The Experiences of Thyroid Cancer Patients Receiving Radioactive Iodine Therapy (갑상선암환자의 방사성요오드 치료 경험 분석)

  • The purpose of this study was to investigate the perceived difficulties during both pre-treatment period and admission period for radioactive iodine therapy (RAIT), the level of information about RAIT provided by medical staffs and the satisfaction of RAIT process in thyroid cancer patients receiving RAIT. Participants were 165 thyroid cancer patients, who had total thyroidectomy and had been offered RAIT. The data were collected using structured questionnaire and open-ended questions and analyzed by descriptive statistics and content analysis. The most frequent perceived difficulties of RAIT was associated with thyroid specific symptoms during both pre-RAIT period and RAIT admission period(38.2% vs 43.0%). The rating of RAIT information provided by medical staffs was evaluated as over moderate level(mean $3.63{\pm}0.80$). The RAIT process satisfaction was investigated as moderate level(mean $6.43{\pm}2.21$). the satisfaction of RAIT was the highest in medical attention and coping procedure (mean $7.64{\pm}2.37$) however it was the lowest in adequacy of RAIT information(mean $5.67{\pm}2.78$). Thus the nursing intervention program for thyroid cancer patient undergoing RAIT should be developed to support needs of thyroid cancer patients related to thyroid specific symptoms and to improve patients' understanding about RAIT procedure so that patients have competence to participate in therapeutic activities efficiently.

Influencing Factors on Stress in Caregivers of Stroke Patients Being Admitted in Rehabilitation Centers (재활전문병원에 입원중인 뇌졸중 환자 주 돌봄 제공자의 스트레스 영향요인)

  • The aim of this descriptive correlation research was to identify the factors influencing stress in caregivers of stroke patients in rehabilitation centers. The data were collected from 200 caregivers at three rehabilitation centers in B City from September 1 to 30, 2015. The data were also collected utilizing the Connor-Davidson Resilience Scale, the Caregiving Mastery Scale, the Medical Outcomes Study (MOS) Scale, and Burden Interview (BI) Scale. The data were analyzed using the SPSS Win 18.0 program. The factor with the greatest impact on stress was social support, which accounted for 9.3% of stress, followed by the duration of giving care to the patient, religious status, economic conditions, caregiver's age, resilience, caregiver's health status, patient's conscious status, and patient's age, all of which accounted for 30% of the stress for caregivers. Therefore, it is essential to develop social support programs that can reduce the stress for caregivers of patients in rehabilitation centers and to develop stress intervention programs, taking all the factors affecting stress into consideration.

Nurses' Perceptions of Person-Centered Care in Long-term Care Hospitals: Focus Group Study (인간중심돌봄에 대한 요양병원 간호사의 인식: 포커스 그룹 연구)

  • This study was conducted to understand and describe Korean nurses' perception of person-centered care for elderly patients in long-term care hospitals. Qualitative data were collected through four focus group interviews consisting of 21 nurses working in four long-term care hospitals in Jeolla and Gyeongsang provinces. Participants completed interviews from July 19 to 30, 2018. All interviews were recorded, transcribed, and analyzed by employing the thematic analysis method. Six main themes for the attributes of person-centered care for elderly patients in the long-term care hospitals were conceptualized: respecting individual needs, walking to the end, supporting hidden dreams, becoming a family partner, helping patients live like they are at home, and changing culture. Person-centered care perceived by nurses was conducted to provide individualized nursing according to elderly's preferences and help them discover the value and meaning of life through various activity programs. Nurses also recognized person-centered care to maintain cooperative relationships with their family members and share their decision-making process, as well as to form a physical environment and organizational culture that respects the rights and autonomy of the elderly. Based on the results of this study, it is necessary to identify the diverse needs of the elderly and develop nursing intervention programs based on person-centered care.

Effects of Korean Elder's Four Major Pains on Suicidal Thought Mediated by Depression: Focused on Gyungrodang Users (노인의 사중고(四重苦)가 우울을 매개로 자살생각에 미치는 영향: 경로당 이용자를 중심으로)

  • The present study empirically confirmed Korean elder's four major pains consisted of poverty, disease, role loss, loneliness and investigated the mediating role of depression between the four major pains and the elder's suicidal thought. To investigate the cause and effect of factors, we conveniently collected 309 samples from 16 Gyungrodangs evenly located in Jeonju and 291 samples, survived the data cleaning such as missing values, outliers, normality and covariance conditions, were analyzed by frequency, factor analysis, reliability, confirmatory factor analysis and structural model analysis. Followed were the selected contributions of the present study. First, the constructs of four major pains such as poverty, disease, role loss, loneliness were predictors of suicidal thought mediated by depression. Second, the elder's poverty, that was the heaviest factor of the four major pain constructs, was a predictor of role loss leading to loneliness. Third, four major pains were predictors of the elder's depression. Note that poverty were not direct but indirect predictor of depression. The present study confirmed the concept of four major pains. Also those who practice in the area of the elderly care should consider the four major pains as well as depression while intervening in the elderly's suicidal thought.

