Delayed Sternal Closure after CABG Using DCP Wide Plate -A Case Report (개심술 후 Plate를 이용한 지연성 흉골 봉합 1예 보고)

  • Severe hemodynamic dysfunction may follow after the closure of the median sternotomy in patients with myocardial edema, cardiac dilatation, and severe postoperative bleeding. In extreme cases, myocardial edema may be so severe that cardiac function is compromised merely by the edges of the open sternum touching the heart. Sternotomy remained open and delayed sternal closure is already described adjunct in complicated cardiac operations. We experi nced a case of delayed sternal closure after 16 hours of CABG using synthes DCP wide plate in a 54-year old obese female patient. Our experience conims that delayed sternal closure Is an effective at simple means of dealing with severe myocardial edema so we report this case with literature review.

Risk Factor, Mortality and Infection Rate of Mediastinum After Delayed Sternal Closure in Congenital Heart Surgery Patients (선천성 심장수술 후 지연 흉골 봉합시 사망률 및 종격동 감염률 그리고 위험인자)

  • Background: Congenital heart surgery may lead to myocardial swelling and hemodynamic instability. Delayed sternal closure may be beneficial in this setting. The purpose of this study was to assess mortality and mediastinal infection rate associated with delayed sternal closure after congenital heart surgery and to evaluate the risk factors which affect mortality and mediastinal infection rate. Material and Method: We retrospectively reviewed 40 patients who underwent delayed sternal closure after repair of congenital heart disease at Yonsei Cardiovascular Hospital, from January 1994 to May 2001. In these patients, we assessed the mortality and mediastinal infection rate, and evaluated their risk factors including operation time, bypass time, aortic cross clamp time, duration to sternal closure and postoperative artificial ventilation time. Mediastinal infection was defined to have positive culture in mediastinum. Result: Hemodynamic instability was the most common indication for delayed sternal closure(n=36) and other indications included postoperative bleeding(n=2) and conduit compression(n=2). The median age at operation was $14.4{\pm}33.4$months old(range, 2days-12years). The patients with postoperative bleeding and conduit compression were much older than the others. The sternum was left open for $4.5{\pm}3.4$ days(range, 1-20days). Overall mortality was 25%(10/40) and mediastinal infection occured in 24.3%(9/37) (3 patients were excluded in mediastinal infection for early death). In risk factor analyses, only aortic cross clamp time had statistical significance for mortality in univariate analyses. However, multivariate analyses revealed that there were no significant predictors for risk of mortality and mediastinal infection. Conclusion: Delayed sternal closure after repair of congenital cardiac disease had relatively high mortality and mediastinal infection rate. But, in patients with hemodynamic instability, postoperative bleeding and conduit compression after repair of congenital cardiac disease, delayed sternal closure may be an effective life saving method.

Cardiac Decompressing Effect by Delayed Sternal Closure Following Open Heart Surgery in Children (소아에서 개심술후 정중흉골절개의 지연흉골폐쇄에 의한 심장압박 방지효과)

  • We retrospectively reviewed a series of 20 patients treated with delayed sternal closure during a 5-year period from 1991 to 1996. Of the 2675 patients with cardiovascular su gery 20 underwent this procedure. Male and female ratio was 11:9, mean age was 6.4 months(range 7 days to 5 years). The indications included unstable hemodynamic profiles after open heart surgery due to myocardial edema and poor lung compliance(15), necessity of mechanical ventricular assist device due to weaning failure(3), and hypoxia after PAB(2). Sternum was closed at a mean interval of 102(range 4 to 213) hours after operation. During delayed sternal closure, central venous pressure was elevated(p<0.05). Mediastinitis and other wound problems did not occur. Sepsis developed in 2 patients and microorganism was confirmed in one of the two patients. Five patients died(mortality 25%). And two of 15 discharged patients died during follow-up period. Cumulative survival rate was 65.0% at 12 months and also 65.0% at 24 months.(Standard error was 10.7%) Delayed sternal closure Is considered to be a good method to decompress the hemodynamically compromised heart. Without that, it is not f asible to come off bypass or to decompress the heart. Of course. careful selection of the indication is imperative.

