One Year Follow-up for Successfully Treated Children with Accommodative Dysfunction (조절이상이 성공적으로 치료된 어린이에 대한 1년 후의 추적검사)

  • Purpose: The purpose of this study is to evaluate the long-term stability of the improved symptoms and accommodative functions after completion of accommodative therapy. Methods: Seven children (mean age${\pm}$SD: $12{\pm}1.41$ years) who were successfully treated with a vision therapy program for either accommodative insufficiency or infacility were followed for 1 year. The visual symptoms of the subjects were measured by the College of Optometrists in Vision Development Quality of Life (COVD-QOL) checklist, and this was followed by measurement of the monocular and binocular accommodative facility with ${\pm}2.00$ D flipper lens. Results: The mean visual symptoms at the 1 year follow-up examination ($15.14{\pm}8.59$) showed a small increase, but there was no significant difference (p=0.446) from post-therapy ($11.86{\pm}7.22$). There was small regression in the monocular (left eye, $13.86{\pm}3.93cpm$) and binocular ($11.14{\pm}3.13cpm$) accommodative facility at the 1 year follow-up examination, but there were no significant different from the monocular ($15.86{\pm}4.14cpm$, p=0.147) and binocular ($13.21{\pm}3.76cpm$, p=0.066) accommodative facility measurements at post-therapy. Also, every subject met the normative values of ${\geq}7$ cpm for monocular accommodative facility and ${\geq}5$ cpm for binocular accommodative facility in the long-term. Conclusions: There was long-term maintenance of the improved visual symptoms and accommodative functions, and so it is clear that the positive therapeutic effects persist with accommodative therapy.

  • 목적 : 본 연구는 업무형태에 따른 측두하악장애의 유병률과 업무 시 노출되는 직무스트레스 및 구강 내 악습관이 측두하악장애와 어떠한 연관성이 있는지 알아보아 보건학적 기초자료를 제공하고자 실시되었다. 방법 : 본 연구의 자료 수집을 위하여 서울과 경기 일부지역에 근무하고 있는 일반사무직, 서비스직, 교직원으로부터 편의 추출된 452명을 대상으로 2010년 1월부터 2010년 4월까지 설문조사를 실시하였고, 수거된 353명을 연구대상으로 하였다. 설문지는 측두하악장애의 증상, 하악사용에 관한 구강 내 악습관, 직무스트레스, 인구사회학적 특성으로 구성되었다. 측두하악장애의 증상의 정도를 구분하기 위해 설문지의 양성응답 수의 빈도에 따라 무증상인 1단계에서 양성응답 수가 가장 많은 4단계 까지 총 4그룹으로 나누었다. 측두하악장애의 유병률을 알아보기 위하여 빈도분석을 시행하였고, 측두하악장애의 증상의 정도에 따른 여러 요인들 간의 연관성 및 관련요인을 알아보기 위하여 교차분석 및 경향성 분석과 다항로지스틱회기 분석을 시행하였다. 결과 : 측두하악장애의 유병률은 75.4%였고, 측두하악장애에 대한 주관적 증상으로는 관절잡음이 56.4%로 가장 주된 증상 중 하나였으나 남녀 간의 차이는 통계적으로 유의하지 않았다. 다음으로는 두통이나 목의 통증이 36.5%이었고, 귀, 관자놀이, 볼 주위의 통증이 22.1%로 높았다. 측두하악장애의 주관적인 증상 수에 따른 인구사회학적 특성은 증상이 없는 경우 여성에서 19.1%, 남성에서 36.6%로 여성에서 더 높은 유병률을 보였다. 연령별로는 40세 이상의 그룹보다 20 - 30대그룹에서 측두하악장애 증상수가 높아지는 경향을 보였다. 하악 사용과 관련된 악습관 및 직무스트레스는 측두하악장애 증상수와 유의한 관련성이 있는 것으로 나타났는데, 하악 사용과 관련된 습관의 개수가 많아질수록 측두하악장애의 증상의 개수도 많아졌고, 습관이 한 가지씩 늘어날수록 측두하악장애 증상이 없는 1단계보다 3단계가 될 위험이 1.45배, 4단계가 될 위험이 1.57배 높아졌다. 스트레스 수준도 가장 하위단계에서 한 단계 높아지면 측두하악장애 1단계에서 4단계가 될 위험이 2.49배, 두 단계 높아지면 3.43배 높아졌다. 결론 : 본 연구의 결과 측두하악장애와 업무특성에 따른 연관성은 설명하지 못하였지만, 직무스트레스가 높은 경우 측두하악장애 증상의 개수 또한 높아짐을 확인할 수 있었다. 이는 측두하악장애의 주관적인 증상을 발생시키는데 있어서 업무형태 보다는 심인적인 부분이 더 중요한 인자임을 의미한다. 그러므로 측두하악장애 평가 시 신체적인 문제뿐 아니라 행동적, 심리 사회적 문제로 예측인자를 폭넓게 인식함으로써 다각적인 접근을 하는 것이 필요하며, 측두하악장애 증상이 발생된 경우 임상적 치료뿐 아니라 행동요법 및 심리 치료와 자가 관리 등이 함께 수반되어 기여요인 조절을 조절하는 것이 중요하다 하겠다.

