A Randomized Controlled Clinical Trial of Topical Herbal Gel Treatment for Chronic Shoulder Pain (만성 어깨 통증의 한약 추출 외용 젤 치료에 대한 임상연구)

  • Objectives : The aim of this study is to evaluate the efficacy of a topical herbal gel application for the treatment of chronic shoulder pain. Methods : We compared the effects of Dapureo gel, which contains several herbal medicines known to improve shoulder pain, with those of placebo gel by double-blind method. 30 participants were randomized - 15 were assigned to treatment group and the other 15 were assigned to control group. Either Dapureo gel(treatment group) or placebo gel(control group) was applied topically by themselves, once a day for 2 weeks. Primary outcome was daily visual analogue scale(VAS) changes for shoulder pain which was self-reported for 2 weeks. Secondary outcome was the difference in the total shoulder pain and disability index(SPADI) which was measured at a baseline and 2 weeks after the treatment. Results : Primary outcome: Subjects of treatment group showed statistically significant improvement in VAS compared to control subjects continuously from the second day(p <0.05) to the fifteenth day(p <0.001). The treatment group showed 31% of pain reduction on the fifteenth day, while the control group showed only 7%. Secondary outcome: In terms of SPADI changes, the treatment group showed improvement compared to the control group(p <0.01). Conclusions : These results suggest that the topical herbal gel treatment used in this study is effective in improving chronic shoulder pain.

Fabrication of anti-reflection thin film by using sol-gel hybrid solution (Sol-gel 하이브리드 용액을 이용한 반사방지막 제조)

  • Anti-reflection (AR) thin films were fabricated on a glass substrate by using an ultrasonic spray. Glycidoxypropyl trimethoxysilane (GPTMS) and tetraethyl orthosilicate (TEOS) were used to synthesize a sol-gel hybrid coating solution. The moving speed of spray nozzle was changed from 15~25 mm/s to control the coating thickness of AR thin film. As the moving speed of spray nozzle increased, the thickness of AR thin film decreased from 138 nm to 86 nm. When the AR thin film was fabricated by nozzle moving speed of 20 mm/s, the refractive index and thickness of AR thin film was measured to be 1.31 and 104 nm, respectively. The average reflectance and transmittance of AR thin film coating glass was measured to be 0.75 % and 94 %, respectively into the visible light range of 380~780 nm.

  • 최근 광대역 서비스에 대한 소비자들의 욕구가 차츰 증가하고 있고 대상콘텐츠도 데이터와 음성 및 비디오까지 포함된 멀티미디어 서비스로 변화함에 따라, 통신서비스 제공업체들은 이에 대한 해결책을 찾는 것이 지상과제가 되었다. xDSL, 케이블 모뎀, 광랜 등 엑세스 네트워크가 잘 발달되어 있는 국내에서도 멀티미디어 서비스의 최종 수신 장치가 될 TV, PC, 오디오 기기 및 전화기 등에까지 네트워크를 연결하기 위해서는 댁내에서 또 다른 홈 네트워크를 구성해야 하는 상황이다. 이러한 홈 네트워크를 가능하게 하는 기술로는 홈 RF, 무선랜, 블루투스 등 무선 홈 네트워크 기술과 IEEE1394, 이더넷, 홈 PNA, 전력선통신과 같은 유선 홈 네트워크 기술로 나눌 수 있다. 무선 홈 네트워르 기술의 경우, 댁내에서의 반사와 감쇄 등의 영향에 의한 음영지역이 존재하는 단점이 있고 또 RF단을 구현해야 하므로 시스템 가격이 올라가게 된다. IEEE1394, 이더넷, 홈 PNA 같은 유선 홈 네트워크기술의 경우에는 댁내 통신을 위해 새로운 선을 포설해야 하는데 이를 위해서는 막대한 시설 투자비가 들어가게 된다. 이 막대한 투자비는 홈 네트워크 구축에 많은 시간이 걸리게 하는 요인이 될 뿐만 아니라, 일반 사용자들이 서비스를 이용하기에는 가격적으로 부담스럽게 된다. 전력선통신 (PLC: Power Line Communication) 은 전기를 공급하는 전력선에 흐르고 있는 상용주파수 50/60Hz의 저주파 전력신호에 고주파 신호를 활용하여 데이터를 실어 나르는 통신기술이다. 집안 곳곳 이미 포설되어 있는 전력선이 이미 하나의 네트워크를 구성하고 있기 때문에 번거롭고 값비싼 추가 배선작업 없이 바로 네트워킹이 가능하다. 이와 같은 이유로 고속 PLC는 설치 용이성, 접근성, 속도 및 비용부분 등에서 경쟁기술에 비하여 여러 장점을 가지고 있다. 젤라인은 국내 전력선통신 표준을 만족하는 24Mbps 고속 전력선통신 칩을 기반으로 다양한 전력선 채널환경 하에서 최적의 통신을 보장하는 전력선 통신시스템을 제공하고 있으며, 이를 소개하고자 한다.