  • We describe a case of successful extracorporeal membrane oxygenation(ECMO) in a small infant with cardiopulmonary resuscitation(CPR) failure after an open heart surgery. A 35-day-old male infant weighing 4.4 kg who had congestive heart failure and pulmonary hypertension underwent patch closure of ventricular septal defect without any intraoperative event. Postoperative course was unremarkable in the intensive care uint for about 5 hours before the junctional ectopic tachycardia developed. Sudden cardiac decompensation with bradycardia occurred about 50 minutes after the development of junctional ectopic tachycardia. He was put on ECMO by arterial cannulation at the ascending aorta and by venous cannulation at the right atrial appendage after 4 hours' CPR. The hemodynamics were stable with enough urine output during ECMO. He was weaned from ECMO 38.5 hours after initiation. Delayed sternal closure was attempted. He was extubated on postoperative day 7 and discharged home on postoperative day 21 without any neurologic sequelae.

Surgical Experience of Open Heart Surgery in Neonates (신생아개심술의 외과적 경험)

  • From January 1993 to April 1995, 27 neonates (under age of 30 days underwent open heart surgery in the Department of Thoracic and Cardiovascular Surgery, Dong-A Medical Center. Mean age and weight were 12.1 days(2days∼306ays) and 3.29 kg(2.6kg∼4.1 kg) respectively. Cardiac anomalies were simple complete transposition of great arteries(TGA) in 11 neonates, TGA with coarctation of aorta(COA) in 1 , total anomalous pulmonary venous connection(TAPVC) in 5, double inlet right ventricle with TAPVC in 1, interrupted aortic arch(IAA) with ventricular septal defect(VSD) in 3, pulmonary atresia(PA) with intact ventricular septum(IVS) in 3, pulmonary stenosis with IVS in 1, Taussig-Bing anomaly with IAA in 1, and hypoplastic left heart syndrome(HLHS) in 1 . Postoperative complications were myocardial and/or pulmonary edema which caused open sternum in 13 patients(54.2%), acute renal failure( RF) in 10(37.0%), Intractable low cardiac output syndrome (LCOS) including weaning failure from cardiopulmonary bypass in 7(25.9%), bronchopulmonary dysplasia in 1, wound infection in 1, and paroxysmal supraventricular tachycardia in 1. Nine of 13 patients with postoperative open sternum were recovered with delayed sternal closure, and seven of 10 patients survived postoperative ARF with peritoneal dialysis. There were 8 operative deaths(29.6%): 3 in the patients with simple complete TGA, 1 In TCA with COA, 1 in PA with IVS, 1 in Taussig-Bing anomaly with IAA, 1 in DIRV with TAPVC, and 1 in HLHS. One late death occurred after arterial switch operation in simple TGA. The mosts common cause of death was low cardiac output syndrome. Our initial experience of open heart surgery in neonates showed high operative mortality and morbidity, especially in complex anomalies.

Delayed Sternal Closure After Heart Surgery in Neonate (신생아 개심술후 지연 흉골봉합)

  • Early repair of complex congenital heart malformation may lead to life-threatening respiratory and hemodynamic embarrassment on sternal closure. We performed delayed sternal closure in nine neonates to avoid a fatal outcome in these situations. Primary elective open sternum was used in 8 [66.7% and primary sternal closure in 4 [33.3% of the 12 patients studied. one patient with primary sternal closure underwent delayed sternal reopening in the intensive care unit. Of the 9 patients with open sternum, 2 patients died of low cardiac output and acute renal failure respectively before delayed sternal closure. 7 patients could undergo delayed sternal closures 3 days after initial operation. The mean age at open cardiac procedure was 14.3 days [range 3 to 30 and mean preoperative weight was 3.4kg [range 2.8 to 4.1 . The aortic cross-clamping time was longer in the group with open sternum than the group with closed sternum [p=0.042 . There was no morbidity and mortality related to delayed sternal closure. Given the low morbidity and potential benifits, this technique should be used in neonates after open heart procedures when postoperative mediastinal compression produces frank low cardiac output or respiratoy compromise during a trial of sternal closure.

