Use of Acoustic Analysis for Indivisualised Therapeutic Planning and Assessment of Treatment Effect in the Dysarthric Children (조음장애 환아에서 개별화된 치료계획 수립과 효과 판정을 위한 음향음성학적 분석방법의 활용)

  • Speech evaluation and treatment planning for the patients with articulation disorders have traditionally been based on perceptual judgement by speech pathologists. Recently, various computerized speech analysis systems have been developed and commonly used in clinical settings to obtain the objective and quantitative data and specific treatment strategies. 10 dysarthric children (6 neurogenic and 4 functional dysarthria) participated in this experiment. Speech evaluation of dysarthria was performed in two ways; first, the acoustic analysis by Visi-Pitch and a Computerized Speech Lab and second, the perceptual scoring of phonetic errors rates in 100 word test. The results of the initial evaluation served as primary guidlines for the indivisualized treatment planning of each patient's speech problems. After mean treatment period of 5 months, the follow-up data of both dysarthric groups showed increased maximum phonation time, increased alternative motion rate and decreased occurrence of articulatory deviation. The changes of acoustic data and therapeutic effects were more prominent in children with dysarthria due to neurologic causes than with functional dysarthria. Three cases including their pre- and post treatment data were illustrated in detail.

  • Velopharyngeal insufficiency (VPI) is a phenomenon that can occur due to anatomical or neurological causes of the soft palate. VPI can make the patient difficult to articulate through hypernasality and nasal emission. There has been needed the customized treatment, as VPI can occur for many causes. We present the case of 21-year-old male who took palate plastic surgery 20 years ago for congenital submucosal cleft palate. As he had poor contraction of both lateral side of velopharynx, he was diagnosed with coronal type VPI. Through sphincter pharyngoplasty, he can obtain improvement of articulation accuracy. We would like to share this challenged case.

Voice onset time in children with bilateral cochlear implants (양측 인공와우이식 아동의 성대진동시작시간 특성)

  • This study aimed to investigate the voice onset time (VOT) of plosives in the VCV syllables by the place of articulation and phonation type spoken by children with bilateral cochlear implants (CIs) in comparison with children with typical hearing (TH). In all, 15 children with bilateral CIs and 15 children with TH participated in this study, aged between 5 to 10 years. All children produced 9 VCV syllables and their VOT were analyzed by the Praat software. There was no significant difference in mean VOT between children with bilateral CIs and children with TH. However, there was a significant difference in mean VOT by the place of articulation, such that the VOT for velars were longer than those for bilabials and alveolars. Additionally, there was a significant difference in mean VOT by the phonation type, such that the VOT of aspirated consonants were longer than those of lenis and fortis consonants. The results of this study suggest that children with bilateral CIs can distinguish the acoustic properties of plosive consonants and control the speech timing between the structures of the larynx and the oral cavity at a similar level as children with TH.

  • This study analyzed 28 domestic and foreign studies related Prompts for Restructuring Oral Muscular Phonetic Targets treatment techniques from 1984 to 2020 to prepare basic data for the development of PROMPT intervention programs and examination tools. According to the analysis, continuous research has been conducted since 1984 when the prompt study was first started, and the method of research was 16 intervention studies, with the highest number of speech disorders, and the target age being 3 to 5 years old, the most frequently conducted for infancy. The treatment was the most frequent in the 16th sessions, and the activities were based on the Motor Speech Hierarchy(MSH), except for the subjects of the non-verbal autism spectrum disorder. According to the analysis of the dependent variables, 'speech production' was the most common, followed by 'speech motor control', 'articulation', and 'speech intelligibility' were highest. Combined with all these studies, it suggests that PROMPT, which are directly useful for exercise spoken word production, are effectively being used outside the country and that it is necessary to develop a PROMPT program that can be applied domestically, in Korea.

Effects of Laryngeal Massage on Muscle Tension Dysphonia: A Systematic Review and Meta-Analysis (근긴장성 발성장애의 후두마사지 효과: 체계적 고찰 및 메타분석)

  • Background and Objectives This study was to investigate the voice quality and articulation effects of laryngeal massage on muscle tension dysphonia (MTD). Materials and Method A systematic review of articles published between January 2000 and December 2020 in Cochrane, PubMed, ScienceDirect, SpingerLink, ERIC, and Naver Academic was conducted. From the total of 2094 articles identified, 10 peer-reviewed articles were included in a meta-analysis. Mean effect sizes of the variables related to voice quality (jitter, shimmer, harmonic to noise ratio or noise to harmonic ratio, high-F0, low-I, cepstral peak prominence) and articulation (F1, F2, F1 slope, F2 slope) were calculated by Hedges'g. Results Meta-analysis of the selected articles showed that laryngeal massage had medium to large effects on all variables of voice quality and articulation except F0-high and F1 slope in the MTD patients. Conclusion This study provided comprehensive clinical evidence that it is highly desirable to apply laryngeal massage to MTD patients.