Comparison of Single and Sandwich Collagen Gel on the Survival and Metabolism of Rat Hepatocytes Primary Cell Culture (쥐 간세포 일차배양 세포의 생존능과 대사능에 단층과 복층 콜라젠 젤이 미치는 영향의 비교)

  • We compared the effects of two different systems of collagen matrix protein application on the survival and the biological functions of cultured primary hepatocytes. The rat liver primary hepatocytes were grown for approximately 40 days in vitro either on single collagen gel or between collagen sandwich gels. The morphological changes were observed for this culture period. While the hepatocytes grown on single gel began to die around at 7 days of culture, the cells grown between collagen gels still maintained their viability and began to die after 15 days. As markers for liver hepatic functions, we determined the biochemical activities of hepatocytes such as the secretions of albumin, fibronectin, fibrinogen, urea, and the reduction of secreted ammonia. We found that the rat hepatocytes cultured between collagen gels maintained fairly good biochemical functions than the hepatocytes cultured on single gel did. Therefore, the application of an extracellular matrix protein, collagen, in sandwich form was confirmed as a better choice for maintaining the functional hepatocytes culture for long term in vitro.

THE EFFECTS OF HYALURONIC ACID GEL ON THE HEALING OF ORAL MUCOSA (히알룬산젤이 구강 창상 치유에 미치는 영향)

  • Purpose: In spite of various advantages of hyaluronic acid in wound healing, there are few research about wound healing process and period in oral and maxillofacial surgery. So, We evaluated the effects of local application of hyaluronic acid gel on wound healing of the oral mucosa using this animal model. Materials and methods: Young adult New Zealand White rabbits, weighting between 2.5 and 3.0kg, were used. Almost uniform round ulcers could be created on the gingival of the rabbits by chemical injury with acetic acid. In experimental group, hyaluronic acid gel was applied daily until the wounds healed and in control group, ulcer lesion was not any treatment. The lengths of ulcers were measured the longest and shortest axes of the ulcers and calculated the areas of ulcer. For histological examination, specimens were made, and observed under a light microscope. Results: The results showed that the number of fibroblasts, new blood vessels and the epithelial thickness from experimental group was higher than from control group. Hyaluronic acid promoted proliferation of the fibroblast, keratinocytes isolated from gingival tissue of rabbits in vitro. Topical application of hyaluronic acid accelerated healing of ulcers created in rabbits. Conclusion: The hyaluronic acid may be effective for wound healing of oral mucosal lesions.

  • It is a gel type high functional toothpaste containing vitamin C, E, propolis extract and the rest of herb with a nanoemulsion state. Vitamin C, E is known as the material with an eminent anti-oxidation effect. Propolis is known as the material with an antimicrobial and anti-inflammatory effect. We have been succeeding in making nanoemulsion of vitamin C, E and propolis through the high pressure homogenizer using stable oil and lecithin and the gel type high functional tooth paste were made from nanoemulsion of vitamin C, E and propolis. We observed the process of wound protecting effect and cure effect for a wound of soft tissue, gingival tissue and mucous membrane showing ulcer and inflammation in oral cavity after applying a gel type high functional toothpaste to patient. As a result, the wound were healed very fast and any side effects were not shown. We confirmed that a gel type high functional toothpaste with nanoemulsion of vitamin C, E and propolis extract has good effect not only for wound healing but also for treatment of ulcer-like lesion in oral cavity. So we report our cases with review of literatures.

