In this study, we evaluated the usefulness of an automatic blood typing analyzer using QWALYS-2 up (Diagast, Loos Cedex, France). During a month( 01OCT2013 - 31OCT2013) we performed 1,636 tests for ABO & RhD blood typing, 1,374 tests for antibody screen & identification tests and compared the results by automatic blood type analyzer with previous manual methods and column agglutination tests. And we analyzed the economic performance by comparison the test unit price between automatic blood type analyzer and manual methods. In ABO & RhD blood typing tests, there were complete concordances between manual and automated blood typing analyzer for 200 clinical samples. In Antibody screen tests, the concordance rate between manual and automated blood typing analyzer was 98.5% and more strong reaction in automated blood typing analyzer than manual methods. Therefore, the introduction of an automated blood typing analyzer, reagents costs were increased but labor costs were decreased. Considering the importance of transfusion safety and economic advantages, the introduction of an automated blood typing analyzer was very useful.

병원에서 사용되는 진단검사는 고가의 분석기를 사용하거나 육안으로 비교표와 확인해야한다. 또한 환자의 진단 기록과 근거가 유지되지 않는 문제가 있다. 이런 문제를 해결하고자 디지털 데이터화된 의료정보를 바탕으로 통합적인 진단검사 처리 환경을 제공하는 의료 정보처리 시스템을 설계 및 구현하였다.
Purpose: In most cases, the main cause of chest or back pain during playing golf is rib fractures. This kind of rib fractures can be easily overlooked in plain radiographs. The purpose of this article is to clarify the value of the ultrasound in detecting rib fractures. Materials and Methods: We compared the findings of chest-X rays (anteroposterior, left or right lateral, both oblique view) with those of the ultrasound in 56 patients between the ages of 20th & 60th who were diagnosed rib fractures. Their chief complaints were chest or back pain which developed during playing golf. The results were analyzed statistically. Results: Among 56 cases of rib fractures, 34 cases were diagnosed with initial X-rays. 51 cases were diagnosed with ultrasound. The sensitivity of radiograph and ultrasound were 60.71% & 91.07%, respectively. Dignosis was ultrasound with statical significance whereas radiography did not(p=0.0004). Conclusion: It was concluded that ultrasound is a valuable tool in the diagnosis of the rib fractures which were undetected with X-rays during playing golf.
Lutheran a antigen ($Lu^a$) is detected in 6 to 8% of Caucasians and Africans. In Korean and other Asian populations, it is very rare or nearly absent. Therefore, although $Lu^a$ has a considerable immunizing capacity, sensitization to $Lu^a$ is a rare event. Here we report on a rare case of anti-$Lu^a$ in a 70 year-old female patient with Lu (a-/b+) phenotype and review the relevant literature. Due to the paucity of $Lu^a$ positive panel cells in antibody screening and identification tests, detection of this rare antibody to $Lu^a$ antigen is not feasible. Therefore, we should keep in mind the possibility of the misleading false negative result in detection of antibody to this low incidence antigen.
Between March 1999 and January 2000, 82 boys with the diagnosis of inguinal hernias (12 bilateral and 70 unilateral hernias), underwent Ultrasound (US) examination of both sides of the groin, a total of 164 inguinal imaging prior to surgery. The patients ages ranged from 3 days to 12 years with a mean of 32.6 months. Ninty four examinations were on the clinically symptomatic side and 70 were on the asymptomatic side. The US criteria for the diagnosis of an inguinal hernia were as follows: 1) visceral hernia, the presence of bowel loops, or omentum in the inguinal canal, 2) communicating hydrocele, the presence of fluid in the processus vaginalis, 3) widening of patent processus vaginalis at the level of internal inguinal ring. The width of patent processus vaginalis at the level of internal inguinal ring over 4 mm is considered an occult hernia. Among the 94 symptomatic groins, US findings showed 31 (33%) visceral hernias, 18 (19%) communicating hydroceles, and 38 (41%) widening of the internal inguinal ring, and 7 (7%) groins without abnormalities. In 70 asymptomatic groins, there were 4 (6%) visceral hernias, 5 (7%) communicating hydroceles, 11 (16%) widening of the internal inguinal ring, and 50 (70%) groins without abnormalities. Among the 70 asymptomatic groins there were US abnormalities in 20 (28%). One hundred and seven groins with positive US findings were surgically explored. Among 107 operated sites, the operative findings were compatible with the US diagnosis in 104, a sensitivity for US of 97.2%. In patients with US findings of widening of internal inguinal ring (>4 mm), there was patent processus vaginalis in 36 out of 38 symptomatic groins and 10 of 11 asymptomatic groins. The sensitivity of US to the operative findings in widening of internal inguinal ring was 93.8%. For visceral hernia and communicating hydrocele, the sensitivity of positive US findings was 100%. Ultrasonography for inguinal hernias appears to be a rapid, reliable, and noninvasive screening diagnostic tool with high positive specificity. Therefore, we recommend the use of US as a routine diagnostic tool in pediatric patients with inguinal hernias and hydroceles.
This study was performed to evaluate diagnostic sensitivity and specificity of sonography on clonorchiasis. During the 9 months from March to November 1994, sonography skin test, stool examination, and medical examination were performed to 609 volunteers of Pohang which is located along the Hyongsan-gang (Rivers and is one of well-known endemic areas of clonorchiasis. The sensitivity of sonography was either 21% if the intrahepatic ducts dilatation was assumed to be the only positive anding or 52% if the periductal echogenicity was also included as one of positive findings. The sensitivity of skin test was 62%. The sensitivity was 46% if the diagnostic criteria were arbitrarily assumed as being positive in skin test and having either sonographic findings. The sonographic finding of fusiform, non-shadowing, weakly echogenic foci in gallbladder or in billiaiy tree indicating the flukes or aggregates of flukes was not obvious in this study. The specificity of sonography was either 95% if the intrahepatic ducts dilatation was assumed to be the only positive finding or 82% if the periductal echogenicity was also included as one of positive findings. The specificity of skin test was 52%. The specificity was 90% if the diagnostic criteria was arbitrarily assumed as being positive in skin test and having either sonographic findings.
We extracted 50 LVH patients out of 30'~80's who performing ECG and echocardiography examination. We used Devereux's theory to examinate LVH with echocardiography and used Sokolow-Lyon's theory to examinate LVH with ECG. We used regression and correlation analysis by SPSS, used ROC curve analysis to decide predominance of two ways of .Age, BMI, SBP and DBP whice are the danger factors of LVH and standard value of LVH diagnosis examination seems correlated. Out of 50 LVH patients, 50 patients were diagnosed LVH by echcardiography examination and only 21 patients were diagnosed LVH by ECG examination. Also echocardiography was AUC 99%, sensitivity 96%, singularity 95%, accuracy 95.5%. And ECG was AUC 76%, sensitivity 62%, singularity 76%, accuracy 68%.By comparing accuracy between echocardiography and ECG in diagnosing LVH, we could tell echocardiography was examination with higher accuracy. Therefore, if one was diagnosed with summit on 1st examination with ECG, considering age, body mass index, systolic blood pressure and dilator blood pressure, should offer echocardiography examination.

