Review of Cancer-Testis (CT) Genes (Cancer-testis (CT) 유전자의 고찰)

  • Cancer-testis (CT) antigens are immunogenic protein antigens with restricted expression in the testes and a wide range of human tumor types, eliciting both humoral and cellular immune responses in cancer patients. They are considered to be ideal targets for vaccine-based immunotherapy, and more than 100 CT antigens, including MAGE, NY-ESO-1, GAGE, BAGE, LAGE, SSX2 and NY-SAR-35 have been identified to date. The CT antigens were identified through various techniques and can be divided in those that are encoded on the X chromosome, the CT-X genes, and those that are not, the non-X-CT genes. CT genes are aberrantly activated and expressed in a proportion of various types of human cancers. The biological role of CT-X in both germ line tissues and tumors remains poorly understood. Cancer vaccine trials based on several CT antigens are currently ongoing. This paper reviews recent advances in and future trends of CT antigens for cancer immunotherapy.

Radioimmunoscintigraphy Using $^{99m}Tc-anti-CEA\;F(ab')_2$ Fragment in Rectal Cancer and a Pilot Study for Radioimmunoguided Surgery (직장암에서 $^{99m}Tc$-항CEA 항체 $F(ab')_2$ 분절을 이용한 수술 전 방사면역신티그라피 및 방사면역지침수술에 관한 기초 연구)

  • Purpose: This prospective study was performed to evaluate the usefulness of preoperative radioimmunoscintigraphy and intraoperative scintimetric examination (radioimmunoguided surgery: RIGS) using $^{99m}Tc-anti-CEA\;F(ab')_2$ fragment. Materials and Methods: Nineteen patients with rectal cancer underwent preoperative whole body planar scintigraphy at 4 hours after injection of $^{99m}Tc-anti-CEA\;F(ab')_2$ fragment and SPECT imaging at 18 hours. Surgical operation was performed at 24 hours after injection. During laparotomy, radioactivities from intraabdominal viscera were measured by gamma probe. The radioactivities from excised tumor and lymph nodes were also measured and compared with pathology. Results: All nineteen patients were confirmed to have adenocarcinomas in the rectum. Twenty-seven of 97 excised lymph node groups had metastasis and 2 patients had liver metastasis in pathology Preoperative radioimmunoscintigraphy detected primary tumors in 11 patients (sensitivity 55%) and it could not detect any lymph nodes or liver metastasis. All patients showed high radioactivity in the kidneys, liver, spleen, and major vessels in intraoperative measurement by gamma probe, and tumor activity was not discriminated from background activity However, radioactivity from excised tumor was higher than normal rectum (T/B ratio; $3.47{\pm}2.25$). When excised lymph node activity/background activity ratio >1.5 was considered as positive criteria of metastasis, sensitivity, specificity, positive and negative predictive values were 78.6%, 73.9%, 55.0% and 89.5%, respectively. Conclusion: Radioimmunoscintigraphy using $^{99m}Tc-anti-CEA\;F(ab')_2$ has no additional value for preoperative staging and use of early RIGS using $^{99m}Tc-anti-CEA\;F(ab')_2$ is inappropriate. For early RIGS using $^{99m}Tc$ labeled antibodies in rectal cancer patients, further development of more specific antibodies and methods to reduce background activity are needed.

  • The ovine squamous cell carcinoma (OSCC)-specific and immunosuppressive properties of OSCC extracts were investigated by using the techniques of lymphocyte blastogenicity, acid dissociation-ultrafiltration and gradient polyacrylamide gel electrophoresis. It was found that OSCC extracts contained two major and one minor protein peaks by Sephadex gel fractionation. Two major peaks bear substantial amount of immunoglobulins, antigen-antibody complex and OSCC-specific fractions, and the minor peak includes immunosuppressive materials. OSCC-specific components were detected at the molecular weights of 10,000 to 100,000 daltons in the major peaks and immunosuppressive materials at the fractions with the molecular weight of 10,000 to 100,000 and < 10,000 daltons in the minor peak. When the fractions were further separated by gradient polyacrylamide gel electrophoresis, the OSCC-specific antigens were found in the slice number 4 to 6 in fraction III, and immunosuppressive materials, in the slice numbers 9 to II in fraction V. The present results were considered to provide a basis for preparation and purification of OSCC-specific and immunosuppressive materials from the crude OSCC extracts.

Medullary Thyroid Carcinoma in a Maltese Dog (말티즈 개에서 발생한 갑상선수질암)

  • An 11-year-old, 2.67 kg female Maltese dog with 3 weeks history of palpable cervical mass near trachea was submitted to a local animal hospital. Radiography and ultrasonography showed radiopaque mass adjacent trachea and vagus nerve. Surgically excised mass was solitary and approximately $3.5{\times}2{\times}0.8cm$ in size. Histopathologically, there were large neoplastic foci admixed with normal thyroid tissues. These neoplastic foci were composed of small to large packets of the neoplastic cells with plasmacytic morphology, and these packets were divided by fine fibrovascular septa. Immunohistochemically, most neoplastic cells in the thyroid mass showed positive reactions for cytokeratin (AE1/AE3), chromogranin A, neuron specific enolase (NSE) and the negative reaction for vimentin. Based on the gross, histopathologic and immunohistochemical characteristics, this dog was diagnosed as medullary thyroid carcinoma.
