The Single-Session Group Education for Advanced & Terminal Cancer Patients and their Family Members (진행암 및 말기암 환자와 가족을 위한 집단 교육 프로그램)

  • Purpose: This research aims to assess the effect of a single session of group education of hospice program at Seoul National University Hospital for the advanced and terminal cancer patients and their family members. Methods: Response to questionnaires from 89 participants were evaluated using SAS and CHISQ analysis. The questionnaires included the following items: 1) the characteristics of participants; 2) the characteristics of patients; 3) the difficulties of patient care; 4) the satisfaction with the program Results: The participants consist of 33 patients (37.5%) and 56 family members (56.2%). Diagnosis included mainly stomach, lung, breast, and colon cancer. Participants of family members consisted of spouse, parent, children, daughter-in-law, and siblings (in decreasing order). The participants were interested in the medical information, nutrition, pain and symptom management, and psychosocial adaptation. They had difficulties in emotional problem, nutrition and symptom management. Even though it was a single session of group education, the level of satisfaction was high (95%). Conclusion: This research shows that even the single session of the group education for the advanced and terminal cancer patients and their family members is very helpful by giving them the necessary information. In order to develop comprehensive care-giving services, more specific informations, more opportunities to participate in such sessions and longer question-answer time is required.

  • Total 28 patients with resectable, locally advanced gastric cancer were entered in our prospective randomized study from June 15, 1988 to Sep. 15, 1990 in Yeungnam University Hospital. This study consisted of curative resection, IORT, external irradiation and combination of chemotherapy. Twenty-four of 28 patients were treated with single dose of 1500 cGy with 9 MeV election intraoperatively. External irradiation of $4300\sim4500$ cGy with 180 cGy per fraction, 5 days per week was started within 4th weeks of postoperative days. Various chemotherapy with or without external irradiation were added for reducing hematogenous and/or peritoneal dissemination and determination of complication of each arm. Duration of follow up was $4\sim31$ months. No serious complication related with radiation were reported compare to resection and chemotherapy only group. Although our follow up period is too short to draw any conclusion, IORT appears to improve local control, hopely further survival. Continuous follow up should be needed for evaluation of real therapeutic gain such as complication vs. improved survival.

  • The majority of choriocarcinomas occur in the uterus as gestational malignant tumors. Rarely, a choriocarcinoma appears in the gastrointestinal tract, and the tumor is assumed to arise from a different histogenetic origin as compared to tumors of other sites. A primary gastric choriocarcinoma is a rare aggressive, widely metastatic malignant tumor, and has a poor prognosis. Reported here is a case of a 69-year-old woman with a primary gastric choriocarcinoma who presented with melena, epigastric pain, and was diagnosed with a poorly differentiated adenocarcinoma based on a preoperative endoscopic biopsy. Gastrectomy with lymph node dissection, followed by postoperative chemotherapy, is the treatment of choice. Therefore, in the case of a poorly differentiated adenocarcinoma with a bleeding tendency, a meticulous examination with the suspicion of a choriocarcinoma should be undertaken.

  • The origin of a submucosal tumor is difficult to determine by endoscopic biopsy. One type of submucosal tumor, which originates in Schwann cell, a schwannoma represents 0.2% of all gastric tumors. We experienced the case of a 57-year-old female patient with a gastric schwannoma presenting with melena and anemia. Computed tomography and upper gastrointestinal endoscopy showed a protruded huge mass from 3 cm below the cardia to angle. Seven endoscopic biopsies indicated only necrotic debri and granulation tissue, chronic gastritis. Because we suspected an advanced gastric cancer, or lymphoma with bleeding, It was performed an operation for an exact diagnosis and a treatment for bleeding. A-post-operative histopathological, immunohistochemical examination led to the final diagnosis of a gastric schwannoma.

Induction of Apoptosis in Human Cancer Cells with Extracts of Taraxacum coreanum, Youngia sonchifolia and Ixeris dentate (흰민들레, 고들빼기, 씀바귀 추출물의 암세포 증식 억제 효과)