Cardiac Rupture after Blunt Chest Trauma (흉부 둔상에 의한 심장파열)

  • A clinical evaluation was performed on 11 cases of the cardiac rupture by blunt chest trauma at the department of thoracic and cardiovascular surgery, Yeungnam University Hospital during the period from July, 1993 to May, 1995. The results were as follows ; The sex distribution was 8 mem and one women, and mean age was 41 years old. The causes of accident was traffic accident in most cases. And then one case was cultivator accident and another one was fall down. The average times from trauma to admission was 139 minutes and the patients that transferred via other hospitals have relatively long average times to 227 minutes. The average times from admission to operation was 117 minutes and we consumed 25 minutes for the preoperative preparation. The sites of injury were 7 cases in the right heart and 3 cases in the left heart. There were symptoms and signs of the cardiac tamponade(dyspnea, chest pain, nausea/vomitus, neck vein distention & hypovolemic shock) at admission and in most of them typical symptoms and signs of a tamponade appeared. Surgical approach was performed with median sternotomy in 10 cases. Subxiphoid pericardial window was created in one case. Another case which was very difficult in surgical procedure was operated under cardiopulmonary bypass and the result was good.

Anatomical Repair of Taussig-Bing Anomaly with Interrupted Aortic Arch and Intramural Left Coronary Artery (벽속관상동맥 기형과 대동맥궁 단절을 함께 동반한 Taussig-Bing심기형의 해부학적 완전교정)

  • We report a case of the successful anatomical correction of the Taussig-Bing anomaly associated with the interrupted aortic arch and intramural left coronary artery for an 38 day-old infant Aortic arch and neoaortic reconstructions were conducted without any prosthetic or pericardial patch. Intramural left coronary was separated from right one after partial detachment of aortic commissure and both coronary artery buttons were transferred separately to the proximal main pulmonary artery(nee-aorta). Delayed sternal closure was done 3 days after the operation and hospital discharge was delayed for a month because of postoperative pneumonia. Now he is 5 months old and free of symptoms and cardiac drugs.

Clinical features and results of recent neonatal cardiac surgery - A review of 82 cases in one hospital (최근 신생아 심장 수술의 특징과 결과 - 단일 병원에서의 82례 고찰)

  • Purpose : The purpose of this study was to investigate the clinical features and outcome in newborns undergoing cardiac surgery. Methods : Eighty two neonates underwent heart surgery for congenital heart defect at Kyungpook National University Hospital between March 2000 and February 2006. Patient characteristics (sex, age, diagnosis), pre-operative conditions, operation type, postoperative complications and mortality were reviewed retrospectively. Results : In 82 patients, 41 (50%) were male. The mean age and weight at operation were 12 days and 3.2 kg, respectively. The common cardiac anomalies were complete transposition of the great arteries (TGA), Tetralogy of Fallot (TOF), pulmonary atresia with intact ventricular septum, and single ventricle variants. Fifty seven operations were performed with cardiopulmonary bypass and corrective surgery was done on 54 patients. Arterial switch operation and modified Blalock-Taussig shunt were most frequently performed as corrective and palliative operations, respectively. The early hospital mortality rate was 7%; the late mortality was 3.9%. Complications were acute renal insufficiency, delayed sternal closure, wound infection, arrhythmia, and brain hemorrhage. Conclusion : During the last 6 years, the outcomes of cardiac surgery for congenital heart defects in neonates improved by progress in perioperative, anaesthetic, surgical, and postoperative care.

