The Effects of Program by PNF on the Articulator Function of Stroke Patients (뇌졸중 환자의 조음기관 기능에 PNF를 이용한 프로그램이 미치는 효과)

  • The purpose of this study was to identify an effects of Proprioceptive Neuromuscular Facilitation(PNF) on articulator function of the stroke patients. This study evaluated pre- and post- articulator function, targeting the test group for whom an program by PNF was applied, the contrast group for which an articulator training program was applied, and the control group for which no intermediation was made. Each program were performed total 24 times based on 3 times per 30~50 minutes. In result, MPT, AMR ,SMR of the test group were enhanced. AMR of the contrast group did not show any significant difference in /tə/, /rə/, and /a/. Therefore, Program by PNF is informed to have a effect in advancing the articulator function of the stroke patients, especially it was effective of the tongue movement.

Effect of Articulation Abilities on the Articulator Strength Training by IOPI of Spasticity Dysarthric Speech (IOPI를 활용한 조음기관 훈련 프로그램이 경직형 마비말장애의 조음 능력에 미치는 영향)

  • Objective : The purpose of this study was to investigate the effects of the IOPI articulator strength training program on articulator(tongue and lip) muscle strength, numbers of /l, s, ʨ/ articulation accuracy, articulatory numbers, articulation regularity and accuracy in the alternate motion rates, and sequential motion rate changes in patients with spastic dysarthria. Methods : Three cases of patients with spastic dysarthria living in Jeju, Korea, were included in this study. A single subject design was selected to study changes in articulator(tongue and lip) muscle strength, numbers of /ㄹ, ㅅ, ㅈ/ articulation accuracy, articulatory numbers, articulation regularity and accuracy in the alternate motion rates and sequential motion rates. Results : After the articulator strength training program was conducted on patients with spastic dysarthria, there were positive changes in articulator(tongue and lip) muscle strength, numbers of /ㄹ, ㅅ, ㅈ/ articulation accuracy, articulatory numbers, articulation regularity and accuracy on the alternate motion rates and sequential motion rates. Conclusion : Our findings suggest that IOPI articulator strength training program could be very useful for the most representative childeren with cerebral palsy if conducted in various subtypes of dysarthric patients and linked with articulatory function training with IOPI at home.

The Effects of Orofacial Training Video Program using Smart Device on Oral Cavity Structure and Function, Diadochokinetic Rate in Acute Stroke Patients with Dysarthria (스마트 기기를 활용한 구강안면 훈련 영상 프로그램이 마비말장애가 있는 급성 뇌졸중 환자의 구강 구조 및 기능과 조음교대운동속도에 미치는 효과)

  • The purpose of this study was investigated the effect of orofacial training video program using smart device on oral cavity structure and function, diadochokinetic rate in acute stroke patients with dysarthria. Present study participated in fourteen acute stroke patients with dysarthria. All subjects assigned that randomized each seven patients in experimental and control group. Subjects of two groups received conventional rehabilitation therapy for 4 weeks. Experimental group performed additionally that orofacial training video program using smart device, supervised under caregivers, during 30 min/day. The outcome measures were the oral cavity structure and function of subscale for KOMSE(: Korean Oro-motor Mechanism Screening Examination), AMR(: Alternating Motion Rate), SMR(: Sequential Motion Rate). In results, Both group showed significant improvements after intervention in all assessments(p<.05). In comparison of change between two groups, experimental group showed significant improvements than control group in oral cavity function, /p ə/ and /tə/ of AMR(p<.05). We suggested that orofacial training video program using smart device expected to positive effects the improvements of oral cavity and articulator function in acute stroke patients with dysarthria.

Therapeutic Singing on Speech Production Parameters in Head and Neck Cancer Patients: Case Studies (치료적 노래부르기를 통한 두경부암 환자의 말산출 기능 향상 사례)

  • This case study investigated the changes in speech intelligibility of patients with head and neck cancers who participated in a therapeutic singing-based intervention. Three patients received a total of twelve 30-minute individual sessions. The intervention consisted of three steps: movements for relaxing breathing muscles, vocalization for increasing the range of articulatory movements, and therapeutic singing. In order to examine the changes in speech intelligibility, the voice quality parameters, diadochokinesis (DDK) and the quadrangle vowel space area (VSA) were measured at pre- and posttest. The recording of what each patient read a written paragraph, which were transcribed by blinded assessors, were also analyzed. The results demonstrated that all of the patients showed positive changes in the voice quality, the rate of repetitive syllable production measured by DDK, and the articulatory working space measured by VSA. Along with these measured changes, increases in positive mood and rehabilitation motivation reported by the patients support that the therapeutic singing-based intervention could induce meaningful changes in terms of speech intelligibility from patients with head and neck cancers. Given that this study was conducted with a small sample size, suggestions for further investigation on the effects of the intervention were also presented.