  • This research examined the effects of Compositae extract on the inhibition of proliferation and apoptosis in human breast and human gastric cancer cells. Compositae extracts which is used in the experiment are Taraxacum coreanum (TC), Youngia sonchifolia (YS) and Ixeris dentata (ID). The proliferation of SK-BR-3, MDA-MB-231 and AGS cells were investigated by MTT assay. ID and YS extracts inhibited proliferation of SK-BR-3, MDA-MB-231 and AGS cells in a dose-dependent manner, but TC have barely affected. In addition, the most effective extract was ID. To assess the apoptosis of ID extract, the nuclei of human cancer cells were stained with DAPI solution respectively. Chromatin condensation, indicated apoptosis, was increased in a dose-dependent manner. We investigated change of ID extract-induced apoptosis proteins on human cancer cells by western blot analysis. The level of Bcl-2 decreased, whereas the level of Bax, cleaved-PARP increased in dose-dependent manner compared with non-treatment. Also Bax/Bcl-2 ratio, which is used in clinical indicator of apoptosis, was increased at ID extract treatment group compared with non-treatment. Moreover the Bax/Bcl-2 ratio of MDA-MB-231 cell was significantly increased as against SK-BR-3, AGS cells. These results indicated that ID extract have anti-proliferation effect better than YS or TC, and induced apoptosis in human breast cancer MDA-MB-231 cell better than human breast cancer SK-BR-3 cell, human gastric cancer. Even if further research is needed, ID can be developed as a chemopreventive or therapeutic agent of breast cancer.

Antioxidant Activity and Anticancer Effects of Rough Rice (Oryza sativa L.) by Germination Periods (발아 기간에 따른 벼(Oryza sativa L.)의 항산화활성과 in vitro 항암활성)

  • This study was conducted on the antioxidant activity and in vitro anticancer effects of rough rice (Oryza sativa L.) by germination periods. Rough rice was germinated at $37^{\circ}C$ for 8 days and then extracted with 70% ethanol. It was then analyzed for total polyphenol content, reducing power, antioxidant activities, and in vitro anticancer effects. Total polyphenol content increased from 3.12 mg/g for raw rough rice to 4.05 mg/g at 4 days of germination. Also reducing power increased from 0.96 to 1.25 (p<0.05). DPPH radical scavenging activity increased from 29.25% at 0 day to 34.82% at 2 days. ABTS radical scavenging activity increased from 3.05 mg AA eq/100 g at 0 day to 3.84 mg AA eq/100 g at 4 days, and then decreased afterward. The anticancer effect of ethanol extract at 4 days on stomach cancer cell line (AGS) and colon cancer cell line (HCT-116) showed higher values compared with raw rough rice extract. These results suggest that the best germination periods for increasing biological activities may be 3~4 days.

Surgical Option for Sufficient Safety Margine in Locally Advanced Type II Cardia Cancer - Left Colon Interposition (국소 진행된 Type II 분문부 선암의 절제연 확보를 위한 수술 방법: 좌측 대장 간치술)

  • Purpose: Radical surgery is the standard therapy for patients with resectable cardia cancer. In the case of type II disease with esophageal invasion, a transhiatal extended radical total gastrectomy is needed or a gastroesophagectomy through an abdomino-thoracotomy, depending on the extent of the esophageal invasion. We analyzed the indications and outcome of left colon interposition as an esophageal substitution. Materials and Methods: Between 1 January 1994 and 31 December 2006, 10 patients underwent left colon interposition after gastroesophagectomy through an abdomino-thoracotomy or the tanshiatal approach for type II cardia cancer at the Department of surgery, Yonsei University College of Medicine. The outcomes of these patients were reviewed and compared, with those who underwent a Roux-en-Y, by gender and age matched analysis, retrospectively. Results: There were nine males and one female with a mean age of 52.5 (range, 16~72). The operation time was $449.00{\pm}87.39minutes$. The mean distance between the proximal resection margin and the cancer was $6.56{\pm}3.65cm$; the maximum size of the tumor was $9.90{\pm}3.97cm$. These measures differed significantly from patients who underwent Roux-en-Y. The patients had a double primary cancer in the cardia and esophagus. There were no events of colon necrosis. However, a pneumothorax occurred in one patient (10%) and a proximal anastomotic stricture occurred in one patient. There were no reports of heartburn, regurgitation, thoracic or epigastric fullness, and one patient even gained weight, 16 kg. Conclusion: Colon interposition after esophagogastrectomy was safe and effective and should be considered as an additional surgical option for locally advanced type II cardia cancer patients with esophageal invasion.