Pseudohypoparathyroidism (PHP) is a rare disorder caused by genetic and epigenetic aberrations in the GNAS complex locus resulting in impaired expression of stimulatory G protein ($Gs{\alpha}$). PHP type Ib (PHP-Ib) is characterized by hypocalcemia and hyperphosphatemia due to renal resistance to the parathyroid hormone, and is distinguished from PHP-Ia by the absence of osteodystrophic features. An 11-yr-old boy presented with poor oral intake and cramping lower limb pain after physical activity. Laboratory studies revealed hypocalcemia, hyperphosphatemia, and increased parathyroid hormone levels. The GNAS complex locus was evaluated using the methylation-specific multiplex ligation-dependent probe amplification (MS-MLPA) assay. Gain of methylation in the NESP55 domain and loss of methylation in the antisense (AS) transcript, XL, and A/B domains in the maternal allele were observed. Consequently, we present a case of PHP-Ib diagnosed using MS-MLPA.

Compressive neuropathy in the upper extremity can be clinically diagnosed by careful history taking, physical examination of the involved nerve. Electrodiagnosis for the suspected nerve informs severity of compression of the involved nerve and indicates specific site of the lesion. In the early stage of the disease, non-operative treatment generally cures the symptom, however, if the conservative treatment fails, confirmation of the exact site of the lesion should be preceded before the operation. Recently, ultrasonography, as a supportive tool for the diagnosis of compressive neuropathy has increasing popularity for its ability to find space occupying lesion, anatomical change of the nerve, and the pathologic change in the nerve itself. For the successful treatment of the compressive neuropathy, these various diagnostic tools have to be introduced in the orthopaedic clinic.
