Study of Quality of Life and Related Factors in Veterans with Physical Dysfunction (신체적 기능장애를 가진 국가유공자의 삶의 질 수준 및 관련 요인 연구)

  • The aim of this study was to examine the quality of life (QOL) and its related factors in veterans with physical dysfunction. The survey data was collected from 412 patients with physical dysfunction managed at five veterans' hospitals in Seoul, Pusan, Daegu, Gwangju, and Daejeon city. The baseline for the quality of life was measured using the SF-36 (Short-Form 36), activities of daily living was measured using a Modified Barthel Index (MBI), pain level was determined using Quadruple Visual Analogue Scale (QVAS), and the depression level was assessed using Korean version of the Geriatric Depression Scale: Short-Form (SGDS-K). The SF-36 scores showed a significant correlation with age (r=-0.132), self-rated current health status (r=-0.545), active daily living (r=0.514), pain level (r=-0.243), and depression level (r=-0.565) (p<0.05). The most influential variables on the QOL were the depression level (B=-.969, p<0.001), QVAS (B=-0.163, p<0.001), and MBI (B=0.140, p<0.001). The results of this study showed that that depression, pain and activities of daily living are significant predictors of the QOL in veterans with a physical dysfunction. Therefore, healthcare providers need to consider these parameters for interventions to improve the QOL in veterans with a physical dysfunction.

Relationships Between Job Stress and Psychosocial Stress Among Nurses in a University Hospital (대학병원 간호사의 직무스트레스와 사회심리적 스트레스의 관련성)

  • This study was intended to measure the level of psychosocial stress among nurses working for a university hospital and to reveal its related factors. The self-administered questionnaires were given to 450 nurses during the period from October 1st to 31st, 2010. As a results, the level of psychosocial stress was 10.2% in normal group, 64.0% in potential stresses group, and 25.8% in high risk stress group. In correlation, psychosocial stress was positively correlated with job demand, but it was negatively correlated with job autonomy, supervisor and colleague support. For the results of hierarchial multiple regression analysis, the level of psychosocial stress was influenced by the variables of regular exercise, quality of sleep, subjective status of health, shift work, fit to the job, job demand, job autonomy and supervisor support. Especially, the psychosocial stress was higher related with the contents of job stress increased with explanatory powers of 18.1% on the psychosocial stress.

Impact of COVID-19-Related Stress and Depression in Public Sector Workers (코로나 19 관련 공공근로자에서의 우울증상 및 스트레스 반응)

  • Objectives : The coronavirus disease 2019 (COVID-19) outbreak is a global medical crisis imposing particular burden on public sector employees. The aim of this study was to investigate the psychiatric distress among public sector workers amid the COVID-19 pandemic. Methods : We conducted a cross-sectional study with 531 public sector workers in Gwangmyung city who completed Korean versions of the Perceived Stress Scale (PSS), Patient Health Questionnaire-9 (PHQ-9), and Impact of Event Scale-Revised-Korean (IES-R-K). Results : The results revealed more than moderate levels of stress (85.2%), depressive symptoms (22.2%), and posttraumatic stress symptoms (38.8%). PSS total score was significantly correlated with PHQ-9, IES-R-K total scores as well as IES-R-K subscale scores. Total scores on the PSS, PHQ-9, and IES-R-K were all inversely correlated with age. Conclusions : COVID-19-related workers experience considerable stress and depressive symptoms, with self-rated stress correlating significantly with depression scores. Age may serve as a protective factor against occupational stress and burnout. These findings highlight the need for adequate psychiatric screening and intervention for public sector workers.

Comparison of Operational Efficiency and Quality Efficiency of Medical Services by Country : Focused on OECD Member Countries (국가별 의료서비스의 운영효율성과 품질효율성 비교: OECD 회원국들을 중심으로)

  • This study analyzed the efficiency of medical services in OECD member countries by dividing it into operational efficiency and quality efficiency. For this purpose, data from 2017-2019 OECD Health Statistics were used. As the analysis method, super efficiency was measured by applying an output-oriented Variable Returns to Scale (VRS) model. As a result of the analysis, Switzerland, Korea, and Italy were included in the high group of operational efficiency, Canada, Greece, Denmark, etc. in the medium group, and Belgium, Germany, and Spain in the low group. Based on quality efficiency, Norway, Switzerland, and Spain are in the high group, and Greece, Denmark, Mexico, etc. are in the medium group, and the Netherlands, Germany, Belgium, etc. were included in the low group. As a result of comparative analysis of efficiency by OECD member countries as of 2018, it was found that Korea's operational efficiency was the most efficient and quality efficiency was inefficient. Korea (0.998) should improve life expectancy by 0.2 (0.2%) and subjective health perception by 44.2 (138.1%) by benchmarking Greece (0.422), Switzerland (0.207), and Spain (0.371) to improve quality efficiency. Unlike most previous studies that focused on operational efficiency, this study measured quality efficiency together and analyzed the efficiency of the medical service industry in each OECD member country. Through this, this study has implications in that it confirmed the international competitiveness of the domestic medical service industry and suggested ways to improve efficiency.