Family Caregivers' Quality of Life, Depression and Anxiety according to Symptom Control in Hospice Patients (호스피스 환자의 증상조절에 따른 가족간병인의 삶의 질과 우울, 불안)

  • Purpose: It is well known that a terminal cancer condition affects not only patient themselves but their family members because the patients experience a variety of symptoms. This study was aimed to investigate modifiable factors that influence family caregivers' quality of life, depression, and anxiety. Methods: From January 2015 through May 2015, a survey was conducted with 61 family caregivers of hospice patients who were hospitalized in two university hospitals and one municipal hospital in Busan. The questionnaire was consisted of characteristics of family caregivers and patients, the Korean version of the Caregiver Quality of Life Index-Cancer (CQOLC-K), Beck's Depression Inventory II (BDI-II), Beck's Anxiety Inventory (BAI), and patient's symptom controlling scores rated by family caregivers. Results: Family caregivers' depression was associated with religion. Quality of life and depression of family caregivers were also influenced by monthly household income. Patient age was inversely related to family caregiver's quality of life ($r_s=-0.259$, P=0.043). Family caregivers' quality of life was associated with patient's anxiety (r=0.443, P=0.001). Family caregivers' depression was affected by patient's constipation (r=0.276, P=0.046), anxiety (r=0.508, P<0.001), and daytime drowsiness (r=0.377, P=0.005). And family caregivers' anxiety was influenced by patients' sleep disturbance (r=0.276, P=0.046), depression (r=0.297, P=0.031), and anxiety (r=0.357, P=0.009). Conclusion: According to our findings, family caregivers had higher quality of life and less depression and anxiety when symptoms in hospice patients were well controlled.

  • The aim of palliative radiotherapy (RT) is to control cancer-related local symptoms with minimal radiation reaction. About one third of all radiation treatments are given with palliative intent. Indications for RT in symptom palliation are as follows: Pain from bone metastasis, pressure symptom from brain and spinal cord, obstruction of bronchus, esophagus, superior vena cava and malignant cancer bleeding from bronchus, urinary tract, uterine cervix and rectum. In hospice palliative care, RT is very effective for symptom palliation and improvement of quality of life without influence on survival.

Relationship between Dietary Habits, Attitudes toward Weight Control and Subjective Fatigue Symptoms in Women College Students (여자대학생의 식생활습관, 체형 및 체중조절 태도와 피로자각증상과의 관련성)

  • The purpose of this research is to access the dietary habits and weight control attitudes in college women, and to correlate them to the subjective fatigue symptoms. The research subjects are 508 women enrolled for studies in undergraduate school from freshmen to senior year in Daejeon city. The survey was conducted with self-administered questionnaire from Apr. 1st to May 1st to May 31st, 2012. In the questionnaire encompasses general characteristics, daily life style factors, dietary habits, attitude toward body type and weight control, and subjective fatigue symptoms. As a results, the affecting factors on the subjective fatigue symptoms were selected various factors such as frequency of the one food, frequency of dining out, having dinner, frequency of instant food intake, frequency of cereal, ice-cream intake, frequency of bean, soybean product intake, frequency of milk, milk product intake, consider excessive intake of sugar, consider excessive intake of salt, self-perception on body type and desire to control weight. Therefore, we can find out that their subjective fatigue symptoms are correlated to the factors in dietary habits, attitude toward body type and weight controls. It is concluded dietary habits and attitude toward body type and weight control themselves cause one's subjective fatigue symptoms, not because of each variables. So, it need efforts of having regular diets, making balanced body and controling body weights by objective ranges.