Estimation of Groundwater Recharge Considering Water Balance and Groundwater Head Variation in Watershed (유역물수지와 지하수위변동을 고려한 지하수 함양량의 추정)

  • 지하수 함양량을 산정하기 위해서 유역물수지 모형 또는 지하수위 변동곡선법등이 활용되어 왔으며, 각각의 장단점에 대한 많은 논의가 있었다. 지역내의 지하수 관리계획을 위해서는 유역물수지 모형이 적합한 반면, 실제(actual) 함양량의 정확성 문제가 있으며, 지하수위 변동곡선법은 실제 지하수의 증감을 통해 실제 함양변화를 잘 표현하는 반면, 그 영향권이 관측공에 의해 제한된다는 단점이 있다. 이와 같은 두 방법의 장점을 살리고 단점을 보완하기 위해 본 연구에서는 유역유출모의와 지하수위 변동을 동시에 고려하여 지하수 함양량을 추정하는 방안을 제시하였다. 수문학적 과정으로 함양을 분석하기 위해 준분포형 수문모형인 SWAT과 완전분포형 지하수 유동해석 모형인 MODFLOW의 결합모형을 이용한 유역단위 유출해석을 수행한다. 실제로 하천변에 위치한 관측공에서의 지하수위는 하천유출시계열과 유사한 양상을 보이며, 상류 경사지에 위치한 관측공의 지하수위는 토양수의 지체와 감수현상을 나타내므로 이같이 시간적으로 상이한 지체 양상을 반영하기 위해 모형내의 비포화대를 통과하는 함양의 거동을 기존의 단일 저수지 모형에서 다단 저수지 모형으로 개선하고 지체함수와 관련된 매개변수를 보정함으로써 모의 함양 시계열과 관측 지하수위 시계열간의 상관성 분석을 수행할 수 있다. 아울러 모의 및 관측 지하수위 분포의 비교를 통해 공간적인 지하수 분포에 대한 검증을 수행함으로써 함양량 산정 절차가 완성된다. 이같은 일련의 절차는 기존의 유역물수지 모형과 지하수위 변동법을 동시에 고려한 새로운 방식의 지하수 함양량 산정기법으로 제시할 수 있을 것이다.성빈맥(1예), 저심박출증(3예), 재수술이 필요했던 출혈(2예), 흉골 지연봉합(2예), 급성 신부전(2예), 폐렴(1예), 대동맥내 풍선펌프로 인한 혈전색전증(1예),수술 후 섬망(2예) 등이 있었다. 생존한 10명의 환자들 중 1명을 제외한 나머지 9명의 환자에서 $38{\pm}40$개월간의 추적관찰이 되었는데, 추적 관찰 기간 중에 3명이 사망하였고 생존한 6명의 환자는 모두 양호한 상태(NYHA 기능등급, $I{\sim}II$)를 보였으며, 그 중 3명에서는 혈역학적으로 큰 의미가 없는 잔여단락이 있었다. 결론: 급성 심근경색증 후 심실중격 결손은 수술위험도가 높은 질환이지만, 수술 전 대동맥내 풍선펌프를 삽입하고 조기에 심실중격 결손부의 infarct exclusion 술식과 함께 관상동맥우회술을 시행함으로써 만족할 만한 수술 및 중기 결과를 얻을 수 있었다.출물 투여로 저하되었으나 NC군보다는 높게 나타났다. 간 중 중성지질 함량은 참나물 에탄올 추출물 투여 용량에 따른 유의차가 없었으나, 총콜레스테롤 함량은 고용량 병합투여한 HC-PBH군만 유의하게 저하되었다. 혈청 및 간 중의 지질 함량 변화는 정상식이를 급여한 NC군과 NC-PB군 간에는 유의차가 없었다. 따라서 고콜레스테롤식이를 급여하면서 참나물 에탄올 추출물을 병합투여 시에만 지질대사 개선 효과가 있는 것으로 여겨지며, 고용량 병합투여 시 효능이 더 큰 것으로 나타났다. 고콜레스테롤식이로 인한 산화적 스트레스가 고콜레스테롤혈증을 유발하였으며, 이는 참나물 에탄올 추출물에 함유된 항산화물질을 포함한 여러 생리활성물질