Health status of Korean disabled people and oral health behavior Relation between the cariogenic experience index and permanent index. (한국 장애인의 건강수준과 구강보건행태 및 우식경험영구치지수와의 융합적 관련성 연구)

  • This study aims to evaluate the overall health of the disabled and oral health behavior to determine its relevance to DMFT index. Based on the National Health Nutrition Survey. The survey was conducted on disabled adults over 19years old age in 2013 the first of 6th phase. The data analysis included a general characteristics, disability factors, social economy factors, health factors, oral health behavior factors were cross-analyzed on a composite sample. The general linear model of the combined sample was analyzed to determine the factors affecting the DMFT index of the disabled. The analysis shows that the DMFT index is higher for men than for women, the higher age, the lower level of education, the worse condition of whole body(EQ-5D), the difficulty of Stomatognathic system, the uncomfortable of chewing and the worse oral health conditions subjectively. Therefore, the study objectively identified the overall oral health condition of the adult disabled in Korea, Through this study, a measure should be made to development of comprehensive oral health education program to promote oral health of disabled people and policy measures should be prepared for prevention and care of oral health of disabled people.

Effect of Stress Level and Stress Vulnerability of Workers on Heart Rate Variability (직장인의 스트레스 수준, 스트레스 취약성이 심박변이도에 미치는 영향)

  • Objectives : The purpose of this study is to identify the effect of office workers' stress level and stress vulnerability on heart rate variability (HRV), confirm the significance of objective stress indicators obtained through subjective stress and heart rate variability devices, and examine their effectiveness as an integrated stress measurement tool in community mental health projects. Methods : From June to July in 2020, 929 workers participated in the stress management and mental health promotion project carried out by K hospital, and their recorded database was used for study analysis with their agreements. Multiple linear regression analysis was performed to find out the effects of general stress level, and stress vulnerability on heart rate variability. Results : All general characteristics were found to have a significant effect on SDNN (ln), RMSSD (ln), LF (ln), and HF (ln), but work experience did not significantly affect RMSSD (ln). Stress level and stress vulnerability did not significantly affect heart rate variability, but stress perception, a sub-factor of stress level, was found to have a significant effect on RMSSD (ln) (β=0.118, p=0.023). Conclusions : In this study, stress perception, a sub-factor of stress level, was found to be a factor affecting RMSSD (ln). This indicates an association with the activity of parasympathetic nerves in stressful situations, and more follow-up studies are needed to use it as a direct indicator of chronic stress and integrated stress in the community mental health field.

Influences of Unilateral Mandibular Block Anesthesia on Motor Speech Abilities (편측 하악전달마취가 운동구어능력에 미치는 영향)

  • There exist patients complaining speech problem due to dysesthesia or anesthesia following dental surgical procedure accompanied by local anesthesia in clinical setting. However, it is not clear whether sensory problems in orofacial region may have an influence on motor speech abilities. The purpose of this study was to investigate whether transitory sensory impairment of mandibular nerve by local anesthesia may influence on the motor speech abilities and thus to evaluate possibility of distorted motor speech abilities due to dysesthesia of mandibular nerve. The subjects in this study consisted of 7 men and 3 women, whose right inferior alveolar nerve, lingual nerve and long buccal nerve was anesthetized by 1.8 mL lidocaine containing 1:100,000 epinephrine. All the subjects were instructed to self estimate degree of anesthesia on the affected region and speech discomfort with VAS before anesthesia, 30 seconds, 30, 60, 90, 120 and 150 minutes after anesthesia. In order to evaluate speech problems objectively, the words and sentences suggested to be read for testing speech speed, diadochokinetic rate, intonation, tremor and articulation were recorded according to the time and evaluated using a Computerized Speech $Lab^{(R)}$. Articulation was evaluated by a speech language clinician. The results of this study indicated that subjective discomfort of speech and depth of anesthesia was increased with time until 60 minutes after anesthesia and then decreased. Degree of subjective speech discomfort was correlated with depth of anesthesia self estimated by each subject. On the while, there was no significant difference in objective assessment item including speech speed, diadochokinetic rate, intonation and tremor. There was no change in articulation related with anesthesia. Based on the results of this study, it is not thought that sensory impairment of unilateral mandibular nerve deteriorates motor speech abilities in spite of individual's complaint of speech discomfort.