Classification and Clinical Implications of Precancerous Lesions in the Stomach (위에서 전암병변의 분류와 임상적 의의)

  • During carcinogenesis, precancers (premalignant lesions) are the morphologically identifiable lesions that precede invasive cancers. In theory, the successful treatment of precancers would result in the eradication of most human cancers. Despite the importance of these lesions, there has been no effort to list and classify all of the precancers. In 2001, the NCI sponsored a workshop on the classification of precancers. When considering all the possible classes of precancers, it is worth noting that not all precancers are neoplastic. In fact, precancers need not progress to cancer, and precancerous lesions often have a high rate of regression. Thus, the following five classes were adopted: 1) acquired microscopic precancers; 2) acquired large lesions with microscopic atypia; 3) Precursor lesions occurring with inherited hyperplastic syndromes that progress to cancer; 4) acquired diffuse hyperplasias and diffuse metaplasias; and 5) currently unclassified entities. In this review paper, precancerous lesions of the stomach are classified and their clinical significance is described.

Importance of integrating Bioinformation and Health Informatics for Healthcare

  • 유전체연구사업단은 국내에서 발병 및 사망빈도가 가장 높은 위암과 간암의 퇴치를 목적으로 국가적 특목전략사업으로 연구를 추진하고 있다. 이와 별도로 보건복지부에서는 22개의 중요 질병별 유전체 연구센터를 전국적으로 추진하고 있다. 따라서, 연구가 성공적으로 진행되면 각 연구소에서 독자적으로 개발하여 제공하는 생명정보의 양은 거의 무한에 이를 것이다. 그러나 생명정보는 환자진료에 도움을 주기 위해서는 궁극적으로 임상정보와 함께 유기적으로 통합되어야 한다. 임상정보와의 통합을 위해서는 의료기관의 진료정보와 연구소의 생명정보가 연계되어 엄밀한 임상실험이 추가적으로 실시되어야 한다. 뿐만 아니라 생명정보학의 발전을 위해서는 연구대상의 임상정보가 공유되어야 한다. 유전체정보를 이용하는 생명정보학(Bioinformatics)은 각 국가마다 전략사업으로 간주하여 막대한 투자가 이루어지는 새로운 분야이다. 현재 선진국에서 개발 사용 중인 시스템의 연간 사용료가 고가이므로 국내 도입은 거의 불가능하거나 또는 매우 비효율적이다. 유전체 또는 생명정보의 임상활용 및 생명정보연구를 위한 임상정보 공유를 위해서는 우선 다음의 사항이 개발되어야 한다. 1) 다음과 같은 개별환자의 정보를 각 의료기관에서 제공 받아 저장 활용한다. - 진찰 및 임상소견, 수술기록, 경과기록, 검사결과 (임상병리, 해부병리, 방사선 등), - 영상정보 (X-ray, CT, MRI, 초음파, 전자현미경, 그래픽 등), - 환자개인기록(병력, 과거력, 가족력, 알러지 등), - 예방접종 기록 2) 각 연구소에서 첨단기술을 이용하여 개발되는 생명정보를 임상에 활용하기 위해서는 유전체연구센타와 병원간에 임상정보와 유전체 분석정보의 공유가 필수적으로 발생하게 됨으로, 유전체 정보와 임상정보의 통합은 미래 의료환경에 필수기능이 될 것이다. 3) 각 생명공학 연구소에서 사용하는 첨단 분석 장비와 생명공학 정보시스템의 자동 연계가 필요하다. 현재 국내에는 전국적인 초고속정보망이 가동되어 웹을 기반으로 하는 생명정보의 공유는 기술적으로 문제가 될 수 없으나 임상정보의 유전체연구에 그리고 유전체연구정보의 임상활용은 다양한 문제를 내포하고 있다. 이에 영상을 포함한 환자정보의 유전체연구센터와 병원정보시스템과의 효율적인 연계통합 운영을 위해 국내에서는 초기 도입단계에 있는 국제적인 보건의료정보의 표준인 Health Level 7 (textural information 공유), DICOM (image 및 wave 공유), 관련 ISO표준, WHO의 ICD9/10 (질병분류), LOINC (검사 및 관련용어), SNOMED International (의학용어) 등을 활용하여야 한다.

  • Objectives : This study was designed to evaluate the effects of traditional oriental therapies(TOT) on advanced gastric cancer(AGC) patient with metastasis to liver. Methods : We retrospectively analyzed the medical record of a case of AGC patient in stage IV with metastasis to liver who had been treated with TOT for 5 months continuously in East-West Cancer Center of Oriental Hospital of Daejeon University from 5 March 2003 through 22 July 2003. Results : After treated with TOT for the 5 months, most of symptoms were disappeared or improved and tumor size was decreased on abdominal CT compared with the previous result. The hematological results and tumor markers were not increased. Conclusions : From the above results, it could be suggested that TOT have effects on improving symptoms and quality of life as a supportive or curative therapy for AGC patients.
