 kg/$m^2$ for BMI, 0.90 and 0.86 for WHR and 140.7 and 138.8 mmHg for SBP, respectively. The mean intakes of energy, Ca, vitamin A, vitamin B$_2$, and vitamin E did not meet Korean RDA for elderly. Intakes of fat and cholesterol were low: the percent energy from fat for male and female subjects were 19.1% and 18.1% and mean cholesterol intakes were 208 mg and 152 mg, respectively. Judging by HEI score, dietary quality was better in females than in male subjects. The mean BMI of normal, over and obese groups were 21.4, 23.9, 26.7 kg/$m^2$ in male subjects and 21.6, 23.9, 27.1 kg/$m^2$ in female subjects. WHR, SBP and TSF were increased with increasing BMI No association was found between BMI and nutrient intakes and/or food consumption pattern. However, a tendency was shown that the overweight group reported higher intakes in most nutrients compared to normal and obese groups. This study implies that with increasing BMI, anthropometric risk factors, such as WHR, TSF, and blood pressure were Increased, however, no significant differences were found in nutrient intakes and food patterns. Energy and fat intakes do not seem to be a cause for obesity in Korean elderly.

  • 한국 및 일본을 포함한 동아시아 지역에 벡터 매개성 바이러스에 의한 소 질병으로 인하여 경제적 손실이 몇년을 주기로 극심하게 나타나고 있다. 그러나 본 질병의 정확한 역학에 대하여는 아직까지 확실한 연구가 되어 있지 않은 상태이다. 본 연구에서는 벡터 매개성 질병에 대한 역학을 좀더 명확하게 밝히기 위하여, 일본 열도중 가장 멀리 떨어져 있으며, 아열대 지역에 속하는 오끼나와 섬에서 사육되고 있으면서 백신접종을 받지 않은 소(Japanese black cattle)를 대상으로 하여 1988년도부터 1992년도에 걸쳐 벡터 매개성 바이러스군에 대한 중화항체가를 측정하고 분석하였다. 소의 유행성 바이러스(bovine ephemeral virus)에 대한 항체 양성율은 연도 및 계절별로 크게 변화를 보였고, Ibaraki virus에 대한 항체양성율은 그 어느 다른 계절보다도 5월에 높은 항체가를 보였으며, Akabane virus에 대한 항체 양성율은 이바라키 바이러스에 대한 항체 양성율과 유사한 수준으로 나타났다. 한편 Chuzan virus에 대한 항체양성율은 계절적인 변화가 심하지 않은 것으로 나타났다.

Infectious Bursal Disease-A Review (전염성 훼브리셔스낭병)

  • Infectious bursal disease, so called Gumboro disease, is found world-wide in areas of intensive poultry farming. The clinical signs of the disease are very indicative, but most infections occur unnoticed due to the age of infection of chicken as well as the degree of virulence of virus affected. Edematous and hemorrhagic lesions in BF at early course of infection and the complete atrophies of BF in later are the most characteristic. The infection is considered highly contagious by direct contact, by fecal material and by contaminated feed and water. The virus is also highly resistant in environment and belongs to Diploma virus with size of 55 to 60nm of Ribovirus group. IBDV grows in embryos, embryonic cells and BF of susceptible chickens. Immune-diffusion using agar gel is the method of a choice to determine IBDV infection in chickens. Maternal immunity is very effective in protecting chickens of critical age when IBDV infection severely damages the function of BF. Immunosuppressive effect of IBDV causes more production losses than direct effects of clinical disease of IBD. Inclusion body hepatitis, infectious anemia and gangrenous dermatitis syndrome are the disease associated with the immunosuppressive condition of chickens.

A Literature Review of Clinical Trials on Alternative Medicine (대체의학 임상실험 현황에 대한 문헌고찰)

  • The aim of this study is to know about condition of alternative medicine. Methods which An literature search in Pubmed January 2000 to March, 2010 was performed. RCT(Randomized Controlled Trial), CCT(Controled Clinical Trials), and OD(efficacy studies with either a controlled or an Other than controlled Design) were included. PubMed databases were searched using key terms relating to alternative medicine. 19 clinical trials were included among 682 studies searched. RCT was 44%, CCT was 16% and OD was 40% in the review. In the all of included studies, the acupuncture and the massage was the biggest part. The using rate was 20% of pain control about Disease of the Musculoskeletal system and Connective tissue, 15% of Certain infectious and parasitic diseases, and 15% of Neoplasms. There was lack of evidence that alternative medicine was effective for progress. Future well-designed clinical trials is needed.