Development of Caregiver Guideline for Participation in Activities of Daily Living for the Elderly with Early Dementia: Focusing on the Delphi Survey (초기 치매 노인의 일상생활 참여 촉진을 위한 보호자 지침의 개발: 델파이 조사를 중심으로)

  • The purpose of this study was to develop the caregiver guideline for participation in activities daily living for the elderly with early dementia in home including easily adaptable and professional strategies as early intervetion. The process of this study was 3 stage. First stage, the development of preliminary caregiver guideline as the first stage of this study was to translate to Korean and reclassify the items about caregiver guideline for participation from reports of foreign association or government related dementia, and it consisted of 3 areas, 128 statements for questionnaire for Delphi investigation. Second stage, the guideline was to conduct the content validity, and delete, reclassify, integrate, revise inappropriate items through 2 rounds and 16 Delphi panels. Third stage, the establishment of the final version of caregiver guideline. It consisted of 8 areas: home modification and assistive device, home activities of daily living, health management, communication, psycho-emotional support, leisure activities, social participation, general strategies. All 68 items was arranged in important order. The content validity, stability, agreement index in this study were 0.81, 0.15, 0.79 respectively. When content validity, stability was above 0.49, below 0.5 in Delphi survey with 15 panels, it was not required additional survey. The result of this study meaned that it was not required to additional Delphi survey, and the result was stable and agreeable. This developed guideline was useful and practical to maintain the elderly's with dementia independent and healthy life as education materials for their caregivers, so it will expect to decrease caregiver's burden. Lastly, it stated limitation and suggestion for further study.

The Effects of an Emotional Intelligence Development Program on the Stress Recognition and the Stress Coping of Elementary School Children (정서지능 향상 프로그램이 아동의 스트레스 인식과 스트레스 대처에 미치는 영향)

  • The purpose of study was to examine the effects of an emotional intelligence development program on the stress recognition and stress coping of elementary school children. The subjects of this study are 24 fourth grade students who were selected based on the level of their emotional intelligence and stress recognition(level under the mean). They were divided into an experimental group and a control group, and each group had 12 students. The quantitative results of this study are as follows: First, the experimental-group increased in the level of emotional intelligence and showed a significant increase in the sub-areas of emotional intelligence(emotional recognition and expression, thought promotion) than the control group. Second, the experimental group decreased in the level of stress recognition and showed a significant decrease in the sub-areas of stress recognition(parents, family environment, friends, schoolworks) than the control group. Third, the experimental group improved in stress coping and showed a significant improvement in the sub-areas of stress coping(active coping, passive/avoidant coping, and social support seeking coping) than the control group. This study shows that emotional intelligence development program can be an effective tool for the change of stress recognition and stress coping of elementary school children.

Effects of Physical Activity Practice Rates and Knowledge Related to Cardiocerebrovascular Disease Prevention on Health Behavior Case Study Focusing on Middle Aged Women with Risk of Central Obesity (중심비만 위험인자를 가진 중년여성의 신체활동 실천율, 심뇌혈관질환 예방관련 지식이 건강행위에 미치는 영향)

  • This study was conducted to evaluate the effects of physical activity practice rates and knowledge related to cardiocerebrovascular disease prevention on the health behavior of middle aged women. Data were collected from Oct to Nov 2017 from 142 middle-aged women living in 24 Eup, Myeon, and Dong areas in North Gyeongsangbuk-do Province using a structured questionnaire. The obtained data were analyzed using descriptive statistics, t-tests, ANOVA, Pearson's correlation, and stepwise multiple regression analysis. The major factors influencing health behavior were found to be alcohol consumption (${\beta}=0.15$, p=0.009), diet (${\beta}=0.16$, p=0.003), vigorous intensity (${\beta}=0.14$, p=0.011), marriage (${\beta}=0.19$, p<0.001), interest in one's own health (${\beta}=0.23$, p<0.001), and health recognition (ill: ${\beta}=0.31$, p<0.001). Alcohol consumption and diet were factors of cardiocerebrovascular knowledge, vigorous intensity was a factor of physical activity practice rate, marriage and interest in one's own health were factors of general characteristics, and health recognition was a factor in health-related characteristics. Health-promotion activity was positively correlated with knowledge regarding cardiocerebrovascular disease prevention (r=0.41, p<0.001) and physical activity practice rate (r=0.44, p<0.001). It will be necessary to develop and apply practical intervention programs based on disease prevention knowledge and physical activity to enhance the health behavior of middle aged women.