  • Decision-making of antibiotics use in infected patients with terminal stage of cancer was difficult for physicians, because of responsibility of solving a medical problem and burden on patients distressed by worthless life expansion. Korean Family Medicine Palliative Medicine Research Group discussed this subject using a case of a 65 year-old male having terminal stage of sigmoid colon cancer with extended cutaneous infection who was treated local antibiotics, improved but expired at the 12th hospital day. We reviewed related literatures and proposed a guide for antibiotics use in inferred patients with terminal stage of cancer. Antibiotics should be used for symptom control as major indication, especially when patients suffered from urinary symptoms. Appropriate antibiotics should be chosen based or sensitivity test. the most important considering factor should be patient and family members' wish about antibiotics use.

Visual Problems and Refractive Error at Video Display Termianls (VDT사용자의 시기능 불편과 굴절이상)

  • The purpose of this study is to evaluate the effects of continuing work on VDT(video display terminal), therefore this study examined visual fatigue, unaided visual acuity, refractive error, accommodation and horizontal phoria of 152 subjects who did two hour long VDT work. For the ocular symptoms, the greatest number was tired eyes accounting for 45.71%. In the visual symptoms, blurred vision was the hightest rate of 80.39% and in case of systemic symptoms shoulder pain was 33.33% marked top ranking. The average of near visual acuity decresed almost 10% from 0.47 to 0.42, but refractive error increased about 0.10D to the direction of myopic shift. The amplitude of accommodation decreased approximately 0.72D from 7.46D to 6.74D. Accommodation facility was delayed from 2.27 second to 2.50 second, the amplitude of positive relative accommodation was decreased from 4.76D to 4.16D and the amplitude of negative relative accommodation was decreased from 2.46D to 2.33D. The horizontal phoria shifted to the direction of esophoria from $1.82{\Delta}$ to $3.24{\Delta}$.

The Moderating Effects of Collectivism in the Relationships between Ambivalence over Emotional Expressiveness, Psychological Maladjustment, and Subjective Well-being (정서표현양가성과 심리적 부적응, 주관적 안녕감의 관계에서 집단주의 문화성향의 조절효과)

  • This study examined the effects of ambivalence over emotional expressiveness (AEE) on psychological maladjustment and subjective well-being. Furthermore, we examined the moderating effect of collectivism on the relationships between AEE, psychological maladjustment and subjective well-being. Many Korean psychologists have suggested a moderating effect of collectivism on the relationship between ambivalence over emotional expressiveness and psychological adjustment, but there are few studies that have examined these relationships. Consequently, it is necessary to investigate these effects. Data concerning AEE, individualism, collectivism, psychological maladjustment (BSI), and subjective well-being were collected from 138 adults in South Korea. To test the moderation effects, multiple regression analyses was conducted. Results found AEE increased psychological maladjustment and decreased subjective well-being. However, the moderating effect of collectivism was not found in the relationship between AEE and psychological maladjustment, as well as AEE and subjective well-being. The difference of emotion suppression and AEE regarding cultural orientation and clinical implication of these findings were discussed.

  • Purpose : This study was conducted to develop primary assessment tools to evaluate the relationship between symptom prevalence and symptom distress in cancer patients of Korea. Methods : A total of 196 patients diagnosed with cancer admitted in 5 general hospitals from January, 6 to January, 20 in 2003, were entered into the study. These patients were asked to complete the MSAS (Memorial symptom assessment scale). We classified these answers into prevalence and distress and then scored them according to grade and frequency. Results : The five most prevalent symptoms were lack of energy (82.1%), dry mouth (73.5%), pain (73.5%), itching (72.4%), and feeling drowsy (68.9%). Among 24 common prevalent symptoms, the ten most distress the symptoms were dry mouth (2.11), itching (2.07), pain (2.03), lack of energy (1.98), difficulty sleeping (1.96), worrying (1.94), "I don't look like myself" (1.91), constipation (1.88), and difficulty concentration (1.76). Conclusion : These 10 symptoms had high prevalence and distress in cancer patients of Korea. We suggest these symptoms to be applied in developing the primary assessment tool in cancer patients.
