Families' Perception and Attitude toward Applied Physical Restraints in General Neurological Wards (신경계 병동에서 억제대를 적용 중인 환자가족의 억제대 적용에 대한 인식과 태도)

  • This study was conducted to identify families' perception and attitude toward application of physical restraint(PR) in general neurological wards. The study is based on subjects from 70 family members who have cared patients applying PR. Data was collected using self-report questionnaires and reviewing of patients' medical records. 68.6% of subjects were female and the mean age was 47.9 years old. Family members were either sons, daughters or spouses. The questionnaires analyzed families' perceptions based on a score of 5, the results showed a mean score of 4.2, and items related to maintaining therapeutic tools scored the highest. The application of PR with two hands was perceived by families as more important than PR applied with one hand. In families' attitude toward PR, there was general agreement that families had the right to decide to apply and remove PR. Continuous education and information regarding PR is required to support families.

A Study on the Functional Efficiency according to Composition of Single Bedroom in Ward - Focused on Distance from NS to Bed - (종합병원 병동부 1인 병실 적용에 따른 기능효율성에 관한 연구 - 간호거점에서 병상간의 거리를 중심으로 -)

  • Purpose: This study examines distance from nurse station to bed according to composition of single bedroom. Methods: This study has two groups of target sample ward planes. The first group has diverse patients bedroom. The other group is 36 single bedrooms and 2 3-bedrooms. The guideline of distance measurement was instituted for consistency. This study has 3 main concepts of the guideline. 1) The distance is shortcut from NS to bed. 2) The traffic line is center line between walls and center of door. 3) The start point is center of NS and the end point is bed. Results: The result of this study can be summarized into two points. The first one is that single bedroom has good privacy and longer distance from NS to bed. The second one is that weak point of single bedroom was asked ward operating system and architectural planning like as patients room, bed per unit, location of NS and plan type. Implications: Consideration Should be taken into account for the effective bedroom composition and allocation in ward. This Stuy hopefully serve as a stepping stone for the standard design of space program in ward planning.

Mediating Effect of Job Burnout in the Relationship between Role Ambiguity and Organizational Effectiveness among Nurses in Comprehensive Nursing Care Service Units

  • The purpose of this study is to determine the relationship between role ambiguity, job burnout, and organizational effectiveness among nurses working in comprehensive nursing care service units and to investigate the mediating effect of job burnout on the relationship between role ambiguity and organizational effectiveness. The participants were 149 nurses working in 10 general hospitals. Data were collected using a self-report questionnaire and analyzed using IBM SPSS Statistics 25.0 and Model 4 of the PROCESS macro program. The results reveal that organizational effectiveness has a significant negative relationship with role ambiguity and job burnout, with job burnout completely mediating the effect of role ambiguity on organizational effectiveness. These findings suggest that interventions to reduce job burnout should be considered to enhance organizational effectiveness.

Factor Associated with Injury Related to Home Mechanical Ventilation in General Ward Patients: A Retrospective Study (가정용 인공호흡기 관련 안전사고 특성 및 손상 영향 요인 분석: 상급종합병원 일반병동 환자 중심으로)

  • Purpose: This study aims to describe the characteristics of safety incidents and factors associated with injury for patients with Home Mechanical Ventilation (HMV) at the hospital. Methods: This is a retrospective study. Data were collected from the work log of respiratory home care nurses and the patients' electronic medical records were investigated. In order to compare group differences, independent t-test and χ2 test were used. Associated factors with injury development were identified by generalized mixed modeling analysis controlling for age and gender. Results: A total of 304 patients on HMV were included in this study, among which 129 (42.4%) experienced 352 HMV-related incidents. Mean frequency of incidents for each patient was 5.11±3.98, ranged from 1 to 15 times. In 19.0% of the incidents, injury was developed. Types of incident and persons involved in the incidents were significantly associated with the patient's injury. In the case of the safety incidents, patient's injury was significantly higher in accidents caused by respiratory circuit problems compared to those caused by problems with the ventilator operation by the medical staff (coefficient=1.25, p=.020). In addition, in the case of those involved in the safety incidents, patient's injury was significantly higher in the accident caused by the patient family members or caregivers than that caused by the medical personnel (coefficient=1.25, p=.019). Conclusion: In order to minimize injury caused by incidents in patients with HMV, hospitals need to provide systemic education to their medical staff and caregivers to enhance awareness of the importance of reporting and safety management.

Impact of Maternal Uncertainty on Coping: Experiences from a General Hospital Pediatric Ward (입원 환아 어머니의 불확실성이 대처에 미치는 영향: 일 개 종합병원 소아과병동을 중심으로)

  • Purpose: The aim of this study was to test whether maternal uncertainty and the general characteristics of mothers and children influenced maternal coping. Methods: In this cross-sectional study, 190 mothers whose children had been admitted to the pediatric ward of a general hospital completed self-report questionnaires during their children's hospitalization. The questionnaires assessed the general characteristics of the mothers and children, maternal uncertainty, and maternal coping. Multiple regression analysis was used to test the research model. Results: In the multivariable model, help from one's spouse (t=3.10, p=.002), religion (t=2.68, p=.008), overall ambiguity (t=2.64, p=.009), and family income (t=2.33, p=.021) were associated with higher coping scores. Conclusion: This research model presents possible guidelines for pediatric nurses to provide comprehensive and accurate information on children's illnesses and treatments for mothers of children hospitalized in general hospitals. In particular, nurses should pay more attention to mothers who are not receiving help from their spouses, are not religious, and have a low family income.

