Nutritional Intake and Timing of Initial Enteral Nutrition in Intensive Care Patients: A Pilot Study (중환자실 환자의 경관영양 공급시기에 따른 영양섭취양상: 예비연구)

  • Purpose: The purpose of this study was to describe the differences between early and delayed enteral nutrition on nutritional intake. Methods: A pilot cohort study was conducted with 45 critically ill adult patients who had a primary medical diagnosis. Energy prescribed and received were collected during the four days after initiation of enteral nutrition. Adequate feeding was defined as the energy intake more than 90% of required energy. Results: A total of 23 patients (52%) were received early enteral nutrition (within 48 hours of admission). Energy intake of early enteral nutrition was less than intake of delayed enteral nutrition during the four days of the study. Although the difference on day one was significantly greater than the differences on day two, the differences on day two were not different from days three or four. No statistical differences in the adequacy of nutritional intake were found between patients in the early and the delayed group. Conclusion: In critically ill patients receiving early enteral nutrition, more aggressive administration from the beginning will improve the nutritional intake. Additional studies including a large multi-centre, randomized clinical trial are recommended.

Antioxidant Compounds and Antioxidant Activities of Different Varieties of Foxtail Millet and Proso Millet according to Cultivation Time (품종 및 재배시기에 따른 조와 기장의 항산화성분 및 항산화활성)

  • Effects of cultivation time on the antioxidant contents and activities of foxtail millet and proso millet were determined. The cultivation times were early-season (ES), normal-season (NS), and late-season (LS), and the cultivated varieties were Hwanggeum-cho (HGC), Cheongcha-cho (CCC), Samdamae (SDM), Gyeongkwan 1 (GK 1), Gyeongkwan 2 (GK 2), Ganghae-cho (GHC), Hwanggeum-gijang (HGG), Manhongchal-gijang (MHCG), Byeoruk-gijang (BRG), Norangchal-gijang (NRG), and Whin-gijang (WG). The total polyphenol, flavonoid, tannin content, and DPPH and ABTS radical scavenging activities of the methanolic extracts of foxtail millet and proso millet showed significant differences according to cultivation variety and time. The highest total polyphenol content of foxtail millet was 29.33 mg of gallic acid equivalent (GAE)/g of sample in GK 1 at LS, whereas that of proso millet was 23.48 mg of GAE/g of sample in BRG at LS. The highest total flavonoid content of foxtail millet was 2.12 mg of catechin equivalent (CE)/g of sample in CCC at ES, whereas that of proso millet was 4.49 mg of CE/g of sample in BRG at LS. The highest total tannin content of foxtail millet was 14.07 mg of tannic acid equivalent (TAE)/g of sample in SDM at LS, whereas that of proso millet was 15.59 mg of TAE/g of sample in BRG at LS. The highest DPPH radical scavenging activity of foxtail millet was 7.71 mg of TE/g of sample in CCC at NS, whereas that of proso millet was 12.66 mg of TE/g of sample in BRG at LS. The highest ABTS radical scavenging activity of foxtail millet was 8.05 mg of TE/g of sample in GK 1 at LS, whereas that of proso millet was 34.46 mg of TE/g of sample in BRG at LS. Generally, HGC, GK 1, and GHC had more beneficial effects at LS than ES or NS, whereas CCC, SDM, and GK 2 had more beneficial effects at NS than ES or LS, and proso millet had more beneficial effects at LS than ES or NS.

Dumping Syndrome in a Child with Gastrojejunal Tube Feeding (위공장 경관식이(Gastrojejunal Tube Feeding) 환아에서 동반된 덤핑 증후군(Dumping Syndrome) 1례)

  • Dumping syndrome is a known complication of gastric surgery in adults, but a very rare disease in the pediatric population. We report on a case of dumping syndrome in a 19-month-old child, who underwent gastrojejunal feeding tube insertion for the treatment and prevention of gastroesophageal reflux and frequent aspiration pneumonia. At 17 months of age, 2 months after the beginning of gastrojejunal tube feeding, postprandial diaphoresis, palpitation, lethargy, bloating, and diarrhea occurred, and a single episode of convulsion with hypoglycemia were noted. Early and late dumping syndrome was confirmed by an abnormal oral glucose tolerance test with early onset hyperglycemia followed by delayed onset hypoglycemia. Diet therapy including uncooked corn starch then improved the postprandial diaphoresis, abnormal glucose levels, and her nutritional status. We conclude that dumping syndrome may be considered as a complication of gastrojejunal tube feeding in a child.