The Effects of Planned Discharge Nursing in Compliance, Knowledge, and Nursing Service Satisfaction for Patients having Video-Assisted Thoracoscopic Surgery (계획된 퇴원 간호 중재가 기흉 수술 환자의 약물복용 및 치료 지시 이행도, 질병에 대한 지식과 간호 만족도에 미치는 효과)

  • This study aimed to develop a planned discharge nursing intervention guideline for patients with pneumothorax, and to examine its influence on drug compliance, knowledge about disease, compliance level with therapeutic regimen, and nursing satisfaction. Methods: The planned discharge nursing intervention guideline was developed based on comprehensive literature reviews and clinical experiences. Patients having video-assisted thoracoscopic surgery at a single general hospital were allocated to either intervention (n=30) or control group (n=29). Participants in the intervention group were administered planned discharge nursing intervention thrice, 30 minutes each, by a cardiovascular nurse. Standard care was provided to patients in the control group. Data were collected from March, 2010 to December, 2010. Results: Participants in the intervention group presented statistically significant improvement in drug compliance (t=-2.05, p=0.044), pill count (t=-2.61, p=0.011), knowledge about disease (t=-4.39, p=0.001), and nursing satisfaction (t=-4.13, p=0.001). No significant difference in compliance levels was observed with standard therapeutic regimen. Conclusion: Planned discharge nursing intervention can be successfully implemented for patients undergoing thoracoscopic surgery. Further research is required to evaluate long-term effects like complication or relapse.

Evaluation of Clinical Usefulness of EIS(Electro Interstitial Scan) (EIS(Electro Interstitial Scan) 방법의 임상적 유효성 연구)

  • Electro interstitial scan shows potential as a non-invasive screening method. It can discriminate some diseases based on electric current response to induce low intensity direct current to limbs or local area of body. DDFAO was invented in France and it is claimed that multi-channel EIS(Electro Interstitial Scan) is useful for various diseases, especially, diagnoses of endocrine system such as diabetics are very effective. In this study, we verified the repeatability and sensitivity of DDFAO by using a RC phantom model and its clinical usefulness using data obtained from normal and diabetes subject groups. As a result, it showed the repeatability and the output change according to change of phantom characteristic, but it was hard to distinguish normal and patient groups non-invasively with just six surface electrodes of DDFAO. The repeatability and the clinical accuracy was not sufficient for screening or diagnostic purposes, as well. In spite of the results with low repeatability and accuracy conducted in this study, we still need further investigations to improve the EIS-based measurement method; EIS is very convenient and simple and it shows potential as a screening tool of the whole body health conditions rather than localized disease diagnosis.

Clinical Significance of Serum Amylase Measurement in Sjögren's Syndrome (쇼그렌 증후군에서 혈청 amylase 측정의 임상적 의의)

  • Serum amylase is a representative enzyme secreted by the salivary gland and pancreas. This study investigates the clinical significance of serum amylase levels in Sjögren's syndrome (SS). Totally, 70 female subjects were enrolled, who were diagnosed as SS and had no accompanying afflictions that affected the serum amylase levels. Unstimulated salivary flow rate (U-SFR) and stimulated SFR (S-SFR), salivary gland scan, and disease activity markers (ESSDAI and ESSPRI), as well as blood tests including ESR, CRP, and amylase, were evaluated. Serum amylase showed significant positive correlation with the U-SFR and S-SFR, and was increased with higher ejection fraction (EF) of the parotid gland. However, there was no significant correlation with disease activity and inflammatory markers. Based on their average amylase levels, subjects were divided into two groups. The group with higher serum amylase levels showed a statistically significant increase in the S-SFR and EF of the parotid gland. Considering the results of the salivary gland scan, we conclude that serum amylase is significantly correlated with SFR and the EF of the parotid gland, thereby indicating that the salivary gland function remains intact in SS.