  • Purpose: The aim of this study was to evaluate the efficiency of nursing units in a hospital using DEA. Methods: Data were collected using the medical information system of a general hospital in a city. Input variables were number of nurses, number of nurse-aides, number of beds, and overhead costs. Output variables included number of admitted patients, rate of bed utilization, satisfaction of discharged patients, and prevention rate of safety accidents and sores. EMS Window version 3.1 was used to measure the efficiency score and descriptive statistics were applied to analyze the general characteristics of variables. Results: The average efficiency score of 18 general nursing units was approximately .99. Nine nursing units had a 1.00 efficiency score. Conclusion: Our findings reveal that nursing units operated very efficiently. To increase efficiency of inefficient nursing units, we recommend results of the DEA slack analysis as a benchmark of the most efficient nursing unit.

Activity-Based Costing Analysis of Nursing Activities in General Hospital Wards (종합병원 일반병동 간호행위의 활동기준원가분석)

  • Purpose: The purpose of this study was to explore the relationship between cost and revenue for inpatient nursing activities in general wards. Methods: Data were collected from 12 medical-surgical wards in one general hospital from January 1 to December 31, 2010. The nursing activities were categorized into 2 groups according to nursing service payment type in terms of the Korea health insurance system. Descriptive statistics were used to identify nursing activities and nursing activity costs. Results: Of 140 nursing activities identified as performed in general wards, payment for 69 items was included in nursing management fees. The percentage of each cost for the nursing units was 90% for labor, 4% for materials, and 6% for operating expenses. The cost for medical support nursing service accounted for 38% of costs and nursing management fees, 62%. The average profit and loss was -237,257,000 won. The cost recovery rate for nursing service was only 44%. Conclusion: The results indicate a need to measure the economic value of nursing activities performed in general wards and use it as a basis for establishing an adequate reimbursement system for nursing service.

  • Purpose: Since the large hospitals are one of the most intensive energy users among building types in Korea, it is important to investigate and apply appropriate energy conservation measures. There are many researches on energy conservation measures for HVAC system in hospitals, but only few useful guidelines for envelope design variables were existed. The building envelope is one of the important factors to building energy consumption and patients' comfort. The purpose of this study is to suggest the most influential envelope design variables for each end-use energy demand. Methods: 100 samples were generated by LHS(Latin Hypercube Sampling) method. After energy performance simulation, global sensitivity analysis was performed by the regression method. DesignBuilder, Simlab 2.2 and JEPlus were used in this process. Results: The most influencing variables are SHGC, SHGC and VT for heating, cooling, and lighting, respectively. However, the most influencing variable for total energy demand is WWR(Window to Wall Ratio). The analysis was conducted based on the coefficient of variance results. Implications: The six envelop design variables were ranked according to the end-use energy demand.

A Study of Convergence Relationship among Nurse's Working Conditions, Posttraumatic Stress Syndrome (간호사의 근로환경, 외상 후 스트레스 증후군과 자아탄력성과의 융복합적 관계)

  • The purpose of this study was conducted to determine the working conditions, post-traumatic stress syndrome and the self-resilience of nurses, and find out the relationship between the post-traumatic stress syndrome and the self-resilience. Data were collected by questionnaire method among 220 General Hospital nurses at Gyeongnam G and J city. Data were analyzed by using SPSS 21.0 program WIN. The results of working conditions of the nurses majority were shift work 70.5%, currently working in the ward 54.6%. Scores were shown as Post-traumatic stress syndrome (28.20) and self resilience (39.98). There is a negative correlation (r =.226, p=.001) between the post-traumatic stress syndrome and the self-resilience. Based on the result, since working conditions of nurses caused psychological stress and nurses with high self-resilience eased the degree of post-traumatic stress, in terms of the personnel management of hospitals, a psychological intervention strategy should be considered.

Risk Factors of Predicting Intensive Care unit Transfer in Deteriorating Ward Patients (병동 급성악화 환자의 중환자실 전동 위험요인 분석)

  • Purpose: When a patient with acute deterioration occurs in a ward, the decision to transfer to intensive care unit (ICU) is critical to improve the patient's outcomes. However, when available ICU resources limited, it is difficult to determine which of the deteriorating ward patients to transfer to the ICU. Therefore the purpose of this study was to identify risk factors in predicting deteriorating ward patients transferred to intensive care unit (ICU). Methods: We reviewed retrospectively clinical data of 2,945 deteriorating ward patients who referred medical emergency team. Data were analyzed with multivariate logistic regression. Results: The solid cancer that diagnosed at hospitalization (odds ratio[OR] 0.39; 95% confidence interval [CI] 0.32-0.47), when the cause of deterioration was respiratory problem (1.51; 95% CI 1.17-1.95), high MEWS (1.22; 1.17-1.28) and SpO2/FiO2 score (2.41; 2.23-2.60) were predictive of ICU transfer. Conclusion: These findings suggest that early prediction and treatment of patients with high risk of ICU transfer may improve the prognosis of patients.