Senior Center Based Diabetes Self-management Program: An Action Research Approach (노인복지관 당뇨병 자기관리 프로그램의 과정과 평가: 실행연구방법)

  • The purpose of this study examined the feasibility and efficacy of a senior center-based diabetes self-management program applying action research approach. The cyclical action research method was applied for this study: plan, act, evaluate, and reflect in delivering three waves of the intervention program. Three waves of a 12 weeks-length small group diabetes self-management program were offered during the period of 15 months in a senior center in Seoul. Planning of $2^{nd}$ and $3^{rd}$ wave program were based on reflection of $1^{st}$ and $2^{nd}$ program evaluation respectively. Among the 46 participants, 93.48% (N=43) completed the program. The quantitative evaluation showed statistically significant improvement in HbA1C(p<.001), fasting plasma glucose(p<.001), BMI(p=.016), waist circumference(p=.001), systolic blood pressure(p=.036), diabetes self-management behavior(p<.001) and health knowledge(p=.008). Qualitative data revealed that individual management was very helpful in empowering and adhering for own diabetes care for the participants. Participants reported high satisfaction towards the program with mean satisfaction score of 65.12. Application of the Diabetes Self-management program with action study strategy was successful in community setting for improving participants' subjective and objective outcomes. Action research method guides the practitioner to tailor the program to respond for the participants and field needs.

A Study on the Evaluation of the Enhancement Program of the Social Work Practice Competency for children of Community Child Center Workers (지역아동센터 종사자의 아동 대상 실천역량 향상 프로그램 평가 연구)

  • This study aimed to implement the enhancement program for the social work practice competency for children of community child center workers and to evaluate the program's effects. For this purpose, the program was implemented for ten community child center workers five times for five weeks(once a week, 120 minutes per session). For the quantitative evaluation, a non-equivalent control group design among quasi-experimental designs was used, and for the qualitative evaluation, the written materials related with the program were collected and analyzed. The results of the quantitative evaluation revealed that the social work practice skills and worker-client relationship empowerment of the community child center workers who participated in the competency enhancement program were significantly improved. In the qualitative evaluation, the subjective experiences were presented, including the improvement of the worker's practice competency and the worker's awareness of the changes in the child, the recognition of the strong points and the points to be improved in the program. This study is expected to raise the expertise and effectiveness of community child center workers, and ultimately, to contribute to the healthy development and the improvement of the mental health of children by improving the social work practice competency of the workers in community child centers, which comprise the representative service delivery system for children in South Korea.

The Relationships among Uncertainty, Appraisal of Uncertainty, Depression, Anxiety and Perceived Health Status in Patients with Atrial Fibrillation (심방세동 환자의 불확실감, 불확실감의 평가, 우울, 불안, 주관적 건강지각 간의 관계)

  • Purpose: The purposes of this study were to explore the concept of uncertainty and to examine the relationships among uncertainty, appraisal of uncertainty, depression, anxiety, and perceived health status in patients with atrial fibrillation. Method: The study utilized a descriptive correlational survey design using a face to face interview method. A convenience sample of 49 subjects were recruited from K university hospital over 8 months. The data were analyzed by t-test, ANOVA, Pearson correlation and partial correlation analysis. Results: 1) Subjects perceived with moderately high uncertainty(M=65.98); moderate physical health(M=39.80), mental health(M=47.38), and general health(M=2.94); moderate anxiety(M= 44.78); and slightly low depression(M=15.33). 2) There were significant differences in uncertainty by gender and education. 3) Uncertainty and danger appraisal were significantly correlated(r=.32, p=.03) while the uncertainty was not associated with opportunity appraisal. 4) Uncertainty was significantly correlated with mental health(r=-.31, p=.04), anxiety(r=.38, p=.01), and depression(r=.37, p=.01). Conclusion: This study was the first trial to explore uncertainty and to examine the relationships among its associated factors in Korean patients with atrial fibrillation. Thus, based on the findings of this study, directions for nursing practice and further nursing research for patients with atrial fibrillation were suggested.