Early Management of Total Parenteral Nutrition Induced Hepatic Dysfunction with Cyclic Parenteral Nutrition (정맥영양 관련 간기능 장애에서 조기 주기성 정맥영양법의 유용성)

  • Purpose: Liver function test abnormalities have been reported frequently in patients receiving total parenteral nutrition (TPN). In adults, it is known that liver complications decrease with the use of cyclic parenteral nutrition (CPN), especially if the shift to cycling was not too late. However, there are few studies about the effects of cycling on liver injury in children beyond the neonatal period. The aim of this study is to evaluate the effect of the early use of CPN on total parenteral nutrition induced hepatic dysfunction. Methods: Twelve sets of CPN in 11 children (2 months to 17 years) were included in this study. Data on underlying diseases, age, length of time on TPN, macronutrient intake, complications, and biochemical parameters were collected from clinical records. All children had received CPN in the early period of persistent transaminase elevation or cholestasis complicated by previous continuous PN. The duration of infusion off-time in CPN was 2 hours in patients less than 3 months of age and 4 hours in the older children. Results: All 12 cases showed elevated aminotransferase and 5 of them also showed cholestasis. Serum total bilirubin concentration was normalized in all 5 cases with median periods of 8 days (p<0.05) after initiation of CPN. ALT either decreased significantly or was normalized in all cases with median periods of 30 days (p<0.05) on CPN. The CPN was well tolerated without significant complication except for one case of hyperglycemia. Conclusion: The early use of cyclic parenteral nutrition had a beneficial effect in improving hepatic dysfunction complicated by TPN in children.

A Randomized, Double-Blind, Placebo-Controlled Trial of Early Ursodeoxycholic Acid Administration for Prevention of Total Parenteral Nutrition-Induced Hepatobiliary Complications (총정맥영양법의 간담도 합병증에 대한 Ursodeoxycholic Acid 조기투여의 이중맹검 위약대조군 연구)

  • Purpose: Ursodeoxycholic acid (UDCA) is known to decrease hepatic injury by promoting the biliary secretion of retained toxic endogenous bile acids in hepatobiliary diseases complicated by total parenteral nutrition (TPN). However, most studies have focused on treatment for complications after TPN. We investigated the preventive role of early administration of UDCA in TPN-induced hepatobiliary complications by a randomized, double-blind, placebo-controlled trial. Methods: Between May 2000 and May 2002, thirteen patients, who were given TPN more than 10 days in the hospital, were assigned randomly to two groups. One was the case group (7 patients) who were given UDCA simultaneously with TPN regimen, and the other, the control group (6 patients) who were given placebo. Their age ranged from 1 day to 13 years. They were affected with diseases impossible for enteral nutrition, such as prematurity, cerebral palsy, chronic diarrhea, anorexia nervosa, pancreatitis, and cyclic vomiting. The duration of TPN ranged from 10 to 70 days. Hematologic parameters including liver function test were measured at regular intervals, and the duration, composition, administration rate, total calorie of TPN were recorded. The serum levels of total bilirubin, aspartate aminotransferase (AST), alanine aminotransferase (ALT), and alkaline phosphatase were compared between groups after cessation of the study. Results: The autoregressive coefficient of the control group was 0.4419 (p=0.0651) in bilirubin, -0.0431 (p=0.7923) in AST, 0.2398 (p=0.2416) in ALT, and 0.2459 (p=0.1922) in alkaline phosphatase by mixed procedure model when the parameters were referred to the case group. Conclusion: The serum level of total bilirubin did not increase in comparison with that of the control group, but statistically insignificant, when both TPN and UDCA were administered simultaneously from the beginning.

Changes of Growth and Flowering Characteristics in Rapeseed Cultivars with Different Sowing Date (파종시기에 따른 유채(Brassica napus L.) 품종별 생육 및 개화특성)

  • This study was carried out to understand the effect of sowing date on growth characteristics, flower duration of a total of 6 domestic rapeseed cultivars, "Sunmang", "Tammiyuchae", "Tamlayuchae", "Naehanyuchae", "Yongsanyuchae" and "Hallayuchae" with different sowing dates (25 Sept., 5 Oct., 15 Oct., 25 Oct., 5 Nov.) in Muan, Jeollanamdo, Republic of Korea. The results obtained were summarized as the follows: The number of seedling stand after overwintering was decreased in all 6 cultivars. It changed little until 5 Oct., and then showed continuously largely decrease as the sowing date being delayed. As the sowing date was being delayed, plant height was decreased while Silique length and thousand seed weight were increased. Seed yield in all 6 cultivars decreased almost linearly with late sowing date. In correlation analysis between the delaying sowing date and growth characteristics, it was inversely correlated with plant height ($r=-0.769^{**}$), No. of branches/plant ($r=-0.760^{**}$), No. of siliqua/panicle ($r=-0.631^{**}$) and seed yield ($r=-0.946^{**}$), while showed a positive correlation with silique length (r=0.635), seed diameter ($r=0.629^{**}$) and thousand grain weight ($r=0.422^*$). No. of seeds/silique and seed set percentage were not significantly correlated with the delaying sowing date. The flower duration was long in order of Sunmang, Tammiyuchae, Yongsanyuchae, Naehanyuchae, Hallayuchae and Tamlayuchae. Varietal variation of flowering date was larger with early sowing date than with delaying sowing date. The range of flower duration across all cultivars was from 1 day of Tamlayuchae to 14 days of Sunmang according to the sowing date, and the end flowering date was able to be extended from 2 to 9 days, compared to normal date of end flowering.