  • Nonlinear inelastic behavior of an HP saddle shell has been examined by a finite element computer program developed on a Cray Y-MP. The mesh convergence is studied using three progressively refined finite element mesh models, $16{\times}16$, $32{\times}32$ and $64{\times}64$, for the elastic and inelastic analyses. It is shown that the $32{\times}32$ mesh model gives a solution that is very close to that given by the $64{\times}64$ mesh model, thus, showing a convergence. The inelastic analysis shows that the shell has a tremendous capacity to redistribute the stresses. At the ultimate, the concrete cracks and the reinforcement yieldings are spread out all over the shell, indicating that the stress distribution in the shell is approaching that given by the classical membrane theory. The present computer program provides a very useful tool for evaluating the nonlinear ultimate behavior of concrete shells during the design process.

Rupture and Intrapleural Migration of a Cohesive Silicone Gel Implant after Augmentation Mammoplasty: A Case Report (코헤시브 실리콘 젤 유방삽입물을 이용한 유방확대술 후 발생한 유방삽입물의 흉강내로의 이탈 및 파열 증례보고)

  • Purpose: Breast implant ruptures and displacement are problematic complications after augmentation mammoplasty. The authors report a patient whose cohesive silicone gel implant ruptured and migrated into the pleural cavity after augmentation mammoplasty. Methods: A 23-year-old female had received augmentation mammoplasty at a local clinic a week before visiting our hospital. When the patient's doctor performed a breast massage on the sixth postoperative day, the left breast became flattened. The doctor suspected a breast implant rupture and performed revision surgery. The implant, however, was not found in the submuscular pocket and no definite chest wall defect was found in the operative field. The doctor suspected implant migration into the pleural cavity, and after inserting a new breast implant, the doctor referred the patient to our hospital for further evaluation. The patient's vital signs were stable and she showed no specific symptoms except mild, intermittent pain in the left chest. A CT scan revealed the ruptured implant in the left pleural cavity and passive atelectasis. Results: The intrapleurally migrated ruptured implant was removed by video-assisted thoracic surgery (VATS). There were no adhesions but there was mild inflammation of the pleura. No definite laceration of the pleura was found. The patient was discharged on the first day after the operation without any complications. Conclusion: Surgeons should be aware that breast implants can rupture anytime and the injury to the chest wall, which may displace the breast implant into the pleural cavity, can happen during submuscular pocket dissection and implant insertion.

A Study on the Architecture and Extension Project for the Church of St. Lorenzo (성 로렌조 교회의 건축과 증축계획에 관한 연구)

  • The purpose of this paper is to study on the architecture and extension project for the Church of St. Lorenzo. This study is composed of a through analysis of the extension elements on the church building and extension type for extension project of the Church of St. Lorenzo. The results of study are as follows: 1. The extension project for the Church of St. Lorenzo was basically consisted of the concept of a horizontal extension plan through both the interior and exterior space of the church building. This project was the plan of concept of additional or affixing extension through existing spaces with necessary spaces to make up for the previous simple space and form as well as functional aspect. 2. The unit spaces of the nave, the aisle, the small chapels bilaterally adorning the aisles, the transept and the chapels around transepts, and the sacristies by the extension project for the Church of St. Lorenzo is composed of regulation, balance and harmony as geometric space composition according to simple proportion system on whole and parts as well as parts and parts, through the definite articulation by each space zoning. 3. The most important innovative aspect different to previous churches in the extension project for the Church of St. Lorenzo is the compositive system of each other symmetrical spaces through long centrical axis of central part, applying the module system for horizontal arrangement. In particular, the use of regular module on repetition and proportion of rhythm from architectural composition system was precisely composed with the alter that consists of visual focus to express more accurate perspective.

  • Pharmaceutical use accounts for a great part of articles and papers on crosslinking of polymers. Crosslinking of polymers used for tissue engineering and drug delivery respects non-cytotoxicity and in situ gelling. The crosslinking of polymers is aimed not only at the improvement of modulus, chemical resistance, and thermal resistance, but also at endowing them with such functions as metal adsorption, antifouling, and ion exchange via crosslinked segments. Smart polymers responding to environmental change, and cosslinking mediated by light, enzyme, natural compound and in aqueous medium in consideration of environment are being studied. Developing new polymeric materials is essential along with the pharmaceutics aiming at the longevity of 120 years old. Functionalization and property adjustment of polymers through crosslinking will be done more delicately. Hydrogels will be focused on injectable and in situ gel forming. In the coating industry crosslinking system with low non-toxicity and low energy consumption will be developed in consideration of workers and environment.