Parenting Stress in Mothers of Premature Infants (미숙아를 출산한 어머니의 양육스트레스)

  • Purpose: The purposes of this study were to assess the parenting stress in mothers of premature infants and stress related characteristics of mothers and infants. Methods: The methodology was a cross sectional survey study using self-report questionnaires. Participants in this study were 36 mothers of infants with corrected ages of 4 months to 12 months who were born prematurely. Data were analyzed using t-test, ANOVA, and Pearson correlation with the SPSS WIN 19.0 program. Results: The mean score for parenting stress in mothers of premature infants was $74.639{\pm}17.570$, indicating that the mothers actually experienced stress. When mothers were able to have some private time, parenting stress was statistically significantly lower. When the residential status was having one's own house and the local community provided informational support regarding child nurturing, there was a statistically significant decrease in the sub-category of particular infant temperament. There was a statistically significant positive correlation between mothers' depression and parenting stress. Conclusion: These results indicate that there is a need for nursing interventions to increase mothers' private time and to include informational support regarding health management of children including developmental status assessment from local communities.

Comparison of the effect of Action Observational training and Task-oriented training on Upper Limb Function and activities of daily living in People with Chronic stroke (만성 뇌졸중환자의 상지 기능 및 일상생활동작에 동작관찰훈련과 과제지향훈련의 효과 비교)

  • This study aimed to compare the effectiveness of action-observational training and task-oriented training on upper limb function and activities of daily living in patients with post-stroke hemiparesis. This study included 12 voluntary participants with post-stroke hemiparesis. Subjects were randomly assigned to either group 1 or group 2, with 6 in each group. Each subject completed the crossover experiment that comprised of action-observational training (A) and task-oriented training (B). The intervention sequence was A-B for the group 1 and B-A for the group 2. Each training was performed for 30 min a day, 5 times a week for 2 weeks (total experimental period of 4 weeks). Assessments were made using the Wolf Motor Function Test (WMFT) and Modified Barthel Index (MBI) thrice: at baseline, after 2 weeks of intervention, and after 4 weeks of intervention. The results showed that the WMFT and MBI scores significantly improved after the completion of each training (p<.05). Therefore, a determination of the superior training method was difficult. The effect sizes of both groups were greater when performed the action-observational training. The findings suggest that both the action-observational training and task-oriented training may be helpful in improving the upper limb function and activities of daily living for patients with post-stroke hemiparesis, and support the clinical feasibility of the action-observational training.

The Effect of Emotional and Informational Support Intervention on Role Stress and Depression of Primary Family Caregivers Caring Stroke Patients (정서$\cdot$정보적 지지중재가 뇌졸중환자 가족원의 역할스트레스와 우울에 미치는 영향)

  • The purpose of this study was to examine the effect of emotional and informational support intervention on role stress and depression of primary family caregivers caring stroke patients and to develop an effective nursing intervention method for them. This study selected the quasi-experiment, based on pre-test and post-test design of non-equivalent control group as a method of study. This study was conducted with 69 primary family caregivers caring stroke patients hospitalized in K and D hospitals in Daegu from August 20 to October 25, 2000. Out of them, 34 were placed in the experimental group, and 35 in the control group. The emotional and informational intervention program was divided into two aspects-emotional support and informational support- and executed three times, using prepared instruments and a guidebook. The intervention program consisted of the 1st intervention for one or one-half hour, the 2nd intervention for 30 minutes in two or three days after the 1st intervention. and the 3rd intervention for 30minutes in another two or three days after the 2nd intervention. Pre-test was carried out just before the 1st intervention, and the post-test was carried out right after the 3rd intervention, in order to collect data. The measuring instrument of role stress used was one that was modified to the role stress instrument of Yang Young-hee(1992) and the stress instrument of Choi Eun-sook (1992). The reliability was Cronbach's $\alpha=.8271$. The measuring instrument of depression used was one that was developed by Beck(1967) and standardized by Han Heung-moo et al(1986). The reliability was Cronbach's $\alpha=.8693$. Data were analyzed with percentage, mean, standard deviation, $x^2-test$, t-test and Paired t-test by using SPSS 9.0 program. The results of this study are summarized as follows: 1. Role stress score of the experimental group was revealed to be significantly lower than that of the control group 2. Depression score of the experimental group was revealed to be significantly lower than that of the control group Accordingly, the emotional and informational support intervention can be a way to reduce role stress and depression of primary family caregivers caring stroke patients.