The Characteristics of Diadochokinesis in 1st and 2nd Grades of Elementary School Students (아동의 조음교대운동 특성: 광주광역시 초등학교 1, 2학년을 대상으로)

  • Diadochokinesis (DDK) aims to identify the evaluating the oral mitor ability and the moter coordination ability. There are few DDK normative data on elementary school students in Korea, The purpose of this study was to investigate the characteristics of the speed and regularity of DDK in first- and second-grade students in elementary school. The subjects were a total of 194 students in first- (45 males, 50 females) and second-grade (47 males, 52 females) in elementary schools in Gwangju Metropolitan City. As evaluation tasks, AMR task 'p?', 't?', and 'k?' and SMR task 'p?t?k?' were performed. The speed and regularity of DDK was measured using Motor speech profile (Model 5141, KayPENTAX) and Praat (v6.0.3.6). The results of this study, First, there was a statistically significant difference by grade in AMR speed for 'p?', 't?', and 'k?' and the AMR speed was faster in second grade group. And, there was no statistically significant. Second, AMR regularity showed a statistically significant difference in 'p?', 't?', and 'k?' according to sex and was found to be more regular in female student group. There was no significant difference in regularity by grade. Third, the SMR speed showed statistically significant difference in 'p?t?k?' by grade and was faster in second grade group. And there was no statistically significant difference by sex. The results of this study showed that the DDK performance ability in first- and second-grade students in elementary school was slightly different according to grade and sex. In future research, it is necessary to investigate the correlation between the articulation accuracy and linguistic intelligibility, and to find out the usefulness of DDK in articulation evaluation.

A Preliminary Study of Developing Korean Oro-motor Mechanism Screening Examination (KOMSE) in Normal Adults (정상성인의 조음기관 구조 및 기능선별검사 제작을 위한 예비연구)

  • This study was purposed to introduce Korean Oro-motor Mechanism Screening Examination (KOMSE) and to provide the standardized scores taken from 120 normal adults aged $18{\sim}59$. KOMSE was composed of 13 items of structure, 17 items of function, 3 items of voice, and 14 items of regularity and accuracy of articulation during diadochokinesis. Additionally, maximum phonation time and diadochokinetic rates were measured. Total score of KOMSE is ranged from 0 to 200. It was significantly higher in the $50{\sim}59$ age group than in the $20{\sim}29$ age group, but was not significantly different in sex. MPT had a significant difference in sex, but no significant difference among different age groups. The rate of DDK was also had no significant difference among different age groups. However, some reducing trend in the rate of DDK was observed as age increased. KOMSE would be very helpful for SLPs to screen the abnormality of oro-motor mechanism.

Preliminary Study on Developing Test Items of Swallowing & Communication Screening Protocols for Patients with Head and Neck Burns (안면부 및 경부 화상 환자의 삼킴 및 의사소통능력 선별 프로토콜 개발을 위한 예비 연구)

  • We have to consider two parts of the evaluation and treatment for the patients with head and neck burns. The primary consideration is swallowing function for nutrition supply for them and the next is speech function for efficient communication and aesthetic impression of them. The purpose of this study is to summarize the preliminary questions of Communication Screening Protocols which can help understand comprehensively on swallowing disorder, motor speech disorder and voice disorder of patients with head and neck burns. We divided the evaluation into 4 evaluation areas including 'oral mechanism', 'respiration/voice', 'articulation', and 'swallowing' by referring to overseas studies dealing with various communication disorders caused by burns, and prepared the final questionnaires by conducting the content validity verification by five expert (speech & language pathologist). The range of Content Validity Index was shown relatively appropriate with .50~.84. There was a conflict of opinions in experts whether the items in the areas of respiration/voice and swallowing may be appropriate, whereas there was no different view of the oral mechanism and articulation area. Through the different characteristics of communication difficulties of patients with head and neck burns, we expect it will be modified appropriately according to the patients through evaluation of burn patients by type and severity.