  • Stroke makes several physical deficits. Dysarthria is one of the most difficult problems in conventional medicine because of the weakness of neuromotor control. The purpose of this study is to find the acoustic characteristics of acupuncture therapy effects on post-stroke dysarthria. Seven patients with stroke(infarction or hemorrhage) were selected by CT or MR imaging. The authors applied acupuncture therapy by inserting needles into 8 acupuncture points, ipsilateral ST4, ST6 and contralateral LI4, ST36 on facial palsy side, and CV23, CV24, bilateral "Sheyu" for 4 weeks. Speech sample were composed of five simple vowels /a,e,i,o,u/ and meaningless polysyllabic words CVCVC(C: stops, affricated, fricative sounds, v: /e/). .VOT, total duration of each speech samples and vowel formant (F1&F2) were analyzed on Spectrogram. The results are as follows: 1. VOT of bilabial and velar stops was decreased post treatment. The VOT of bilabial glottalized pre and post treatment were statistically significant (p < 0.05). 2. Total duration of polysyllabic words was decreased post treatment. Decrement of total duration containing the bilabial was statistically significant (p<0.05). 3. First formant of round vowel /o/ pre and post treatment was statistically significant (p<0.05).

Acoustic Duration of Consonants and Words by Phonetic Complexity in Children with Functional Articulation and Phonological Disorders (기능적 조음음운장애 아동의 조음복잡성에 따른 자음과 단어의 음향학적 길이)

  • Purpose : This study was conducted to investigate whether phonetic complexity affected the type and frequency of articulation errors and the acoustic duration of consonants and words produced by children with functional articulation and phonological disorders. Methods : The participants in this study were 20 children with functional articulation and phonological disorders and 20 children without such disorders who were between 3 years 7 months old and 4 years 11 months old. The participants were asked to name what they saw in pictures and their responses were recorded. The types and frequencies of articulation errors and the acoustic duration of words were analyzed and words were categorized as being of either 'high' or 'low' phonetic complexity. The acoustic duration of initial and final consonants and vowels following initial consonants were compared between the groups according to articulatory complexity. Results : Children with functional articulation and phonological disorders produced articulation errors more frequently when saying high complexity words and had longer word duration when saying low-complexity words than children without such disorders. There was no major difference in initial and final consonant duration between the groups. but the main effect of articulatory complexity on the duration of both consonants was significant. Conclusion : These results suggest that the articulatory-phonic structure of words influences the speech motor control ability of children with functional articulation and phonological disorders. When articulating consonants, children with functional articulation and phonological disorders had speech motor control ability similar to that of children without such disorders.

A Study On Voice and Articulation in Children with Hearing Impairment (청각장애아동의 음성 및 조음 특성 연구)

  • The purpose of this study was to investigate the fundamental frequency(Fo) of voice signal, the first to the third(F1-F3), and duration in children with hearing impairment. Each subject made a recording of sustained /i/ and /a/, four VbV as and four VsV. The Praat 4.1.6. was used for analysis. The results of this study were as follows: First, F0 of children with hearing impairment were higher than normal children. Second, /a/ vowel was showed that F1, F2 and duration were higher than normal children. Third, /i/ vowel was showed that F1 and duration were higher than normal children. However, F2 was lower than normal children. Therapeutic implications have been drawn.

  • 복잡한 음성장애를 이해하기 위해서는 음성관에 대한 여러 단계에서의 정량적인 검사가 이루어져야 한다. 이를 위하여 여러 가지 검사 법이 이용되고 있는데 예를 들면 음성의 인지적 검사(perceptual intelligibility), 음향음성학적검사(acoustic analysis), 공기역동학적 검사(aerodynamic study), 후두구조물의 운동 관찰, 그리고 근과 신경의 기능 검사(electromyographic study)등이 있다. 이중 인지적 검사는 청취자 동의 문제와 검사 법에 대하여 문제점이 제기 되기도 하며 발화 중 후두기능의 병태생리에 관한 추론적인 정보만을 제공한다는 문제점이 있다. 음향음성 검사는 이미 잘 알려진 상태로서 많은 parameter들이 측정되어온 것이 사실이나 그 유용성에 대해서도 아직 논란이 있으며 단지 성대의 진동에 의해 나타나는 현상만을 이용한 검사로서 일종의 정지성 연구에 불과한 것이 사실이다. Effects of Metaphon Intervention on a Phonological Ability of Preschool Children with Articulation-Phonological Disorders (상위음운 중재가 취학 전 조음음운장애 아동의 음운 능력에 미치는 효과)

  • The purpose of this study was to find an effect of Metaphon Intervention on the improvement of speech intelligibility of preschool children with articulation-phonological disorders. Subjects were 4 preschool children with articulation-phonological disorders. A multiple baseline design across subjects was used to examine the effect of the program. The program consisted of 2 steps. The first step was composed of concept level, sound level, phoneme level, and word level. The second step was on sentence level. Results were as follows: First, metaphon ability of all subjects was improved after the Metaphon Intervention. Second, speech intelligibility of all subjects was improved after Metaphon Intervention. From the results above, Metaphon Intervention can be effective to improve not only phonological awareness and metaphon but also overall speech intelligibility of preschool children with articulation-phonological disorders.
